Surgical Algorithm for Obstructive Sleep Apnea: An Update

Stanley Yung Chuan Liu,Robert Wayne Riley,유명상 대한이비인후과학회 2020 Clinical and Experimental Otorhinolaryngology Vol.13 No.3

Sleep surgery is part of a continuum of care for obstructive sleep apnea (OSA) that involves medical, pharmacologic, and behavioral therapy. Upper airway surgery for OSA can significantly improve stability by way of modulating the critical negative closing pressure. This is the same mechanism of action as positive airway pressure or oral appliance therapy. The updated surgical algorithm in this review adds precision in three areas: patient selection, identification of previously unaddressed anatomic phenotypes with associated treatment modality, and improved techniques of previously established procedures. While the original Riley and Powell phase 1 and 2 approach to sleep surgery has focused on individual surgical success rate, this algorithm strives for an overall treatment success with multi-modal and patient-centric treatments.

      • Use of Emoji as a Marketing Tool: An Exploratory Content Analysis

        ( Stanley Mathews ),( Seung-eun Lee ) 복식문화학회 2018 Fashion, industry and education Vol.16 No.1

The purpose of this exploratory study was to enhance the understanding of how brands tilize emojis in their marketing practices. A content analysis was conducted utilizing Google News as a search tool to access articles containing information pertaining to the use of emojis by brands. The combination of keywords used for the search were “emoji”, “business”, and “marketing”. The search was narrowed down to the period of January 1st, 2014 - November 29th, 2017. This method generated a total of 604 trade publications with 55 of them providing information pertaining to specific brands and their use of emojis in their marketing strategies. A content analysis of trade publications has revealed that a variety of marketers have utilized emojis in their brand marketing practices. The entertainment, service, and food/drink industries have predominantly utilized emojis in their marketing practices, and their primary purpose for using emojis was to increase consumer engagement. Brands applied most of these emoji marketing strategies to an online or digital setting, whether it was social media pages, mobile applications, or any other form of computer-based marketing. Although there are limitations to this exploratory research in terms of its methodology, the findings of this study provide interesting insights into the potential of emojis as a marketing tool.

        U . S . Forest Service Research : Its Administration and Management

        Stanley L . Krugman 한국산림과학회 1987 한국산림과학회지 Vol.76 No.3

The U.S. Forest Service administers the world`s largest forestry research organization. From its modest beginning in 1876, some 30 year, before the United States national forest system was established, the research branch has devoted its effort to meet current and future information needs of the forestry community of the United States, not just for the U.S. Forest Service. The research branch is one of three major administrative units of the U.S. Forest Service. The others being the National Forest System and State and Private Forestry. Currently the National Forest System comprises 155 national forests, 19 national grasslands, and 13 utilization projects located in 49 states. Puerto Rico, and the Virgin Islands. 1 he National Forest System manages these areas for a large array of uses and benefits including timber, water, forage, wildlife, recreation, minerals, and wilderness. It is through the State and Private Forestry branch that the U.S. Forest Service cooperates and coordinates forestry activities and programs with state and local governments, forest industries, and private landowners. These activities include financial and technical assistance in disease, insect, and fire protection : plan forestry programs ; improve harvesting and marketing practices : and transfer forestry research results to user groups. Forestry research is carried out through eight regional Forest Fxperiment Stations and the Forest Product Laboratory. Studies are maintained at 70 administrative sites, and at 115 experimental forest and grasslands. All oh the current sciences that composed modern forestry are included in the research program. These range from forest biology (i. e. silviculture, ecology, physiology, and genetics)to the physical, mathematical, engineering, managerial, and social sciences. The levels of research range from application, developmental, and basic research. Research planning and priority identification is an ongoing process with elements of the research program changing to meet short-term critical information needs(i.e. protection research)to long-term opportunities(i.e. biotechnology). Research planning and priority netting is done in cooperation with National Forest Systems, forest industries, universities, and individual groups such as environmental, wilderness, or wildlife organizations. There is an ongoing review process of research administration, organization, and science content to maintain quality of research. In the U.S. Forest Service the research responsibility is not completed until the new information is being applied by the various user group : i.e. technology transfer program. Research planning and development in the U.S. Forest Service is a dynamic activity. Porgrams for the year 2000 and beyond are now in the planning stage.

Effect of ethanolic extract of Chromolaena odorata on the kidneys and intestines of healthy albino rats

        Stanley Anyanwu,Imeobong J. Inyang,Enosakhare A. Asemota,Okechi O. Obioma,Dorathy C. Okpokam,Victoria O. Agu 한국한의학연구원 2017 Integrative Medicine Research Vol.6 No.3

Background: The use of plants and plant products for medicinal purposes is an age-long practice in traditional communities and is becoming prominent globally. This study was performed to evaluate the effect of ethanolic extract of Chromolaena odorata on the kidney and intestine of albino rats. Methods: Twenty growing albino rats with an average weight of 54 g were used in this study. They were grouped into four groups. Groups 1, 2, and 3, known as the test groups, were given 50 mg/kg, 100 mg/kg, and 250 mg/kg ethanolic extract of C. odorata, respectively, while the control group was given distilled water orally. The experiment was performed for 6 weeks. The animals were killed using chloroform suffocation. The kidneys and the intestine were harvested and fixed in 10% neutral buffered formalin for histological analysis. Blood samples were collected from the animals by heart puncture for estimation of creatinine and urea levels. Results: The creatinine, urea, aspartate aminotransferase, alanine transaminase, and alkaline phosphatase levels of blood sample from the test group were significantly different when compared with the control (p < 0.05). The histological sections of the kidneys in this study showed no signs of degeneration. Infiltration of inflammatory cells and epithelial erosion were observed in the histology sections of the intestine of all the test groups. Conclusion: The results from this study revealed that uncontrolled use of this plant extract has an adverse effect on the kidney function and on the histology of the intestine of the rats used in this study.

Risk of Neuroleptic Malignant Syndrome with Vesicular Monoamine Transporter Inhibitors

Stanley N. Caroff 대한정신약물학회 2020 CLINICAL PSYCHOPHARMACOLOGY AND NEUROSCIENCE Vol.18 No.2

Objective: Vesicular monoamine transporter-2 (VMAT2) inhibitors have been proven to be effective for the treatment of tardive dyskinesia and their use is likely to increase. The evidence base of published clinical reports was reviewed to evaluate the possible risk of neuroleptic malignant syndrome (NMS) with these drugs. Methods: Pubmed, Embase, Web of Science and PsycINFO databases were queried for all years using terms for “neuroleptic malignant syndrome”, “hyperthermia” AND “vesicular monoamine transporter inhibitors”, “reserpine”, “tetrabenazine”, “valbenazine” or “deutetrabenazine” Results: Thirteen clinical cases, ten of which involved tetrabenazine, were identified in which VMAT2 inhibitors were prescribed in patients with current or past NMS episodes. In most cases, the association was confounded by limited reporting of clinical data, variable temporal correlation with VMAT2 inhibitors, polypharmacy with antipsychotics, and uncertain differential diagnoses. Conclusion: While rare cases of NMS meeting consensus criteria have been reported primarily with tetrabenazine, the risk with recently developed VMAT2 inhibitors may be even less. Evidence of causality of NMS with VMAT2 inhibitors is confounded by concomitant treatment with antipsychotics and diagnostic uncertainties in patients susceptible to basal ganglia dysfunction. Nevertheless, clinicians should remain vigilant for early signs of NMS in all patients treated with any drugs that affect brain dopamine activity.

Effect of Varenicline on Tardive Dyskinesia: A Pilot Study

        Stanley N. Caroff,Alisa R. Gutman,John Northrop,Shirley H. Leong,Rosalind M. Berkowitz,E. Cabrina Campbell 대한정신약물학회 2021 CLINICAL PSYCHOPHARMACOLOGY AND NEUROSCIENCE Vol.19 No.2

Objective: Although evidence implicates striatal cholinergic impairment as a mechanism underlying tardive dyskinesia, trials of nonspecific cholinergic agents have been inconclusive. As a partial agonist at specific nicotinic receptor sub-types, varenicline reduces drug-induced dyskinesias in animal models suggesting promise as a treatment for tardive dyskinesia. Methods: Three schizophrenia patients with tardive dyskinesia who were smokers underwent an open trial of varenicline. After a 2-week baseline, subjects received varenicline 1 mg twice daily. Changes from baseline on the Abnormal Involuntary Movement Scale were measured after a 4-week varenicline stabilization period, and 6 weeks after the smoking quit date in one patient. Results: Varenicline had no effect on mean Abnormal Involuntary Movement Scale scores after 4 weeks. Although smok-ing decreased after 4 weeks on varenicline and diminished further in one patient after 10 weeks, this also appeared to have no effect on ratings of tardive dyskinesia. Conclusion: In contrast to animal models, no significant change in tardive dyskinesia occurred in response to varenicline replacement in three schizophrenia patients. Further investigations of cholinergic mechanisms in tardive dyskinesia are worthwhile as agents for specific cholinergic targets become available for treatment. In addition, treatment trials of tardive dyskinesia should control for smoking status, while patients on antipsychotics receiving nicotine replacement therapies for smoking should be studied further for changes in movement.
