        급성 골수성 백혈병(M6)에 합병된 중추성 요붕증 1 예

        Leukemia is known to be an extremely rare cause of diabetes insipidus. We report a case of diabetes insipidus as a complication of leukemia. An 18 year-old man was admitted because of polyuria polydipsia and fever. Bone marrow aspiration and biopsy revealed the diagnosis of acute erythroleukemia (AML, M6). On the water deprivation test, the diabetes insipidus was confirmed to be of the central type. He was managed with intranasal vasopressin for diabetes insipidus with good response. Induction chemotherapy was administered without response, and the patient died of septicemia and respiratory failure 2 months after initial diagnosis. A short review of the literature related to this subject is also presented.

        요추천자후 두통증에 대한 혈장봉합의 적용례

        At present, spinal anesthesia is often recommanded for a safe operation and the management of pain. However the complications from the spinal anesthesia, such as hypotension, dyspnea, nausea and vomiting, pulmonary embolism, headache, auditory and visual disturbances, lumbago, urinary difficulty and neurologic sequelae have often been reported from time to time. Thus an attempt to study the complications of spinal anesthesia, particularly the differences of complications between needle size(22 gauge and 25 gauge), has been done by our department. The following results were observed; 1) The most common sequelae of spinal anesthesia was hypotension(35.6%) and, in order frequency, urinary difficulty(23.3%), headache(16.7%), lumbago(15.3%), nausea of and vomiting(12.8%), dyspnea(8.9%), auditory and visual disturbances(0.83%) and minor neurologic sequelae(0.56%). 2) The incidence of headache and lumbago was more frequent in the 22G. group, but there were no statistically significant differences(p>0.05). 3) The incidence of headache was higher in the females than the males and there were statistically significant differences(p<0.01). 4) The incidence of lumbago was higher in the fourth decade(21.7%), and females showed a higher incidence than in males and there were statistically significant differences(p<0.05). 5) The incidence of lumbago and headache and the degree of headache was without correlation to the number of punctures in both groups(22G group and 25G group). 6) The time to postoperative urination had no correlation to the level of anesthesia.

        Ketamine 정맥마취가 호흡기능에 미치는 영향

        An increasing interest in intravenous anesthetic techniques has resulted from the availability of more efficacious intravenous agents, possible discomfort of the patient on endotracheal intubation and the concern over anesthetic pollution in the operating room. This study was done to investigate the effect of intravenous anesthesia with ketamine on the respiratory system by comparing arterial blood gas analysis before and after the procedure. Analysis of arterial tlood for PCO_2, PO_2, pH, and base excess were carried out. Heart rate and blood pressure were monitored on 15 paienta in ASA class I for diagnostic or short procedures. Each patient was premedicated with atropine 0.01 mg/kg and Talium 0.2 mg/kg intramuscularly 30 minutes before the procedure. Ketamine was adnlinisteree intravenously 1.0-1.5 mg/kg or interamuscularly 3-5 mg/kg for induction of anesthesia. The anesthesia was maintained with ketamine 0.5~l.Omg/kg and Valium 0.1 mg/kg every 5 to 10 minutes. The results of this study showed that ketamine anesthesia soomed not to cause any untoward effect on respiratory function. In other words, ketamine aeems to be a safe and good intravenous anesthetic agent for diagnostic or short surgical procedures.

        말기 신부전증을 동반한 전신성 홍반성 낭창 환자에서 동종 신장이식 증례

        In systemic lupus erythematosus renal involvement is very common and it accounts for the leading cause of death of the disease. Since SLE is a systemic disease involving many organs and it is caused by altered immunological background many physicians tend to hesitate in employing hemodialysis and renal transplantation. We report the first renal transplantion in Korea which was performed in a patient with end-stage renal failure due to SLE. The patient had positive LE pre- parations, low serum complement, antibody to native DNA and renal biopsy showing diffuse proliferative lupus nephritis but she manifested decreasing clinical activity during hemodialysis. The patient received kidney allograft from living related donor after unilateral nephrectomy. There was no clinical or his- tological evidence of rejection or recurrence of lupus nephritis in the grafted kidney. The patient maintains normal renal function 18 months after renal trans- plantation.

      • Improvement of surface roughness in silicon-on-insulator wafer fabrication using a neutral beam etching

        <P>Silicon-on-insulator (SOI) wafers were etched by an energetic chlorine neutral beam obtained by the low-angle forward reflection of an ion beam, and the surface roughness of the etched wafers was compared with that of the SOI wafers etched by an energetic chlorine ion beam. When the ion beam was used to etch the silicon layer of the SOI wafers, the surface roughness was not significantly changed even though the use of higher ion bombardment energy slightly decreased the surface roughness of the SOI wafer. However, when the chlorine neutral beam was used instead of the chlorine ion beam having a similar beam energy, the surface roughness of the SOI wafer was significantly improved compared with that etched by the chlorine ion beam. By etching about 150 nm silicon from the SOI wafer having a 300 nm-thick top silicon layer with the chlorine neutral beam at the energy of 500 eV, the rms surface roughness of 1.5 Å could be obtained with the etch rate of about 750 Å min<SUP>−1</SUP>.</P>

        Three monophyletic branches are strongly supported in a phylogenetic analysis of the Teloschistaceae based on combined data sets of internal transcribed spacer and large subunit nrDNA and 12S small subunit mtDNA sequences. These are described as new monotypic genera: Jasonhuria S. Y. Kondr., L. Lokos et S. -O. Oh, Loekoesia S. Y. Kondr., S. -O. Oh et J. -S. Hur and Olegblumia S. Y. Kondr., L. Lokos et J. -S. Hur. Three new combinations for the type species of these genera are proposed.

      • Differentially Expression Genes of Normal and Cloned Bovine Placenta

        Offspring have been produced from somatic cells in a number of species. This biotechnology introduced a new phenomenon in reprogramming and differentiation of somatic cell, namely totipotency. However, birth of oversized calves and perinatal abnormalities such as increased gestation length, lack of spontaneous parturition, higher incidence of dystocia, and reduced perinatal viability of offspring are frequently observed in pregnancies of cloned bovine fetuses. Disturbance of feto-placenta has been proposed as likely causes for abnomal growth. However. Little is known the mechanism responsible for the perinatal problems. Therefore, we focused on gestation length in somatic cell nuclear recipient cows. To solve this issues, placental tissues of control and cloned bovine were obtained by a cesarean section (C-section) before 5 days of paturition. Total RNA from control and cloned bovine placenta was extractd by TRIzol reagent. GeneFishing DEG kits (Seegene) were used to identify differentially expression genes. Total RNA (3 ug) were synthesized by M-MLV reverse transcriptase (200 u/ul) with 10 uM dT-annealing control primer (ACP1) at 42C for 90 min. Then, first-strand cDNA (50 ng) was amplified using the 5 uM arbitary ACP (1-20) and 10 uM dT-ACP2 primers. Some specific expression genes were amplified, Now, we are cloning and sequencing. These finding strongly can be support to solve the problems for parturition delay in nuclear transfer cows, suggest that placenta specific proteins are key indicators for the aberration of gestation and placental function in cows.

        The Intracisternal Administration of MEK Inhibitor Attenuates Mechanical and Cold Allodynia in a Rat Model of Compression of the Trigeminal Ganglion

        The present study investigated the role of ERK in the onset of mechanical and cold allodynia in a rat model of compression of the trigeminal ganglion by examining changes in the air-puff thresholds and number of scratches following the intracisternal injection of PD98059, a MEK inhibitor. Male Sprague Dawley rats weighing between 250 and 260 g were used. Under anesthesia, the rats were mounted onto a stereotaxic frame and received 4% agar (10 μl) solution to compress the trigeminal ganglion. In the control group, the animals were given a sham operation without the application of agar. Changes in behavior were examined at 3 days before and at 3, 7, 10, 14, 17, 21, 24, 30, and 40 days after surgery. Compression of the trigeminal ganglion significantly decreased the air-puff thresholds. Mechanical allodynia was established within 3 days and persisted over postoperative day 24. To evaluate cold allodynia, nociceptive scratching behavior was monitored after acetone application on the vibrissa pad of the rats. Compression of the trigeminal ganglion was found to produce significant cold allodynia, which persisted for more than 40 days after surgery. On postoperative day 14, the intracisternal administration of 1 μg or 10 μg of PD98059 in the rat model significantly decreased the air-puff thresholds on both the ipsilateral and contralateral side. The intracisternal administration of 10 μg of PD98059 also significantly alleviated the cold allodynia, compared with the vehicle-treated group. These results suggest that central ERK plays an important role in the development of mechanical and cold allodynia in rats with compression of the trigeminal ganglion and that a targeted blockade of this pathway is a potential future treatment strategy for trigeminal neuralgia-like nociception.
