        Relationships of Fall Risk With Frailty, Sarcopenia, and Balance Disturbances in Mild-to-Moderate Alzheimer’s Disease

        hence it commonly coexists with balance issues. Frailty and sarcopenia are intertwined and prevalent in dementia, and are closely related to falls. We aimed to determine the relationships of the fall risk with balance disturbances, sarcopenia, and frailty in mild-to-moderate Alzheimer’s disease (AD). Methods The study enrolled 56 patients with probable AD. A comprehensive geriatric assessment was performed, and muscle strength and mass, performance status, gait, and balance were evaluated. All parameters were compared between fallers and nonfallers with AD. Results Fallers comprised 53.6% of the study population. The demographic features and AD stages did not differ between fallers and nonfallers. Fallers were more frail than nonfallers (p<0.05). Frailty was found to be independently associated with fall history (odds ratio=2.15, 95% confidence interval=1.20–3.82, p=0.031). We found that falls were not associated with AD stage, muscle mass and function, balance and geriatric syndromes except urinary incontinence in patients with AD (p>0.05). Conclusions We found that falls were not influenced by AD stage. Both physical and cumulative frailty were strongly associated with falls in patients with mild-to-moderate AD.

        Surgical importance of the tympanic bone: multidetector computed tomography findings

        Objectives: To measure tympanic bone thickness (anterior-superior, anterior-inferior, and inferior wall), external ear canal length, and tympanomandibular distance that can be useful in cases that undergo tympanic bone resection. Materials and Methods: The temporal computed tomography (CT) images of 349 patients were retrospectively evaluated. The anterior-inferior, anterior-superior, and inferior wall thicknesses; tympanomandibular distance; and external auditory canal (EAC) bone canal length were measured from the narrowest part of the canal. The shapes of the EAC in the coronal and sagittal planes were also examined. Results: The numbers of female and male patients were similar, and the mean age was 49.45±13.95 years. The anterior-superior, anterior-inferior, and inferior wall thicknesses were 1.92±0.60, 2.54±0.74, and 9.16±2.20 mm, respectively. The anterior-superior and anterior-inferior wall thicknesses and canal lengths were greater on the right side (P<0.001). All measurement values were higher in males, except right tympanomandibular distance (P<0.05). A non-significant negative correlation was found between the age of the participants and the left anterior-inferior wall and tympanomandibular distance on both sides. Intra-observer agreement was high for all measurements. We observed four main shapes in the external ear canal in the coronal plane: Type 3, Type 2, Type 1, and Type 4 in order of frequency on the right, and Type 2, Type 3, Type 1, and Type 4 on the left. In the sagittal plane, we detected three shapes: oval (74.4%), triangular (16.3%), and round (9.4%). Conclusion: The anterior wall thicknesses and tympanomandibular distance should be measured on preoperative temporal bone CT to safely perform tympanic bone anterior resection, which is required in some otological procedures, and also to prevent temporomandibular joint damage.

        Mirtazapine Augmentation for Selective Serotonin Reuptake Inhibitor-Induced Sexual Dysfunction: A Retropective Investigation

        The aim of the present study was to retrospectively identify sexual dysfunction changes in the patients under mirtazapine-augmented serotonin reuptake inhibito (SSRI) treatment. The study comprised medical records of 20 outpatients, under mirtazapine-augmented SSRI treatment for their major depressive disorder, who had been selected among the patients that had developed sexual dysfunction to previous treatment as monotherapy, with SSRI for at least six weeks. These drugs were maintained and mirtazapine were added (15-45 mg/day). There was a significant difference in scores between baseline and week 4 or week 8 on the both Hamilton Depression Rating and Arizona Sexual Experience Scale. According to Clinical Global Impression-Improvement, 68.4% of the patients were responders. The use of low-dose mirtazapine as an add-on treatment to SSRIs appears to be an effective and well-tolerated augmenttaion for sexual dysfunction caused by SSRIs.

        Effects of Plant Extracts on Lipid Oxidation in Fish Croquette during Frozen Storage

        The aim of this study was to investigate the effects of tomato and garlic extracts on lipid oxidation in fish croquette during frozen storage. The fish for croquette was purchased from the main fish market in Antalya,Turkey. Commercial tomato and garlic extracts were added into the croquette formulation. Lipid quality of frozen croquettes was analyzed at monthly intervals. There was no difference in free fatty acids and UV absorbance values of treatment groups. Treatment of tomato and garlic extracts kept oxidation at low levels. The results for thiobarbutiric acid, para-anisidine, and conjugated-diene values showed that tomato extract was the most effective in delaying lipid oxidation than garlic extract.

      • Twinning induced reduction of seismic anisotropy in lawsonite blueschist

        Lawsonite is an important mineral to understand seismic anisotropy in subducting oceanic crust because of its large elastic anisotropy and prevalence in cold subduction zones. However, there is a lack of knowledge on how lawsonite twinning affects seismic anisotropy despite previous reports showing the existence of twins in lawsonite. We thus investigated the effect of twins in lawsonite on lattice preferred orientation (LPO), fabric strength, and seismic anisotropy of lawsonite using the lawsonite blueschists from Alpine Corsica (France) and Sivrihisar Massif (Turkey). LPOs of minerals were measured by using the electron backscattered diffraction (EBSD) facility attached to scanning electron microscope. The EBSD analyses of lawsonite revealed that {110} twinning in lawsonite is developed and [001] axes are strongly aligned subnormal to the foliation and both [100] and [010] axes are aligned subparallel to the foliation. It is found that the existence of twins in lawsonite could induce a large reduction of seismic anisotropy, especially for the maximum S-wave anisotropy up to 3.67% in lawsonite and 1.46% in the whole rocks. Therefore, lawsonite twinning needs to be considered in the interpretation of seismic anisotropy in the subducting oceanic crust in cold subduction zones.

        Lumbar Disc Herniation in a Patient With Congenital Vertebral Body Anomaly: A Case Report

        Lumbar disc herniation is characterized with low back and leg pain resulting from the degenerated lumbar disc compressing the spinal nerve root. The etiology of degenerative spine is related to age, smoking, microtrauma, obesity, disorders of familial collagen structure, occupational and sports-related physical activity. However, disc herniations induced by congenital lumbar vertebral anomalies are rarely seen. Vertebral fusion defect is one of the causes of congenital anomalies. The pathogenesis of embryological corpus vertebral fusion anomaly is not fully known. In this paper, a 30- year-old patient who had the complaints of low back and right leg pain after falling from a height is presented. She had right L5-S1 disc herniation that had developed on the basis of S1 vertebra corpus fusion anomaly in Lumbar computed tomography. This case has been discussed in the light of literature based on evaluations of Lumbar Computed Tomography (CT) and Magnetic Resonance Imaging (MRI). This case is unique in that it is the first case with development of lumbar disc herniation associated with S1 vertebral corpus fusion anomaly. Congenital malformations with unusual clinical presentation after trauma shouldbe evaluated through advanced radiological imaging techniques.

      • Tertiary Cytoreduction for Recurrent Epithelial Ovarian Cancer: a Multicenter Study in Turkey

        Background: The purpose of this study was to determine the benefit of tertiary cytoreductive surgery (TC) for secondary recurrent epithelial ovarian cancer (EOC), focusing on whether optimal cytoreduction has an impact on disease-free survival, and whether certain patient characteristics could identify ideal candidates for TC. Materials and Methods: Retrospective analysis of secondary recurrent EOC patients undergoing TC at three Turkish tertiary institutions from May 1997 to July 2014 was performed. All patients had previously received primary cytoreduction followed by intravenous platinum-based chemotherapy and secondary cytoreduction for first recurrence. Clinical and pathological data were obtained from the patients' medical records. Survival analysis was caried out using the Kaplan Meier method. Actuarial curves were compared by the two tailed Logrank test with a statistical significance level of 0.05. Results: Median age of the patients was 49.6 years (range, 30-67) and thirty-eight (72%) had stage III-IV disease at initial diagnosis. Twenty six (49%) had optimal and 27 (51%) suboptimal cytoreduction during tertiary debulking surgery. Optimal initial cytoreduction, time to first recurrence, optimal secondary cytoreduction, time interval between secondary cytoreduction and secondary recurrence, size of recurrence, disease status at last follow-up were found to be significant risk factors to predict optimal TC. Optimal cytoreduction in initial and tertiary surgery and serum CA-125 level prior to TC were independent prognostic factors on univariate analysis. Conclusions: Our results and a literature review clearly showed that maximal surgical effort should be made in TC, since patients undergoing optimal TC have a better survival. Thus, patients with secondary recurrent EOC in whom optimal cytoreduction can be achieved should be actively selected.

        Neutrophil to Lymphocyte Ratio Predicts Left Ventricular Remodeling in Patients with ST Elevation Myocardial Infarction treated with Primary Percutaneous Intervention

        Background and Objectives: It has been demonstrated that the neutrophil/lymphocyte ratio (NLR) might be a useful marker to predict cardiovascular risk and events. We aimed to investigate the role of the NLR to predict ventricular remodeling (VR) in patients with anterior ST-elevation myocardial infarction (STEMI) who were treated with primary percutaneous coronary intervention. Subjects and Methods: We prospectively included 274 consecutive anterior STEMI patients. Echocardiography was performed during admission and at six months after myocardial infarction. VR was defined as at least 20% increase from baseline in left ventricular end- diastolic volume. Patients were divided into two groups according to their VR status: VR (n=67) and non-VR (n=207). Total and differential leukocyte count, N-terminal pro-brain natriuretic peptide (NT-proBNP) and other biochemical markers were measured at admission and 24 hours later. Results: Compared with the non-VR group, peak creatine kinase MB (CK-MB), NT-proBNP (24 h), neutrophil/lymphocyte ratio, presence of diabetes, no-reflow frequency and wall motion score index were significantly higher in patients with VR (p<0.05 for all). On multivariate logistic regression analysis, NLR (β=2.000, 95% confidence interval=1.577-2.537, p<0.001) as well as peak CK-MB, NT-proBNP (24 h), WMSI and diabetes incidence were associated with VR. The cutoff value of the neutrophil/lymphocyte ratio obtained by receiver operator characteristic curve analysis was 4.25 for the prediction of VR (sensitivity: 79 %, specificity: 74%). Conclusion: In patients with anterior STEMI, initial NLR and NT-proBNP measured 24 hours after admission may be useful for predicting adverse cardiovascular events including left VR.
