      • 기능성 Catechol-O-Methyltransferase 유전자 다형성과 한국인 정신분열병의 관련성

        연구목적 : Catechol-O-Methyltransferase(COMT)는 카테콜아민 신경전달물질의 불활성화를 담당하는 효소로서, 정신분열병과 관련된 후보 유전자로서 알려져 왔다. 최근, 기능성 COMT유전자 다형성과 정신분열병의 관련성이 제기되었는데, 한국인 정신분열병에서도 이러한 관련성이 있는지를 살피기 위해 본 연구를 시행하였다. 방 법 : 정신분열병으로 진단된 환자군 103명과 성별 및 연령별로 1:1 짝짓기 대조군 사이에 유전자형과 대립유전자의 분포를 비교하였다. 또한, 환자군은 공격적 행동, 가족력, 아동기 발병 여부에 따라 두 군으로 나누어 비교하였다. DNA의 다형성 부의를 중합효소연쇄반응에 의해 증폭한 후 ,NlaⅢ제한효소법과 전기영동을 실시하여 유전자형을 판별하였다. 결 과 : 환자군 전체와 대조군 사이에 유전자형과 대립유전자 빈도의 차이가 없었다. 가족력이 있는 환자군을 대조군과 비교하였을 때는, 대립유전자 L을 포함하는 유전자형과 그렇지 않은 유전자형의 분포에 있어 유의한 차이를 보였다(p=0.02, OR=3.9, 95% Cl=1.1∼14.3). 결 론 : 한국인 정신분열병과 기능성 COMT 유전자 다형성 사이에 유의한 관련성이 없었다. 따라서, 기능성 COMT 유전자 다형성이 한국인 정신분열병의 발병에 영향을 주지 못한다. 그러나, 정신분열병 가족력과 기능성 COMT유전자 다형성의 관련성이 시시돠어, 향후 본 연구에서 나타난 방법론적인 문제점을 보완하고 추가 대상자를 확보한 연구가 필요하다. Objectives : Catechol-O-methyltransferase(COMT) is involved in the degradation of catecholamine meurotransmitters and has been investigated as a candidate gene in schizophrenia. Recently, possible relationship between functional COMT gene polymorphism and schizophrenia has been suggested. To address the possible role of functional COMT gene polymorphism in the predisposition to schizophrenia, we carried out an association study in Korean schizophrenic patients and controls. Methods : One hundred and three Korean inpatients diagnosed as schizophrenia and 103 age and sex matched controls were selected as study subjects. Patients were subgrouped into two groups on the basis of history of aggressive behavior, family history of schizophrenia and related disorders, and age at onset. We determined COMT genotypes using PCR of the relevant region followed by digestion with NlaⅢ and electrophoresis. Results : No significant differences of allele and genotype frequencies were noted between patients and controls. However, when patients were categorized by the presence of family history of schizophrenia and related disorders, patients with family history showed almost 4 fold higher frequency of having COMT L allele containing genotype compared to controls(p=0.02, OR=3.9, 9.5% CI=1.10-14.33). Conclusion : Although our results do not support an association between functional polymorphism of COMT gene and schizophrenia overall, the findings suggest an association between functional COMT gene polymorphism and familial schizophrenia. Further studies with large samples are needed to confirm this association.

      • 인지도 조사에 의한 건국의대 의학과 교육목적 및 교육목표의 평가

        This study was conducted to survey the recognition and accuracy for educational goal and objectives of College of Medicine, Konkuk University on students and professors with using 18 items of questionnaire from May 15 through June 30, 2004. The recognition for educational goal of College of Medicine, Konkuk University in participating students and professors were 93-95%. And recognition for 3 objectives of College of Medicine, Konkuk University on participating students and professors were 84-87%, 95% respectively. However the accuracy for detail educational objectives using other college of medicine were 46-59% in first student survey, 44-67% in second student survey and 58-72% in professor survey. Therefore it is need to advertize 3 objectives to students and professors. And it is also necessary to refine the contents of educational objectives to more suggesting directions than broad goal of ideal.

        연 폭로가 남성호르몬에 미치는 영향

        목 적 : 본 연구는 직업성 연 폭로근로자에서 연폭로가 남성 생식기계 독성중 성호르몬에 미치는 영향과 그 정도를 평가하고자 시도되었다.방 법 1998년 하반기에 실시한 일반, 특수건강진단자중 경기도 지역 직업성 연 폭로 근로자 33명과 과거 직업력에서 연 폭로가 없었던 사무직 근로자 33명을 연구대상으로 선정하였다. 연의 생물학적지표로는 혈색소, ZPP, 혈중 연, 및 요중 연을 이용하였으며, 성호르몬으로는 LH, FSH, testos-terone, SHBG 및 FTI를 이용하였다. 연 폭로 지표와 성호르몬과의 상관관계를 조사하였다. 결 과 : 연의 생물학적 지표중 혈색소는 폭로군에서 대조군보다 유의하게 낮았으며, ZPP, 혈중 연및 요중 연등은 폭로군에서 대조군보다 유의하게 높았다. 성호르몬에서 FSH, testosterone, SHBG및 FTI는 모두 폭로군에서 대조군보다 낮았으나 통계적 유의성은 없었다. 폭로군의 성호르몬중에서 연령은 FSH 및 FTI, ZPP는 FSH, 혈중 연은testosterone과 유의한 상관관계가 있었다. 대조군에서 연령은 FSH, testosterone, SHBG 및 FTI와 흡연량은 FSH와 유의한 상관관계가 있었다. 연령군별 폭로군과 대조군의 성호르몬 비교에서 30-39세군의 FTI에서 폭로군이 대조군보다 유의하게 낮았으며 그 밖의 호르몬에서는 모두 유의한 차이가 없었다. 결 론 : 연 폭로군의 FSH, Testosterone, SHBG 및 FTI가 대조군에 비교하여 낮았으나 통계적 유의성은 없었다. 그러나 폭로군에서 연의 생물학적 지표와 일부 성호르몬간에 유의한 상관관계를보였으며 이는 본 연구 대상 근로자의 연 폭로 수준이 일부 성호르몬에 영향이 있음을 보여주는 것으로 판단된다. 향후 이에 대한 지속적인 연구가 진행되어 야 할 것이다. Objectives : This study was performed to evaluate the relationship between biological markers of lead exposure and level of sex. hormones in men occupationally exposed to lead. Methods : In this study, 33 male workers who employed at smelting and battery factories were compared with 33 male workers who were not exposed to lead. As biological markers of lead exposure, hemoglobin, ZPP, blood lead and urine lead were measured. As sex hormones, FSH, LH, testosterone, SHBG and FTI were determined. Results : Hemoglobin level was significantly lower in exposed than in nonexposed group. Level of ZPP, blood lead, and urine lead were significantly higher in exposed than in nonexposed group. There was a trend that level of FSH, testosterone, SHBG, and FTI was lower in exposed than in nonexposed group respectively, but there were no statistical significance. In exposed group, FSH level was correlated with age, ZPP, and blood lead; testosterone level was correlated with hemoglobin, blood lead, urine lead, and SHBG ; and FTI level was correlated with age. Conclusions : Level of some sex hormones were lower in exposed than in nonexposed group but there were no statistical significance between the exposed and the nonexposed group. But level of sex hormones were correlated with several biological markers of lead in exposed group. It suggests that occupational lead exposure might affect sex hormones.

        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

      • Compact and multiplexible hydrogen gas sensor assisted by self-referencing technique.

        <P>We have experimentally implemented a multiplexible but compact fiber sensor system suitable for multipoint sensing of hydrogen gas leakage. By making dual cavities along an optical fiber and coating a palladium film only at the end of the fiber tip, the measurement errors induced by the optical source power fluctuation and the mechanical perturbation in the lead fiber could be compensated. By adjusting the length of the dual-cavity, the capability of multiplexing several hydrogen sensors could be achieved. The experiment results showed that the response speed of the sensor was increasing with temperature, but at a low temperature the response amplitude became large.</P>

        Characteristics of Fuel Pellets Made by Mixing Paper Sludge and Forest Byproducts

        In this study, for reusing the sludge generated in a fine paper mill as fuel, the characteristics of the pellets prepared by mixing auxiliary raw materials such as pulp mill chip rejects, ca- mellia oilseed cake, Sancho pruning remnants, and mulberry pruning remnants were evalu- ated. The pellet yield decreased as more sludge containing about 50% of the inorganic fillers was mixed with each auxiliary material, but the camellia oilseed cake, unlike the others, was of great help in improving the pellet yield. The Van Krevelen diagram showed that, as more sludge was added to the woody biomass in preparing the pellets, they tended to have a simi- lar elemental composition to that of coal or lignite. The sludge addition to the woody biomass had a negative effect on the calorific value of the pellets, but the lower calorific value of 2400-3000 kcal/kg was acquired even when more than 50% of sludge was added. The sludge pellets containing the camellia oilseed cake had a faster ignition point than those mixed with the other woody biomass, but the flame duration was shorter. This study showed that the sludge from fine paper mills containing about 50% of cellulose fibers could be used to pro- duce economical solid fuels if mixed with other woody biomass in an appropriate ratio.

        Development of a minipig physical phantom from CT data

        <P><B>ABSTRACT</B></P><P>Quantification of pathological progression of radiation-induced injury is essential in development of treatment methods, and a proper animal model is necessary for relevant radiological and medical studies. A minipig is a current animal model selected because of its similarities to humans in anatomy and pathology. In the present study, a minipig physical phantom was developed using computed tomography (CT) data. For dosimetry purposes, the minipig physical phantom was constructed on a slice-by-slice basis, with an array of holes to accommodate dosimeters. The phantom is constituted of three major organs, i.e. bone, lung, and remaining soft tissue, and the organs are clearly distinguishable on each 20-mm-thick axial slice. The quality of the tissue-equivalent (TE) substitutes was analyzed in terms of the atomic compositions and Hounsfield units (HUs). The density (in g/cm<SUP>3</SUP>) and effective atomic number of TE substitutes for the bone, lung, and soft tissue are 1.4 and 7.9, 0.5 and 10.0, and 1.0 and 5.9, respectively. Although the TE substitutes have slightly different physical properties, we think the phantom is acceptable because the HU values of the TE substitutes lie in the HU range of real tissues.</P>

        The acute effects of hydrocortisone on cardiac electrocardiography, action potentials, intracellular calcium, and contraction: The role of protein kinase C

        <P><B>Abstract</B></P> <P>Hydrocortisone exerts adverse effects on various organs, including the heart. This study investigated the still unclear effects of hydrocortisone on electrophysiological and biochemical aspects of cardiac excitation–contraction coupling. In guinea pigs’ hearts, hydrocortisone administration reduced the QT interval of ECG and the action potential duration (APD). In guinea pig ventricular myocytes, hydrocortisone reduced contraction and Ca<SUP>2+</SUP> transient amplitudes. These reductions and the effects on APD were prevented by pretreatment with the protein kinase C (PKC) inhibitor staurosporine. In an overexpression system of <I>Xenopus</I> oocytes, hydrocortisone increased hERG K<SUP>+</SUP> currents and reduced Kv1.5 K<SUP>+</SUP> currents; these effects were negated by pretreatment with staurosporine. Western blot analysis revealed dose- and time-dependent changes in PKCα/βII, PKCε, and PKCγ phosphorylation by hydrocortisone in guinea pig ventricular myocytes. Therefore, hydrocortisone can acutely affect cardiac excitation–contraction coupling, including ion channel activity, APD, ECG, Ca<SUP>2+</SUP> transients, and contraction, possibly via biochemical changes in PKC.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Hydrocortisone decreased QT interval and action potential duration in cardiomyocytes. </LI> <LI> Hydrocortisone reduced amplitude of contraction and Ca<SUP>2+</SUP> transient in cardiomyocytes. </LI> <LI> Hydrocortisone increased hERG K<SUP>+</SUP> current and decreased Kv1.5 K<SUP>+</SUP> current. </LI> <LI> PKC inhibitor prevented the effects of hydrocortisone on cardiac electrophysiology. </LI> <LI> Hydrocortisone activated PKCα/βII and PKCε in cardiomyocytes. </LI> </UL> </P>

        Is endoscopic ultrasonography essential for endoscopic resection of small rectal neuroendocrine tumors?

        <P><B>AIM</B></P><P>To evaluate the importance of endoscopic ultrasonography (EUS) for small (≤ 10 mm) rectal neuroendocrine tumor (NET) treatment.</P><P><B>METHODS</B></P><P>Patients in whom rectal NETs were diagnosed by endoscopic resection (ER) at the Pusan National University Yangsan Hospital between 2008 and 2014 were included in this study. A total of 120 small rectal NETs in 118 patients were included in this study. Histologic features and clinical outcomes were analyzed, and the findings of endoscopy, EUS and histology were compared.</P><P><B>RESULTS</B></P><P>The size measured by endoscopy was not significantly different from that measured by EUS and histology (<I>r</I> = 0.914 and <I>r</I> = 0.727 respectively). Accuracy for the depth of invasion was 92.5% with EUS. No patients showed invasion of the muscularis propria or metastasis to the regional lymph nodes. All rectal NETs were classified as grade 1 and demonstrated an L-cell phenotype. Mean follow-up duration was 407.54 ± 374.16 d. No patients had local or distant metastasis during the follow-up periods.</P><P><B>CONCLUSION</B></P><P>EUS is not essential for ER in the patient with small rectal NETs because of the prominent morphology and benign behavior.</P>

        Port-site metastasis after laparoscopic radical pancreatosplenectomy in left-sided pancreatic cancer

        Despite debates regarding the safety of well-selected left-sided pancreatic cancer, minimally invasive distal pancreatosplenectomy is considered safer and more effective than open distal pancreatosplenectomy in well-selected patients. Previous studies have shown that minimally invasive surgery yields comparable oncologic outcomes to open surgery. While patients who undergo minimally invasive distal pancreatosplenectomy also experience recurrences and metastases after surgery, port-site metastasis is particularly rare. In this report, we report an extremely rare case of port-site metastasis following minimally invasive distal pancreatosplenectomy for left-sided pancreatic cancer.
