한국 사회는 1인 가구 비중이 급증하고 있으며, 특히 만성질환을 가진 중장년 1인 가구는 '만성질환', '1인 가구', '중장년'이라는 취약성이 교차하는 집단으로 주목받고 있다. 이들은 질병의 지속적 관리가 요구되나 가족 지원체계의 부재로 적절한 의료 이용에 구조적 한계를 겪는다. 사회적 배제, 특히 노동 및 주거 배제는 건강 불평등의 핵심 지표이나, 기존 연구는 이들의 인과적 경로와 복합적 메커니즘 규명에 한계가 있었다. 이에 본 연구는 사회적 배제가 의료 이용량에 미치는 영향을 종단적으로 분석하고, 특히 투파트 모형을 통해 패널 고정효과 회귀 분석에서 드러나지 않는 심층 효과를 규명하고자 하였다.
본 연구는 한국의료패널(2014-2022)자료를 활용, 만 40~64세의 만성질환 보유 1인 가구 695명(1,987 person-years)을 패널 고정효과 모형 및 투파트 모형으로 분석하였다. 주요 연구 결과는 다음과 같다. 첫째, 주거 배제(아파트에서 연립/다세대 이주)는 응급실 방문을 유의하게 감소시키는(p<.05) 것으로 나타났다. 둘째, 투파트 모형을 통한 심층 분석 결과, 패널 회귀분석에서 관찰되지 않았던 상쇄 효과(Suppression Effect)가 규명되었다. 주거 배제(기타 주거)는 외래 이용량(강도)을 약 59.0% 유의하게 증가시키는(p<.05) 상반된 경로가 확인되었다. 셋째, 성별 조절효과 분석 결과, 주거 배제(연립/다세대 이주)가 입원일수에 미치는 영향에서 유의미한 성별 차이가 발견되었다(p<.05).
본 연구는 만성질환 중장년 1인 가구의 의료 이용 문제가 노동 배제와 주거 배제라는 사회 구조적 문제와 깊이 연관되어 있음을 실증적으로 밝혔다. 특히 투파트 모형을 통해, 주거 배제(이용 증가)가 다른 요인과 상쇄될 수 있는 복합적 메커니즘을 규명하였다. 정책적으로는 본 연구의 상쇄 효과 발견에 근거하여 이원화된 접근이 필요하다. 주거 배제 집단(이용 증가 경향)에게는 찾아가는 건강 관리 서비스 연계 등 예방적 접근이 요구되며, 노동 배제 집단에게는 의료 접근성 보장 정책이 필요함을 시사한다. 셋째, 노동-주거-건강을 통합적으로 다루는 정책 패러다임의 전환이 강조된다.

Korean society is experiencing a rapid increase in the proportion of single-person households, and middle-aged single-person households with chronic diseases are attracting attention as a group where the vulnerabilities of "chronic disease," "single-person household," and "middle age" intersect. While these individuals require continuous disease management, they face structural limitations in accessing appropriate medical care due to the absence of family support systems. Social exclusion, particularly labor and housing exclusion, is a key indicator of health inequality; however, existing research has been limited in identifying their causal pathways and complex mechanisms. Therefore, this study aimed to longitudinally analyze the impact of social exclusion on healthcare utilization and to identify in-depth effects not revealed in panel fixed-effect regression analysis, particularly through the Two-Part Model.
This study utilized data from the Korea Health Panel (2014-2022), analyzing 695 single-person households aged 40-64 with chronic diseases (1,987 person-years) using panel fixed-effect models and two-part models. The main findings are as follows. First, housing exclusion (moving from apartments to multi-family housing) significantly reduced emergency room visits (p<.05). Second, in-depth analysis through the two-part model revealed a suppression effect that was not observed in panel regression analysis. A contradictory pathway was confirmed where housing exclusion (other housing types) significantly increased outpatient utilization intensity by approximately 59.0% (p<.05). Third, gender moderation analysis revealed significant gender differences (p<.05) in the impact of housing exclusion (moving to multi-family housing) on hospitalization days.
This study empirically demonstrated that healthcare utilization problems among middle-aged single-person households with chronic diseases are deeply associated with social structural issues of labor exclusion and housing exclusion. In particular, through the two-part model, this study identified complex mechanisms in which housing exclusion (increased utilization) can be offset by other factors. Policy-wise, based on the discovery of suppression effects in this study, a dual approach is needed. Preventive approaches such as outreach health management services are required for housing-excluded groups (with increased utilization tendencies), while policies ensuring healthcare accessibility are needed for labor-excluded groups. Third, a paradigm shift in policies that integrate labor, housing, and health comprehensively is emphasized.
