Lower Eyelid Epiblepharon Associated with Lower Eyelid Retraction

Purpose: To describe a series of patients with lower eyelid epiblepharon associated with lower eyelid retraction. Methods: We retrospectively reviewed the medical records of patients who underwent surgery for lower eyelid retraction, epiblepharon, or thyroid-associated ophthalmopathy (TAO) between October 1999 and March 2007. Patients with both lower eyelid retraction and epiblepharon on preoperative examination were included in this study. Results: Twenty-seven eyelids of 20 patients with both lower eyelid retraction and epiblepharon were enrolled. The underlying causes of lower eyelid retraction included congenital retraction (seven eyelids), congenital fibrosis of the extraocular muscles (CFEOM; seven eyelids), TAO (seven eyelids), post-operative cicatricial retraction (five eyelids), and facial nerve palsy (one eyelid). Eight of 27 eyelids were successfully corrected after the repair of retraction without the repair of epiblepharon, regardless of the cause of lower eyelid retraction. Another four eyelids with epiblepharon associated with TAO resolved after only orbital decompression. Cilia-everting sutures were additionally applied for epiblepharon in another 14 eyelids, 12 of which did not require the excision of a skin fold or the orbicularis muscles. Only one eyelid with mild retraction and epiblepharon underwent simple epiblepharon repair. Recurrence of retraction or epiblepharon developed in three eyelids during follow-up. Conclusions: In cases with both lower eyelid retraction and epiblepharon, the retraction should be repaired first, and then the epiblepharon can be corrected selectively according to the severity of the case.
다공성 폴리에틸렌 안와 삽입물의 피복 재료로서 신전된 폴리테트라플루오로에틸렌의 이용

Purpose: To investigate the possibility of expanded polytetrafluoroethylene (e-PTFE) as a wrapping material for porous polyethylene orbital implant. Methods: Two groups of 6 female New Zealand albino rabbits underwent enucleation with placement of a 12 mm porous polyethylene orbital implant wrapped in simple e-PTFE in one group (group 1) and macro-porous e-PTFE in the other group (group 2). Histopathologic sections of the implants obtained at 4, 6, 8 weeks were compared on the extent of fibrovascular ingrowth, percentage of the area of proliferated vessel, and grade of fibrosis between the two groups. Results: There was no implant exposure or severe inflammation in group 1 and 2 during the observation. The extent of fibrovascular ingrowth differs only at 4 weeks. Group 1 showed less fibrovascular ingrowth (until outer 1/3) at anterior half compared with posterior one, but there was no difference between anterior and posterior half in group 2 (until middle 1/3). The percentage of the area of the proliferated vessels was higher in group 2 than group 1 at both anterior and posterior half, and the anterior/posterior ratio of the area of vessels was also higher in group 2 than group 1. The grade of fibrosis was higher in group 2 than group 1 at 4, 6, and 8 weeks. Conclusions: Macro-porous e-PTFE almost didn`t disturb fibrovascular ingrowth into the orbital implant, so macro-porous e-PTFE can be used safely as a wrapping material for porous polyethylene orbital implant.
Polytetrafluoroethylene as a Spacer Graft for the Correction of Lower Eyelid Retraction

Purpose: To evaluate the efficacy of porous expanded polytetrafluoroethylene (e-PTFE, Goretex??) containing large pores made with a 21-gauge needle as a graft for the correction of lower lid retraction. Methods: e-PTFE grafts were implanted between the tarsus and lower lid retractor via a transconjunctival approach with/without amniotic membrane transplantation, or via a transcutaneous approach. Rabbits were examined and assessed for corneal and conjunctival complications and for e-PTFE graft status. Rabbits were sacrificed for a histological study at 8 weeks postoperatively. Results: e-PTFE grafts were uniformly extruded 3 weeks postoperatively in eyelids operated on via the transconjunctival approach. However, rabbits operated on via the transcutaneous approach demonstrated e-PTFE graft retention; in addition, dense fibrovascular ingrowths into the large pores of e-PTFE were observed histologically. Conclusions: e-PTFE is a good substitute for other graft materials as a spacer in lower lid retraction operations, especially as an interpositional graft using a transcutaneous approach.
