혈관성 치매 집단에서의 우울증과 인지기능 및 병소의 관계 = Relationships among depression, cognitive functions, and lesion sites in vascular dementia

국문 초록 (Abstract)

본 연구는 혈관성 치매 집단에서 우울증과 인지기능의 관계를 밝히고, 치매집단의 두뇌 병소를 확인하여 우울증의 위험을 증가시키는 병소를 찾기 위하여 수행되었다. 초기 단계의 혈관성 치매로 진단받은 노인 35명과 정상 노인 35명이 연구에 참여하였고, 두 집단 간에 인구학적인 변인에 있어서 유의미한 차이는 없었다. Seoul Neuropsychological Screening Battery로 종합적인 인지기능을 평가하였고, Geriatric Depression Scale, Cornell Scale for Depression in Dementia 및 DSM-IV(American Psychiatry Association, 1994)의 기준을 사용하여 우울 수준을 측정하였다. 연구 결과 혈관성 치매 환자들이 정상 노인에 비하여 우울 수준이 유의미하게 높다는 사실이 발견되었고 우울증의 수준은 인지 기능 중에서 언어적 기억력, 주의 집중 능력, 정신운동 속도 및 집행 기능과 유의미한 부적 상관관계가 있음이 밝혀졌다. 또한 위계적 회귀 분석 결과 우울증은 음소적 유창성에 유의미한 영향을 미치고 있는 것으로 밝혀졌는바, 좌반구의 전두엽 영역과 우울증의 밀접한 관련성이 시사되었다. 이러한 결과를 바탕으로 혈관성 치매 환자들의 병소를 좌반구의 전두엽 영역에 손상이 있는 집단과 그 밖의 영역에 손상이 있는 집단으로 나누고 두 집단의 차이를 살펴본 결과 좌반구 전두엽 영역에 손상이 있는 집단은 정상 노인 집단보다 우울 수준이 유의미하게 더 높았고, 이 집단이 임상적으로 유의미한 우울 수준을 나타낼 확률이 정상 노인의 6.44배에 이른다는 사실을 발견하였다. 그러나 같은 혈관성 치매 집단이라도 좌반구 전두엽 영역 이외의 영역에 손상이 있는 집단의 우울 수준은 정상 노인의 우울 수준과 유의미한 차이를 보이지 않았다.
