    With the development of medical science, molecular imaging has become a core technology for early diagnosis and effective therapy in modern diagnostic medicine. Recent developments in molecular imaging technologies has seen nuclear medicine and medical ultrasound (US) imaging make advancements using amphiphilic phospholipid-based shells for selective and targeted drug delivery. The aim of the present study was to develop and evaluate a new radiopharmaceutic process for the acquisition of improved positron emission tomography (PET) images of integrin αvβ3-expressing tumors, and a perfluorocarbon nanodroplets (PFC NDs) based PET-US dual-modality carrier to selectively deliver diagnostic/therapeutic radiopharmaceuticals using ultrasound with a dual imaging probe for hybrid PET-US technology.
    This study’s radiopharmaceutical investigation focused on integrin αvβ3 which plays an important role in tumor metastases and angiogenesis. Arginine-glycine-aspartate (RGD) peptide motif has the disadvantage that despite a high binding affinity with integrin αvβ3, due to inherent hydrophilic properties, they are rapidly excreted from the circulatory system.. The purpose of this study was to develop a novel 68Ga labeled cyclic RGD peptide which could enhance PET tumor imaging with improved pharmacokinetics, by adapting biphenyl groups between chelator and RGD peptides. 68Ga-NOTA-biphenyl-c(RGDyK) was 37% less hydrophilic than 68Ga-NOTA-c(RGDyK). 68Ga-DOTA-biphenyl-c(RGDyK) demonstrated a 12% higher lipophilicity level than 68Ga-DOTA-c(RGDyK) which was used as a reference compound. In small animal in vitro PET images, 68Ga-NOTA- biphenyl-c(RGDyK) and 68Ga-DOTA-biphenyl-c(RGDyK) represented relatively lower blood-clearance and increased signal to noise ratio compared with 68Ga-NOTA-c(RGDyK) and 68Ga-DOTA-c(RGDyK), respectively.
    In the study of PET-US dual-modality carriers, perfluoropentane (PFP) NDs were fabricated by mixing liquid-phase PFC with a phospholipid solution consisting of 1,2-dipalmitoyl-sn-glycero-3-phosphocholine (DPPC) and 1,2-dipalmitoyl-sn-glycero-3-phosphate sodium salt (DPPA). The main source of 64Cu was synthesized 64Cu-diacetyl-bis (N4-methylthiosemicarbazone) (64Cu-ATSM). 64Cu-ATSM was encapsulated into the NDs in a size-controlled manner, by exploiting the hydrophobicity of 64Cu-ATSM and using a vial mixer and an extruder.
    The fabricated 64Cu-ATSM-loaded PFC NDs (64Cu-PFC NDs) were evaluated using in vitro/in vivo PET-computed tomography (PET-CT) and US imaging, particle size analysis, and transmission electron microscope (TEM). The acquired in vitro PET images were quantitatively evaluated using mean ROI (region of interest) radioactivity after reconstruction with an ordered subset expectation maximization 2D (OSEM 2D) algorithm. US images were obtained using an US image scanner with a 5 MHz linear probe in B mode in a degassed water tank. The acquired US images were evaluated based on the average brightness of the ROI. The in vivo PET-CT images of the 64Cu-PFC NDs were acquired over 30 min as dynamic and delayed images (4 h) post direct intratumoral injection into a nude mouse model bearing subcutaneously xenografted human gastric cancer NCI-N87 cells. In the acquired in vitro PET images, 64Cu-PFP NDs were observed as a hot spot in the lower section of the test tube where the drug was present, whereas 64Cu-ATSM exhibited wide distribution without a specific hot spot. The radioactivity of 64Cu-PFP NDs in the lower section was substantially increased by 445 % and 397 % compared to the upper and middle sections, respectively. 64Cu-PFP NDs in the acquired in vitro US images also demonstrated strongly contrast-enhanced US images. In the acquired mouse model PET images, the tumor uptake of the 64Cu-PFP NDs was relatively low (approximately 65%), compared to 64Cu-ATSM, determined by PET-delayed imaging analysis.
    Consequently, this study developed a biphenyl-conjugated c(RGDyK) which was a potential candidate for the imaging of integrin αvβ3-expressing tumors and 64Cu-PFC NDs which have the potential to be activated by ultrasound as a PET-US dual imaging probe. The dual-functional concept of theragnostic radiopharmaceuticals-encapsulated PFC NDs and a noble biphenyl group conjugated cyclic RGD peptides with improved pharmacokinetics are expected to contribute to research of selected/localized drug delivery systems with hybrid PET-US technology.

    분자영상기술은 의학과학 기술의 발전과 함께 현대의학의 조기 진단과 효과적인 치료 분야에 있어서 핵심기술로 성장해 왔다. 대표적인 분자영상 기술 중 하나인 핵의학과 초음파 영상 기술은 선택적, 표적 지향적 약물전달을 위하여 양친매성 인지질 기반의 소포체를 이용한 다양한 연구를 수행하여 왔다. 본 연구의 목적은 인테그린 αvβ3가 과발현된 종양의 진단에 있어 향상된 양전자 단층 촹영 영상의 획득이 가능한 새로운 방사성 의약품을 개발하고 이를 평가하고자 하였으며, 양전자 단층촬영-의료용 초음파 (PET-US) 융합기술을 위하여 이중 영상 프로브로서의 기능과 함께 초음파를 이용한 진단 및 치료용 방사성 의약품을 선택적으로 전달 할 수 있는 과불화탄소 나노액적 기반의 수송체를 개발하고 이를 평가하고자 하였다. 방사성 의약품 연구에서, 본 연구는 종양의 전이와 혈관 재형성 과정에 있어서 인테그린 αvβ3가 중요한 역할을 수행하는 것에 초점을 두었다. RGD 펩타이드는 이러한 인테그린 αvβ3와 효과적으로 결합함에도 불구하고, 고유의 친수성 특성에 의하여 체내 순환기 시스템으로부터 빠르게 배출 되는 단점을 가지고 있다. 본 연구의 목적은 킬레이터와 RGD 펩타이드 사이에 바니페닐 그룹을 적용 시킴으로써 향상된 약물의 동태학적 특성과 함께 강조된 종양 양전자 단층촬영 영상의 획득할 수 있는 새로운 68Ga 표지 싸이클릭 RGD 펩타이드를 개발하고자 하였다. 68Ga-NOTA-biphenyl-c(RGDyK)는 바이페닐이 합성되지 않은68Ga-NOTA-c(RGDyK)과 비교하여 친수성이 약 37%정도 감소하였으며, 68Ga-DOTA-biphenyl-c(RGDyK)은 68Ga-DOTA-c(RGDyK)과 비교하여 소수성 특성이 약 12% 증가하는 것으로 평가 되었다. 체외 검사와 함께 수행된 소동물 영상에서, 68Ga-NOTA-biphenyl-c(RGDyK)과 68Ga-DOTA-biphenyl-c(RGDyK)는 68Ga-NOTA-c(RGDyK)과 68Ga-DOTA-c(RGDyK)과 비교하여 각각 상대적으로 낮은 혈액 내 배출을 보였으며, 증가된 신호 대 잡음 비를 보여주었다.     제작된 64Cu-PFP NDs은 원형형태로서 평균직경은 약 224 ± 78.6 nm로 평가되었고, 원심분리 후 실험관 하단에서 펠렛(pellet) 형태로 응집되었다. 체외 PET 영상에서, 순수 64Cu-ATSM은 실험관 전 영역에서 균일하게 분산된 방사능 분포를 보여준 반면 64Cu-PFP NDs은 약물이 존재하는 하단영역에서 강한 방사능 집적을 보여주었다. 하단 영역의 64Cu-PFP NDs방사능은 상단과 중간 영역과 비교하여 각각 445 % and 397 %로 증가되었다. 64Cu-PFP NDs은 또한 초음파영상에서 강한 에코 영상을 보여주었다. 소동물 PET 영상에서, 64Cu-PFP NDs은 64Cu-ATSM 과 비교하여 약 65 % 정도의 낮은 종양 섭취율을 보였다.

    분자영상기술은 의학과학 기술의 발전과 함께 현대의학의 조기 진단과 효과적인 치료 분야에 있어서 핵심기술로 성장해 왔다. 대표적인 분자영상 기술 중 하나인 핵의학과 초음파 영상 기술은 선택적, 표적 지향적 약물전달을 위하여 양친매성 인지질 기반의 소포체를 이용한 다양한 연구를 수행하여 왔다. 본 연구의 목적은 인테그린 αvβ3가 과발현된 종양의 진단에 있어 향상된 양전자 단층 촹영 영상의 획득이 가능한 새로운 방사성 의약품을 개발하고 이를 평가하고자 하였으며, 양전자 단층촬영-의료용 초음파 (PET-US) 융합기술을 위하여 이중 영상 프로브로서의 기능과 함께 초음파를 이용한 진단 및 치료용 방사성 의약품을 선택적으로 전달 할 수 있는 과불화탄소 나노액적 기반의 수송체를 개발하고 이를 평가하고자 하였다. 방사성 의약품 연구에서, 본 연구는 종양의 전이와 혈관 재형성 과정에 있어서 인테그린 αvβ3가 중요한 역할을 수행하는 것에 초점을 두었다.
