노인장기요양보험제도의 치매 정책에 관한 고찰 = A Study on Policy for Dementia in Long-term Care Insurance System for the Elderly

국문 초록 (Abstract)

본 연구의 목적은 외국의 사례들을 통하여 치매의 요양인정개념을 알아보고, 이를 근거로 한국의 장기요양보험제도에서 치매지원을 위한 등급판정요인을 고찰하여, 치매노인 및 가족의 어려움을 제도적으로 관리하고 지원할 수 있는 적극적인 방안을 모색하고자 하는 것이다. 한국의 장기요양보험은 일상생활활동(Activity of Daily Living: ADL) 중심의 신체기능장애정도를 중심으로 등급판정이 이루어져, 치매의 정도는 크게 고려되지 않고 중증의 신체수발 중심의 서비스가 제공된다. 하지만 치매노인은 경증이라 하더라도 장시간의 보호가 필요하며, 중증으로 악화되지 않도록 하는 것은 물론 초기 전문적 치료와 보호가 절실하다. 선진국들의 사례에서처럼 장기요양보험제도의 치매지원관련 활성화를 위하여 전문적인 치매평가도구를 사용하여 인지문제로 인한 일상생활수행능력의 장애를 정확하게 평가하고 치매로 인하여 발생하는 문제, 주변가족들의 부양부담을 명확하게 하는 것이 필요하다. 또한, 재가서비스와 주야간보호서비스의 다양한 활용, 치매관련 상담기능의 확대, 치매 초기 발견과 빠른 치료에 대한 지원 등이 선행되어 이루어져야 할 것이다.

The purpose of this study is to analyze the support for dementia patients in Long-term Care Insurance System of Korea and other nations. The classification criteria in Korea focus on the capabilities of ADL and the degree of dementia is considered a little. But senile dementia`s inappropriate conditions have always been under danger in deed of protection. The family is responsible for senile dementia and the access of social protection of the anguishing family is very low. In order to provide for the early establishment of policy about dementia, it is necessary to evaluate senile dementia with evaluation kit. It is important to evaluate ADL failure through this evaluation kit. And it is necessary to clarify the burden of families by dementia. And the law must be presented in elders protections for social insurance to settle the home service, day-night care service and counseling system.
