미술치료가 소아암 아동의 공포와 어머니의 불안에 미치는 효과 = The effect of art therapy on fear of child and anxiety of mother with the pediatric cancer patient

The purpose of this study is to explore the effect of Art Therapy on fear of child and anxiety of mother with the pediatric cancer patient.
The participants in this study were a 6 year-old pediatric cancer patient and his mother. The program of mother and son Art Therapy consisted of 20 sessions and each 90minute session was ca— 278 —
rried out once a week. The treatment of materials were used to investigate the change of the facial-expression fear scale(FEFS), state anxiety inventory(STAI) and House-Tree-Person Test(HTP) before and after the Art Therapy was carried out.
First of all, the pediatric cancer patient and his mother in each session were analysed to reveal the effect of Art Therapy. Then fear of child and anxiety of mother derived from their fear and anxiety as well as the point change of treatment of materials were examined.
The results of this study are as follows:
First, The fear of child and anxiety of mother with the pediatric cancer patient and his mother showed a positive change through Art Therapy.
Second, Art Therapy affected to reduce the fear of child. The score of the facial-expression fear scale(FEFS) decreased from 48 to 28 suggesting a 33.3% reduction and the score of House-Tree-Person Test(HTP) decreased from 27 to 9 suggesting a 32.7% reduction in fear of child.
Third, Art Therapy affected to reduce the anxiety of mother. The score of the state anxiety inventory(STAI) decreased from 54 to 36 suggesting a 22.5% reduction and the score of House-Tree-Person Test(HTP) decreased from 18 to 7 suggesting a 20.0% reduction in anxiety of mother.
Therefore, according to the study, it has been to light that mother and son Art Therapy would be effective for the fear of child and anxiety of mother with the pediatric cancer patient and his mother .

국문 초록 (Abstract)

본 연구는 의료적 장면에서 미술치료를 실시하여 소아암 아동의 질병이나 의료적 처치에 대한 불안이나 공포를 감소시키고, 아동의 치료에 가장 영향력이 있는 어머니도 함께 실시하여 아동의 질병과 그로 인한 불안을 이해하고 현실적인 상황을 수용하여 아동의 치료에 도움을 주기 위한 기초적인 임상자료를 제공하는 데 그 의의가 있으며, 미술치료가 소아암 아동의 공포 및 어머니의 불안에 미치는 효과를 살펴보고자 하였다.
본 연구의 대상은 K대학병원 소아과에서 “급성 림프구성 백혈병”으로 소아암 진단을 받은 6세 남자 아동과 그를 병원에서 직접 돌보고 있으며 본 연구의 목적과 방법을 듣고 참여하기를 승낙한 어머니이며, 미술치료를 6개월 동안 주 1회 90분씩 총 20회기를 실시하였으며, 각 회기별로 그림과 행동에서의 질적 분석을 하였다. 그리고, 아동에게는 얼굴표정 공포 측정도구(FEFS)와 HTP검사, 어머니에게는 상태불안 측정도구(STAI)와 HTP검사를 통한 사전․사후의 변화를 측정하여 양적 분석을 하였다.
본 연구의 결과 다음과 같은 결론을 얻었다.
첫째, 미술치료가 소아암 아동의 공포 감소에 효과적인 것으로 나타났다. 회기별 질적 분석과 얼굴표정 공포 측정도구로 측정한 총점과 변화율의 감소, HTP 분석 기준 중 불안(공포) 측정 지표수와 변화율도 감소되었다.
둘째, 미술치료가 소아암 아동 어머니의 불안 감소에 효과적인 것으로 나타났다. 회기별 질적 분석과 상태불안 측정도구로 측정한 총점과 변화율의 감소, HTP 분석 기준 중 불안 측정 지표수와 변화율도 감소되었다.
즉 미술치료를 통해 소아암 아동과 어머니가 질병이나 치료과정에 대한 감정과 지각을 표현하고 그림을 통해 대화함으로서 소아암 아동과 어머니가 가지게 되는 공포나 불안을 이해시켜 소아암 아동의 공포 및 어머니의 불안 감소에 효과적인 것으로 나타났다.
