        Effect of adherence to carbohydrate counting on metabolic control in children and adolescents with type 1 diabetes mellitus

        Purpose: Carbohydrate counting provides better glycemic control and flexibility than other food planning methods. Consistent adherence to such a complex method is difficult, especially for youth. However, studies that determine adherence to this method and whether it alters metabolic control are limited. The aim of the current study was to determine adherence to this method and investigate its effect on metabolic control, anthropometric measurements, insulin dose, and energy intake. Methods: In this prospective cross-sectional study, 53 children and adolescents with type 1 diabetes mellitus aged 2 to18 years and receiving intensive insulin therapy were trained and followed for 6 months. Demographics, anthropometrics, insulin requirements, hemoglobin A1c (HbA1c), fasting lipids, and food records at baseline and study conclusion were evaluated. At the end of the study patients were divided into adherer and nonadherer groups according to carbohydrate estimate deviations from standardized daily sample menus and calculations for accurate insulin doses. More than 10-g variation in daily consumed carbohydrate amount or failure to decide bolus insulin dose was defined as a nonadherer. Results: The mean HbA1c, low-density lipoprotein cholesterol, and body mass index standard deviation score changed after the carbohydrate counting training while the mean HbA1c between groups was significant (P<0.05). Total daily insulin doses increased, and the mean high-density lipoprotein cholesterol levels decreased in both groups. There were significant correlations between HbA1c and carbohydrate deviation scores as well as HbA1c and caregiver's education level. Conclusion: Since adherence to carbohydrate counting may affect metabolic control, health professionals should evaluate and monitor carbohydrate counting skills of caregivers and patients in order to improve efficiency.

        Adult vaccine-related knowledge, attitudes, and behaviors in Turkey

        Purpose: Adult vaccination, which is among the duties of family physicians, is an important issue that reduces morbidity and mortality. In this study, it was aimed to evaluate knowledge, attitudes, and behaviors of adults about adult vaccines. Materials and Methods: This study is a descriptive, cross-sectional research; which was performed by a questionnaire including sociodemographic data and questions about adult vaccination to the patients and their relatives who applied to Şişli Hamidiye Etfal Training and Research Hospital Family Medicine Polyclinics for any reason. Mean, standard deviation and chi-square tests were used for the analysis; p<0.05 was considered significant. Results: The study included 182 people, 93 of them were women (51.1%) and 89 (48.9%) were men. The mean age was 32.9±12.8 years; most of them (n=144, 79.1%) were high educated and 38.5% were married. The most known vaccine was tetanus (n=154, 84.6%), the least known was zona vaccine (n=30, 16.5%). Health care professionals and television (n=60, 33%) were the most information sources about vaccines (n=78, 42.9%). Vaccination was mostly (26.9%) recommended by family physicians 144 (79.1%). The students had more knowledge about human papilloma virus (HPV), zoster, and measles, mumps, and rubella vaccines (p≤0.05), and mostly the information was got from internet and school (p≤0.05). As education status increased, knowledge about tetanus, hepatitis A, pneumonia, and HPV vaccines increased significantly, whereas only tetanus vaccination was performed in practice (p≤0.05). As the income increased, knowledge about pneumonia and HPV vaccines increased, but vaccination was not performed as expected. Conclusion: Age, occupation, education, and income level are directly related to knowledge and attitudes about adult vaccination. Adult vaccines are still not known and performed enough in Turkey. It is important to encourage and increase the number of trainings via media on this subject.

        Cellulose Monoacetate/Tetraethyl Orthosilicate Hybrid Nanofibers for Electrochemical DNA Biosensors

        Cellulose monoacetate/tetraethyl orthosilicate (CMA/TEOS) hybrid nanofibers were produced with differentratios via electrospinning and used for guanine oxidation analysis in the single strand deoxyribonucleic acid (ssDNA)molecules by electrochemical method. Nanofiber (NF) diameters for pure CMA dramatically decreased from between 2.4 μ-306 nm to between 958-42 nm with addition of hydrochloric acid (HCl) catalyzer into the electrospinning solution. UniformCMA nanofibers morphologies transform to more defect structures containing particular structures with the addition andincrease of TEOS content in CMA electrospinning solution. Also, nanofibers' diameters range became more fluctuated formand ultrafine nanofibers (diameters below 100 nm) existed more in the nanofiber mat. Even though the melting point wasseen at CMA nanofibers, melting points were not detectable in CMA/TEOS NFs since TEOS addition does not allow anappropriate crystallization. Thermal analysis results revealed that residual contents after TGA measurement in nitrogenatmosphere were gradually increased by increasing TEOS ratio in CMA/TEOS nanofibers. ssDNA molecules were immobilizedon the as-spun nanofibers and differential pulse voltammetry (DPV) measurements were carried out to investigate theguanine oxidation in ssDNA. Guanine oxidation signal intensities decreased with the initial addition of TEOS to CMAnanofibers and increases again by increasing TEOS content in CMA/TEOS hybrid nanofibers and then gradually decreasesagain with increasing TEOS content. The prepared CMA/TEOS hybrid nanofibers could be a promising candidate as anelectrode interface for genetic molecule detection via electrochemical methods.

        The role of serum lipoxin A4 levels in the association between periodontal disease and metabolic syndrome

        Purpose: An unresolved inflammatory state contributes to the pathogenesis of periodontal disease and metabolic syndrome (MetS). Therefore, the purpose of this study was to evaluate the role of lipoxin A4 (LXA4), a proresolving lipid mediator, in the association between periodontal disease and MetS. Methods: Sixty-seven patients with MetS and 65 patients without MetS were included in the study. Sociodemographic information was obtained via a questionnaire, and detailed medical diagnoses were made. Periodontal parameters (plaque index [PI], gingival index [GI], probing pocket depth [PD], and clinical attachment level [CAL]) and metabolic parameters were measured, and serum LXA4 levels were determined. The associations among MetS, periodontal parameters, and serum LX levels were evaluated by adjusted multivariate linear regression analyses. Results: Patients with MetS were older and had a higher body mass index than patients without MetS. Periodontal parameters (PI, GI, PD, and CAL) were higher in patients with MetS than in those without MetS. Serum LXA4 levels were higher in patients without MetS. Multivariate linear regression analysis indicated a positive association between MetS and periodontal parameters (PD and CAL). Negative associations were established between MetS and LXA4 levels, and between LXA4 and periodontal parameters (PI, PD, and CAL). Conclusions: The presence of higher values of periodontal parameters in patients with MetS and the negative relationship of LXA4 with MetS and periodontal disease may support the protective role of proresolving lipid mediators in the association between periodontal disease and MetS.

        Vascular Cell Adhesion Molecule 1, Intercellular Adhesion Molecule 1, and Cluster of Differentiation 146 Levels in Patients with Type 2 Diabetes with Complications

        Background: Type 2 diabetes mellitus (T2DM) is a multisystemic, chronic disease accompanied by microvascular complicationsinvolving various complicated mechanisms. Intercellular adhesion molecule 1 (ICAM-1), vascular cell adhesion molecule 1(VCAM-1), and cluster of differentiation-146 (CD146) are mainly expressed by endothelial cells, and facilitate the adhesion andtransmigration of immune cells, leading to inflammation. In the present study, we evaluated the levels of soluble adhesion moleculesin patients with microvascular complications of T2DM. Methods: Serum and whole blood samples were collected from 58 T2DM patients with microvascular complications and 20 agematchedhealthy subjects. Levels of soluble ICAM-1 (sICAM-1) and soluble VCAM-1 (sVCAM-1) were assessed using enzymelinkedimmunosorbent assay, while flow cytometry was used to determine CD146 levels. Results: Serum sICAM-1 levels were lower in T2DM patients with microvascular complications than in healthy controls (P<0.05). No significant differences were found in sVCAM-1 and CD146 levels between the study and the control group. Although patientswere subdivided into groups according to the type of microvascular complications that they experienced, cell adhesion molecule levelswere not correlated with the complication type. Conclusion: In the study group, most of the patients were on insulin therapy (76%), and 95% of them were receiving angiotensin-convertingenzyme (ACE)-inhibitor agents. Insulin and ACE-inhibitors have been shown to decrease soluble adhesion molecule levels viavarious mechanisms, so we suggest that the decreased or unchanged levels of soluble forms of cellular adhesion molecules in our studygroup may have resulted from insulin and ACE-inhibitor therapy, as well as tissue-localized inflammation in patients with T2DM.

      • Human Papilloma Virus Frequency and Genotype Distribution in a Turkish Population

        Objectives: To determine human papillomavirus (HPV) frequency, genotypes and the relation between cervical smear results, risk factors and types in women living in Manisa, Turkey. Materials and Methods: A total of 410 women were included in the study. Cervical specimens were obtained for linear array HPV genotyping and pathological testing. Conventional Pap test and Bethesda system were used for evaluation of cytology specimens. Results: A total of 410 women with a mean age of 34.9 years were tested. A positive result of any HPV was found in 35 patients (8.5%). Among them, 26 different serotypes of HPV were identified and the most frequent type was HPV 16 (28.5%) followed by type 45 and 53 (11.4%). Patients were infected by 65.7% high risk, 11.4% probable high risk and 22.9% low risk HPV types. Multiple HPV positive results were found in 13 patients (37.1%). Patients with single partner, history of abnormal smear or condyloma had positive HPV results and this was statistically significant (p<0.05). Correlation analysis showed a statistically weak relation between positive HPV and abnormal smear results (r=0.120). Conclusions: Determining HPV types of genital HPV infections is important for epidemiological studies. We have found the rate of positive HPV as 8.5% which implies the need for extended screening programs in order to diagnose oncogenic HPV at an early stage.

      • Marine Cargo Insurance Exclusions: Comparative view between Turkish Law and Japanese Law

        In this article, the author make analysis on the cargo insurance exclusions listed under the Institute of Cargo Clauses (ICC). While making this analysis, the author explicitly focuses on delay and insolvency default exclusions as these exclusions bear many uncertainties as to how the coverage is provided and when is provided. For example, delay is generally accepted as a cargo coverage mainly related with the results of another peril. Therefore, it is an awkward concept of cargo coverage. On the other hand, insolvency default exclusion, even though under ICC 2009 is an exclusion clause that is covered, bring some questions on case to case basis. The author in this work analyzes briefly these questions and makes short comparison between Japanese Law and Turkish Law.

      • Predictive Value of Malignancy Risk Indices for Ovarian Masses in Premenopausal and Postmenopausal Women

        Background: To evaluate the predictive role of a risk of malignancy index in discriminating between benign and malignant adnexal masses preoperatively. Materials and Methods: A total of 408 patients with adnexal masses managed surgically between January 2010 and February 2014 were included. The risk of malignancy indices (RMI) 1, 2, 3 and 4 were calculated using findings for ultrasonography, menopausal status, and CA125 levels. Histopathologic results were the end point. ROC analysis was used for the sensitivity and the specificity of the models. Results: Some 37.6 % of the cases were malignant in the postmenopausal group while 7.9 % were malignant in the premenopausal group. Pelvic pain was the most common complaint, and the majority of the cases were diagnosed at stage 3. The RMI 1, 2, 3 and 4 yielded percentage sensitivities of 76.1, 79.1, 76.1 and 76.1 and specificities of 91.5, 89.1, 90.6, 88.6, respectively. RMI 1 was the most reliable test in the general population according to AUC levels and Kappa statistics. From ROC analysis results of post/premenopausal women, the RMI 1 (cut off: 200) yielded sensitivities of 84.0/60.9 and specificities of 87.7/92.5. With RMI 2 they were 88.6/60.9 and 80.0/91.0, with RMI 3 84.0/60.9 and 87.7/91.8, and with RMI 4 (cut off:400) 81.8/47.8 and 83.6 /44.0. Although test performance of RMI methods were good in a general population and postmenopausal women, the RMI inter-agreement validity was only moderate or fair in premenopausal women. Conclusions: Our study confirms the effectiveness of RMI algorithms in postmenopausal women. However, more sensitive tests are needed for premenopausal women.
