        두개저 골절에 대한 임상적 고찰

        65 consecutive patients with basal skull fracture admitted to Eul-Ji General Hospital, between March 1990 and March 1993 was carried out to study it's clinical and radiological presentations, complications, and outcomes. 1) Two thirds of the patients were between 20 and 40 years old and 82% were male. 2) 78% of the injuried were by traffic accidents and 17% were by fall down. 3) The outcomes of treatment were related to initial GCS(Glasgow coma scale) of the patients. 4) Clinical features were ear bleeding -54%, Racoon's eye -35%, hemotympaneum -32%, hearing loss -25%. 5) The combined lesions were skull fractures -43%, facial bone fractures -31%, subdural hematoma -19%, epidural hematoma -14%, and pneumocephalus -14%. 6) The combined cranial nerves injuried were facial nerve -32%, vestibulocochlear nerve -12%, oculomotor nerve -12%, abducens nerve -12%. 7) The location of basal skull fracture according to clinical feature and CT bone window were middle cranial fossa -54%, on anterior cranial fossa -22%, anterior and posterior cranial fossa -12%, posterior cranial fossa 12%. 8) Clinical features associated with types of petrous bone fractures were analyzed. 9) The time of onset and the degree of facial palsy had a influence on recovery of facial nerve function and the types of hearing loss had a role in recovery of hearing function. 10) CSF leakage were present in 16 cases and immediate type was more than delayed type. 11) Three patients(4.6%) developed meningitis, especially associated with CSF leakage and it's rate was higher when CSF leakage persisted over 1 week.

        This paper proposed a optimal scale factors technique of a neuro-fuzzy controller for a load frequency control of two areas power system. The optimal scale factors control technique is optimize from an initial fuzzy logic control rule, and then is learned with an error back propagation learning algorithm of the neuro-fuzzy controller. In application two areas the load frequency control of the power system, it hopes to have response characteristic better than the conventional control technique.

      • 말뚝재하시험에 의한 허용지력 비교 연구

        The estimation of pile bearing capacity is important since the design details are determined from the result. There are numerous ways of determining the pile design load, but only few of them are chosen in the actual design. According to the recent investigation in Korea, the formulas proposed by Meyerhof based on the SPT N values are most frequently chosen in the design stage. In the study, various static and dynamic formulas have been used in predicting the allowable bearing capacity of a pile. Further, the reliability of these formulas has been verified by comparing the perdicted values with the static and dynamic load test measurements. Also, in most cases, these methods of pile bearing capacity determination do not take the time effect consideration, the actual allowable load as deter mimed from pile load test indicates severe deviation from the design value. The principle results of this study are summarized as follows : As a result of estimate the reliability in criterion of the Davisson method, it was showed that Terzaghi & Peck > Chin > Meyerhof > Modified Meyerhof method was the most reliable method for the prediction of bearing capacity. Comparisons of the various pile-driving formulas showed that Modified Engineering News was the most reliable method. However, a significant error happened between dynamic bearing capacity equation was judged that uncertainty of hammer efficiency, characteristics of variable, time effect etc... was not considered. As a result of considering time effect increased skin friction capacity higher than end bearing capacity. It was found out that it would be possible to increase the skin friction capacity 2.0 times higher than a driving. As a result of considering 7 day’s time effect, it was obtained that Engineering News, Modified Engineering News, Hiley, Danish, Gates, CAPWAP analysis for relation, respectively, Q_(u(Restrike)) / Q_u(EOID)) = 0.971t^(0.1), 0.968t^(0.1), 1.192t^(0.1), 0.880t^(0.1), 0.889t^(0.1), 0.966t^(0.1).

        In this paper, we are considered various possible disturbance conditions in order to design controller, H∞ optimal controller is successfully designed to control in such as situations. To do this, we are determined weighting function and design parameter γ to method of trial and error by Glover-Doyle algorithm. To compare with H∞-PSS and conventional-PSS which is applied in nominal system and load variations, the dynamic characteristics of H∞-PSS controller was verified which has a good response.

        Diethylnitrosamine 으로 유도된 간암발생 과정에서의 γ - Glutamyl Transpeptidase 양성 병소에 대한 입체형태학적 분석 연구

        Background/Aim - Hepatocarcinogenesis of microscopically altered foci could be shown to be progressed into a trabecular pattern of hepatocellular carcinoma. And it is reported that down-regulation of TGF beta II receptor and up-regulation of TGF alpha and c-myc reveal the progression of diethylnitrosamine-induced foci into liver cell cancer. Up-regulation of TGF beta II receptor, however, causes apoptosis of foci. To determine characteristic morphology and growth kinetics of putatively precancerous y glutamyl transpeptidase (GGT) positive foci and hyperplastic nodules, a stereological quantification was attempted in the Peraino's neonatal rat model initiated by diethylnitrosamine and promoted by phenobarbital. Materials/Methods - Fifteen Sprague-Dawley rats were I.p. injected with 0.15 pmole/g of body weight of diethylnitrosamine mixed in corn oil at one day after birth. From weaning at 4 weeks of life, the rats were continuously fed 0.035% phenobarbital in drinking water and sacrified 5 rats at each time point of 8 weeks, 16 weeks, and 32 weeks. Teklad standard diet was fed after weaning. The livers obtained were fixed in freshly prepared, cold ethanol-acetic acid (99:1 vo1%). For the GGT histochemical staining, Rutenberg's method was modified, and counterstained with H & E or toluidine blue. For the stereological analysis GGT positive foci and nodules were traced in 200 consecutive tissue sections and quantified the 3 dimensional volumes by computer assisted planimetry. Either spheroidal or non-spheroidal morphology was determined by parabola 2nd degree equation ' y=ax+bx+c (sphere a=-P,). Results - Thirty nine (55.71%) out of 70 representative lesions were nonspheroidal. Especially at 8 weeks, the 28 out of 40 GGT positive foci were irregular, nonspheroidal shape. Later times, however, GGT positive foci and reprogrammed nodular lesions were become spheroidal. Lilliefors probabilities test for spheroidal frequency was statistically significant (p$lt;0.05). Conclusion ' Stereologically non-spheroidal characteristics of the early GGT positive foci limit growth kinetic estimation by 3 dimensional volume quantitation but permit in later times in spheroidal, GGT positive foci and reprogrammed nodules showing fade-out of GGT activity. In other words, GGT positive foci may be clonally selected for growing into hyperplastic nodules and hepatocellular carcinoma or regressed by apoptosis.         감비(減肥) 3호방(3號方)이 고지방식이(高脂肪食餌)로 비만이 유도된 백서(白鼠)에 미치는 영향

        Objectives: In order to investigate the effects of Gambibang-3(here in after referred to GBB3) on the obese gene and obese inhibitory, C57BL/6 mice were induced by high fat diet. Methods: C57BL/6 mice were divided into four groups(normal, high fat diet with control, high fat diet with Reductil(here in after referred to RDT), high fat diet with GBB3 extract) and fed for 8 weeks. And observed that, the weight change of the adipocytes in body and liver, total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, glucose, leptin change in the serum, the expression of β3AR and leptin gene in 3T3 cell and primary adipocyte cell. Results & conclusions: The following results were obtained in this study. 1. GBB3 and RDT group showed that the revelation of β3AR in primary adipose cell and 3T3 cell were increased considerably, and that the revelation of leptin in primary adipose cell and 3T3 cell were decreased considerably. 2. GBB3 and RDT group showed that the weight of adipocyte and liver were not different with control group significantly. 3. GBB3 and RDT group showed that the amount of HDL-Cholesterol were increased considerably, the amount of glucose, LDL-Cholesterol and Triglyceride were decreased considerably. 4. GBB3 group showed that the amount of leptin in the serum were decreased considerably.

      • 차세대지능망에서 주문형 서비스를 위한 다중스레드형 서비스제어기능의 구현 및 성능분석

        차세대지능망은 서비스 가입자가 기존에 제공하고 있는 서비스 범위내에서 서비스 특징을 선택하거나 데이타를 수정할 수 있는 주문형 서비스(Customized Service)를 제공할 수 있어야 한다. 이러한 주문형 서비스를 제공하는 서비스 제어기능의 구현을 위해 기존의 지능망서비스제어 시스템처럼 주문형 서비스마다 하나의 프로세스를 할당하는 형태로 서비스 로직을 구성하게 되면 주문형 서비스 수만큼 프로세스 수도 증가되어 문맥 교환, 프로세스간 통신등과 같은 운영체제 오버헤드로 인하여 차세대지능망시스템의 실시간 특성상 성능에 문제점이 많다. 본 논문에서는 이러한 문제점을 해결하기 위해 운영체제 오버헤드를 줄일 수 있는 경량프로세스(Light-Weight Process : LWP) 개념의 다중스레드형 주문형 서비스 처리 모델을 도입하여 SunOS Release 4.0에서 운용되는 경량프로세스 라이브러리를 이용하여 구현하였으며, 서비스제어 기능을 다중스레드화(Multi-threaded) 시켰을 때 프로세스형태로 구성된 모델에 대한 상대적인 성능향상 효과 예측을 위하여 각 성능 모델에 대하여 객체지향 시뮬레이션 언어인 DEVSIM++을 이용하여 시뮬레이션을 수행하였다. 또한 기존의 다중스레드 모델인 주/종속 스레드 모델의 경우 트래픽 증가에 따른 병목현상이 발생되어 이를 완화시킬 수 있는 새로운 다중스레드 모델을 제시하고 기존의 주/종속 스레드모델보다 성능이 우수함을 시뮬레이션을 통하여 보였다. The Advanced Intelligent Network (AIN) must have the capability to provide customized services, which is to provide service subscribers with more flexible services by selecting different service features of their choices. If the customized services are implemented in the service control function with each service logic configured in single-threaded structure with one process per one type of service, the performance may be a major problem in the real-time AIN system. The reason for performance degradation is that as the number of various services increases. so does the number of processes. This causes to increase operating system overhead, such as context switching and inter-process communication(IPC). In this paper. we implement a multi-threaded S/W model for customized service which adopts the concept of the light-weight process to reduce the operating system overhead using the user-level multi-threaded library in SunOS Release 40. We analyze the effect of the performance improvement of multi-threaded service logic models over single-threaded service logic models by a computer simulation based on DEVSIM++ object-oriented simulation language. Also multi-threaded model has a problem of performance degradation due to bottleneck with the traffic increase. In order to solve the problem, we present a multi-threaded S/W model and analyze the effect of the performance improvement of proposed model.

        Therapeutic effect of dichloroacetate against atherosclerosis via hepatic FGF21 induction mediated by acute AMPK activation

        Dyslipidemia-induced atherosclerosis, which has a risk of high morbidity and mortality, can be alleviated by metabolic activation associated with mitochondrial function. The effect of dichloroacetate (DCA), a general pyruvate dehydrogenase kinase (PDK) inhibitor, on in vivo energy expenditure in ApoE−/− mice fed a western diet (WD) has not yet been investigated. WD-fed ApoE−/− mice developed atherosclerotic plaques and hyperlipidemia along with obesity, which were significantly ameliorated by DCA administration. Increased oxygen consumption was associated with heat production in the DCA-treated group, with no change in food intake or physical activity compared with those of the control. These processes were correlated with the increased gene expression of Dio2 and Ucp-1, which represents brown adipose tissue (BAT) activation, in both WD-induced atherosclerosis and high-fat-induced obesity models. In addition, we found that DCA stimulated hepatic fibroblast growth factor 21 (Fgf21) mRNA expression, which might be important for lowering lipid levels and insulin sensitization via BAT activation, in a dose- and time-dependent manner associated with serum FGF21 levels. Interestingly, Fgf21 mRNA expression was mediated in an AMP-activated protein kinase (AMPK)-dependent manner within several minutes after DCA treatment independent of peroxisome proliferator-activated receptor alpha (PPARα). Taken together, the results suggest that enhanced glucose oxidation by DCA protects against atherosclerosis by inducing hepatic FGF21 expression and BAT activation, resulting in augmented energy expenditure for heat generation.

      • 데이타 불균형 분포를 고려한 병렬 데이타베이스 시스템 구조간의 병렬 조인 비용에 대한 비교 분석

        데이타베이스 연산 중에서 가장 많은 처리 시간을 요구하는 조인 연산에 대한 연구는 병렬 데이타베이스에서도 지속적으로 이루어지고 있으며, 이러한 병렬 조인 연산은 병렬 데이타베이스 시스템의 구조에 따라 그 성능에 영향을 받을 수 있다. 기존의 병렬 데이타베이스 시스템 구조에 관한 연구에서는 부하 균등에 단점을 가진 비공유 구조가 디스크 공유 구조에 비해 더 좋은 성능을 보이는 것으로 평가되어 왔다. 그러나 트랜잭션내의 병렬성, 동적 부하균등 그리고 DSS(Decision-Support System)형 트랜잭션 처리의 관점에서 비교한다면 디스크 공유 구조가 비공유 구조에 비해 성능 향상을 가져올 수 있다. 본 논문에서는 비공유 구조의 부하 균등을 위한 해쉬 조인 알고리즘을 디스크 공유 구조에 적용하도록 수정하였으며, 각각의 성능을 비교 · 분석하기 위한 성능분석 모델의 제시 및 분석을 수행하였다.
