    서론: 폐결핵 이후 만성 폐질환 발생의 변화 양상 및 이의 빈도나 치료 이후 임상양상의 변화에 대해서는 아직까지 명확히 밝혀지지 않았다. 본 연구에서는 폐결핵 환자에서 향후 만성 폐질환 발생의 양상과 이와 관련한 위험 요인들을 살펴보고자 하였다.

    방법: 국민건강보험 맞춤형 자료를 이용하여 2010년 1월 1일부터 2019년 12월 31일까지 폐결핵을 진단받고 치료를 받은 환자 68,515 명을 대상으로 나이, 성별, 입적 년도 기준 1:1 짝짓기를 시행하였다. 대상자 중 폐결핵 환자는 53,368명, 폐외결핵 환자 12,463명, 폐결핵과 폐외결핵이 동반된 환자는 2,684명이었다. 주요 만성 호흡기질환 중 만성폐쇄성폐질환, 기관지천식, 간질성폐질환, 기관지확장증, 그리고 폐암 발생에 대한 위험도를 주요 연구결과 지표로 설정하였다. 임상 정보 등을 바탕으로 다변량 콕스 회귀분석을 통해 호흡기질환 발생에 영향을 미치는 요인들을 살펴보았다. 또한 폐외결핵 동반 여부에 따른 민감도분석을 통해 만성 호흡기질환 발생 여부의 차이를 살펴보았으며, 다변량 모델에서 해당 위험요인들의 영향을 살펴보았다.

    결과: 양군의 추적관찰 기간은 5.7년이었다. 추적관찰 기간 동안 각 호흡기질환의 신규 발생률은 1000인년 당 만성폐쇄성폐질환은 5.01, 기관지 천식 8.31, 간질성폐질환 0.38, 기관지확장증 2.64, 폐암은 1.56 이었으며, 각 호흡기 질환의 대조군에서의 신규 발생률은 순서대로 1.40, 5.89, 0.14, 0.47 그리고 0.92 였다. 만성폐쇄성폐질환의 발생 위험도는 대조군에 비해 2.68 (95% 신뢰구간 2.39-3.02)배 높았으며, 기관지 천식의 경우 1.38 (95% 신뢰구간 1.30-1.47)배 발생 위험이 높았다. 폐결핵 환자군에서 간질성폐질환 위험도는 2.50 (95% 신뢰구간 1.82-3.44)배, 기관지확장증 4.45 (95% 신뢰구간 3.80-5.20)배, 폐암의 경우 1.46 (95% 신뢰구간 1.27-1.68)배 높았다. 위험요인에 대한 하위군 분석에서 젊은 연령군에서, 그리고 동반질환 점수가 낮은 군에서 폐결핵 이후 만성폐쇄성폐질환, 기관지확장증 발생 위험도가 뚜렷하게 관찰되었다. 또한 남성에서 여성보다 폐결핵 군에서 폐암 위험도가 뚜렷하게 높게 관찰되었다. 그 외 다른 호흡기질환과의 연관성을 살펴보았을 때, 나이, 성별, 흡연 등 주요 위험요인의 영향은 뚜렷하지 않았다. 폐외결핵을 포함하여 분석한 경우 폐결핵 환자에서 관찰된 호흡기질환 발생 상대위험도와 큰 차이 없이 관찰되었다.

    Background: Concern has been growing regarding post-tuberculosis (TB) morbidities. However, the association between post-TB status and the risk of future lung diseases has been evaluated in only few studies. Therefore, in the present study, the risk of chronic respiratory disease was investigated in a nationwide population-based cohort.

    Methods: Using the Korean National Health Insurance Service (KNHIS) database, this study included TB patients (n = 68,515) and matched non-TB population (n = 68,515) for age, sex, and the year of index date. Of them, 53,368 patients with pulmonary TB (PTB), 12,463 individuals had extrapulmonary TB and 2,684 PTB patients with combined extrapulmonary TB. The risk of subsequent chronic lung diseases, including chronic obstructive pulmonary diseases (COPD), bronchial asthma, interstitial lung diseases (ILD), bronchiectasis and lung cancer was estimated using Cox proportional hazards regression, and stratified analyses for related factors were performed.

    Results: During the median 5.7 years of follow-up, the incidence of COPD was 5.01, 8.31 for bronchial asthma, 0.38 for ILD, 2.64 for bronchiectasis and 1.56 for lung cancer per 1000 person-years for TB patients and 1.40, 5.89, 0.14, 0.47, and 0.92 respectively, for non-TB populations. The overall risk of COPD was 2.68 (95% confidence interval (CI) 2.39–3.02) fold higher in PTB patients than in the matched non-TB group. For bronchial asthma, 1.38 (95% CI 1.30–1.47)-fold higher risk was found in PTB patients than in the matched non-TB populations. The risk of ILD (aHR 2.50, 95% CI 1.82-3.44), bronchiectasis (aHR 4.45, 95% CI 3.80-5.20) and lung cancer (aHR 1.46, 95% CI 1.27-1.68) was higher in PTB patients compared to those in the non-TB population. The presence of extrapulmonary TB did not affect the association between PTB and respiratory diseases. In the stratified analysis for risk factors, PTB patients in younger age groups had higher risk of COPD, and bronchiectasis than those found in older age groups. In addition, lower CCI scores showed a relatively higher risk of COPD and bronchiectasis development in PTB patients than in the matched non-TB group. The strength of the association between PTB and lung cancer was prominent in men than in women compared to non-TB populations.

    Conclusion: PTB patients had a significantly higher risk of respiratory diseases including COPD, bronchial asthma, bronchiectasis, ILD and lung cancer than the general population. The associations were consistently found in PTB patients regardless of combined extra-pulmonary TB. Possible effect modifiers including younger age, and lower CCI in PTB patients showed prominent association with defects in lung function or structure as well as men had higher risk of lung cancer than in women.
