        전국 응급의학과 수련병원의 응급실 병력지에 대한 분석

        Background: This study was designed to analyze the current emergency department(ED) medical records of teaching hospitals in Korea. Methods: The five-item questionnaires were mailed to the EDs of 40 hospitals. Among them, 27 questionnaires and 35 ED medical records were returned for reply rates of 67.5% and 87.5%, respectively. Results: 1) The actual number of data elements in the ED medical records used by each hospital varies widely. It ranges from 1 to 15 data elements with an average of 7.5 data elements. 2) Thirteen data elements, signature of nurse, checklist style in review of systems, checklist style in physical examination, neurologic examination, figure of face, Glasgow coma scale, trauma scale, treatment plan, mode of transfer, condition on transfer, documents sent with patient, condition on discharge or discharge instruction, use of pediatric chart and vaccination history are used by less than 50% of the medical records examined. 3) There was no difference in the total number of data elements or in redesign and computerization of ED medical record based on the location of the hospital, the type of hospital administration, or the number of years since the start of EM residency program. 4) There was a statistically increased number of data elements in redesigned medical records. 5) In the survey, 89% of the residents replied that medical records needed to be redesigned. With respect to uniformity, 58% of the residents disagreed. A well-designed checklist chart rather than a descriptive chart was preferred by 89% of the residents. Conclusion: The currently used ED medical records have much room for improvement. The age of the ED had little impact on the quality of ED medical records. More attention and effort in this field are needed. In addition, The Korean Society of Emergency Medicine should provide guidelines for ED medical records.

      • 産業技能訓練의 패턴 硏究

        As our society advances economically, and technologically, the need for technical manpower becomes more critical. Recently, the industrial development in Korean has grown so rapidly that the need for industrial training is becoming more and more important. As a result, effective and efficient industrial education is in great demanded. The purpose of this study was to seek effective and efficient instructional methods for industrial training. The procedures of the study were : 1) to survey the present status and problems of industrial training in Korea 2) to seek effective an efficient instructional patterns for industrial training by applying the systematic approach to instruction and the individualized instructional system pattern. The results of the survey were analyzed within the following six areas : A) Administration and organization of industrial training institutes B) the goals and objectives of industrial training, C) the curricula presented in industrial training D) instructional methods, materials and facilities of industrial training, F) instructional systems of industrial education. It was found that there exist many problems to be solved in industrial training. 11.9% of 150 respondents answered that they have applied the systems approach to their teaching-learning process. It indicated that the instructional method of industrial training has not yet been systemized. It seems to be clear that industrial training need the employment of a systems approach to instruction for effective and efficient industrial training. Since this study is mainly aimed at seeking effective and efficient instructional methods, two developed instructional patterns were suggested: A) a systematic approach to industrial training and B) an individualized instructional system to industrial training. A. Systematic approach to industrial training Instruction must be designed based on what the learner is to do, produce or become. By employing a systems approach, the diversity of elements in the teaching-learning process will be organized usefully and effectively. The pattern which was applied for this study was developed by Ely & Gerlach of the U.S.A. This systematic approach pattern has ten elements in its system: 1) specification of objectives, 2) selection of content, 3) the assessment of entering behaviors, 4) the strategy which will be employed, 5) the organization of students into groups, 6) the allocation of time, 7) the allocation of learning spaces, 8) the selection of appropriate learning resources, 9) the evaluation of teacher and learner performance, 10) an analysis of feedback by the teacher and the learner. B. Individualized instructional system for industrial training Industrial educators explore the need for and utilization of individualized instruction in industrial education. The development of an individualized approach to instruction is based on several components which serve to integrate and structure the learning environment for each student. The following components should be considered in the development of individualized instructional systems: 1. Selecting the instructional content for adoption to an individualized instructional system. 2. Specifying course objectives in behavioral terms. 3. Developing learning sequences that describe the learning paths students can travel in order to fulfill each course objective. 4. Identifying and selecting a variety of instructional media suitable for an individualized instructional approach. 5. Developing instructional materials as required to supplement these materials readily available. 6. Preparing evaluative instruments and procedures that diagnose and monitor individual progress through the learning sequences. Industrial educators should derive tangible benefits from efforts involved in the development of individualized instruction. The instructional strategy proposed could be applied to industrial and technical education at the elementary, secondary and postsecondary school levels. According to some industrial educators, the experience with individualized instruction indicated the approach was successful in allowing students to pace learning according to their individual abilities and to utilize time more effectively.

      • 선경축소에 따른 슈퍼데크의 구조안전성 평가에 관한 실험적 연구

        SuperDeck is as the deck of assembling Kaiser, cubical rebar for slab, with corrugated zinc galvanized sheet iron at manufactory, given the improvement on design, manufacture, and performance for construction work of cast-in-place reinforced concrete slab by enabling to cast concrete directly without the form work. Nowadays, in general, simplified 2D analysis and allowable stress design are used as for the method of analyzing and designing of SuperDeck System. Since this method contradicts with 3D analysis, used for the development of SD300 at The Korean Structural Engineers Association, and limit state design, the comparison and examination of these two methods and the maximum span, which allows for construction, are needed. To compare the simplified 2D analysis applied to allowable stress design with 3D analysis applied to limit state design, 8 specimen are manufactured. Main variables include the depth of slab, the length of span, the diameter of bottom bar and lattice bar, and the presence of corrugated zinc galvanized sheet iron. From the comparison result of test and analysis, in case of using under D10 as bottom bar, 3D analysis is more appropriate.

      • 도시 하수오니에 의한 고분자 물질의 생분해도 측정

        Factors influencing the biodegradability of polymer materials by municipal sewage sludge were studied using starch, polyethylene glycol(PEG) and polyvinyl alcohol(PVA). The degree of biodegradability was determined by measuring evolved carbon dioxide as a function of time, when the polymer material is exposed to activated sewage sludge. The biodegradability of starch reached around 70% and 40% after 30day incubation in summer and autumn, respectively. This is caused by the activity of microorganism and incubation temperature. The biodegradability of PEG 8000 and PVA was 1/7 and 1/14 of starch, respectively. Biodegradation of PEG 8000 after 30 day incubation in summer was slightly higher than that in spring while lag phase was significantly longer in spring. This shows that the activity of microorganism as well as incubation temperature is important for biodegradation.

      • 151종 생약제 추출물이 B형 간염 바이러스 표면 항원(HBsAg)과 항체(HBsAb)의 결합반응 그리고 HBV DNA polymerase 활성에 미치는 효과 : 한국산 각종 생약제를 이용한 B형 간염치료제의 개발

        세계인구 43억명 중 B형 간염 바이러스 보균자는 2억 1천 5백만명이며 이중 78%인 1억 6천만명 이상이 아시아 대륙에 집중되어 있다. 한국도 B형 바이러스 간염의 침윤지대이다. 본 연구는 B형 바이러스 보균자 혹은 만성간염을 치료할 수 있는 생약제를 탐색할 목적의 일환으로 한국에서 시판하는 생약제 151종을 구입하여 증류수로 추출하고 여과한 후 냉동건조하여 HBs 항원에 대한 결합능, HBV DNA polymerase억제능, tumor necrosis factor(TNF) 유리에 미치는 영향, 그리고 galactosamine 유발 간염에 대한간장보호 작용을 조사하였다. 151종의 생약제중 0.16㎍/㎖ 농도에서 간염 vaccine 내에 함유된 HBs 항원과 결합하는 생약제는 지유, 가자, 복분자, 대황, 정향, 빈량, 산수유, 소목, 마황, 쇄양, 후박, 목과, 양강, 오약, 산사, 목단피 등 16종이었다. 이들 생약제중 대황(Rheum palmatum L. 大黃), 가자(Terminalia chebula R. 訶子), 지유(Sanguisorba officinalis L. 地楡), 복분자(Rubus coreanus M. 覆盆子)는 HBV DNA polymerase 활성도를 비교적 강하게 억제하였으며 빈량(Areca catechu L. 빈랑), 정향(Eugenia caryophyllata T. 丁香), 목과(Chaenomeles japonica L. 木瓜), 산수유(Cornus officinalis S. 山茱萸)는 약하게나마 HBV DNA polymerase 활성을 억제하였다. 또한 HBV DNA polymerase를 억제한 대황, 가자, 지유, 복분자, 빈량, 정향, 마황, 소목, 산수유를 대상으로 TNF 유리에 미치는 영향을 조사한 결과 소목을 제외한 8종의 생약제 모두가 대식세포를 자극하여 TNF를 유리시켰다. 그리고 장차 임상에 사용함을 전제로 HBV DNA polymerase를 억제한 대황, 가자, 지유, 복분자, 빈량, 정향, 마황, 소목, 산수유를 galactosamine 유발 간염(백서)에 투여하여 간기능 검사에 미치는 효과를 조사하였으나 간기능 검사상으로는 특별한 보호작용을 나타내지 못하였으며 생약제 단독 투여로서는 간기능에 대하여 나쁜 영향을 주지 않았다. Hepatitis B virus infection is major cause of acute and chrome hepatitis, cirrhosis of liver, and hepatocellular carcinoma. There are over 200 million carriers of hepatitis B virus in the world, nearly 80 % of whom live in the South East Asia, Korea is also highly endemic area of hepatitis B virus infection. This study was performed to evaluate the effect of extracts of traditional Korean herbal medicines on hepatitis B virus. 151 traditional herbs were extracted, filtered and lyophilized. Using 151 herbal extracts we tested binding capability to the HBs Ag, inhibition of HBV DNA polymerase, release of tumor necrosis factor (TNF), and protection against galactosamine induced liver damage. Among 151 herbs, 16 herbs (Sanguisorba officinalis L., Terminalia chebula R., rubus coreanus M., Rheum palmatum L., Eugenia caryophyllata T., Areca catechu L., Cornus officinalis S., Caesalpinia sappan L, Ephedra sinica S., Cynomorium songaricum R., Machilus thunbergii S., Chaenomeles japonica L., Alpinia officinarum H., Lindera strychnifolia V., Crataegus pinnatifida B., Paeonia suffruticasa A. ) had binding capability to HBs Ag which contained in the commercial hepatitis vaccine, at 16 ug/㎖ of concentratioa Among the above 16 herbs, Rheum palmatum L., Terminalia chebula R., Sanguisorba officinalis L., Rubus coreanus M. significantly inhibited the HBV DNA polymerase activity, and Areca catechu L., Eugenia caryophyllata T., Ephedra sinica S., Caesalpinia sappan L., Chaenomeles Japonica L., Cornus officinalis S. also inibited the HBV DNA polymerase activity but less effective. All of the herbs, except Caesalpinia sappan L., which inhibited the HBV DNA polymerase activity, actively stimulated the mononuclear cells to release tumor necrosis factor(TNF) in whole blood culture system. The herbs which inhibited the HBV DNA polymerase activity, did not show any protective effect in galactosamine-induced hepatitis in rats, but there was no direct toxic effect when herbs were administered alone.

      • 適應 ARMA 모형이 적용에 관한 고찰

        ARMA 모형에 의한 음성 분석에 있어서 가장 곤란점의 하나인 입력추정에 대해서 검토하고 보조수단으로서 임펄스 추정 알고리즘을 제안하였다. 제한된 조건하에서 제 안된 알고리즘은 극히 우수한 특성을 보였으나 조건의 변화에 따라 그 특성이 급격히 열화되는 것이 관측되었으며 일반적 적용은 타당하지 않음이 판명되었다. 아울러 파라메터 추적과정에서 수렴에 적절한 신호의 진폭과 수렴판정의 척도에 대해서 평가와 제안을 실었다. The major difficulty in analyzing the voice signal by ARMA model lies on the fact that the excitation signal can not be separated from the estimation error. An algorithm appropriate to estimate isolated impulse type excitation signal is presented along with the brief discussion of recent work related to the topic. The proposed algorithm exhibits highly reliable performance under the favorable conditions. However it has been found the algorithm is so sensitive to the change of condition that it's applicability is limited to the special purposes. Also, the effect of the signal amplitude on the convergence, and the measure of convergence are discussed, and suggestions are given for better use of ARMA model for voice signal analysis.

      • Benzene과 n-Heptane 混合物의 液膜分離에 있어서 界面活性濟의 影響

        In the separation of benzene-n-heptane mixture by emulsion liquid membrane in a batch system, the effect of surfactants on the separation factor and stability of liquid membrane was investigated over the surfactant concentration ranging from 0.1 to 1.5wt% at the emulsification time of 5 and 10 minutes, the contact time of 5 and 10 minutes and the settling time of 5 and 10 minutes. The surfactants used were triethanolamine lauryl sulfate, sodium laurylether sulfate and sodium lauryl sulfate. In the concentration range used sodium laurylether sulfate showed better separation than the other two surfactants and the separation factor reached its maximum at the surfactant concentration of 0.5wt% for all three surfactants except several experimental conditions only for sodium laurylether sulfate. In the range of contact time used the separation factor increased as the contact time increased for all three surfactants, however, in the case of sodium laurylether sulfate the separation factor decreased after the contact time of 20 minutes.

      • 一部都市地域 靑少年의 成長發育과 最大成長發育年齡에 관한 類似縱斷的 硏究

        This study was carried out to know physical growth and development, physical and nutritional indices, and maximum growth accelerating phenomena by semi―longitudinal method to measure body height, body weight, chest girth and sitting height of 427 of high school 3rd grade students who were living in Anyang city and born from March 1 in 1973 to February 28 in 1974. The result Were as follows : 1. Physical growth and development Body height grew one―step direct―development and body weight, chest―girth and sitting height grew two―step direct―development. Female excelled made in two―step development. Intersection phenomena of male and female showed in case of body height : 9.5 ∼ 13.5, body weight : 10.0 ∼ 14.0, chest―girth :11.5 ∼ 15.5, sitting height : 9.5 ∼ 14.0. During this periods, female excelled male but after these periods, male excelled female again. 2. Physical and nutritional indices In all cases of relative body weight, relative chest girth and relative sitting height it was found to be increasing thereafter with advancing ages. In cases of Ro¨hrer index and Kaup index it was found to be reaching to normal state thereafter with advancing ages. In either cases of Vervaeck and Pelidisi index, if was found to be increasing and teaching to normal state thereafter with advancing ages. 3. Maximum growth age of physiques M.G.A.'s were 12.31 for male and 10.42 for female which showed that M.G.A. for female appeared about 2 years earlier than that for male, in body height. M.G.A.'s were 12.73 for male and 11.65 for female which showed that M.G.A. for female appeared about 1 years earlier than that for male, in body weight. M.G.A.'s were 12.75 for male and 11.54 for female which showed that M.G.A. for female appeared about 1.2 years earlier than that for male, in chest ― girth. M.G.A.'s were 12.52 for male and 10.44 for female which showed that M.G.A. for female appeared about 2 years earlier than that for male, in sitting height.
