      • 溫經湯에 關한 文獻的 考察

        Through the study of successive literatures form the Han-dynasty(漢代) when 《Keumjweyeuliak(금궤요략)》 was written to modern age, we inspected many types of Onkyungtang(溫經湯) and prescriptions including the term of ' Onkyung(溫經)' or Chokyung(調經)', and look into their composition and symptoms respond to administration of this medicine. The results of this study were as follows; 1. The medicine which makes up Onkyungtang was 38 kinds from the Han-dynasty to the Chung-dynasty(淸代), and up to the present it summed into 43 kinds. 2. Onkyungtang which was composed of different kinds of medicine was 10 types from the Han-dynasty to the Chung-dynasty and added 11 types in modern days. Therefore it sumed up into 21 types. 3. No Onkyungtang is seen more common than that of the same composition as in 《Keumjweyeuliak》 in successive literature. The next is the same constituents as in 《Buinyangbangdaejeon(婦人良方大全)》. Others were rare. 4. Onkyungtang in《Keumjweyeuliak》was used to treat cold syndrome of the penetration and conception vessles or uterus of deficiency type and that in 《Buinyangbangdaejeun》was for cold syndrome of the penetration and conception vessels or uterus of excess type. 5. Other names of Onkyungtang in《Keumjweyeuliak》were Daeonkyungtang(大溫經湯), Chokyungsan(調經散), Chokyungtang(調經湯), Chunkeumchokyungtang(千金調經湯), Chunkeumchokyungsan(千金調經散) and Soonkyungtang(小溫經湯). And the prescription in the same constitution was also called Chungkyungonkyungtang(仲景溫經湯), Keumkweonkyungtang(금궤온경탕) and Sibbionkunghwan(十二溫經丸). 6. Onkyungtang in 《Buinyangbangdaejeon》was called Soonkyungtang(小溫經湯) and the very prescription was also called in the name of Yangbangonkyungtang(良方溫經湯), Jinmionkyungtang(指迷溫經湯), Kungkwiinkyungtang and Jinssionkyungtang(陳氏溫經湯).

        사망진단서(사체검안서) 상의 선행사인으로부터 사망통계의 원사인이 선정되는 비율 : 3개 대학병원에서 교부된 사망진단서를 중심으로

        Background : To exatnine the problems intolved in writing practice of death certificates, we cotnpated the determination of underlying cause of death for wital statistics using recorded underlying cause of death in issued death statistics. Methods : We collected 688 rnortality certificates issue in year of 2,000 from 3 university hospitals. And we also collected vital statistics from ministry of statistics. The causes of death were coded by experienced medical record wpecialists. And causes of death determined at ministry of statistics for national vita statistics were mapped to causes of death recorded at each death certificates. The rate that underlying causes of death for vital statistics were derived from underlying causes of death recorded at issued death certificaties sere analysed. Results : 64.5% of underlying cause of death for could be derived from underlying cause of death recorded at issued death certificates, 8.6% derived from intermediate cause of death, and 3.9% derived from direct cause of death. In 23% of cases, underlying cause of death could not be derived using issued death certificates. The rate that underlying cause of death for vital statistics could be derived from underlying cause of death recorded at death certificates was different between 3 university hospitals. Ane the rate was also different between death certificates and postmortem certificates. We classified the causes of death using 21 major categories. The rate was different between diseases or conditions tha caused death too. Conclusion : When we examined the correctness of death certificate writing practice using abpve methods, cortectness of writing could not be told as satisfactory. There was difference in correctness of writing between hospotals, between death certificates and postmortem certificates, and between diseases and conditions that caused death. With this results, we suggested some strategy to improve the correctness of death certificate writing practice.

      • 제 2형 당뇨병 환자에서 혈중 Metalliproteinase-9의 농도

        Purpose : Diabetic nephropathy is a serious complication of diabetes and accounts for much of its morbidity and mortality. It is well known that the overt clinical phase of diabetic nephropathy is preceded for many years by the presence of microalbuminuria. Microalbuminuria is thought to be a marker of widespread vascular damage and may underlie the propensity toward severe extrarenal vascular disease in microalbuminuric patients. Metalloproteinase-9 has a broad substrate specificity for native collagens as well as proteoglycan and elastin. One study reported that an increased plasma metalloproteinase-9 concentration precedes the development of microalbuminuria in type 2 diabetic patients. Another study reported that a high glucose concentration cause a abnormality on the activity of metalloproteinase by the mesangeal cell. The purpose of the present study was to examine whether palsma metalloproteinase-9 concentrations are altered at various complications in type 2 diabetic patients. Methods : Sixty patients with type 2 diabetes and thirty healthy control were recruited for the present study. No patient had any malignancy or history of heart disease, liver disease, collagen disease. We recorded age, duration of diabetes, and blood pressure. After an overnight fast, blood was drawn from an antecubita1 vein of measurement of glucose, metalloproteinase-9, HbA1C, and serum creatinine. To measure plasma concentration of metalloproteinase-9, a one step sandwich enzyme immunoassay using two monoclonal antibodies was used. Patients were divided into two group according to the presence or abscence of microalbuminuria. The groups were then compared with regard to clinical and laboratory characteristics and serum concentration of metalloproteinase-9. Results : The mean serum levels of metalloproteinase-9 in diabetic patients (62.32±25.83ng/㎖) were significantly higher than in healthy controls(12.92±4.97ng/㎖) (p<01). The mean serum levels of metalloproteinase-9 in diabetic patients with microalbuminuria(81.16±41.43ng/㎖) were significantly higher than in those with normoalbuminuria(31.48±7.34ng/㎖) (p<01) and much more than in healthy control(12.92±4.97ng/㎖) (p<05). Furthermore, the severity of nephropathy were closely associated with the serum metalloproteinase-9 level and the serum metalloproteinase-9 level was significantly correlated with microalbuminuria. The mean serum level of metalloproteinase-9 in diabetic patients with retinopathy(61.74±39.07ng/㎖) was significantly higher than in those with no retinopathy(34.98±9.02ng/㎖) (p<05). But, in diabetic patients with normoalbuminuria, there is no difference between the groups with retinopathy versus no retinopathy. Conclusion : In summary, a sustained increase in serum metalloproteinase-9 in patients with type 2 diabetes is associated with a high risk for the development of microalbuminuria. The results of this study suggest that the serum rnetalloproteinase-9 was a useful and noninvasive marker for the development of diabetic nephropathy. It could be also used to predict the development of diabetic complications and to assess therapeutics or the prognosis of diabetes. The usefulness of this model will be clarified by a larger scale and serial study.

      • P4G 서울 정상회의와 우리나라의 기후외교

        우리나라가 개최한 최초의 기후ㆍ환경 다자 정상회의인 P4G 서울 정상회의가 지난 5월 31일 서울선언문 채택과 함께 성공적으로 막을 내렸다. 총 67명의 국가 정상 및 국제기구 수장과 더불어 200여 명의 정부, 국제기구, 기업, 학계, 시민사회, 미래세대 대표가 참석한 이번 정상회의는 국제사회 최대 현안인 기후변화와 환경문제를 다루는 녹색올림픽이었다고 할 수 있다. 국제사회는 파리협정 이행 원년인 2021년 우리나라가 P4G 서울 정상회의 개최를 통해 기후변화 및 환경 문제 대응을 위한 국제사회의 연대와 협력 강화에 기여한 글로벌 기후리더십을 높이 평가하였다. P4G 정상회의 참가국은 우리의 그린뉴딜 정책, 2050 탄소중립 선언을 비롯, 최근 발표한 해외석탄 발전 공적 금융지원 중단, 2030 국가 온실가스 감축목표(NDC) 추가 상향 등 기후행동 강화 정책을 환영하고 평가하였다. 미국과 중국을 비롯한 광범위한 참여 국가와 국제기구가 지지한 서울선언문의 채택은 올해 11월 영국 글래스고에서 개최될 제26차 기후변화당사국총회(COP26)를 앞두고 국제사회의 기후변화 대응 논의가 더욱 활발하게 이뤄지도록 기여할 것이다. 최빈국에서 경제선진국으로 성장한 우리나라는 국제사회의 기후변화 대응 강화를 위해 개발도상국과 기후선도국간 가교 역할을 할 수 있는 자격과 능력을 갖추고 있다. 앞으로 우리나라의 포용적 기후리더십 확립을 위해서는 11월 COP26에서 야심찬 수준의 NDC 발표가 필요하다. 향후 G7, G20, COP26 등 다양한 국제무대의 기후변화 논의 과정에서 우리나라가 기후선도국과 개발 도상국의 가교 역할을 할 수 있도록 적극적으로 참여할 것을 기대한다. The P4G Seoul Summit, the first multilateral summit on climate and the environment hosted by the Republic of Korea, successfully concluded on May 31 with the adoption of the Seoul Declaration. The summit brought together 67 heads of state and government and more than 200 participants from governments, international organizations, businesses, academia, civil society and the young generation. It may well be dubbed the “Green Olympics” as it provided such wide-ranging participants with an opportunity to exchange their views on the urgent global issues of climate change and the environment. The international community appreciated that the ROK had shown its global climate leadership by hosting the P4G Seoul Summit that contributed to promoting global solidarity and cooperation for responding to climate change and environmental issues. The participating countries welcomed the ROK’s initiatives for strengthened climate action such as the Korean Green New Deal policy, 2050 Carbon Neutrality goal, commitment to stop public financing for new overseas coal-fired power plants and plan to announce a further ambitious NDC target. A wide range of countries and international organizations including the U.S. and China endorsed the Seoul Declaration, which will contribute to more active and enhanced discussions on global response to climate change ahead of COP26 to be held in November this year in Glasgow, UK. Once one of the poorest countries in the world, the ROK has transformed itself into an advanced economy, gaining the status and ability to play a bridging role between developing and developed countries in renewing the global efforts for climate change response. The ROK needs to announce its updated NDC with even greater ambition at the upcoming COP26 so as to consolidate its inclusive climate leadership in the world. The ROK will continue to actively engage in international fora such as the G7, the G20, and COP26 and take part in climate discussions to play such a bridging role.

        임상연구 : 난치성 만성 통증에 대한 척수자극술의 효과 분석: 5년간이 경험

        Background: We investigated the effect of spinal cord stimulation on intractable chronic pain retrospectively and report our 5-year experience. Methods: 49 patients with the medical history of trial spinal cord stimulation have been retrospectively analyzed. They consist of 34 men and 15 women, ranging in age from 22 to 89 years. Causes of intractable chronic pain included postherpetic neuralgia, failed back surgery syndrome, complex regional pain syndrome, chronic back and leg pain, cauda equina syndrome, perineal pain, diabetic polyneuropathy, and spinal cord injury. Results: The pain due to failed back surgery syndrome, complex regional pain syndrome, chronic low back and leg pain, and spinal cord injury was well controlled by using spinal cord stimulation system. After a trial period of stimulation, 20 patients had permanent stimulators implanted. Most of them reported satisfactory pain relief for maximum 60 months and minimum 9 months. Noticeable complications included electrode displacement and hemorrhage. Conclusions: We may suggest that spinal cord stimulation is an effective and safe therapy for chronic intractable pain.         진행성 위암 환자에서 발생한 Cisplatin에 의한 말초신경병증

        Background/Aims: Since cisplatin was widely used for the treatment of advanced gastric cancer, prevention and management of its side effects have become an important issue. Among the various side effects, neurotoxicity still remains as the major dose-limiting side effect of cisplatin. To investigate the frequency and the prognostic factors of neurotoxicity associated with cisplatin- containing chemotherapy, we performed this retrospective analysis. Methods: We analyzed clinical data of 317 patients with advanced gastric cancer treated with cisplatin-containing (20 mg/m2/day, D1-5 per cycle) chemotherapy in Korea Cancer Center Hospital from 1992 to 1996. Results: Peripheral neuropathy was developed in 44 patients(13.9%; 95% C.I, 10-17.8%): 28 patients in who grade I and 16 patients in who grade II. The incidence of peripheral neuropathy was increased as the cumulative dose of cisplatin increased. Age(p=0.0458) and cumulative dose of cisplatin(p 0.0001) were significant prognostic factors for the development of peripheral neuropathy in multivariate analysis. Conclusions: Peripheral neuropathy was a side effect of cisplatin-containing chemotherapy observed frequently in patients with advanced gastric cancer. Its development was closely related with cumulative cisplatin dose and patient's age. Once peripheral neuropathy developed clinically, it did not improve in most patients even after the administration cisplatin was discontinued.

        EphA8-ephrinA5 Signaling and Clathrin-Mediated Endocytosis Is Regulated by Tiam-1, a Rac-Specific Guanine Nucleotide Exchange Factor

        Recent studies indicate that endocytosis of Eph-ephrin complexes may be one of the mechanisms by which a high affinity cell-cell adhesion is converted to a repulsive interaction. In this study, we show that EphA8 undergoes clathrin-mediated endocytosis upon treatment with ephrin-A5, and that EphA8 is associated tightly with Tiam-1, a Rac-specific guanine nucleotide exchange factor. Analysis of EphA8 deletion mutants revealed that a juxtamembrane region in EphA8 is critically involved in endocytosis of EphA8-ephrinA5 complexes. An EphA8 mutant lacking this juxtamembrane portion was defective for endocytosis with ephrinA5, and also displayed a weak association with Tiam-1. Expression of an endocytosis-defective version of EphA8 resulted in a low level of Rac activity in response to ephrin-A5 stimulation. More importantly, down-regulation of Tiam-1 resulted in inefficient endocytosis of EphA8-ephrinA5 complexes. These results suggest that Tiam-1 plays a role in clathrin-dependent endocytosis of EphA8-ephrinA5 complexes.
