      • Roles of E-cadherin and Cyclooxygenase Enzymes in Predicting Different Survival Patterns of Optimally Cytoreduced Serous Ovarian Cancer Patients

        The relation between cyclooxygenase enzymes and E-cadherin, along with the roles of these markers in the prediction of survival in optimally cytoreduced serous ovarian cancer patients was investigated. Individuals who underwent primary staging surgery and achieved optimal cytoreduction (largest residual tumor volume <1 cm) constituted the study population. Specimens of 32 cases were immunohistochemically examined for cyclooxygenase-1, cyclooxygenase-2, and E-cadherin. Two could not be evaluated for E-cadherin and cyclooxygenase-1. Overall, 14/30, 19/30, and 15/32 cases were positive for E-cadherin, cyclooxygenase-1, and cyclooxygenase-2, respectively. The expressions of E-cadherin and cyclooxygenase-2 were inversely correlated (p:0.02). E-cadherin expression was related with favorable survival (p<0.001). The relation between the expression of cyclooxygenase enzymes and poor survival did not reach statistical significance. On multivariate analysis, E-cadherin appeared as an independent prognostic factor for survival. In conclusion, E-cadherin expression is strongly linked with favorable survival. E-cadherin and cyclooxygenase 2 may interact with each other during the carcinogenesis-invasion process. Further studies clarifying the relation between E-cadherin and cyclooxygenase enzymes may lead to new preventive and therapeutic targets in ovarian cancer.

        Monte Carlo analysis of earthquake resistant R-C 3D shear wall-frame structures

        The theoretical background and capabilities of the developed program, SAR-CWF, for stochastic analysis of 3D reinforced-concrete shear wall-frame structures subject to seismic excitations is presented. Incremental stiffness and strength properties of system members are modeled by extended Roufaiel-Meyer hysteretic relation for bending while shear deformations for walls by Origin-Oriented hysteretic model. For the critical height of shear-walls, division to sub-elements is performed. Different yield capacities with respect to positive and negative bending, finite extensions of plastic hinges and P-${\delta}$ effects are considered while strength deterioration is controlled by accumulated hysteretic energy. Simulated strong motions are obtained from a Gaussian white-noise filtered through Kanai-Tajimi filter. Dynamic equations of motion for the system are formed according to constitutive and compatibility relations and then inserted into equivalent It$\hat{o}$-Stratonovich stochastic differential equations. A system reduction scheme based on the series expansion of eigen-modes of the undamaged structure is implemented. Time histories of seismic response statistics are obtained by utilizing the computer programs developed for different types of structures.

        Utilization of Waste Loquat (Eriobotrya Japonica Lindley) Kernels as Substrate for Scleroglucan Production by Locally Isolated Sclerotium rolfsii

        Present study mainly focused on investigating the feasibility of waste loquat kernels as substrate in submerged culture of Sclerotium rolfsii MT-6 for scleroglucan production. Loquat kernel contained high protein (22.5%)and total carbohydrate (71.2%) contents. Dried and powdered kernels were subjected to acid hydrolysis with 2N HCl. The obtained hydrolysate was used for the preparation of loquat kernel extract (LKE) and detoxified loquat kernel extract (DLKE). S. rolfsii MT-6 was isolated from fermented squash (Cucurbita pepo). Optimal conditions for scleroglucan production were initial pH 5.0, shaking speed 150 rpm, 28℃, and cultivation time of 72 hr. Production media prepared with DLKE or LKE gave maximum biomass concentrations of 17.06 and 16.21 g/L,and maximum scleroglucan concentrations of 12.08 and 10.53 g/L, respectively. DLKE was also favorable substrate for mycelial growth in a uniform pellet form. This is a first report on the application of waste loquat kernels as scleroglucan production substrate and on the use of a local S. rolfsii isolate for this purpose.

        Static analysis of simply supported porous sandwich plates

        In this study, it is aimed to analyze the bending of porous sandwich plates using the four-variable shear deformation theory. The core of the sandwich plate is assumed to be functionally graded, and face sheets are assumed to be isotropic. The pore distribution of the sandwich plate is considered even and uneven type of porosity distribution. Displacement fields are defined with four variable shear deformation theory. Equilibrium equations of porous sandwich plates are derived from virtual displacement principle. An analytical solution is obtained by Navier’s approach. Results are presented for uniformly and sinusoidally distributed loaded porous sandwich plates. Face sheet -core thickness ratio, porosity distribution, amount of porosity is investigated.

        Quality of Life in Older Adults with Cochlear Implantation: Can It Be Equal to That of Healthy Older Adults?

        Background and Objectives: This study aimed to evaluate the audiologic results after cochlear implantation (CI) in older patients and the degree of improvement in their quality of life (QoL).Subjects and Methods: Patients over 65 years old who underwent CI at implant center in Bozyaka Training and Research Hospital were included in this study (n=54; 34 males and 20 females). The control group was patient over 65 years old with normal hearing (n=54; 34 males and 20 females). We administered three questionnaires [World Health Organization Quality of Life-BREF (WHOQOL-BREF), World Health Organization Quality of Life-OLD (WHOQOL-OLD)], and Geriatric Depression Scale (GDS) to evaluate the QoL, CIrelated effects on activities of daily life, and social activities in all the subjects. Moreover, correlations between speech recognition and the QoL scores were evaluated. The duration of implant use and comorbidities were also examined as potential factors affecting QoL.Results: The patients had remarkable improvements (the mean score of postoperative speech perception 75.7%) in speech perception after CI. The scores for the WHOQOL-OLD and WHOQOL-BREF questionnaire responses were similar in both the study and control groups, except those for a two subdomains (social relations and social participation). The patients with longer-term CI had higher scores than those with short-term CI use. In general, the changes in GDS scores were not significant (<i>p</i><0.05).Conclusions: The treatment of hearing loss with CI conferred significant improvement in patient’s QoL (p<0.01). The evaluation of QoL can provide multidimensional insights into a geriatric patient’s progress and, therefore, should be considered by audiologists.

        Background: Gastric Xanthalesmas are rare benign incidental yellow-white lesions between 0.5-10 mm in size. They are detected more frequently in female patients and the frequency of these lesions increases with age. Although the etiology of these lesions are unclear, chronic gastritis, gastrointestinal anastomosis, intestinal metaplasia, and H. pylori infection are found to be associated with GI xanthalesmas. They are usually located in antrum of the stomach in gastrointestinal tract, especially along the lesser curvature. In contrast to cutaneous xanthalesmas there is no evident association between GI xanthalesmas and hyperlipidemia. Methods: Here we present endoscopic and histological features of gastric xanthalesma in a 65 year old man with a history of chronic gastritis and iron defi ciency anemia. Results: Patient presented with iron defi ciency anemia. Upper GI endoscopy and colonoscopy has been done for investigation of the etiology. Colonoscopy showed diverticular disease of the colon. Upper GI endoscopy of the patient showed a 4mm foamy yellowish lipamatous lesion at the lesser curvature of the stomach (Image). Multiple biopsies are obtained from the lesion. The histology showed large foamy histiocytes containing a mixture of lipids in the mucosa and the submucosa. The multiple biopsies obtained from the antrum showed intestinal metaplasia and chronic gastritis with H.plylori positivity. Conclusion: Gastric xanthalesmas are asymptomatic benign and incidental endoscopic fi ndings. Even though they can be found in every part of Gastrointestinal tract from esophagus to colon, they are frequently observed in stomach. They are usually associated with chronic gastritis, Helicobacter pylori infection and intestinal metaplasia. Although the clinical signifi cance is unclear, gastric xanthalesmas are important lesions and biopsies should always be performed because they are frequently confused with malignant lesions.

        Lateral Locked Plating or Dual Plating: A Comparison of Two Methods in Simple Bicondylar Tibial Plateau Fractures

        Background: In this study, our aim was to compare the results of the dual locking plate fixation technique and lateral locking plate fixation technique for tibial bicondylar plateau fractures without posteromedial fragment. Methods: We evaluated 20 patients who underwent surgical treatment due to bicondylar tibial plateau fracture between 2010 and 2015. Ten patients were included in group 1, in which a dual locking plate was employed, whereas 10 patients were included in group 2, in which a lateral locking plate was used. In both groups, functional and clinical outcomes after treatment were rated according to the Knee Society Knee Scoring System, Rasmussen functional score, and Rasmussen radiological score. Results: The mean follow-up time was 24 months. There were no significant differences between the groups with respect to functional and radiographic outcomes at the final follow-up. Conclusions: In bicondylar tibial plateau fractures without posteromedial fragment, the lateral locking plate fixation technique showed the similar clinical and radiological outcomes as the dual locking plate fixation technique.

        The effect of disease-modifying antirheumatic drugs on sleep and quality of life in older patients with rheumatoid arthritis

        Objective: The sleep quality is worse in rheumatoid arthritis (RA) patients than in healthy controls and it is more difficult to achieve a satisfactory quality of life after treatment with age. Our aim is to assess the quality of life and sleep in elderly onset RA patients and to analyze the effect of disease-modifying agents on sleep and quality of life. Methods: Thirty-four older patients with RA patients and 30 healthy controls are included in the study. Sleep quality was evaluated with the Pittsburg sleep quality index and quality of life with Short Form-36. Parametric/non-parametric tests and Spearman/ Pearson correlation analysis were applied for the data according to the distribution. Results: While the rate of poor sleep quality before treatment was 67.6%, the rate was 26.5% after treatment. There was a statistically significant difference before and after treatment in terms of subjective sleep quality, sleep latency, sleep duration, sleep efficiency, and scores for sleep disturbance. The mean steroid dose and Disease Activity Score-28 were higher in patients with poor sleep quality than in patients with good sleep quality. Patients with poor sleep quality had lower mean physical function, pain, general health, social function, emotional role difficulties, and energy/vitality values than patients with good sleep quality. Conclusion: Both sleep and quality of life improved after treatment in older patients with RA patients. In older patients, it should be regularly evaluated in terms of sleep and quality of life and appropriate treatment should be provided.
