장애인복지에 대한 연구 : 장애인 정상화 방안을 중심으로

근대적 산업화 정책이 시작된 1960년대 이후 우리나라의 경제적 부는 급속히 성장했다. 그러나, 이러한 경제적인 부는 수많은 교통사고와 산업재해 등 후천성 장애요인을 유발시키는 요인이 되기도 하였다. 이러한 후천성 장애요인이 전체 장애인구중에서 차지하는 비중(89.4%)은 급속히 증가하였고, 다양한 사회 · 경제적 여건에 의해 사회적 취약계층이 증가하면서 사회복지의 이념적 배경을 동반한 복지정책의 정립이 필요해졌다. 장애인복지법 제1조에서는 장애인의 인간다운 삶과 권리를 보장하고 예방과 의료, 교육, 직업재활, 생활환경개선 등 사업을 정함으로써 장애인 복지대책의 종합적 추진을 도모하고, 장애인의 자립, 보호 및 수당의 지급 등에 관하여 필요한 상황을 정함으로써 생활안정에 기여 및 장애인의 복지증진 및 사회활동의 참여증진에 기여함을 목적으로 한다고 되어있다. 장애인의 사회활동의 참여의 증진을 위하여 1990년 1월 13일에는 장애인 고용촉진 등에 관한 법률이 공포되었고 이후 1990년 9월에는 한국장애인고용촉진공단도 설립되었다. 그러나 이렇게 등장한 장애인고용촉진정책은 장애인들의 기대감에 비해 많은 문제점을 가지고 있다. 누구에게나 직업은 사회적 위치와 자기실현에 필수적인 것이다. 장애인에게 직업을 통한 사회참여는 자기실현의 근거 및 비장애인과의 사회통합에 있어서도 중요한 의미가 있다. 사회통합에 있어서 '사회적 위치와역할'과 '사회적 권리와 의무'가 장애인이든 비장애인이든 직업에 의해가능하기 때문이다. 본 논문에서는 사회통합적 복지를 통하여 장애인이 일반적인 환경과 생활방식에 가장 가깝도록 보장하기 위하여 사회 전반적인 장애인 정상화 요소를 검토, 분석하고 대안을 제시하고자 한다.
중증장애인의 탈시설화를 위한 장애인활동지원사업의 활성화 방안
With the abolition of the disability grading system from July 1, 2019, the concept of recognition of disability from 'disability grade' to 'disability degree' has been changed for persons with disabilities. Although a simple system change cannot bring about a big change at once, it can be meaningful that the concept of the severely disabled, which was limited to grades, has been expanded a little more comprehensively. People with severe disabilities who are supported by the Activity Support Project for the Disabled will have a strong desire to receive continuous and high-quality services at their home, and through the expansion of application qualifications, more people with disabilities will want to receive help for independent living. The traditional perception of the welfare of the disabled in Korea is that since the deinstitutionalization movement developed in the sense of accommodating and protecting the disabled in a safer space and focusing on facilities, many people with disabilities should closely relate and cooperate with the local community to live in harmony. It can be said that it is a relatively recent trend to change to a communal character. It is necessary that even the severely disabled themselves be able to express their lives more actively and live their daily lives based on their own desires, even if they are considered as individuals from families outside the center of facilities, to be systematically protected and demanded from within the community. Therefore, this study aims at deinstitutionalization of the severely disabled and examines the current status and problems of the activity support project for the disabled by reviewing major theories and prior studies on the revitalization plan of the activity support project for the disabled, one of social services, and deinstitutionalization of the severely disabled. It was intended to suggest a plan to activate the activity support project for the disabled. First, in terms of activation measures in terms of users, first, the introduction of 24-hour activity support service, second, the expansion of salary diversity, and third, the expansion of family service. In terms of revitalization in the service system, first, development of activities to support the disabled in connection with community care, second, reinforcement of de-institutionalization policy, third, expansion of service support comprehensive survey function, fourth, improvement of treatment and working conditions of active support personnel , Fifth, long-term care insurance service for the elderly and service linkage could be summarized. Key words: severely disabled, deinstitutionalization, normalization theory, support for activities of the disabled, reorganization of the grading system for the disabled. 2019년 7월 1일부터 장애인등급제 폐지로 인해서 장애인 당사자에게는 ‘장애등급’에서 ‘장애정도’의 장애의 인정 개념이 전환되었다. 단순하게 제도의 변경으로 한 번에 큰 변화를 가져올 수는 없겠지만, 등급으로 한정되어있던 중증장애인의 개념이 조금 더 포괄적으로 확대되어 나간 것에 의의를 둘 수 있겠다. 장애인활동지원사업을 지원받는 중중장애인들은 원가정에서 지속적이고 양질의 서비스를 받고 싶은 욕구가 클 것이며, 신청자격 확대를 통해서도 더 많은 장애인 당사자가 자립생활에 도움을 받고자 할 것이다. 한국의 장애인복지에 대한 전통적인 인식은 장애인을 더 안전한 공간에서 시설중심으로 수용 및 보호하는 차원에서 탈시설화 운동이 전개된 이후로 많은 장애인이 지역사회와 긴밀히 관계하고 함께 협력해서 한 동네에서 어우러져 살아가야 한다는 공동체적 성격으로 전환되는 것도 비교적 최근의 경향이라고 할 수 있겠다. 중증장애인 스스로도 자기 삶을 더 능동적으로 표현하고 일상생활을 본인의 욕구에 기반한 생활해 나갈 수 있는 것은 시설중심에서 벗어난 한 가정 개인으로 보더라도 당당하게 지역사회 안에서 제도적으로 보호받고 요구할 수 있어야 할 필요성이 크다고 사료된다. 이에 본 연구에서는 중증장애인의 탈시설화를 지향하고 사회서비스 중 하나인 장애인활동지원사업의 활성화 방안에 대해서 주요이론 및 선행연구 검토를 통해 장애인활동지원사업의 현황 및 문제점을 살펴보고 중증장애인 탈시설화를 위한 장애인활동지원사업의 활성화 방안을 제시하고자 하였다. 먼저 이용자 측면에서의 활성화 방안은 첫째, 24시간 활동지원서비스 도입, 둘째, 급여의 다양성 확대, 셋째, 가족서비스 확대를 들 수 있었다. 서비스 제도적 측면에서의 활성화 방안으로는 첫째, 커뮤니티케어와 연동한 장애인활동지원사업의 전개, 둘째, 탈시설 정책 강화, 셋째, 서비스지원종합조사 기능 확대, 넷째, 활동지원인력의 처우 및 근로조건 개선, 다섯째, 노인장기요양보험서비스와 서비스 연동으로 정리할 수 있었다. 위와 같은 목적에 따라서 연구한 결과에 대한 함의를 다음과 같이 요약해 우리나라 실정에 맞는 장애인활동지원사업 정책적 실현방안을 제시하고자 하였다. 첫째, 장애인활동지원급여에 대한 ‘확대’와 ‘보장’이 필요하다. 둘째, 활동지원제도와 개인예산제도의 한국형 모델개발이 필요하다. 셋째, 중증장애인의 주거공간 마련을 위한 주거제도 확충이 필요하다. 넷째, 장애인활동지원인력의 처우개선이다. 주제어: 중증장애인, 탈시설화, 정상화이론, 장애인활동지원, 장애인등급제 개편.
중증장애인의 탈시설화 실현방안 연구 : 발달장애인을 중심으로
불가능할 것 같이 여겨졌던 시설보호중심의 한국사회에서 정부가 ‘커뮤니티 케어(Community care)’를 추진함으로써 장애인 탈시설화가 진일보를 내딛게 되었다. 지금까지 한국의 장애인복지에 대한 인식은 장애인을 시설에서 수용·보호하는 차원이었으며, 장애인에 대한 정책 또한 장애인을 통제하거나 배제하는데 가세하고 있다. 또한 장애인은 사회의 소외계층이며 낙인과 차별의 대상이 되어 대부분 그 가족의 책임으로 전가되는 경향이 컸다. 이러한 장애인 문제를 사회문제로 인지해야하는데 그 이유는 장애인 자립의 문제가 한 개인의 문제로 치부(置簿)하기에는 그 범위가 너무 넓고 크기 때문이다. 장애인에 대한 근본적인 문제 인식이나 장애인을 위한 진정한 의미의 복지를 본격적으로 논의하게 된 것은 비교적 최근의 일이다. 발달장애인은 독립된 인격체로서 사회로부터 필요한 지원을 받아 자신의 삶을 선택하고 통제하며 능동적 주체로 살아갈 수 있어야 한다. 발달장애인 탈시설은 속해있는 서비스 전달체계의 변동이 아니라 한 사람의 삶의 변화로 이해할 필요가 있다. 이에 본 연구에서는 중증장애인 탈시설의 궁극적 목표인 일상적 삶을 실현하기 위한 방안을 모색하기 위해 외국의 장애인 탈시설화 정책사례들을 살펴보고, 중증장애인이 한 사회의 시민으로서 지역사회 내에서 주민들과 함께 일상적 삶을 영위할 수 있도록 정상화 이론에 기반하여 중증장애인의 탈시설화 실현방안을 제시하고자 하였다. 위와 같은 목적에 따라 연구한 결과에 대한 제언은 다음과 같이 요약해 보고자 한다. 첫째, 장애인에 대한 지역주민들의 인식개선이 절실하다. 둘째, 탈시설 및 자립생활을 위한 서비스 및 프로그램이 개발되어야 한다. 셋째, 중증장애인에 대한 차별화된 서비스가 제공되어야 한다. 넷째, 장애인 거주시설의 종사자 처우개선이 필요하다.
통합교육이 비장애 고등학생의 장애인에 대한 경험과 태도에 미치는 영향

국가통계포털에 공개된 자료에 따르면 2014년 국내 등록 장애인 수는 전체 인구의 5% 수준이다. 현재 대한민국 국민 중 100명당 5명이 장애인이며, 국내 학령기의 장애학생 중 일반학교에서 비장애학생과 함께 통합교육(inclusive education)을 받고 있는 비율은 전체 장애학생의 70.4%에 이른다. 1970년대에 이르러 세계 장애 통합교육의 큰 흐름은 Bank-Mikkelsen과 Wolfensberger가 주창한 정상화(normalization) 개념에 입각해 장애학생을 가능한 정상에 가까운 생활을 할 수 있게 하며, 적어도 평균수준의 시민 생활을 할 수 있도록 하는 것을 목표로 한다. 이 연구는 장애학생과 비장애학생이 통합교육 환경에서 함께 교육받는 환경이 특수교육의 주요한 교육방식이 된 현 시점에서 통합교육을 받는 비장애 고등학생과 통합교육을 받지 않는 비장애 고등학생들이 주변 장애인과 관련하여 어떻게 다른 경험과 태도를 형성하는지, 그리고 통합교육 경험 여부에 따른 장애인에 대한 경험과 태도에 차이가 있다면 그 이유는 무엇인지를 살피는 것을 목적으로 한다. 장애인에 대한 비장애학생들의 태도를 측정하는 도구로 R. F. Antonak이 1982년과 1980년에 실시한 장애인에 대한 태도 설문조사 SADP와 ATDP를 적용한다. 연구 대상자들은 서울시에 위치한 통합교육 실시 A 여고 비장애학생 105명과 비통합교육 B 고등학교의 비장애학생 125명, 비통합교육 C여고 비장애 학생 61명이다. 추가적으로 A여고 1학년 비장애학생 3명과 A여고, B고 교사 1명씩을 인터뷰해 학생들의 학교생활, 교우 관계, 개인적 특성들에 대해 이야기 들었다. 첫 번째, 통합교육 실시에 따른 비장애 고등학생의 주변 장애인과 관련된 경험의 차이에서 통합교육 실시 A여고 학생들은 통합교육 미실시 B고와 C여고 학생들에 비해 현재 장애인 친구가 있다고 대답하는 비율이 현격히 높다. 고등학교 이전 통합교육 경험과 관련해서는 설문에 참여한 세 학교 모두 65% 이상의 학생들이 통합교육 경험이 있다고 답했다. 장애인의 도움을 받은 긍정적인 경험을 한 비율은 통합교육 실시 A여고 학생들이 가장 높다. 두 번째, 통합교육 여부에 따른 고등학생들의 장애인에 대한 태도 차이는 장애인에 대한 태도 설문조사(SADP·ATDP) 통계 분석 결과, 통합교육을 실시하는 A여고 학생들의 장애인에 대한 태도가 통계적으로 유의미한 정도로 가장 부정적이었다. 더불어 학생들은 성적이 높을수록 장애인에 대해 긍정적인 태도를 가지고 있으며, 생활수준이 낮은 학생들이 장애인에 대해 긍정적인 태도를 가지며, 장애인 가족을 가진 학생들이 그렇지 않은 학생들에 비해 유의미하게 긍정적인 장애인에 대한 태도를 보인다. 장애인의 도움을 받은 경험도 비장애학생의 장애인에 대한 긍정적인 태도 형성에 유의미한 영향을 끼치는 것으로 밝혀졌다. 세 번째, 통합교육 실시 여부에 따른 비장애 고등학생의 장애인과 관련한 경험의 차이는 현재 학생들이 다니는 학교 유형과 직결된다. A여고 학생들은 2014년 2학기 현재 장애학생 14명과 함께 통합학급 형태로 수업을 받고 있어 나머지 두 학교 학생들보다 장애인을 친구가 있다고 대답한 학생들의 비율이 현격히 높다. B고 학생들은 자율형사립고라는 학교 특성상 성적이 우수한 학생들의 비율이 높아 학생들이 대학입시에 더욱 확고한 목표를 가져 장애인 관련 봉사활동 참여 비율이 A여고 학생들보다 더 높다고 풀이할 수 있다. 마지막으로 통합교육 실시 고등학교의 비장애학생들과 통합교육 미실시 고등학교의 비장애학생들은 왜 장애인에 대한 다른 경험을 가지는가에 대한 연구 결과는 다음과 같다. A여고 학생들은 장애 학생들과 통합학급에서 수업 받으면서 그들과 이야기하고 서로 도움을 주기도 하지만, 실상 장애학생들을 자신들과 다른 온순한 존재이거나 때로는 개인 특성에 따른 문제행동을 일삼는 존재로 보고 있어 현재 통합교육을 받지 않는 고등학생들에 비해 유의미하게 장애인에 대한 부정적인 태도를 가지고 있다고 분석된다. 연구 결과, 지금까지 고등학교 통합교육 현장은 비장애학생들의 장애인에 대한 태도 형성과 관련해 긍정적이고 구체적인 교육적 효과를 도출시키는 데는 성공하지 못했다고 평가된다. 비장애인과 장애인들의 지속적인 교류와 서로에 대한 긍정적인 태도 형성을 위해서 보다 실현가능하고 효과적인 제도와 정책이 개발되고 시행되어야 한다. 통합교육의 실행과 평가와 관련한 후속 연구에서 이러한 부분에 대해 보다 구체적이며 충분한 고찰이 이루어지기를 기대한다. The total number of the government-registered disabled is about 5% of the whole Korean population according to data by Kosis in 2014. It means that currently five of every one hundred Korean are disabled, and the ratio of disabled pupils who are endowed with the inclusive education service with non-disabled ones at general schools accounts for 70.4% of the entire number. Since 1970s, the main stream of the world disabled inclusive education aims to 'let the mentally retarded obtain an existence as close to the normal as possible' by Bank-Mikkelsen and 'let them live as citizens in the normal level at least' by Wolfensberger on the basis of their principle of the normalization. The purpose of this research is to experiment how different experiences and attitudes high school students at inclusive and non-inclusive schools have obtained with the disabled around them, and why they have had such differences with their experiences in their school environment. The research tool for measuring attitude of the non-disabled toward the disabled is ‘The Scale of Attitudes toward Disabled Persons(SADP)’in 1982 and ‘Attitude Toward Disabled Persons(ATDP)’in 1980 by R. F. Antonak. Subjects are 105 non-disabled students at inclusive A girls' high school, 125 non-disabled ones at non-inclusive B high school and 61 non-disabled ones at non-inclusive C girls' high school in Seoul. In addition, the researcher interviewed three 1st grade students of A girls' high school, one teacher at A school and another teacher at B school to comprehend students' routine at classes, their peer relationship, individual features and so on. First, on the difference in experiences with the adjacent disabled of non-disabled high school students concerning participation into the inclusive education, the ratio of students at inclusive A girls' high school who responded to have their own disabled friends is far more frequent than those of non-inclusive schools B and C. Hence, over 65% of subjects at all of three schools answered that they have ever experienced the inclusive education environment before high school. The ratio of who have positive experience being helped by the disabled records the highest among A school students. Second, on the difference in attitudes of non-disabled high school students toward the disabled, the attitude of inclusive A school students recorded the most negative with statistical significance in result of the analysis on survey SADP & ATDP. Furthermore, the higher grade students obtain, the more positive attitude on the disabled they have. Those who live in lower economic status have the most positive attitude on the disabled. Those who have disabled family members show significantly more positive attitude toward the disabled rather than those who don't have disabled families. The experience getting helped by the disabled is also revealed to influence on the formation of positive attitude on them significantly. Third, the reason why the difference on the attitude of non-disabled students toward the disabled directs to the type of their schools. Students at school A are studying at inclusive classes with fourteen disabled ones during the 2nd semester in 2014, so they responded to have disabled friends with much higher ratio than their counterparts at other two schools. Students of school B have participated in volunteer programs for the disabled more frequently than counterparts of inclusive school A. That is explained because students of school B, an autonomic private school, tend to have higher grade than those of school A and they have stronger purpose on the entrance exam for college. Finally, the reason why students of inclusive and non-inclusive high school get different experiences with the disabled are as follows. Students at A girls' high school talk to the disabled peers and cooperate with them at inclusive classes, but in reality, they regard the disabled peers as someone naive who are different from themselves or trouble-makers with their own abnormal feature. Thus non-disabled students of A school are analysed to own negative attitude toward the disabled rather than students with non-inclusive schools with statistical significance. As a result of the research, domestic high school inclusive education field is estimated not to have achieved the positive and concrete educational fulfillment in building favourable attitude toward the disabled of non-disabled students until now. To accomplish sustainable communication and mutual positive attitude between the disabled and the non-disabled, more realizable and effective regulation and policy are desired to be developed and installed. It is anticipated that following researches on the practice and evaluation of the inclusive education to undertake more concrete and enough consideration on these aspects.
