국문요약 성 명 : 김대운 학 과 : 임플란트 지도교수 : 류재준 문제제기 인쇄 방향은 적층가공 과정의 초기 단계에서 인쇄 생산물의 물리적인 특성뿐만 아니라 인쇄 정확도에 영향을 미치기 때문에, 중요하게 결정되어야 할 주제 중의 하나이다. 또한, 인쇄 방향은 표면의 기하학적 구조를 형성하는 층들이 쌓이는 방향과 계층별 구성을 결정한다. 목적 이 연구는 3D 프린팅된 denture base resin의 표면 특성, 미생물의 반응에 대한 인쇄 방향의 효과를 평가하기 위해 수행하였다. 재료 및 방법 표본은 denture base를 제작하는 polymethyl methacrylate (PMMA)를 3가지 인쇄 방향(0°, 45°, 90°)으로 프린팅하였다. 거칠기, 친수성, 표면 에너지, Candidaalbicans(C.albicans)에 대한 반응을 평가하였다. 결과 45°방향의 표본은 다른 그룹과 비교할 때, 더 높은 거칠기와 표면 에너지를 보여 준다 (p < 0.001). C. albicans 의 더 높은 비율은 90° < 45° < 0° 의 순서로 통계적으로 유의하게 증가하며 발견된다. 결론 인쇄 방향은 거칠기, C.albicans에 대한 반응에 유의하게 영향을 준다. 그래서, 인쇄 방향은 의도한 특성으로 생산물을 제조하기 위해 세심하게 결정되어야 한다. 생산물의 적절한 특성을 얻기 위해, 인쇄 변수들을 인쇄 방향에 의존하여 조정해야 한다. 임상적 의미 임상적인 목적으로, 높은 재현성, 낮은 거칠기, 감소된 C. albicans 의 부착을 얻을 수 있기 때문에, 90° 각도로 인쇄한 denture base를 제안한다. 그러나, 제작하는 동안 층사이의 접착 강도를 증가시키는 고려가 아마 필요하다.

Study of heat transfer to the implant-bone interface induced by grinding of occlusal surface of implant nonprecious metal prosthesis Name : Jisu Yang, D.D.S., Department : Department of Esthetic Restorative Dentistry, Graduate school of Clinical Dentistry, Korea University. D.D.S., M.S.D, Ph.D Objective: The aim of this study is to verify amount and phase of heat transferred to implant-bone interface induced by grinding the surface of implant nonprecious metal prosthesis with comparing usefulness of cooling methods. Materials and methods: To manufacture thermometer, 16 K-type thermocouples were attached to internal fixation type implants and embedded into acrylic resin. 10 samples were made with dental nonprecious metal and abutment for casting which composes 3 concavo-convex occlusal side and they were connected to the implants with connecting screws, setting into constant-temperature water tank maintaining 37℃. During grinding the concavo-convex side of prosthesis for 30 seconds in three conditions-noncooling group, air pressure cooling group, water spray cooling group- temperatures were recorded at an interval of 0.05 second. Results were analyzed by 1-way ANOVA, and verified for further reference with Turkey HSD in 95% level of significance. Results: The temperature of implant-bone interface in noncooling group elevated over 47℃, significantly higher than other control groups (P<.05). The temperatures of air pressure cooling group and water spray cooling group were maintained under 47℃, and there were no significant difference between cooling methods. Grinding occlusal surface on implant nonprecious metal prosthesis may cause heat over critical temperature which could interrupt osseointegration. So it is important to do the procedure in cooling condition and both air pressure and water spray cooling method seems to be effective.
상아질에 레진코팅시 self adhesive 레진 시멘트와 간접수복용 복합레진간의 전단결합강도 변화에 관한 연구

Shear bond strength of a self-adhesive resin cement to resin-coated dentin Jee youn Hong, DDS Estheric Restoration Dentistry Graduate School of Clinical Dentistry, Korea University (Director: Professor Jae-Jun Ryu, DDS,MSD,PhD) ABSTRACT Purpose: The aims of this study were 1) to evaluate the effect of a resin coating on the shear bond strength of indirect composite restoration bonded to dentin with a self adhesive resin cement and 2) to compare the shear bond strength with that of a conventional resin cement. Materials and Methods: Thirty six extracted non-carious human molars were prepared: 1) the calculus and periodontal ligament were removed from the teeth. 2) the teeth were stored in normal saline 3) Occlusal enamel of each tooth was removed by using high-speed coarse diamond burs under water cooling. Finally 4) the teeth were flattened by 600 grits silicone carbide paper disks. They were divided into 3 groups: 12 for Unicem (group 1), 12 for Unicem with resin coating(group 2), and 12 for Panavia F(group 3). The dentin surfaces of group 1 and 3 were left uncoated, while the surfaces of group 2 were resin coated with a dentin bonding system, Clearfil SE bond and a flowable resin composite, Metafil flo. The specimens were temporarized for 24 hours. Indirect composite resin blocks (Tescera) were bonded to dentin by resin cements according to the manufacturer’s specifications: Unicem for group 1 and group 2, Panavia F for group 3. After 48 hours of water storage, shear bond strengths were measured by Universal testing machine with a cross head speed of 1mm/min. The data was analyzed with one-way analysis of variance and multiple comparison test(Tukey method). Results: The shear bond strengths of Unicem applied to resin coated dentin surfaces were significantly higher than those of Unicem and Panavia F used to uncoated dentin surfaces.(p<0.0001). Conclusion: Application of a resin coating to the dentin surface significantly improved the shear bonding strength of a self adhesive resin cement in indirect restoration. Thus, the resin coating technique could be highly recommended to supplement the weakness of self adhesive resin cements. 약한 골질에서 임플란트 종류별 초기 안정성의 비교에 관한 연구

The comparison of primary stability by implant type in poor density bone Purpose : The purpose of this study is to compare the primary stability by measuring initial torque, ISQ value, and removal torque according to implant type. Material and methods : Sawbone blocks were used. GSⅢ Implant and TSⅢ Implant in Osstem™ and CMI Implant (IS type) in Neobiotech™ were selected among domestic implants. All implant size was Ø4.0×10.0mm. Kavo Implant engine (INTRAsurg300), Kavo monitoring system, Osstell Mentor and MGT12 torque gauge were used for measuring initial torque, ISQ value, removal torque, respectively. Result : There were significant differences in initial torque among three implants(p<.05). There were significant differences in ISQ value between TSⅢ Implant and CMI Implant (IS type)(p<.05). There was no significant difference in removal torque(p>.05). And there was no correlation among initial torque, ISQ value, removal torque in D3 density bone. Conclusion : Different initial torque, ISQ value, and removal torque are shown by each implant type. 인공뼈의 골질의 차이와 연한 골질에서의 드릴링 차이에 따른 임플란트 초기안정성의 평가

Purpose : The aim of this study was to evaluate the implant primary stability by different densities of artificial bone using an initial and removal torque machine, as well as resonance frequency analysis device. In addition when bone quality is poor, the present study evaluates how different drilling methods affect primary stability. Material and methods : TSⅢ implant which has a diameter of 4.0 mm and lengh of 10 mm were placed the three types of polyurethane artificial bone block using 20 for each and then the insertion torque, implant stability quotient (ISQ) and removal torque were measured. The primary stability of the implant was also evaluated using the manufacturer's recommended drilling sequence in type 4 bone. Results : 1) Insertion torque, ISQ and removal torque were high at the bone with good quality. 2) It had a significant correlation between insertion torque, ISQ and removal torque. 3) Initial stability was obtained higher when it has performed the manufacturer's recommended drilling sequence in poor type 4 bone. Discussion & Conclusion : It is easy to get good primary stability at the bone with good quality. When the implant is placed on poor quality bone, it may get better primary stability by following the manufacturer's recommended using a drilling size one down.
임시수복물즉시수리시 수리재료에 따른 전단결합강도의 비교

Shear bond strength of immediately repaired provisional restoration materials Soo Young Lee, D.M.D. Esthetic Restorative Dentistry, Graduate School of Clinical Dentistry, Korea University (Directed by Professor Jae Jun Ryu, D.D.S., M.S.D., Ph.D.) Purpose : The aim of this study was to evaluate the bond strength between PMMA and bis-GMA resin to provisional restoration materials with different repair methods. Materials and Methods : Ten discs(10mm in diameter and 10mm thick) were fabricated for each provisional resin base materials, including a self-cured methacrylate(Alike), light-cured bis-GMA(Filtek Z-350 XT Flowable Restorative), dual-cured bis-Acrylate(Luxatemp Automix Solar) and divided seven experimental groups. Group 1 is composed of base as Alike and repair material as Z-350. In Group 2, base is Alike and applied monomer liquids and repaired with Z-350. In Group 3, base is Alike and applied a bonding agent(Clearfil S3 bond) and repaired with Z-350. In Group 4, base is Alike and applied monomer liquids and repaired with Alike. In Group 5, base is Z-350 and repaired with Alike. In Group 6, base is Luxatemp Automix Solar and repaired with Alike. In Group 7, base is Luxatemp Automix Solar and repaired with Z-350. The shear bond strengths were measured with a universal testing machine and fracture surfaces were examined under an optic microscope. Results : The mean shear bond strength was 0.79MPa(Group 1), 2.92MPa(Group 2), 13.01MPa(Group 3), 13.35MPa(Group 4), 9.82MPa(Group 5), 10.27MPa(Group 6), and 18.53MPa(Group 7). One-way ANOVA revealed that the shear bond strengths among experimental groups were significant differences(P<0.05). Dunnett’s T3 multiple comparison showed that the shear bond strength of Group 3 was no significant difference with that of Group 4,5,6(P<0.05). The highest shear bond strengths were measured in Group 7. Group 4 and 7 tended to cohesive failure. Conclusion : Group 1,2 were not appropriate for repair provisional restoration due to low shear bond strength. Group 3,4,5,6 may be used to repair provisional restoration. However, additional study was needed to verify clinical acceptance. The highest shear bond strength was measured in Group 7.
Polymerization shrinkage-strain kinetics of elastic impression materials
Purpose The purposes of this study were to measure the polymerization shrinkage-strain and to investigate the polymerization shrinkage-strain kinetics of the impression materials. Materials and Methods Two condensation-cured silicones (Verone Putty Plus, Verone Perfecting Paste), two addition-cured silicones (Doric Soft Putty, Doric ES Flo-light), one alginate (Fidelity) and one polyether-based (Impregum) were investigated. The polymerization shrinkage-strains and their kinetics were measured by using the Bonded-disc method at 5, 10, 30, 60 and 120 minutes after the start of mixing at the ambient temperature of 23 oC. All data were statistically analyzed by one-way ANOVA. Results The shrinkage-strain kinetic graphs showed that all materials shrank immediately and then the rate of shrinkage-strain decreased gradually with time except Fidelity and Impregum that expanded for the initial few minutes. After that, the shrinkage-strain increased in magnitude up to 120 minutes. The shrinkage-strain values of the silicone impression materials and the polyether impression material were 0.24-0.62 % and 0.04 % respectively at the first 5 minutes after the start of mixing. The irreversible hydrocolloid impression material (Fidelity) expanded for the first 5 minutes, and exhibited a delayed start to shrinkage but a continuous shrinkage. At 10 minutes, Impregum, regardless of the lowest values (0.09 %), was not significantly different from Fidelity (P > 0.05), but Fidelity showed the significantly higher shrinkage-strain value of 1.60 % than the other materials (0.21-0.77 %) at 120 minutes. Significance Most shrinkage of the elastomeric impression materials occurred in the first 10 minutes after mixing. Hence, elastomeric impression materials should be left in the mouth at least 10 minutes. Keywords Impression materials, Shrinkage-strain kinetics, Bonded-disc method

- Abstract - Evaluate shear bond strength between bis-acryl provisional restoration and bis-acryl repair material after light-curing, pressure, water-immersion and heat Jin Man Park, D.D.S. Esthetic Restoration Major, Graduate School of Clinical Dentistry, Korea University (Directed by Professor Jae Jun Ryu, D.D.S., M.S.D., Ph.D.) Purpose : The purpose of this study was to evaluate the effect of light-curing, pressure application, heat treatment, and water-immersion on the shear bond strength between bis-acryl resin base and bis-acryl repair material Materials and Methods : A total of 90 specimens were fabricated using bis-acryl resin base and repair materials and divided into 9 groups depending on the different treatments: light-cure, under pressure of 0.2 MPa, heat treatment at 20, 60, 100 and 140oC, and water immersion. All the specimens were evaluated for shear bond strength between base and repair resin materials. The obtained data were statistically analyzed for significance depending on the different treatments. Results : There were no statistically significant differences on shear bond strength between groups at room temperature regarding to light-curing, pressure application, and water immersion. For heat treatment, shear bond strength showed statistically significant increase up to the temperature of 100oC. Chemical deformation of bis-acryl resin was observed over the temperature of 100oC. All the groups under heat treatments showed cohesive failure. Conclusion : Light-cure, pressure application, and water immersion have no effect on shear bond strength between bis-aryl resin base and repair materials. CEREC CAD/CAM system으로 제작한 single crown의 변연 및 내면 적합도 평가

본 연구에서는 Cerec CAD/CAM system에서 software version의 차이와 spacer parameter 설정값의 차이가 보철물의 변연 및 내면 적합도에 미치는 영향을 알아보고자 하였다. software version은 Cerec software (SW) 3.8과 Cerec SW 4.2를 사용하였고 spacer parameter는 40 ㎛와 80 ㎛로 설정하여 single crown을 제작한 후 변연 및 내면 적합도를 측정, 비교하였다. 상악 좌측 제 2 소구치를 지대치로 하는 모형을 제작하여 구강 내 scanner를 이용하여 40개의 디지털 인상을 채득하였다. 채득된 이미지를 software version과 spacer parameter 설정값에 따라 4개의 그룹으로 나누어 crown을 제작하였다. 제작한 crown의 내면에 Fit checker (GC dental, Tokyo, Japan)를 채우고 모형에 위치시켜 경화시킨 후 내면에 부가중합형 인상재 examixfine regular type (GC dental, Tokyo, Japan)를 넣어 경화시켰다. 협설, 근원심으로 두 번의 절단을 가하여 각 8 개씩 16 포인트의 Fit checker 두께를 Optical video measuring system (seven ocean optical technology, Dongguan, China)을 이용하여 측정하였다. 각 지점의 software version의 차이, spacer parameter 설정값의 차이에 따른 적합도 차이를 살펴보기 위해 Friedman Two Way ANOVA를 시행하였고 사후분석으로 multiple comparisons를 시행하였다. 또한 변연 외측, 변연 내측, 측벽, 교합면 부위로 나누어 paired t-test를 시행하여 부위별 유의성을 평가하였다. 실험 결과 변연 외측의 적합도는 software version의 차이나 spacer parameter 설정값의 차이에 따른 유의한 결과를 보이지 않았고 측정치의 평균이 35㎛로 임상적으로 허용 가능한 범위에 있었다. 내면에서는 평균적으로 Cerec SW 4.2에서 Cerec SW 3.8에 비해 더 작은 간극을 보였고 spacer parameter 설정값을 작게 부여할수록 더 작은 간극을 보였다. 이상의 실험 결과로 볼 때 Cerec CAD/CAM system 사용 시 더 나은 보철물의 내면 및 변연 적합도를 얻기 위해서는 가능하면 최신의 software를 사용하고 spacer parameter 설정값을 작게 부여하는 것이 추천된다. Purpose : The purpose of this study is to evaluate the marginal and internal fitness of single crowns fabricated by Cerec CAD/CAM system. Material and Method : 40 digital impressions of an upper second premolar of dentiform were made with the intra-oral scanner and divided into 4 groups depending on software version and spacer parameter. 3.8 and 4.2 software versions were used and spacer parameter was set at 40 ㎛ and 80 ㎛. Crowns were designed and fabricated using Cerec CAD/CAM system. Marginal and internal fitness of the crowns were measured using the replica technique, which employs Fit checker to simulate the cement layer thickness. The data was analyzed using Friedman Two Way ANOVA with multiple comparison and paired t-test at significant level of p<.05. Results : There was no significant difference in the marginal fitness regarding to software version and spacer parameter. In terms of internal fitness, crowns fabricated using 3.8 software version showed larger gap than crowns fabricated using 4.2 software version. Also, to set spacer parameter at 80 ㎛ showed larger gap than to set spacer parameter at 40 ㎛. Conclusions : Single crowns fabricated using Cerec CAD/CAM system provided clinically acceptable marginal and internal fitness. Using the latest software version with smaller spacer parameter provided better internal fitness.
최근 3차원 디지털 테크놀러지의 발전과 함께 치과 수복학에서 전통적으로 쓰이던 다양한 아날로그식 과정을 대체하는, 디지털 치과학에서는 기본적으로 CAD/CAM에 바탕을 두고 있다. 1985년 독일 시로나사에서 개발된 CEREC 시스템이 그 최초라고 할 수 있는데, 현재에는 관련 기술들의 비약적인 발전을 통해서 그 보급의 속도가 매우 빨라졌다. 현재로서는 정확도나 실용성 면에서 전통적인 Conventional procedure를 완전히 대체할 만한 수준에 이르기에는 검증도 더 필요하고 시간이 다소 필요할 것으로 보인다. 치의학 영역에서 사용되는 CAD/CAM을 이용한 수복용 시스템은 크게 3가지 구성요소를 가지고 있는데 그 세가지 요소는 data acquisition unit, processing software 그리고, automated milling machine 이다. Data acquisition은 크게 direct와 indirect로 나뉠 수 있는데, 여기서 direct라 함은 환자의 구강에서 인상을 바로 채득하는 방법이고, indirect라고 하면 환자의 구강에서 전통적인 인상재를 이용해서 인상을 채득하고 stone을 부어 만든 석고 모형을 디지털로 인상을 채득하는 방법을 말한다. Indirect method는 치과에서의 프로시저는 전통적인 방법과 완전히 동일하다. 반면, direct method에서는 구강 안에서 바로 digital mpression을 채득해야 하므로 필수적으로 Intraoral scanner가 필요하게 되고, 이 방법이 현재 수복치과학에서 주요 관심사이자 본 논문의 연구 주제라 할 수 있다. 본 연구에서는 iTero system에 대하여 진행하였다. 이 시스템의 특징을 간단히 정리하면, CEREC과 LAVA의 경우 구강내 스캔을 할 때, 전용 파우더를 뿌려야 하지만, iTERO는 파우더를 뿌릴 필요가 없는 장점이 있다. 파우더의 두께가 인상의 정확성에 영향을 미칠 수 있다는 보고가 있다. 또한 다른 시스템과는 달리 open system으로 스캔된 object를 범용 파일인 stl로 제공하므로 이 파일을 이용하여 여러 다른 소프트웨어에서 사용이 가능하다. 또 다른 특징은 Miling center를 통해 폴리우레탄 재질로 되어 있는 Working cast를 제공해 준다는 점인데, Chairside CAD/CAM을 이용한 보철물의 경우 working cast가 없어 환자에게 바로 try in 하면서 adaptation을 해야 하지만, working cast를 제공하는 경우 working cast 상에서 보철물의 fit을 맞춘 다음 환자에게 시적할 수 있는 장점이 있다. 기존의 방법이든 새로운 방법이든간에 치과수복에서 일련의 모든 과정의 최종 목표 중 하나는 최종 보철물의 정확한 passive fit을 이루는 것이고, 이를 위해서 Conventinal impression에서 그랬듯이 Digital impression 역시 인상의 정확도가 중요하다. 인상의 정확도는 인상 자체의 정확도도 중요하지만, 그 인상체를 이용해서 작업모형, working cast를 만들게 된다면, 이것의 정확도 역시 확보되어야 최종 보철물의 passive fit을 이룰 수 있다. 따라서 본 연구의 목적은 iTero의 digital impression system을 통해 제작된 작업 모형의 정확도를 평가해 보고자 하는 것이었으며, 이를 위해서 오리지날 석고 모형, 가상 모형, 그리고 가상 모형을 통해 제작된 RP 모형 등과 비교를 해 보았다. 실험 재료와 방법으로는, 치과보철학 시간에 실습용으로 상호 채득한 총 100명의 치과대학생들의 상하악 석고모형 중 임상적으로 stable한 교합을 보이는 54개를 선별하여 사용하였다. 3 mm 이상의 총생 혹은 공극이 있거나 제3대구치가 아닌 다른 치아를 상실하였거나 구치부에 수복물 혹은 치료되지 않은 병소를 지닌 치아를 가지고 있는 것으로 판단되는 모형은 모두 제외하였으며 이들을 iTero intraoral scanner를 이용하여 digital pression을 채득하였고, 이를 범용 3D file인 stl 포맷으로 변환하였다. 이 stl file을 먼저 iTero center에 전송하여 폴리우레탄 재질의 작업모형을 만들었다. 또 이 stl file을 사용하여 rapid prototyping achine으로 RP model을 제작하였다. 이로써 original stone casts, virtual casts in form of STL file, polyurethan casts, RP casts의 비교 가능한 네가지 casts 가 제작된다. 각 모델들의 비교 측정을 위해서 Horizontal measurement와 Vertical mesurement를 실시하였다. Horizontal measurement의 경우,cusp tip 사이 거리를 측정하였는데, 전치의 경우는 distal incisal tip을 기준점으로 하였고, 소구치의 경우는 buccal cusp tip, 대구치의 경우는 mesiobuccal cusp tip을 기준으로 삼았다. Vertical measurement의 경우는 견치와 제 2소구치에서 buccal side에서 보이는 cusp tip과 clinical crown의 가장 하방점 치은연 부위를 연결한 직선의 길이를 측정하였다. 앞서 말씀드린 바와 같이 4가지 형태의 모델들에서 실물 모델에서는 캘리퍼스를 이용해서 측정하고, 가상 모형에서는 Rapidform 2004라고 하는 3D 프로그램 내에서 dimension을 측정한 후 상호 비교하였다. 그 결과를 살펴보면, 총 4가지 타입의 모델이 있다보니, 상호간의 비교를 할 때에는 4C2 = 6, 총 6개의 비교조합이 가능하다. ttable1 에서 우선 본 논문의 주된 관심사였던 iTero system을 이용한 Working cast와 다른 model간의 차이에 대한 비교를 앞 쪽 3 collumn에 배치해 보았다. 전반적으로 iTero working 모델과 다른 모델과의 비교에서 icc가 낮게 나오고 있으며, root mean square error로 표시되는 상호간의 차이가 커서 통계적으로 유의한 표시가 있는 부분이 많음을 보실 수 있다. 이것은 다른 말로 하면, 나머지 세 타입의 모델은 서로 차이가 별로 없었지만, iTero working 모델의 경우는 나머지 세 모델과 차이가 좀 있다는 이야기가 된다. Table2에서 다른 방법의 비교를 해 본 결과이다. 이 테이블에서는 OC 즉 stone으로 된 original cast를 gold standard로 하고, 나머지 세가지 모델과 비교한 것에 대해서 차이의 절대값의 평균과 표준편차 측정한 것이다. 역시 iTero working model에서의 차이가 나머지 두 모델에 비해서 상당히 크다는 것을 확인할 수 있다. 이상에서 알 수 있듯이 iTero system을 사용해서 채득한 전자 인상 (digital pression)과 이를 reconstrucion한 가상 모델은 우수한 정확성을 보였다. 그러나 이를 iTero center에 의뢰하여 제작해온 작업 모형의 경우에는 다른 방법으로 제작된 모형과의 비교에서 만족할 만한 재현성을 보이지 못하였으며, 따라서 이를 이용해서 보철물을 adapation 하거나 조정할 때에는 주의가 필요할 것으로 생각된다.
