        요추부 추간판탈출증에서 경피적 자동수핵제거술 치료의 예후 인자

        Percutaneous automated lumbar discectomy as it is now practiced appears to be an extremely safe procedure. One hundred fourty eight patients with lumbar disc herniation were treated with automated percutaneous lumbar discectomy(A.PL.D.). The postoperative results in 118 patients were evaluated at more than 3 months after the operation. Total satisfactory result was 74% excellent 16%, good 58%, fair 22%, poor 4% by Macnab' s criteria. For further satisfactory result, more meticulous crteria is needed. The results indicated that several prognostic factors were considered : 1) less than 30 years of age, 2) more than 4ml of aspirated disc volumn, 3) L4-5 of disc level, 4) follow up X-ray change(59%), but no CT change and possible MRI change.

      • Gene profile of fibroblasts identify relation of CCL8 with idiopathic pulmonary fibrosis

        <P><B>Background</B></P><P>Idiopathic pulmonary fibrosis (IPF) is characterized by the complex interaction of cells involved in chronic inflammation and fibrosis. Global gene expression of a homogenous cell population will identify novel candidate genes.</P><P><B>Methods</B></P><P>Gene expression of fibroblasts derived from lung tissues (8 IPF and 4 controls) was profiled, and ontology and functional pathway were analyzed in the genes exhibiting >2 absolute fold changes with <I>p</I>-values < 0.05. <I>CCL8</I> mRNA and protein levels were quantified using real-time PCR and ELISA. <I>CCL8</I> localization was evaluated by immunofluorescence staining.</P><P><B>Results</B></P><P>One hundred seventy eight genes differentially expressed and 15 genes exhibited >10-fold change. Among them, 13 were novel in relation with IPF. <I>CCL8</I> expression was 22.8-fold higher in IPF fibroblasts. The levels of <I>CCL8</I> mRNA and protein were 3 and 9-fold higher in 14 IPF fibroblasts than those in 10 control fibroblasts by real-time PCR and ELISA (<I>p</I> = 0.022 and <I>p</I> = 0.026, respectively). The <I>CCL8</I> concentrations in BAL fluid was significantly higher in 86 patients with IPF than those in 41 controls, and other interstitial lung diseases including non-specific interstitial pneumonia (<I>n</I> = 22), hypersensitivity pneumonitis (<I>n</I> = 20) and sarcoidosis (<I>n</I> = 19) (<I>p</I> < 0.005, respectively). Cut-off values of 2.29 pg/mL and 0.43 pg/mL possessed 80.2 and 70.7% accuracy for the discrimination of IPF from NC and the other lung diseases, respectively. IPF subjects with <I>CCL8</I> levels >28.61 pg/mL showed shorter survival compared to those with lower levels (<I>p</I> = 0.012). <I>CCL8</I> was expressed by α-SMA-positive cells in the interstitium of IPF.</P><P><B>Conclusions</B></P><P>Transcriptome analysis identified several novel IPF-related genes.         In this study, the experimental study has carried out to select pitch angle on the backward flow in an axial fan that has adjustable pitch blades. With the change of pitch angle of axial fan with adjustable blade, air flow rate, pressure and air flow direction can be changed. Because of this merit, adjustable axial fan can be used in the backward flow. For the selection of the backward flow pitch angle, fan performance test method is selected by KS B 6311. Dynamic pressure, static pressure, electric current and voltage are measured in each pitch angles of axial fan that are 36°, -16°, -21°, -26°, -31° and -36°. In the result of test, fan performance curves at several pitch angle has been investigated. Finally, pitch angle of -26° has been selected to get largest flow rate at backward flow situation.

        Zero-energy construction is being emphasized due to environmental pollution. However, in the case of external insulation using organic insulation with good insulation performance, there are many studies on inorganic insulation as it shows limitations on fire stability. In addition, as the demand for stone for exterior walls increases, Cellular Light -weight Concrete(CLC) with polymer is used to supplement fire stability and insulation performance, and the construction of stone is complemented by combining organic insulation, inorganic insulation, and stone. In this study, the compressive strength and adhesion in tension of CLC are studied. As a result of the experiment, the compressive strength of 28 days according to the polymer addition rate did not change. The adhesion in tension according to the polymer addition rate tends to increase as the addition rate increases. The target adhesion in tension is 0.8 MPa, but the maximum value of the experiment did not reach the target value, and further research was needed to combine to maintain the density and improve the adhesion in tension.

        만성 신부전증 환자의 중증 고혈당에 따른 의식변화 결핍에 대한 연구

        In diabetic patients who were being treated with hemodialysis, coma and other neurological deficits did not occur in spite of extremely elevated serum glucose levels. In this report, we compared diabetic patients with renal failure in severe hyperglycemia with nonketotic hyperosmolar coma patients with normal renal function to know what affect mental changes. Mental changes were not present in diabetic patients with renal failure. These patients with renal failure showed more severe hyperglycemia, but corrected serum sodium concentration and calculated effective serum osmolality were low. So it is suggested that corrected serum sodium concentration and effective serum osmolality are more important factor affecting mental changes than high blood glucose levels in diabetic patients with renal failure, due to absence of osmotic diuresis.

      • Electro-hyperthermia up-regulates tumour suppressor Septin 4 to induce apoptotic cell death in hepatocellular carcinoma

        <P>Purpose: Modulated electro-hyperthermia (mEHT) has been shown to be effective against various types of human tumours, including hepatocellular carcinoma (HCC). Here we aimed to investigate the molecular mechanism underlying the cytotoxic effects of mEHT to HCC cells. Materials and methods: Human liver cancer cell lines, Huh7 and HepG2, were treated with mEHT (42 degrees C/60 min) three times at 2-day intervals. Growth inhibition and apoptotic induction were evaluated using MTS, microscopic analysis, a clonogenic assay, annexin V/PI staining and a ccK18 ELISA. Global changes in gene expression were examined using RNA sequencing to obtain insights into molecular changes in response to mEHT. For in vivo evaluation of mEHT we used HepG2 HCC xenografts grown in nude mice. Results: mEHT suppressed HCC cell proliferation and long-term colony formation through induction of apoptosis. The growth inhibitory effects are induced through a subset of molecular changes. Notably the expression level of septin 4 (SEPT4) (involved in pro-apoptotic activity and growth suppression) was up-regulated, whereas a key regulator of invasiveness G-Protein coupled receptor 64 (GPR64) was repressed. Subsequent Western blotting confirmed that the common increase in tumour suppressor SEPT4 in both Huh7 and HepG2 cells is accompanied by the restoration of cyclin-dependent kinase (CDK) inhibitor p21 and decrease in pro-caspase 7 and pro-caspase 3, thereby accelerating apoptotic signalling in HCC cells. Additionally, mEHT significantly inhibited the growth of human HCC xenografts in nude mice. Conclusions: These findings suggest that apoptotic cell death induced by mEHT is mediated by the up-regulation of tumour suppressor SEPT4 in human HCC cells.</P>

      • Control of Reversible Self-Bending Behavior in Responsive Janus Microstrips

        <P>Here, we demonstrate a simple method to systematically control the responsive self-bending behavior of Janus hydrogel microstrips consisting of a polymeric bilayer with a high modulus contrast. The Janus hydrogel microstrips could be easily fabricated by a simple micromolding technique combined with an initiated chemical vapor deposition (iCVD) coating, providing high flexibility in controlling the physical and chemical properties of the microstrips. The fabricated Janus hydrogel microstrip is composed of a soft, pH-responsive polymer hydrogel layer laminated with a highly cross-linked, rigid thin film, generating a geometric anisotropy at a micron scale. The large difference in the elastic moduli between the two layers of the Janus microstrips leads to a self-bending behavior in response to the pH change. More specifically, the impact of the physical and chemical properties of the microstrip on the self-bending phenomena was systematically investigated by changing the thickness and composition of two layers of the microstrip, which renders high controllability in bending of the microstrips. The curvature of the Janus microstrips, formed by self-bending, highly depends on the applied acidity. A reversible, responsive self-bending/ unbending exhibits a perfect resilience pattern with repeated changes in pH for 5 cycles. We envision that the Janus microstrips can be engineered to form complex 3D microstructures applicable to various fields such as soft robotics, scaffolds, and drug delivery. The reliable responsive behaviors obtained from the systematic investigation will provide critical information in bridging the gap between the theoretical mechanical analysis and the chemical properties to achieve micron-scale soft robotics.</P>

        This study is a late study of “An Exploratory Study on STI policy impact assessment”. In the research of 2015, in principle, it is necessary to evaluate not only R&D investment but also general policies such as various promotion systems, standards and regulations by putting the term “policy impact assessment” in front of it We emphasized the points. However, it is practically very difficultto collect and analyze detailed data of all STI policies. In addition, it is also restricted to allow policy researchers to access data within a certain range. Therefore, this study focuses on the impact assessment of R&D investment, which accounts for the largest percentage of STI policies. In this research, the impact assessment system of R&D investment is analyzed by classifying it into evaluation of economic impact and social impact assessment. Also, the evaluation of economic impact was analyzed separating into a micro viewpoint and a macro viewpoint. First, The analysis of the economic impact assessment of the micro viewpoint relates to how the government’s R&D investment affects enterprises. Specifically, the main analysis target was the employment and investment of companies, the impact on sales and profits. Second, The Analysis for macroeconomic impact assessment on the economy is related to the influence of government’s R&D investment such as national economic growth, employment etc. We also analyzed the spillover effect by industry according to R&D investment. Third, The social impact assessment is to analyze how R&D investment has impacted technology-based social change. This study is a very difficult attempt. However, we analyzed from the point of view of the consumer considering all social change areas as much as possible. The results of the study are summarized as follows. First, the study of economic impact evaluation shows the effect of R&D investment and it can be used as a basis to improve the efficiency and goal of R&D investment in future sectors such as employment, economic growth and investment. Second, through research on social impact assessment, government R&D investment has focused on specific areas in terms of improving the quality of life and social development of people, and confirmed that most social sectors are neglected. In addition, we proposed policy priorities for specific social change indicators and social sectors and proposed long-term R&D investment strategies. Finally, this study is expected to contribute to the establishment of future strategy of private enterprise through the measurement of social change index, the social influence of private research and development, and the influence of technology shock from abroad.

      • 만성 췌장염에서 Foipan^R 투여시 효과와 안정성

        To evaluate the clinical effect and safety of long-term administration of Camostat Mesilat (FOIPAN^R) in chronic pancreatitis, we had given Foipan 200㎎ 샹 orally to twenty patients with chronic pancreatitis for 4 weeks. Foipan improved the subjective symptoms and objective signs of patients with chronic panceatitis without signficant hematologic and biochemical changes. There were no serious side effects excent only mild thirst and Constipation in two cases. In conclusion, Fiopan^R seems to bo a fairly safe and effective drug for relief of accompanying symptoms in chronic pancreatitis.
