The purpose of this study is to demonstrate the potential enhancement of the flavonoid contents from Agastache rugosa, which can be obtained as raw materials for functional products in the food medicine industry by identifying important factors for efficient preparation to save costs and time in terms of economic factors. For this reason, response surface methodology using Box-Behnken design was used to optimize the extraction conditions for the maximum yield of seven major compounds from A. rugosa. The optimum conditions were obtained with an ethanol concentration of 60.0%, a temperature of 50 oC, and an extraction time of 33.6 min, meaning that the regression analysis fits the experimental data well. Under these conditions, the seven major compounds 1-7 had observed values of 2.169, 2.135, 0.697, 2.485, 0.105, 1.247, and 0.551%, respectively. These results show that the observed values are in good agreement with the predicted values in the regression model. This process for optimization study exhibited a basic protocol for obtaining stable ingredients from A. rugosa that are appropriate for the development of effective functional products.

<P><B>Abstract</B></P> <P>Liver fibrosis is a chronic liver disease caused by viral infection and/or metabolic, genetic and cholestatic disorders. The inhibition of hepatic stellate cell (HSC) activation and the selective apoptosis of activated HSCs can be a good strategy to treat liver fibrosis. The activated HSCs are known to be more susceptible to tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) induced apoptosis than normal HSCs because death receptor 5 is overexpressed on the cell surface. In this work, a target-specific and long-acting hyaluronic acid (HA)–TRAIL conjugate was successfully developed for the treatment of liver fibrosis. The HA–TRAIL conjugate was synthesized by a coupling reaction between aldehyde-modified HA and the N-terminal amine group of TRAIL. The biological activity of the HA–TRAIL conjugate was confirmed by an in vitro anti-proliferation assay and caspase-3 expression in human colon cancer HCT116 cells. In vivo real-time bioimaging exhibited the target-specific delivery of near-infrared fluorescence dye-labeled HA–TRAIL conjugate to the liver in mice. According to pharmacokinetic analysis, the HA–TRAIL conjugate was detected for more than 4days after single intravenous injection into Sprague–Dawley (SD) rats. Finally, we could confirm the antifibrotic effect of HA–TRAIL conjugate in an <I>N</I>-nitrosodimethylamine-induced liver fibrosis model SD rats.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

The purpose of this study was analyze the relationship between perfectionism, self - management and self - confidence of Taekwondo athletes and to examine the role of self - management in self - management and self - confidence. For this purpose, 243 Taekwondo players who were registered as athletes in the Korea sports council gathered data through perfectionism test, self - management questionnaire and confidence questionnaire. In order to analyze the collected data, the following main results were obtained through exploratory factor analysis, reliability analysis and correlation analysis using SPSS 18.0 statistical program. First, the correlation analysis between the variables all of the variables except socially oriented perfectionism and interpersonal relationship showed a positive correlation. second, perfectionism In the relationship between self-direction and confidence Between self-management factors partial mediation effect. third, perfectionism social orientation in the relationship between self-confidence training management showed complete mediating effect. fourth, perfectionism social orientation in the relationship between self-confidence Interpersonal management there was no mediation effect. fifth, perfectionism social orientation in the relationship between self-confidence body management showed complete mediating effect. Seventh, perfectionism other oriented in the relationship between self-confidence training management showed complete mediating effect. Eighth, perfectionism other oriented in the relationship between self-confidence interpersonal management partial mediation effect. Ninth, perfectionism other oriented in the relationship between self-confidence mental management partial mediation effect. The tenth, perfectionism other oriented in the relationship between self-confidence body management partial mediation effect. These results suggest that the higher self - directed perfectionism positively affects self - management and self - confidence. Through this study, the positive effect of self - orientation will help to improve self - confidence and performance by self - management.

Background and Objectives: Neck metastasis is one of the most important prognostic factors in treating tonsillar cancer. Incidence and pattern of lymph node metastasis of tonsillar squamous cell carcinoma are the basic knowledge of treatment decision. Occult metastasis rate of tonsillar cancer and pattern of metastasis, failure pattern, survival were retrospectively analyzed. Patients and Methods: Seventy six patients who underwent surgery for tonsillar squamous cell carcinoma as an initial treatment from 1992 to 2004 were evaluated. Charts, imaging studies and pathologic reports were reviewed. Results: At the time of surgery, 78% of patients with tonsillar cancer had neck metastasis and 66% had multiple node metastasis. Occult neck metastasis was in 26%. There was high incidence of neck metastasis even in early stage of primary lesion. Conclusion: High incidence of lymph node metastasis was confirmed histopathologically in tonsillar cancer. All tonsillar cancer patients may need elective treatment of the neck. Tonsillar cancer had relatively good prognosis even though its neck metastasis rate is very high.
The disease burden of bronchiectasis in Korea: a national database study

Background: The prevalence and disease burden of bronchiectasis in comparison to those of chronic obstructive pulmonary disease (COPD) have not been well elucidated using a nationally representative database. Methods: We compared respiratory symptoms, physical activity, quality of life, and socioeconomic status in subjects with bronchiectasis versus those with COPD or control subjects participating in the Korea National Health and Nutrition Examination Survey 2007-2009. Participants were classified as physician- diagnosed bronchiectasis, COPD, and control (those without COPD or bronchiectasis). Results: The prevalence of bronchiectasis in subjects aged 40 years or older was 0.8%. Compared to COPD subjects, bronchiectasis subjects were more likely to be younger (median 59.0 years, p=0.001), female (47.6%, p=0.049), and never-smoker (50.1%, p=0.049) and have had history of pulmonary tuberculosis (40.5%, p < 0.001) and osteoporosis (19.1%, p < 0.001). However, as in COPD subjects, bronchiectasis subjects had low family income (p < 0.001) and the proportion of subjects working as manager/professional/ office workers (6.4%, p < 0.001) was smaller than that of control subjects. After adjusting for covariables, compared to control, bronchiectasis subjects but not COPD subjects were more likely to have respiratory symptoms (adjusted odds ratio [OR] = 7.96, 95% confidence interval [CI]: 2.10-30.12), limitation in physical activity (adjusted OR=9.43, 95% CI=1.06-83.79), and low family income (adjusted OR=3.61, 95% CI: 1.75-7.47). Conclusion: The prevalence of bronchiectasis in Korea was 0.8%. Respiratory symptoms, limitation in physical activity, and low family income were significantly associated with bronchiectasis.
Two cases of Antley-Bixler syndrome caused by mutations in different genes, FGFR2 and POR

Antley-Bixler syndrome (ABS) is a rare form of syndromic craniosynostosis with additional systemic synostosis, including radiohumeral or radioulnar synostosis. Another characteristic feature of ABS is mid-facial hypoplasia that leads to airway narrowing after birth. ABS is associated with mutations in the FGFR2 and POR genes. Patients with POR mutations present with either skeletal manifestations or congenital adrenal hyperplasia with ambiguous genitalia. We report here two cases of ABS caused by mutations in FGFR2 and POR. Although the patients had craniosynostosis and radiohumeral synostosis in common and cranioplasty was performed in both cases, the male with POR mutations showed an elevated level of 17α-hydroxyprogesterone during newborn screening and was diagnosed with congenital adrenal hyperplasia by adrenocorticotropic hormone stimulation. This patient has been treated with hydrocortisone and fludrocortisone. He had no ambiguous genitalia but had bilateral cryptorchidism. On the other hand, the female with the FGFR2 mutation showed severe clinical manifestations: upper airway narrowing leading to tracheostomy, kyphosis of the cervical spine, and coccyx deformity. ABS shows locus heterogeneity, and mutations in two different genes can cause similar craniofacial and skeletal phenotypes. Because the long-term outcomes and inheritance patterns of the disease differ markedly, depending on the causative mutation, early molecular genetic testing is helpful.
