Background/Aims: Anisakiasis is frequent in Jeju Island because of the people`s habit of ingesting raw fish. This study evaluated the clinical characteristics of patients with small bowel anisakiasis and compared them with those of patients with gastric anisakiasis. Methods: We retrospectively reviewed the medical records of 109 patients diagnosed with anisakiasis between May 2003 and November 2011. Results: Of the 109 patients diagnosed with anisakiasis, those with suspicious anisakiasis (n=38) or possible anisakiasis (n=12) were excluded. The age and gender distributions did not differ between patients with small bowel anisakiasis (n=30; age, 45±13 years) and those with gastric anisakiasis (n=29; age, 46±10 years). The mean duration of hospitalization was 5.4±4.3 days for patients with small bowel anisakiasis and 0.5±1.7 days for patients with gastric anisakiasis. Small bowel anisakiasis was accompanied by leukocytosis (76.7% vs 25.5%, p=0.003) and elevated C-reactive protein levels (3.4±3.2 mg/dL vs 0.5±0.3 mg/dL, p=0.009). Contrast-enhanced abdominopelvic computed tomography showed small bowel wall thickening with dilatation in 93.3% (28/30) of patients and a small amount of ascites in 80.0% (24/30) of patients with small bowel anisakiasis. Conclusions: Compared with gastric anisakiasis patients, small bowel anisakiasis patients had a longer hospitalization time, higher inflammatory marker levels, and small bowel wall thickening with ascites. Recently, diffusion of Telematics service and explosive increase of using in-vehicle display device such as CNS(Car Navigation System) are becoming the new causes of traffic accidents during driving. This is caused by 'Inattention' from increase of driver's mental workload while he/she is driving. It is the real situation that many countries including Japan are considering a control of In-vehicle device during driving through legislation. This research is studied about the effect of information system at voice or display unit on drivers through the Driver Eye Movement Analysis and measurement of performance ability. The experiment is carried out in the driving simulator environment, and the result presents through the comparative analysis of the driver's visual field via the gaze tracking device and average of correct-answer rates in addition arithmetic performance.

Background/Aims: Rabbit anti-thymocyte globulin (ATG) is usually incorporated in hematopoietic stem cell transplantation (HSCT) to reduce the incidence of graft-versus-host disease (GVHD). This study aimed to find optimal ATG doses in patients undergoing human leukocyte antigen (HLA)-mismatched allogeneic HSCT. Methods: We retrospectively collected medical records from 352 consecutive patients with acute myeloid leukemia (n = 214), acute lymphoblastic leukemia (n = 62), or myelodysplastic syndrome (n = 76) in eight centers of Korea between 2005 and 2015. All patients received busulfan-based conditioning without total body irradiation (TBI) and received stem cells from HLA-mismatched donors. Results: In the current study, 5-year overall survival rates of patients receiving low to medium doses of ATG (2.5 to 7.5 mg/kg) were higher than those receiving other doses of ATG (hazard ratio [HR], 0.528; 95% confidence interval [CI], 0.311 to 0.897; p = 0.018). The incidence rates of extensive chronic GVHD (ecGVHD) after administration of low to medium doses of ATG were lower than those after other doses of ATG (HR, 0.447; 95% CI, 0.224 ton 0.889; p = 0.022). Conclusions: The low to medium doses of ATG may be associated with improving survival outcomes and reducing incidence of ecGVHD without enhancing the chances of relapse in patients with acute leukemia or myelodysplastic syndrome undergoing non-TBI-based HLA-mismatched allogeneic HSCT.

Purpose Folate, folic acid, has a role in mitigating inflammatory reactions in the human body. This study aimed to evaluate the association of serum folate levels with lung function in chronic obstructive pulmonary disease (COPD) patients. Method Of 8,149 participants of the 2016 Korean National Health and Nutrition Examination Survey (KNHANES), 311 subjects (192 males and 119 females) having COPD defined by the lower fifth percentile of reference population were selected. Pearson’s correlation coefficient was used to investigate the relationship between serum folate level and lung function measurements. The association between serum folate level and lung function in patients with COPD was evaluated using multivariable linear regression analysis after adjustment for age, sex, height, high sensitivity C-reactive protein, total calorie intake, residence, smoking status and smoking pack-years, education, and household income. Result Serum folate level showed a positive correlation to the predicted percentage of forced expiratory volume in one second (FEV1%). In males, a trend for positive correlation with serum folate level was observed in predicted FEV1%, FEV1 value, predicted percentage of forced vital capacity (FVC%), FVC value, and peak expiratory flow (PEF). No significant correlation between serum folate level and lung function in females was observed. In the multivariable linear regression model, serum folate level was associated with an increase in predicted FEV1%, FEV1 value, predicted FVC%, FVC value, and PEF; however, the significance was only observed in males, especially among current smokers. Conclusion High serum folate level was positively associated with lung function measurements in male COPD patients who are current smokers. Further longitudinal studies are needed to elucidate underlying mechanisms.

Background: Ferritin is a multi-functional protein. In chronic inflammatory state, serum ferritin level is elevated. Cigarette smoke causes chronic inflammation and elevates serum ferritin concentration. The present study is aimed to evaluate the association between serum ferritin level and pulmonary function test (PFT) in former smokers using nationally representative data from 2010 to 2012. Methods: Among 25,534 populations, eligible 1,556 subjects aged over 40 without current infection, tuberculosis, cancer, or hyperferritinemia were selected. Association between serum ferritin level and pulmonary function parameters was evaluated by complex sample general linear regression analysis. Multivariate logistic regression model was used to estimate the odds ratio (OR) for obstructive lung disease and restrictive lung disease according to serum ferritin concentration. Results: In general linear regression model adjusted after age, height, smoking amount, non-smoking period, daily iron intake, TIBC, serum iron, residence area, and education, serum ferritin concentration was negatively associated with predicted forced expiratory volume in one second (FEV1) and forced vital capacity (FVC) in male former smokers (P-value 0.036 and P-value 0.007, respectively). In logistic regression analysis, OR in high serum ferritin group (>300 ug/L) were 1.325 (95% CI: 0.747-2.348) for FEV1/FVC <0.7 and 1.438 (0.826-2.504) for FEV1/FEV6 <0.75, but there were no statistically significant differences (P-value 0.335 and P-value 0.199, respectively). Conclusions: High serum ferritin is significantly associated with decreased FEV1 and FVC in male former smokers. Further studies are needed to clarify the prognostic effect of serum ferritin on chron ic obstructive lung disease or restrictive lung disease.
Lemierre syndrome with thrombosis of sigmoid sinus following dental extraction: a case report

Lemierre syndrome is caused by an infection in the oropharyngeal region with subsequent thrombophlebitis in the internal jugular vein. The thrombus from the thrombophlebitis can invade other vital organs, such as liver, lungs, or joints, resulting in secondary infection, which further exacerbates the fatal prognosis of this syndrome. Lemierre syndrome, also called postanginal sepsis or necrobacillosis, was first reported by Dr. Lemierre in 1936. In his report, Lemierre mentioned that out of 20 patients who suffered from this syndrome, only two survived. He also stated that all of the 20 patients complained of infections in the palatine tonsils and developed sepsis and thrombophlebitis in the internal jugular vein. Once called a “forgotten disease,” this syndrome showed a very high mortality rate until usage of antibiotics became prevalent. In this case report, the authors present a 71-year- old female patient who suffered from Lemierre syndrome with thrombosis extended to the right sigmoid sinus.
