        만성 폐쇄성 폐질환 환자에서 장기간의 스테로이드 복용 중 발생한 중증 분선충 감염 1예

        Strongyloides stercoralis is a soil-transmitted intestinal nematode that may cause long-lived auto-infection in the host. It is distributed worldwide, especially in the tropical and subtropical regions, but has been rarely reported in Korea. Chronic infections by S. stercoralis are mostly inapparent infections that carry nonspecific gastrointestinal and pulmonary symptoms. However, In immunocompromised patients such as those receiving long-term steroid therapy and patients with AIDS or malignant tumors, S. stercoralis can induce hyperinfection by autoinfection. This may lead to increased rate of complications such as resistance to chemotherapy and sepsis, In such cases mortality rate of up to 87% has been reported. We report a case of severe strongyloidiasis in a patient with chronic obstructive pulmonary disease who was receiving long-term steroid therapy. The chief complaint was repeated dyspnea and hematochezia, and strongyloidiasis was diagnosed by the presence of rhabditiform larvae of S. stercoralis in the fecal smear and isolation of filari-form larvae from the stool culture. The patient developed septic shock during treatment with albendazole and showed clinical signs of hyperinfection of S. stercoralis. He eventually died despite aggressive treatment.

        In this paper, we propose a neural network with fuzzy input signal representation scheme not only for improving the classification accuracy but also for being able to classify objects whose attribute values do not have clear boundaries. The fuzzy input signal representation scheme is included as a preprocessing module. It transforms imprecise input in linguistic form and precisely stated numerical input into multidimensional numerical values. The transformed input is processed in the postprocessing module. The experimental results indicate the superiority of the backpropagation network with fuzzy input signal representation scheme in comparison to the conventional backpropagation network.

        딕카이트로부터 스멕타이트의 수열합성

        딕카이트[Al₂Si₂O_(5).(OH)₄]를 출발물질로 사용하여 이팔면체형 스멕타이트를 수열합성하였다. 시료를 활성화시키기 위해 Na₂O 성분을 첨가하고 800℃에서 4시간 열처리하였다. 합성실험을 Na-0.7 바이델라이트의 화학반응식에 의한 화학양론적 조성에 따라 SiO₂성분을 첨가하였으며 300℃, 70 ㎏f/㎠ 이하의 조건에서 온도, 압력, 시간 등을 변화시키면서 실험을 수행하였다. 합성실험을 위해 약 1리터 용량의 밀폐형 강철재 압력용기를 사용하였다. 스멕타이트를 합성할 수 있는 최적 조건은 반응온도 290℃, 반응시간 48시간, pH 10 및 60 ㎏f/㎠ 의 압력조건인 것으로 확인되었다. 온도, 압력조건 외에 원료물질의 활성화, 반응시간, 반응용액의 초기 pH 등은 결정도에 영향을 미치는 주요 요인으로 작용하였다. 합성결과물에 대한 X-선 회절분석, 에틸렌글리콜 처리, 'Greene-Kelly' 측정법 등의 실험결과, 합성된 스멕타이트는 Na-바이델라이트임이 확인되었다. A hydrothermal process was used to synthesize dioctahedral smectite from dickite [Al₂Si₂O_(5).(OH)₄]. Dickite was previously activated by heating at 800℃ for 4 hours with Na₂CO₃. After the heat-treatment, Sift was added for stoichiometrv. The autoclaving was carried out in closed stainless steel vessel (about 1 liter) at the condition of various temperature, pressure, time etc. High quality smectite could be obtaind by heating at 290℃ under the pressure of 60 ㎏f/㎠ for 48 hours. This experiment reveals that pH of the solution was an important factor and should be maintained at 10 to 11 for the formation of dioctahedral smectite. Be synthesized smectite was identified as Na-beidellite by the treatment of ethylene glycol and Greene-Kelly test.

        외래에서 항암화학요법을 받는 암환자들의 영양불량 위험도 연구

        Although it is well known that cancer patients suffer from malnutrition, there are few published studies on malnutrition in outpatients receiving chemotherapy in Korea. This study aimed to evaluate nutritional risk in oncology outpatients receiving chemotherapy and to show the baseline data to set up nutritional management programs for cancer patients. This is a retrospective observational analysis on 1,962 patients referred for nutritional education before or during chemotherapy at Seoul National University Hospital Cancer Center from January 2006 to May 2007. According to a malnutrition screening tool, the proportion of patients having malnutrition risk was 23.0%. In the case of upper gastrointestinal cancer, more than 50% of patients were assessed as being at the risk of malnutrition. They showed more than 7% weight loss compared to their usual body weight and poor oral intake; energy intake was less than 100% of Basal Energy Expenditure (BEE) and protein intake was less than or equal to 0.77 g/㎏/d. However, only 6.3% of breast cancer patients had risk of malnutrition and their oral intake was better; energy intake was 121% of BEE, and protein intake was 0.90 g/㎏/d. Outpatients receiving chemotherapy had different nutritional risk depending on their cancer site. Nutritional management program should be conducted differently, depending on the cancer site and upper gastrointestinal cancer patients at high risk of malnutrition should basically have nutritional assessment and intervention.

        느타리속 버섯류의 영양성분 및 생리활성

        일반 느타리 13품종과 색상 느타리 5품종을 사용하여 아미노산 및 polyphenol, β-glucan 함량을 분석하고, 생리활성으로 항산화 및 항암, 항고혈압, 항혈전, 항당뇨, 항염활성을 측정하였다. 느타리버섯 18종의 아미노산 분석결과 전반적으로 감칠맛을 내는 glutamic acid 함량이 비교적 많이 함유되어 있었고, 필수아미노산 성분도 고르게 분포되어 있었다. Polyphenol 함량에서는 전 품종에서 20 mg% 함량이상을 나타냈으며, 노랑느타리(R)가 39.13±0.82 mg%로 가장 높았다. β-glucan 함량은 노랑느타리(R)에서 37.67±0.22%로 가장 높았으며, 그 외에 원형1(C), 장안PK(A)에서 각각 28.75±0.61%, 27.95±0.33%의 순으로 나타났다. 전자공여능에서는 노랑느타리(R) 버섯의 DPPH IC50값이 2.93±0.44 mg/mL로 가장 낮아 항산화 활성이 가장 우수한 것으로 나타났으며, 세포독성 실험에서는 노랑느타리(R) 에탄올 추출물 1% 처리시 신장 암세포에 대해 36.90%의 세포 억제율을 보였다. ACE 저해활성의 경우 노랑느타리(R) 에탄올 추출물 1%농도에서 60.5±0.2%의 저해율이 측정되었고, 흑평(B) 56.7±1.1%, 여름(H) 52.4±1.3% 수준으로 나타났다. 항혈전 활성에서는 3%농도에서 흑평(B)과 삼복(G)을 제외한 나머지 느타리버섯 에탄올 추출물에서 50%이상의 용해 활성을 보였으며 노랑느타리(R)에서 거의 plasmin과 동등한 활성을 나타냈다. 항당뇨 활성에서는 노랑느타리(R)의 경우 50.5±0.8%의 비교적 높은 효소저해율이 측정되었고, 항염활성에서는 노랑느타리(R)에서 68.4±0.3%의 억제율이 측정되었다. 이상의 결과로 일반 느타리 13 품종과 육종 재배된 색상 느타리 5품종 중 노랑느타리(R)가 가장 우수한 생리활성을 나타내 향후 기능성 소재로의 활용가능성이 기대되었다. In this study, the anti-oxidant, anti-tumorigenic, anti-hypertensive, anti-thrombic, anti-diabetic, and antiinflammatory properties of 18 different species of genus Pleurotus were investigated. In addition, the amino acid, β-glucan, and polyphenol content were also measured. All species contained more than 20 mg% of polyphenol with the highest contents found in Pleurotus cornucopiae var. citrinopileatus (yellow pleurotus) (39.13±0.82 mg%). The β-glucan contents was also the highest in yellow Pleurotus (37.67±0.22%) followed by Won-Hyeong1 (C, 28.75±0.61%) and Jang-an PK (A, 27.95±0.33%). The yellow Pleurotus exhibited the highest antioxidant activity as assessed by the DPPH scavenging rate with an IC50 value of 2.94±0.44 mg/mL. Ethanol extracts from the yellow Pleurotus treated at 1% concentration showed cytotoxic activity up to 36.9% in the human embryonic kidney 293T cell lines. The yellow Pleurotus also showed the highest inhibitory effects on ACE activity (60.52±0.2%). Finally, the yellow Pleurotus exhibited anti-diabetic and antiinflammatory properties as shown by inhibition of α-amyloglucosidase activity (50.5±0.8%) and nitric oxide production (68.4±0.3%). Taken together, our data indicate the yellow pleurotus is a promising functional food ingredients.

        The Positional Effect of Solute Functional Group among Positional Isomers of Phenylpropanol in Hydroxyl Group-Solvent Specific Interactions in Methanol/Water Mixed Solvents Monitored by HPLC

        We have evaluated the hydroxyl group-solvent specific interactions by using a Lichrosorb RP18 stationary phase and by measuring the retention data of carefully selected solutes in 50/50, 60/40, 70/30, 80/20, and 90/10(v/v%) methanol/water eluents at 25, 30, 35, 40, 45, and 50 oC. The selected solutes are 3 positional isomers of phenylpropanol, that is, 1-phenyl-1-propanol, 1-phenyl-2-propanol, and 3-phenyl-1-propanol. There exist clear discrepancies in Ho (solute transfer enthalpy from the mobile to the stationary phase) and TSo (solute transfer entropy) among positional isomers. The difference in Ho and TSo between secondary alcohols (1-phenyl-1-propanol and 1-phenyl-2-propanol)is negligible compared to the difference between the primary alcohol (1-phenyl-3-propanol) and secondary alcohols. The TSo values of 3-phenyl-1-propanol are close to those of butylbenzene while the TSo values of secondary alcohols are close to those of propylbenzene. The difference in Ho (specific solute-mobile phase interaction enthalpy) between the primary alcohol and the secondary alcohol decreases with increase of methanol content in the mobile phase. A unique observation is an extremum for 1-phenyl-3-propanol in the plot of TSo vs. methanol volume %. The positive sign of TSo of 3-phenyl-1-propanol implies that the entropy of 3-phenyl-1-propanol is greater than that of the hypothetical alkylbenzene (the same size and shape as phenylpropanol) in the mobile phase.
