Photodynamic Therapy for Esophageal Cancer

Photodynamic therapy, a curative local treatment for esophageal squamous cell carcinoma, involves a photosensitizing drug(photosensitizer) with affinity for tumors and a photodynamic reaction triggered by laser light. Previously, photodynamic therapywas used to treat superficial esophageal squamous cell carcinoma judged to be difficult to undergo endoscopic resection. Recently,photodynamic therapy has mainly been performed for local failure after chemoradiotherapy. Although surgery is the most promisingtreatment for local failure after chemoradiotherapy, its morbidity and mortality rates are high. Endoscopic resection is feasible forlocal failure after chemoradiotherapy but requires advanced skills, and its indication is limited to within the submucosal layer bydepth. Photodynamic therapy is less invasive than surgery and has a wider indication than endoscopic resection. Porfimer sodium(a first-generation photosensitizer) causes a high frequency of side effects related to photosensitivity and requires the long-termsun-shade period. Talaporfin (a second-generation photosensitizer) requires a much shorter sun-shade period than porfimersodium. Photodynamic therapy will profoundly change treatment strategies for local failure after chemoradiotherapy.

Japanese national health expenditure was 8.0% of GDP in 2004, and it was lower than average of OECD countries. But it has increased rapidly in recent years. Japan has relatively many acute care beds and high-price medical equipments, and the average length of stay is long. Japanese government is trying healthcare reform to contain healthcare expenditure, increase the efficiency of management and improve the quality of healthcare. As healthcare policies for hospitals such as DPC (Diagnosis Procedure Combination) for acute care beds, reduction of long-term care beds, and functional differentiation and liaison among healthcare institutions are implemented, the number of hospitals in financial difficulties is increasing. The serious situation urges hospitals to adapt to changes and search new directions of management. They need to establish and implement appropriate positioning strategy, and increase management efficiencies. Korean healthcare system has similarities with Japanese in many aspects. The recent reform and changes in Japanese healthcare system and hospitals give suggestions to Korean hospitals as to how they can prepare for environmental changes and improve management.

This paper describes a fast circuit simulation technique using the latency insertion method (LIM) with a parallel and distributed leapfrog algorithm. The numerical simulation results on the PC cluster system that uses the cloud computing system are shown. As a result, it is confirmed that our method is very useful and practical.
