혼종모형을 이용한 암묵적 간호지식의 개념분석 = Concept analysis of tacit nursing knowledge by hybrid model

국문 초록 (Abstract)

본 연구는 혼종모형을 이용하여 우리나라 임상현장에 실재하는 암묵적 간호지식의 본질을 알아보고 그 개념의 정의와 속성을 밝히고자 수행된 질적 연구이다. 이론적 단계에서의 광범위한 문헌고찰을 통해 확인된 암묵적 간호지식의 속성과, 현장작업 단계에서 경력간호사들을 대상으로 수행된 심층면담자료의 분석을 통해 확인된 암묵적 간호지식의 속성을 비교분석하여 최종분석단계에서 암묵적 간호지식의 속성과 정의를 규명하였다.
암묵적 간호지식은 임상 상황 맥락 속에서 사람 및 업무와 관련된 지식으로 3가지 차원 즉, 자기 영역, 타인 영역, 업무영역에서 나타났다. 자기 영역의 속성으로는 선 체득, 인지, 반성적 사고 속성과 타인 영역의 대상자 이해, 상호작용 능력 속성, 업무영역에서는 자득 획득, 행동지향, 업무 성취 속성으로 나타났다. 각 속성의 범주로는 자기영역에서 7개, 타인 영역에서 5개, 업무영역에서 23개로 총 35개로 도출되었다.
암묵적 간호지식의 정의는 ‘전문적 간호지식을 바탕으로 반복된 임상실무경험을 통해 비공식적으로 습득’되고 ‘체화되어 무의식적 행동양상’으로 나타나는 ‘개인적 간호업무지식 및 예술적 기술’로 나타났다. 또한, 암묵적 간호지식은 개인적 요소를 기초로 하여 다차원적이고 비구조적인 상황에서 시행착오와 반성을 통해 발달되며 신속한 상황파악, 통합적 이해와 재해석, 추론력, 정확하고 신중한 합리적 직관적 판단, 임상적 의사결정, 조직력, 업무자신감, 유연한 대응력 및 상호작용능력을 제공하여 실무효율성을 높이는 유용성이 매우 높은 지식으로 나타났다. 한편, 이러한 지식은 불완전함으로 인한 오류가능성이 있으므로 적용 시 적절성과 정확성에 주의가 요구되는 것으로 확인되었다. 그럼에도 불구하고 암묵적 간호지식은 임상에 만연되어 각각의 기준으로 사용되고 있어 임상간호에 직· 간접적으로 크게 영향을 미치고 있는 것으로 나타났다. 따라서 건전한 암묵적 간호지식의 생성, 공유와 집단지성 및 동료검토 등을 이용한 검증과 명시화를 통해 바르게 적용되도록 하기 위한 개인적, 조직적 노력이 요구된다.

다국어 초록 (Multilingual Abstract)

The study is qualitative to investigate the nature of tacit nursing knowledge practiced in the clinical field in korea using hybrid models and clarify the definitions, attributes of the concept. The nature and definition of tacit nursing knowledge identified through a review of the extensive literature at the theoretical stage, the analysis of the in-depth interview data conducted by the career nurses in the field work stage it is compared and analyzed.
The tacit nursing knowledge appeared in three dimensions as the knowledge related to the person, the work context of the clinical situation, the self, others, and task.
The attributes of the self domain were as follows: pre-embodiment, cognition, and reflective thinking. The attributes of the others domain were as follows: understanding the subject and ability of interaction. The attributes of the task domain were as follows: self acquisition, action orientation, and task achievement. Each attribute was categorized as 7 in the self domain, 5 in the other domain, and 23 in the task domain.
The definition of tacit nursing knowledge appeared to be personal nursing knowledge and artistic skills that were learned informally and embodied through repeated clinical practice experience based on professional nursing knowledge and appeared as unconscious behavior patterns. In addition, tacit nursing knowledge is developed through trial and error the reflection in a multidimensional, unstructured situation based on individual factors, and is used for prompt grasp of the clinical situation, integrated understanding and reinterpretation, inference ability, accurate and considerate rational and intuitive judgment, Organizational ability, confidency in practice, flexible responsiveness and interaction ability. On the other hand, it is confirmed that this knowledge requires attention to the appropriateness and accuracy when applied as the possibility of error due to incompleteness. Nevertheless, tacit nursing knowledge has been widely used in clinical practice and has been shown to have a great impact directly or indirectly on clinical nursing. Therefore, individual and organizational efforts are needed to ensure that they are properly applied through validation and clarification using the generation, sharing and collective intelligence and peer review of sound implicit nursing knowledge.
