Needle insertion robots have been actively studied as a means to improve the instability of manual procedures through remote and robotic control. However, significant challenges remain in avoiding collisions and mechanical interferences in C-arm–based X-ray environments. To address these issues, this study proposes a cable-driven parallel robot (CDPR)–based needle insertion system for interventional pain procedures. The proposed system was designed to operate within the limited workspace imposed by C-arm imaging systems and incorporates a virtual Remote Center of Motion (RCM) mechanism to maintain a fixed pivot point at the skin entry site during needle insertion. A cable interference avoidance strategy was implemented to enable stable end-effector motion under constrained spatial conditions. The system performance was experimentally evaluated through phantom and cadaver studies. In addition, a cable-tension–based external force estimation method was introduced to monitor resistance changes during needle insertion. Rather than directly measuring needle tip forces, the proposed approach utilizes variations in cable tension to estimate the resultant external force acting on the end- effector, enabling real-time detection of the Loss of Resistance (LOR) phenomenon associated with tissue penetration. Experimental results demonstrated that the proposed system can reliably track RCM-based insertion motions and detect LOR events without additional sensors at the needle tip. These results confirm the feasibility of the proposed CDPR-based robotic system for interventional pain procedures and highlight its potential to enhance procedural safety and intuitiveness in C-arm–guided clinical environments.
