        Traditional Unani Medicine in Flu-like Epidemics and COVID-19 during Pregnancy: A Literary Research

        Background: The pandemic COVID-19 caused by a novel coronavirus SARS-COV-2 has spread like a forest fire. This disease may have serious consequences for pregnant women. Presently, no specific drugs or vaccines exist to battle this disease and researches are underway. Unani medicine has a unique role in prevention and management during epidemics. Here, we reviewed the overview of COVID-19 infection and pregnancy, concept and practices in Unani medicine for flu-like epidemics in general and pregnancy, and safety of Unani drugs for the prevention and treatment of mild symptomatic cases of COVID-19 during pregnancy. Methodology: Unani classical texts and pharmacopoeia were meticulously explored for concepts and practices for flu-like epidemic diseases. Further, we browsed scientific databases such as PubMed, Scopus and others for an overview, epidemics and Unani medicine, effectiveness and safety of Unani drugs in COVID-19 and pregnancy. Results: Unani medicine includes prevention and management of flu-like epidemic include quarantine and isolation, aromatic herbal drugs fumigation and spraying for environmental disinfection, ilaj bid tadbir for health promotion and use of health-protecting drugs and symptom-specific drugs in general and related to pregnancy. Lahsun, asalussus, behidana, banafsha, zanjabeel, unnab, etc are in use since antiquity for the prevention and treatment of asymptomatic and mildly symptomatic pregnant women during infectious and epidemic diseases. Conclusion: Currently, the aforementioned plants are proven for antiviral, antioxidant, immunomodulatory and anti-inflammatory activities, probably useful in the COVID-19 pandemic. Additionally, scientific studies have provided new insight into the mechanism underlying the therapeutic effect of Unani medicines that are safe in pregnancy.

        Orosomucoid Serum Concentrations and Fat Depot-Specific mRNA and Protein Expression in Humans

        Obesity is associated with chronic low-grade inflamma-tion, which contributes to systemic metabolic irregularities and obesity-linked metabolic disorders. Orosomucoid (ORM), an acute phase reactant protein, was shown to be produced in response to metabolic and inflammatory signals in the adipose tissue of obese mice, which protects them from severe inflammation and subsequent metabolic dysfunction. In this study, we examined whether there are site-specific differences between visceral and subcutaneous adipose tissue (VAT and SAT, respectively) ORM gene and protein expression from individuals with a wide range of obesity and the relationship between expressed and circulating ORM levels and measures of adiposity, insulin resistance, and pro- and anti-inflammatory markers and adipokines. The level of circulating ORM correlated positively with BMI, body fat mass, and serum leptin. It also correlated with fasting insulin, HOMA-IR values and C-reactive protein in men. There were no site-specific differences in ORM mRNA and protein expression between VAT and SAT, nor did we find a relationship between circulating ORM levels and its mRNA expression in either fat depot. We found that ORM mRNA expression correlated with mRNA expression of TNF-, IL-6, and adiponectin in VAT, and with TNF- and adiponectin in SAT. These observations are the first description linking adipose tissue ORM and pro- and anti-inflammatory molecules in humans. The close links of ORM and measures of adiposity, insulin resistance, and adipose tissue inflammation in humans reinforce previous experimental data and warrant further studies to explore a possible role of ORM in the patho-genesis of obesity-associated metabolic derangements.

        Hydrothermal carbonization of oil palm trunk via taguchi method

        Hydrothermal carbonization (HTC) and its parameters show a significant role in the quality of HTC products and the distribution of yield. The present study investigates the optimal conditions that are suitable to produce maximum yield products of solid, liquid, and gas, from HTC of oil palm trunk (OPT), by following the Taguchi method. Moreover, all the three products of HTC were analyzed using various characterizations. The optimum runs for hydrochar yield, liquid yield, and gaseous yield were run 1 (R1), run 4 (R4), and run 9 (R9), respectively. The reaction temperature was found to be the most influential parameter that affected the yield distribution during HTC, where low temperature supported solid production, intermediate temperatures favored liquid yield, and high temperature produced higher gaseous yield. Elemental analysis, H/C and O/C atomic ratios, higher heating value (HHV), and energy density values of hydrochar recommended that the HTC process has significantly converted OPT into better energy fuel. The energy densification value of hydrochar ranged between 1.28 and 1.40, which confirmed the significance of the HTC process. Two characteristic peaks from FTIR were observed at 3,430 cm1 and 2,923 cm1 hydrochar. SEM analysis confirmed that the porosity of hydrochar was higher than OPT after HTC. However, the major organic matter in the bio-oil traced by GC-MS analysis was acetic acid, accounting for about 59.9-71.7%, and the outlet gaseous product consisted of 0.87-9.17% CH4, 3.88-29.02% CO2, 1.07-7.89% CO, and 0.31-1.97% H2, respectively, as shown by GC-TCD.

        Differential Patterns of Serum Concentration and Adipose Tissue Expression of Chemerin in Obesity: Adipose Depot Specificity and Gender Dimorphism

        Chemerin, a recognized chemoattractant, is expressed in adipose tissue and plays a role in adipocytes differentia-tion and metabolism. Gender- and adipose tissue-specific differences in human chemerin expression have not been well characterized. Therefore, these differences were assessed in the present study. The body mass index (BMI) and the circulating levels of chemerin and other inflammatory, adiposity and insulin resistance markers were assessed in female and male adults of varying degree of obesity. Chemerin mRNA expression was also measured in paired subcutaneous and visceral adipose tissue samples obtained from a subset of the study subjects. Serum chemerin concentrations correlated positively with BMI and serum leptin levels and negatively with high density lipoprotein (HDL)-cholesterol levels. No correlation was found between serum chemerin concentrations and fasting glucose, total cholesterol, low density lipoprotein (LDL)-cholesterol, triglycerides, insulin, C-reactive protein or adiponectin. Similarly, no relation was observed with the homeostasis model assessment for insulin resistance (HOMA-IR) values. Gender- and adipose tissue-specific differences were observed in chemerin mRNA expression levels, with expression significantly higher in women than men and in subcutaneous than visceral adipose tissue. Interestingly, we found a significant negative correlation between circulating chemerin levels and chemerin mRNA expression in subcutaneous fat. Among the subjects studied, circulating chemerin levels were associated with obesity markers but not with markers of insulin resistance. At the tissue level, fat depot-specific differential regulation of chemerin mRNA expression might contribute to the distinctive roles of subcutaneous vs. visceral adipose tissue in human obesity.

        A Randomized Trial Comparing the Effect of Unani Formulation with Metronidazole in Bacterial Vaginosis

        Background and objectives: Bacterial vaginosis (BV) is recognized as the most prevalent type of vaginal infection, impacting approximately 19-24% of women in their reproductive years. The recurrence rate of BV is significant, negatively impacting the well-being of affected women. This study aimed to compare the therapeutic effects of a polyherbal Unani formulation and metronidazole in treating bacterial vaginosis. Methodology: In this prospective patient blinded standard controlled trial, a total of 40 individuals with a clinical diagnosis of bacterial vaginosis were randomly assigned to receive either an active control treatment (n = 20) or a test drug (n = 20). In the test drug combination of Acacia catechu, Azadirachta indica and Quercus infectoria in tablet (1g) form in the dose of 2 tablets orally twice daily with water was administered for 3 weeks. In the active control standard drug, metronidazole 400 mg tablet, orally twice daily was given for one week. The primary outcome measure was clinical cure; H. negative Amsel’s criteria and a reduction in subjective symptoms, while the secondary outcome measure was an improvement in SF-36 quality of life (QOL). Results and conclusion: Both the experimental treatment and the metronidazole demonstrated a significant clinical cure for bacterial vaginosis as well as an increase in health-related quality of life. Based on these findings, it appears that the test medication is a potent Unani formulation for the treatment of bacterial vaginosis. A well conducted trial with a bigger sample size is required to corroborate these findings.

      • Binding Pattern Elucidation of NNK and NNAL Cigarette Smoke Carcinogens with NER Pathway Enzymes: an Onco-Informatics Study

        Cigarette smoke derivatives like NNK (4-(Methylnitrosamino)-1-(3-pyridyl)-1-butanone) and NNAL (4-(methylnitrosamino)-1-(3-pyridyl)-1-butan-1-ol) are well-known carcinogens. We analyzed the interaction of enzymes involved in the NER (nucleotide excision repair) pathway with ligands (NNK and NNAL). Binding was characterized for the enzymes sharing equivalent or better interaction as compared to +Ve control. The highest obtained docking energy between NNK and enzymes RAD23A, CCNH, CDK7, and CETN2 were -7.13 kcal/mol, -7.27 kcal/mol, -8.05 kcal/mol and -7.58 kcal/mol respectively. Similarly the highest obtained docking energy between NNAL and enzymes RAD23A, CCNH, CDK7, and CETN2 were -7.46 kcal/mol, -7.94 kcal/mol, -7.83 kcal/mol and -7.67 kcal/mol respectively. In order to find out the effect of NNK and NNAL on enzymes involved in the NER pathway applying protein-protein interaction and protein-complex (i.e. enzymes docked with NNK/NNAL) interaction analysis. It was found that carcinogens are well capable to reduce the normal functioning of genes like RAD23A (HR23A), CCNH, CDK7 and CETN2. In silico analysis indicated loss of functions of these genes and their corresponding enzymes, which possibly might be a cause for alteration of DNA repair pathways leading to damage buildup and finally contributing to cancer formation.

        Biogenic fabrication and characterization of silver nanoparticles using aqueous-ethanolic extract of lichen (Usnea longissima) and their antimicrobial activity

        Background: Biogenic fabrication of silver nanoparticles from naturally occurring biomaterials provides an alternative, eco-friendly and cost-effective means of obtaining nanoparticles. It is a favourite pursuit of all scientists and has gained popularity because it prevents the environment from pollution. Our main objective to take up this project is to fabricate silver nanoparticles from lichen, Usnea longissima and explore their properties. In the present study, we report a benign method of biosynthesis of silver nanoparticles from aqueous-ethanolic extract of Usnea longissima and their characterization by ultraviolet–visible (UV-vis), Fourier transform infrared (FTIR) spectroscopy, transmission electron microscopy (TEM) and scanning electron microscopy (SEM) analyses. Silver nanoparticles thus obtained were tested for antimicrobial activity against gram positive bacteria and gram negative bacteria. Results: Formation of silver nanoparticles was confirmed by the appearance of an absorption band at 400 nm in the UV-vis spectrum of the colloidal solution containing both the nanoparticles and U. longissima extract. Poly(ethylene glycol) coated silver nanoparticles showed additional absorption peaks at 424 and 450 nm. FTIR spectrum showed the involvement of amines, usnic acids, phenols, aldehydes and ketones in the reduction of silver ions to silver nanoparticles. Morphological studies showed three types of nanoparticles with an abundance of spherical shaped silver nanoparticles of 9.40–11.23 nm. Their average hydrodynamic diameter is 437.1 nm. Results of in vitro antibacterial activity of silver nanoparticles against Staphylococcus aureus, Streptococcus mutans, Streptococcus pyrogenes, Streptococcus viridans, Corynebacterium xerosis, Corynebacterium diphtheriae (gram positive bacteria) and Escherichia coli, Klebsiella pneuomoniae and Pseudomonas aeruginosa (gram negative bacteria) showed that it was effective against tested bacterial strains. However, S. mutans, C. diphtheriae and P. aeruginosa were resistant to silver nanoparticles. Conclusion: Lichens are rarely exploited for the fabrication of silver nanoparticles. In the present work the lichen acts as reducing as well as capping agent. They can therefore, be used to synthesize metal nanoparticles and their size may be controlled by monitoring the concentration of extract and metal ions. Since they are antibacterial they may be used for the treatment of bacterial infections in man and animal. They can also be used in purification of water, in soaps and medicine. Their sustained release may be achieved by coating them with a suitable polymer. Silver nanoparticles fabricated from edible U. longissima are free from toxic chemicals and therefore they can be safely used in medicine and medical devices. These silver nanoparticles were stable for weeks therefore they can be stored for longer duration of time without decomposition.
