        Prospective Clinical Trial of Corneal Reconstruction With Biomaterial-Free Cultured Oral Mucosal Epithelial Cell Sheets

        PURPOSE:: To investigate the efficacy and safety of transplantation with biomaterial-free cultured oral mucosal epithelial cell sheets (COMECs) for ocular reconstruction in subjects with total limbal stem cell deficiency. METHODS:: A prospective clinical trial (NCT02149732) was conducted in 8 subjects with total limbal stem cell deficiency after approval from the institutional review board of Seoul National University Hospital (H-0707-043-213) and the Ministry of Food and Drug Safety of Korea. COMECs were prepared in a culture system without the use of any temperature-sensitive polymers or carriers. The COMECs were transplanted without suture fixation. Four subjects underwent penetrating keratoplasty after stabilization of the COMEC transplant. Stable epithelialization, changes in visual acuity, and postoperative complications were evaluated for 6 months. Corneal cytokeratins (K) of 4 subjects who underwent penetrating keratoplasty were stained with an immunofluorescent agent. RESULTS:: The ocular surface was successfully reconstructed in 6 eyes. Complete stable epithelialization was achieved within a mean of 53.6 days. Visual improvement (≥2 lines) was achieved in 62.5% of the eyes. K12 (corneal phenotype), K4, and K13 (mucosal phenotype) were well expressed in grafts after keratoplasty, whereas K1, K8, and K19 were barely expressed. No ocular infections, local tumor formation, or remarkable systemic complications were observed. Ocular reconstruction using COMECs failed in 2 eyes, which had full symblepharon in 4 quadrants. CONCLUSIONS:: Transplanting biomaterial-free COMECs seems to be an efficient and safe procedure to reconstruct the ocular surface in patients who are completely limbal stem cell deficient without a full symblepharon.

      • 소아에서 발병한 human metapneumovirus와 respiratory syncytial virus에 의한 호흡기 감염의 임상 양상

        목적: 국내에서 호발하는 hMPV와 RSV 감염의 임상 양상의 차이가 아직 명확하지 않아 이를 확인하고자 한다. 방법: 2006년 8월부터 2007년 7월까지 분당차병원에 하부 호흡기 감염으로 입원한 환아 1,104명을 대상으로 비인강 흡입법과 RT-PCR을 이용하여 바이러스를 검출하였다. 이중 hMPV와 RSV 감염 양성인 환자 각각 51명과 138명을 대상으로 후향적인 의무기록분석을 통해 검출 시기, 연령, 성별, 임상 진단명, 임상 양상 및 검사 소견을 비교 분석하였다. 결과: hMPV감염은 4월에, RSV감염은 11월에 가장 많이 발생하였다. 두 군 모두 1세 미만의 영아가 가장 많았으나 6개월 이하의 군에서 hMPV는 25% RSV는 40%를 차지하였다. 2세 이상의 군에서는 hMPV가 33.4%, RSV는 19%였다. 진단명은 두 군 모두 폐렴, 급성 세기관지염, 급성 기관지염 순으로 차이를 보이지 않았다. 말초 혈액 백혈구수는 RSV 감염시 더 높았고, 입원 당시 열이 있는 환아의 비율은 hMPV군에서 더 높았다. 결론: 본 연구에서 hMPV는 영유아에서 바이러스성 하부 호흡기 감염의 네 번째로 흔한 원인으로, hMPV 감염의 임상양상은 RSV 감염과 유사하였다. 향후 이러한 연구가 호흡기 바이러스 감염의 효율적인 관리에 도움이 될 것이라 여겨진다. Purpose: To identify the clinical features of human metapneumovirus (hMPV) and the respiratory syncytial virus (RSV) infection in children. Methods: The participants of our study were 1,104 children who were admitted to Bung-dang CHA hospital for lower respiratory infection from August 2006 through July 2007. Nasopharyngeal swabs were taken from the patients, and viruses were identified by RT-PCR. The clinical features of 51 patients with hMPV infection and 138 patients with RSV infection were compared by retrospective review of their medical records. Results: The peak incidence of hMPV infection was noted in April, and that of RSV was noted in November. Both viruses had the highest incidence in patients age <1 year and hMPV infections occurred in 40% and 25% of patients age <6 months, respectively. In the hMPV group, 33.4% of the patients were age ≥2 years, while in the RSV group, 19% were age ≥2 years. In both groups, pneumonia was the most common clinical diagnosis, followed by acute bronchiolitis, acute bronchitis, and asthma. The white blood cell counts were higher in the RSV group, and fever was more frequent on admission in the hMPV group. Conclusion: hMPV was the fourth most common virus causing lower respiratory tract infections in children. The clinical features of hMPV infection were similar to those of RSV infection.         Prognostic Impact of Elective Supraclavicular Nodal Irradiation for Patients with N1 Breast Cancer after Lumpectomy and Anthracycline Plus Taxane-Based Chemotherapy (KROG 1418): A Multicenter Case-Controlled Study

        <P><B>Purpose</B></P><P>This study was conducted to evaluate the impact of supraclavicular lymph node radiotherapy (SCNRT) on N1 breast cancer patients receiving post-lumpectomy whole-breast irradiation (WBI) and anthracycline plus taxane-based (AT) chemotherapy.</P><P><B>Materials and Methods</B></P><P>We performed a case-control analysis to compare the outcomes of WBI and WBI plus SCNRT (WBI+SCNRT). Among 1,147 patients with N1 breast cancer who received post-lumpectomy radiotherapy and AT-based chemotherapy in 12 hospitals, 542 were selected after propensity score matching. Patterns of failure, disease-free survival (DFS), distant metastasis-free survival (DMFS), and treatment-related toxicity were compared between groups.</P><P><B>Results</B></P><P>A total of 41 patients (7.6%) were found to have recurrence. Supraclavicular lymph node (SCN) failure was detected in three patients, two in WBI and one in WBI+SCNRT. All SCN failures were found simultaneously with distant metastasis. There was no significant difference in patterns of failure or survival between groups. The 5-year DFS and DMFS for patients with WBI and WBI+SCNRT were 94.4% versus 92.6% (p=0.50) and 95.1% versus 94.5% (p=0.99), respectively. The rates of lymphedema and radiation pneumonitis were significantly higher in the WBI+SCNRT than in the WBI.</P><P><B>Conclusion</B></P><P>We did not find a benefit of SCNRT for N1 breast cancer patients receiving AT-based chemotherapy.</P>

        녹차씨 압착박 열수 추출물의 지방세포 분화 억제 효과

        본 연구는 녹차씨 압착박의 활용성을 증가해 보고자 녹차씨를 압착한 후 남은 박을 추출하여 총 페놀, 조사포닌, 카테킨, 나린게닌 함량을 확인하고 3T3-L1 세포에서의 지방세포분화 억제 관련 효과를 확인하였다. 또한 기존에 체지방 감소 효능이 있는 것으로 알려진 녹차와도 비교하였다. 녹차씨 압착박 추출물과 녹차추출물의 페놀, 조사포닌, 카테킨, 나린게닌 함량을 비교한 결과 녹차씨 압착박 추출물에서 나린게닌 함량이 높음을 알 수 있었다. 3T3-L1 세포에서 Oil Red O 염색법을 이용한 지방 축적저해 정도 및 세포내 중성지방 함량을 측정한 결과, 녹차씨 압착박 추출물에서 지방 축적저해 및 중성지방의 함량이 감소하는 것을 확인하였다. 녹차씨 압착박이 지방합성 및 에너지 조절 효소에 미치는 영향을 알아보기 위해 단백질 발현 실험을 실시한 결과, 녹차 또는 녹차씨 압착박 추출물에 의해서 AMPK 및 ACC의 인산화 증가 및 FAS의 발현이 감소하는 것을 확인 할 수 있었으며, 이러한 효능은 녹차 추출물보다 녹차씨 압착박 추출물에서 더 강하게 나타나는 것을 알 수 있었다. 이상의 결과로부터 녹차씨 압착박 추출물이 녹차 추출물 보다 지방세포분화 억제 효과가 높게 나타나는 것이 확인되어져, 녹차씨는 추후 체지방 감소 효능을 갖는 식품 소재로도 활용이 가능 할 것으로 생각되어진다. The effect of the hot water extract of defatted green tea seed cake (GTSE) on lipid metabolism and the underlying mechanisms of lipolysis in mature 3T3-L1 adipocytes were investigated. In this study, we found that the naringenin content of GTSE was 5.5 ㎎/g; however, catechins were not detected. The intracellular lipid droplets were stained with Oil Red O dye and quantified. Compared to the control, lipid accumulation was significantly decreased by 52%, and intracellular triglyceride (TG) level was reduced by 33% after treatment with GTSE at a concentration of 40 ㎍/mL. To determine the mechanism of reduction in TG content, we determined the level of fatty acid synthase (FAS), phosphorylation of adenosine monophosphate-activated protein kinase (AMPK), and acetyl-coenzyme A carboxylase (ACC) in the cell model. Incubation of the 3T3-L1 adipocytes with GTSE stimulated AMPK and ACC phosphorylation in a dosedependent manner, and decreased the expression of FAS.

        난소 적출 동물모델에서 고흥산 석류 농축액의 갱년기 증상 개선 효과

        본 연구는 난소 적출 동물 모델을 이용하여 고흥산 석류 농축액 시험군과 함께 이란산 석류 농축액을 주성분으로 하는 시판 제품의 혈청 생화학적 지표, 골밀도 등 몇 가지 지표를 분석하여 갱년기 증상 개선 효과를 비교하였다. 고흥산 석류 농축액과 시판 제품은 난소 절제 동물 모델의 골밀도(bone mineral density, BMD) 및 percent bone volume(bone volume/tissue volume), bone surface density(bone surface/tissue volume)를 증가시켰으며, 특히 고용량의 고흥산 석류 농축액(PE3)은 통계적으로 유의하게 골밀도 및 골볼륨을 증가시켰다. 시판 제품을 투여한 군도 골밀도를 증가시키는 양상을 보였으나 통계적 유의성은 없어 고흥산 석류 농축액이 이란산 석류 제품보다 골밀도 등 뼈 건강에 관련한 갱년기 증상 개선에 더 효과적이라고 사료된다. 고흥산 석류 농축액은 난소 절제에 의한 체중 증가 및 복부/내장지방 축적을 개선하였으며, 농도 의존적으로 HDL-C(high density lipoprotein cholesterol)는 증가시키고 low density lipoprotein cholesterol은 감소시켰다. 또한 고농도 석류 농축액(PE3)의 HDL-C/total cholesterol이 통계적으로 유의하게 증가하여 고흥산 석류 농축액이 갱년기 여성의 비만 및 혈중지질을 개선할 수 있을 것으로 기대된다. 불안장애 실험에서는 고흥산 석류 농축액 투여로 인해 불안장애가 개선되는 경향을 보였으나 통계적 유의성이 없었고, 이란산 시판제품 투여 시에는 대조군에 비해 유의하게 개선시키는 것으로 나타났다. 우울증 또한 시판 제품 투여군에서는 대조군에 비해 유의하게 개선 효과를 나타내었으나 고흥산 석류 농축액 투여군에서는 우울증 개선 효과가 확인되지 않았다. 이상과 같은 결과를 통해 고흥산 석류 농축액은 난소를 적출한 rat의 골 소실을 현저히 개선하고 복부/내장지방 및 혈중지질을 농도 의존적으로 개선하여 골다공증, 비만 및 심혈관 질환과 같은 갱년기 증상에 효과가 있음을 확인하였다. 그러나 고흥산 석류 농축액 투여군에서 대표적인 갱년기 증상인 우울증과 불안장애 개선에 유의성 있는 효과가 나타나지 않는 것은 동물실험 모델에서 사람에게 나타나는 다양한 갱년기 증상을 모두 평가하기에 한계가 있는 바, 향후 추가적인 연구를 통해 고흥산 석류 농축액이 갱년기 여성에게 나타나는 여러 가지 증상 중 특정 증상에 대해 선택적으로 효과를 나타내는지에 대해 임상시험을 통해 검증할 필요가 있겠다. In this study, the effect of Goheung pomegranate extract on postmenopausal syndrome was evaluated both in vitro and in vivo in ovariectomized Sprague-Dawly (SD) rats. Sixty female SD rats were divided into six groups: sham, sham operation and distilled water; OVX, ovariectomized and distilled water; PE1, ovariectomized and pomegranate concentrate (0.75 mL/twice/d); PE2, ovariectomized and pomegranate concentrate (1.5 mL/twice/d); PE3, ovariectomized and pomegranate concentrate (2.2 mL/twice/d); and CE, ovariectomized and commercial pomegranate concentrate (2.2 mL/twice/d). Percent bone volume (bone volume/tissue volume) and trabecular thickness (Tb.Th) improved in a dose-independent manner in PE1, 2, and 3. Especially, bone mineral density was significantly improved in PE3 (P<0.05) compared to OVX. Pomegranate extract reduced body weight and visceral fat mass. High density lipoprotein cholesterol (HDL-C) level slightly increased in a dose-independent manner in the experimental group. In addition, HDL-C/total cholesterol level of PE3 significantly increased (P<0.05) compared with OVX. These results show that pomegranate concentrate improved blood lipid levels and bone metabolism in ovariectomized rats. Therefore, Goheung pomegranate concentrates are expected to improve cardiovascular and bone-related diseases in menopausal women.

        구제역바이러스 신속진단을 위한 pan-serotype reverse transcription loop-mediated isothermal amplification (RT-LAMP) 진단법

        In this study, we developed a sensitive and specific reverse transcription loop-mediated isothermal amplification (RT-LAMP) assay for rapid visual detection of foot-and-mouth disease virus (FMDV) circulated in Korea. The RT-LAMP was completed in 40 min at 62°C and the results of the assay were directly detected by naked eye without any detection process. The assay specifically amplified all 7 serotypes of FMDV RNAs but not amplified other viral and cellular nucleic acids. The sensitivity of the RT-LAMP was 10<sup>2</sup>, 10<sup>3</sup> and 10<sup>3</sup> TCID<sub>50</sub>/mL for serotype O, A and Asia 1 FMDV, respectively, which was comparable to conventional reverse transcription polymerase chain reaction (RT-PCR) and relatively lower than that of real time quantitative RT-PCR (qRT-PCR). Clinical evaluation of the RT-LAMP using different serotypes of Korean and foreign FMDV strains showed a 100% (35/35) agreement with the results of the RT-PCR and qRT-PCR. These results indicated that RT-LAMP assay developed in this study could be a valuable diagnostic method for FMDV monitoring and surveillance.

        320x240 적외선 배열검출기를 이용한 고분해능 열상 신호처리기 구현

        This paper describes the design principles and methods of electronic processor for thermal imager with 320$\times$240 staring array infrared detector. For the detector's nonuniformity correction and excellent image quality, we have designed the multi-point correction method using the defocusing technique of the optics. And to enhance the image of low contrast and improve the detection capability, the new technique of histogram processing has been designed. Through these image processing techniques, we have developed the high quality thermal imager and acquired a satisfactory thermal image. The result of MRTD(Minimum Resolvable Temperature Difference) is $0.1^{\circ}C$ at 4cycles/mard.

        This paper has presented to the design results of the analog and digital signal processor for the 2nd generation thermal imaging system using $480\times6$ infrared focal plane array In order to correct non-uniformities of detector arrays, we have developed the 2-point correction method using the thermo electric cooler. Additionally, to enhance the image of low contrast and improve the detection capability, we developed the new technique of histogram processing being suitable for the characteristics of contrast distribution of thermal imagery. Through these image processing techniques, we obtained a high qualify thermal image and acquired good result.

        소아에서 후방인공수정체 삽입술 후 발생한 후낭혼탁에 대한 야그레이저 후낭절개술의 효과

        Purpose: To evaluate the effect of Nd:YAG laser capsulotomy for posterior capsular opacification (PCO) after posterior chamber intraocular lens (PC-IOL) implantation in children. Methods: Thirty eyes of 23 children underwent Nd:YAG laser capsulotomy for PCO after PC-IOL implantation. The frequency of laser capsulotomy, elapsed time between cataract surgery and capsulotomy, laser parameters, pre and postoperative visual acuity, complications and recurrence were reviewed. Results: From 108 eyes with intact posterior capsule after lensectomy, 30 (27.8%) were treated with Nd:YAG laser capsulotomy. The mean age at Nd:YAG laser capsulotomy was 7 years, the mean follow-up was 27 months, and the time interval between cataract operation and Nd:YAG laser capsulotomy was 13 months. Visual acuities of 20/40 or better were attained in 73% of eyes and visual acuities of 20/60 or less in 10% of eyes. PCO recurred in 12 eyes (40%), 10 of which were treated by performing a second laser capsulotomy, but the other two required a third laser capsulotomy. There was no relationship between the recurrence and the delay to initial laser capsulotomy, the amount of energy used for laser capsulotomy, or the patient age. One eye (3%) had corneal erosion, a bleeding from the pupillary margin and increased IOP; however, all responded well to medical treatment. Conclusions: Nd:YAG laser capsulotomy for PCO after PCL implantation in children offers a noninvasive and safe capability to create a clear visual axis. Although considered effective, there is a high likelihood that Nd:YAG laser capsulotomy will require revision.
