만성질환자의 주관적 건강상태에 미치는 영향 요인 = Factors of Influencing Subjective Health Status in the Adults with Chronic Disease - Focusing on Socio-Economic Status of Diabetics

국문 초록 (Abstract)

현대사회에 증가하고 있는 만성질환은 완치가 어렵고 평생 지속적인 자기관리가 핵심으로, 대표적인 만성질환인 당뇨병의 경우 약물, 식사, 운동 등 생활 전반에 걸쳐 질병에 대한 자기관리가 합병증을 예방하고 건강한 삶을 살아가는 데 필수적이다. 하지만 소득수준, 교육 정도, 직업영역에서의 낮은 사회경제적 지위는 일상생활에서 꾸준히 요구되는 당뇨병 자기관리를 수행하는 데 장애가 되고, 나아가 당뇨병 환자가 지각하는 건강상태에 영향을 미친다. 본 연구에서는 전국조사를 통해 완성된 한국의료패널데이터를 활용하여, 사회경제적 지위가 당뇨병을 가진 성인의 주관적 건강상태에 미치는 영향을 살펴보았고, 주요 발견으로 소득을 나타내는 의료보장형태가 유의미한 변인으로 나타났다. 이러한 결과는 첫째, 사회경제적 지위를 나타내는 지표를 한국의 성인 당뇨병 환자에게 적용할 때 민감성 및 문화적 고려가 요구된다는 점, 둘째, 저소득층 당뇨병 환자를 대상으로 하는 차별화된 보건복지정책 및 실천전략이 필요함을 시사한다. 주요 결과를 바탕으로 사회복지 현장에서 저소득층 당뇨병 환자 개입을 위해 고려해야 할 함의를 논하였다.

Living with diabetes like other chronic disease requires self-management in terms of medication, diet and exercise in daily life. But this is often challenge for diabetics with low Socio-Economic Status(SES). Because they have various barriers to diabetes self-management due to the lack of personal and social resources. This study is intended to examine the effect of SES on influencingubjective health status in the adults with diabetes. Data came from the 2011 Korea Health Panel(KHP) and current analysis utilized data on 564 adult diabetics(age ≥ 20 years). One of major findings is that income is the significant variable effecting on the subjective health status of adult diabetics. Accordingly, this study shows that both the sensitivity and cultural considerations in exploring the variables of SES and the differentiated health promotion intervention should be requested to chronic disease patients with low SES.
