장기요양보호노인의 수발만족과 노인의 우울증의 상관관계에 관한 연구 = Relationship between impaired elders satisfaction with care and their depression

This study aims to investigate relationship between impaired elders’ satisfaction with care and their depressive symptoms, using a sample of Survey of Impaired Elderly and Their Service Utilization conducted in Seoul. Guided by the social support literature, the authors propose that depressive symptoms of impaired elderly will be associated with their satisfaction with care after controlling (1) socio-demographic characteristics of impaired elderly (gender, marital status, coresidence with children, educational attainment, and subjective economic status); (2) health status of impaired elderly (limitations in activities of daily living, and the number of mental health problems); (3) social support (frequency of visits from children, and emotional support). Controlling socio-demographic characteristics, health status, and social support of impaired elderly, multiple regression analyses indicate that greater depressive symptoms of impaired elderly are associated with lower satisfaction with physical care and quality of relationship with the caregiver. In conclusion, the authors suggest that support and services should be provided for both impaired elderly and family caregivers in order to reduce depressive symptoms of impaired elderly.

국문 초록 (Abstract)

본 연구는 1998년에 실시된 장기요양보호노인 생활과 서비스 실태조사 자료를 이용하여, 장기요양보호노인의 수발에 대한 만족과 노인의 우울증의 상관관계에 대해 분석해 보았다. 본 연구는 사회적 지지(social support)이론을 근간으로 하여, 장기요양보호노인의 수발에 대한 만족(주수발자의 신체적 수발에 대한 만족, 주수발자의 정서적 지원에 대한 만족, 주수발자와의 관계의 질, 가족수발 전반에 대한 평가)이 다음 3가지의 변인집단을 통제한 후에도 노인의 우울증과 상관관계를 가지는 것으로 설정하였다: (1) 노인의 사회인구학적 특성(성별, 유배우상태, 자녀와의 동거형태, 교육수준, 주관적 경제상황), (2) 노인의 건강상태(일상생활수행능력의 제한, 정신적 건강상태), (3) 노인의 사회적 지지(비동거자녀의 방문빈도, 정서적 지지). 다중회귀분석결과에 의하면(n=406), 장기요양보호노인의 사회인구학적 특성, 건강상태, 사회적 지지를 통제한 이후에도, 주수발자의 신체적 수발에 대한 만족이 낮고, 주수발자와의 관계가 수발을 받기 이전에 비해 나빠진 경우, 노인이 더 우울한 것으로 나타났다. 또한 경제적 상황이 어려울수록, 일상생활수행능력에 제한이 있을수록, 정신적 건강상태가 양호할수록 노인은 더 우울한 것으로 나타났다. 결론에서는 본 연구가 갖는 정책적 함의와 제한에 대해 논하였다.
