충청권 희귀난치성질환 지역거점병원 네트워크 구축, 2008 = Establishment of local rare disease center network in Chungcheong area

국문 초록 (Abstract)

목표 : 비수도권에 거주하는 희귀난치성질환자들에게 양질의 지속적인 의료서비스를 제공하고 희귀난치성질환 전문 의료 인력 양성 및 체계적인 진료협력체계를 구축하고자 본원에서는 희귀난치성질환의 대부분을 차지하는 유전 질환 및 유전대사질환을 중심으로 희귀난치성질환 지역거점병원을 운영하고자 한다.
내용 및 방법 : 희귀난치성질환 전문클리닉 운영하여 지역 내에 거주하는 희귀난치성질환 환자들에서 양질의 의료 서비스를 제공하고 상담실에 상담 간호사가 상주하여 희귀난치성질환 환자들에게 질병 정보 제공 및 교육, 상담을 시행한다. 지역 내 병원 및 보건소에 홍보하여 지역 내 환자들이 지역거점병원을 이용할 수 있도록 하고 보건소에 희귀난치성질환으로 등록하여 의료비 지원을 받을 수 있도록 한다. 희귀난치성질환 지역전문가 워크샵 및 증례연구회를 시행하여 지역 내 의료진에 대한 교육 및 홍보를 시행한다. 환자 및 보호자 모임을 통하여 질환에 대한 교육 및 정보 제공, 환자 및 보호자들간의 유대 형성 및 경험을 공유할 수 있도록 한다.
결과 : 희귀난치성질환 전문클리닉 및 상담실을 운영하여 대전충청지역의 희귀난치성질환 환자들에게 의료서비스를 제공하고 있으며 현재까지 총 177명의 환자들을 추적 관찰 중이다. 이중 수도권 지역에서 54명의 환자가 전원되었다. 총 46명의 환자들을 보건소에 등록하여 의료비 지원을 받을 수 있도록 하였으며 총 56명의 환자들에게 상담 서비스를 제공하였다. 원내에서 시행이 불가능한 효소 분석, 유전자 분석 등을 병원내 각 과에서 의뢰받아 총 71건의 외부 수탁 검사를 시행하였다. 희귀난치성질환의 교육 프로그램 개발 및 홍보를 위해 전문가 워크샵을 연 2회 시행하였으며, 희귀난치성질환 증례 연구회를 3회 시행하여 소그룹으로 각 병원의 희귀 질환 증례를 발표하고 토의하였다. 또한 환자 및 보호자 모임을 연 2회 주관하여 환자에 대한 교육 및 경험을 공유할 수 있는 기회를 제공하였다.
고찰 및 결론 : 희귀난치성질환 지역거점병원을 운영하면서 환자들에게 양질의 의료서비스를 제공함으로써 수의 환자들이 수도권 지역에서 전원 되었으며, 상담간호사와의 상담을 통하여 의료비 지원, 정신적인 지지를 받는데 도움이 되었다. 지역전문가 워크샵 및 희귀난치성질환 증례연구회를 통해 지역 내 의료인에 대한 교육에 이바지하였으며, 환자 및 보호자 모임을 통한 교육 및 보호자들간의 유대 관계 형성에 도움이 되었다.

다국어 초록 (Multilingual Abstract)

Purpose : This project was initiated to provide high quality medical service to the patients with rare disease who live in the rural district, to educate medical specialist and to build up medical collaboration system. This activity was carried out by managing the patients with genetic diseases and inherited metabolic diseases.
Methods : We established the special clinic for rare diseases and consultation office to provide high quality medical service, disease information, education, and counseling. A nurse counselor has an important role in these services. Also, we made a notice about local rare disease center hospital to the local clinics and public health centers and tried to register lots of patients to the public health centers to get special medical insurance support for rare disease from government. Rare disease workshop for local medical staffs, case conferences and patients support group meetings were organized to train medical specialist and to offer medical information about rare disease.
Results : One-hundred and seventy seven patients with rare disease have been followed up in the rare disease special clinic. Fifty-four patients were transferred from the hospital located in the national capital region. Forty-six patients were registered in the local health center to get special medical insurance support for rare disease from government. Fifty-six patients visited consultation office for counseling. Seventy-one special tests, consulted from each department of the hospital such as enzyme study and DNA analysis, were requested to the outside laboratory. Medical specialist workshops (2 times), rare disease case conferences (3 times), and patients support group meetings (2 times) were held.
Conclusions : A lot of patients were transferred from the hospital in the Capital area by providing high quality services in the local rare disease center. The role of nurse counselor became essential for patient' care and psychological support. Local rare disease center contributed the education of local medical staffs, patients and their families.
