최근 대사성 질환의 발병 연령이 낮아지는 추세를 고려할 때, 노화의 영향이 상대적으로 적고 사회적 활동이 활발한 초기 성인기 인구에서 전신 염증 상태와 청력 변화 간의 관련성을 규명할 필요가 있다. 본 연구는 20-39세 한국 성인을 대상으로 전신 염증 지표인 고감도 C-반응성 단백(high-sensitivity C-reactive protein, hs-CRP) 수치와 순음평균역치(pure-tone average, PTA) 간의 연관성을 분석하여, 초기 성인기의 청력 관리에서 전신 염증 상태를 함께 고려할 필요성을 검토하고자 하였다.
제9기 국민건강영양조사(2022-2023년) 자료를 활용하여 만 20-39세 성인 2,047명을 최종 분석하였다. 복합표본 설계를 반영한 다변량 선형회귀분석을 통해 연령, 성별, 이어폰 사용 시간, 흡연 상태를 보정한 기본모형과, 음주, BMI, HbA1c, 심혈관계 약물 복용 여부 및 중이염 진단 이력을 추가로 보정한 확장모형을 구축하였다. 또한 hs-CRP 사분위수 분석, 연령군별 분석, BMI 및 성별 층화분석을 수행하였다.
분석 결과, 기본모형에서는 hs-CRP 농도가 증가할수록 저주파, 전주파 및 고주파 PTA가 모두 유의하게 상승하는 양의 연관성이 관찰되었다. 그러나 확장모형에서는 저주파 PTA와의 연관성만 통계적으로 유의하게 유지되었다(β=0.335, p=0.02). hs-CRP 사분위수 분석에서도 저주파 PTA에서만 유의한 선형 경향이 관찰되었으며(p for trend=0.03), 최상위 사분위수에서 제1사분위수 대비 저주파 PTA가 유의하게 높았다(β=0.994, p=0.02). 연령군별 분석에서는 hs-CRP와 저주파 PTA 간의 양의 연관성이 20-39세 초기 성인기뿐 아니라 40-59세 및 60세 이상 연령군에서도 일부 관찰되었다. BMI 및 성별 층화분석에서는 일부 하위집단에서 양의 연관성이 관찰되었으나, 상호작용 검정 결과는 통계적으로 유의하지 않았다.
본 연구는 20-39세 초기 성인기 인구에서 hs-CRP와 저주파 PTA 간의 유의한 연관성을 확인하였으며, 이러한 연관성은 생활습관, 대사적 요인, 심혈관계 약물 복용 여부 및 중이염 진단 이력을 보정한 이후에도 유지되었다. 이는 초기 성인기의 미세한 저주파 청력역치 변화가 소음 노출만으로 설명되기보다, 전신 염증 및 대사·혈관 건강 상태와도 관련될 수 있음을 나타낸다. 특히 저주파 PTA는 임상적 난청으로 분류되기 이전 단계의 초기 청각 기능 변화를 반영할 가능성이 있으므로, 청년층의 청력 저하 예방에서는 외부 음향 자극 관리와 함께 금연, 적정 체중 유지, 신체활동, 혈당 관리 등 전신 건강을 개선하기 위한 예방적 접근이 함께 고려될 필요가 있다. 다만 본 연구는 단면연구이므로 인과관계 해석에는 제한이 있으며, 향후 종단연구를 통해 hs-CRP 상승이 저주파 청력역치 변화 또는 청력 저하의 진행을 예측하는지 확인할 필요가 있다.

Considering the recent trend toward earlier onset of metabolic diseases, it is necessary to examine the association between systemic inflammation and hearing changes in early adulthood, a period in which the influence of aging is relatively limited and social activity is high. This study aimed to investigate the association between high-sensitivity C-reactive protein (hs-CRP), a marker of systemic inflammation, and pure-tone average (PTA) among Korean adults aged 20-39 years, and to examine whether systemic inflammatory status should be considered in hearing management during early adulthood.
Using data from the 9th Korea National Health and Nutrition Examination Survey (KNHANES, 2022-2023), a total of 2,047 adults aged 20-39 years were included in the final analysis. Multivariable linear regression analyses accounting for the complex sampling design were performed. A basic model was adjusted for age, sex, earphone use time, and smoking status, while an extended model was additionally adjusted for alcohol consumption, body mass index (BMI), HbA1c, cardiovascular medication use, and history of otitis media. Additional analyses were conducted using hs-CRP quartiles, age-stratified models, and stratified analyses by BMI category and sex.
In the basic model, higher hs-CRP levels were significantly and positively associated with low-frequency, all-frequency, and high-frequency PTA. However, in the extended model, only the association with low-frequency PTA remained statistically significant (β=0.335, p=0.02). In the hs-CRP quartile analysis, a significant linear trend was observed only for low-frequency PTA (p for trend=0.03), and participants in the highest quartile had significantly higher low-frequency PTA than those in the lowest quartile (β=0.994, p=0.02). In the age-stratified analyses, positive associations between hs-CRP and low-frequency PTA were observed among adults aged 20-39 years as well as in some middle-aged and older age groups. In the stratified analyses by BMI category and sex, positive associations were observed in some subgroups; however, the interaction tests were not statistically significant.
This study identified a significant association between hs-CRP and low-frequency PTA among adults aged 20-39 years, and this association remained after adjustment for lifestyle factors, metabolic factors, cardiovascular medication use, and history of otitis media. These findings suggest that subtle changes in low-frequency hearing thresholds during early adulthood may be related not only to noise exposure but also to systemic inflammation and metabolic or vascular health status. In particular, low-frequency PTA may reflect early auditory function changes before the development of clinically defined hearing loss. Therefore, approaches to preventing hearing decline in young adults should consider not only the management of external acoustic stimuli but also preventive strategies aimed at improving overall health, such as smoking cessation, maintaining a healthy body weight, physical activity, and blood glucose control. However, because this study used a cross-sectional design, causal inference is limited. Future longitudinal studies are needed to determine whether elevated hs-CRP predicts changes in low-frequency hearing thresholds or the progression of hearing decline.
