Despite the use of aggressive local and systemic treatment including autologous stem cell transplantation in multiple myeloma, extramedullary recurrences are common and the prognosis of these patients is poor. Many novel drugs such as thalidomide, lenalidomide and bortezomib improve the response of treatment of multiple myeloma, but some reports failed to describe thalidomide has effect in extramedullary plasmacytoma. Recent data report on the successful treatment plasmacytomas with bortezomib in patients with advanced multiple myeloma. We treated 4 relapsed or refractory extramedullary plasmacytomas with bortezomib at our institution. We recognized all these extramedullary plasmacytomas decreased and showed more than partial response. This report lends support to the efficacy of bortezomib in the treatment of plasmacytoma and describes the safe use of bortezomib. Responses may, however, be of short duration. Therefore, despite our limited experience, we propose that bortezomib may be considered a therapeutic option for such patients who have risk of radiation therapy
식이섬유질원이 포함된 식이가 흰쥐의 장내 균총조성에 미치는 영향

This study was performed to investigate the influence of dietary fiber from the whole food on the composition of intestinal microflora in rats. The fiber sources of experimental diets were prepared by drying and milling of small water dropwort(WD), mugwort(MW), butterbur(BB) and apple(AP). Each fiber sources was mixed at the 15% level of the diet. The level of dietary fiber in WD, MW, BB diets was simillar but that in AP diet was slightly low. Male rats of Sprague-Dawley strain were blocked into 5 groups: WD, MW, BB, AP, FF(fiber free), The animal was fed ad libitum each of experimental diets for 37 days. Total viable counts and the numbers of Lactobacillus, Bifidobacterium, Bacteroidaceae, Eubacterium, Peptococcaceae, Streptococcus, Clostridium, Staphylococcus, E. coli were determined by nonselective and various selective media. Food intake of AP and FF groups were lower than the other groups, FF group was lower than the other groups in flood efficiency ratio. Total viable counts were the highest in FF group. The number of Bacteroides, Eubacterium were higher in AP group compared with other groups and that of streptococcus was lower in MW, BB, AP groups than others. The number of Lactobacillus was higher in WD, MW, BB groups than the other groups and that of Bifidobacterium was higher in BB, AP groups. A decrease in the intestinal population of Clostridium was shown in MW, BB groups. These findings suggests that the samll water dorpwort, mugwort butterbur are effective in improving intestinal flora of rats.
Interleukin-1β에 의하여 치주인대세포에서 유리된 cytokine이 파골세포형성에 미치는 영향

Tooth movement is induced by bone remodeling during orthodontic treatment. Bone remodeling is regulated by various cytokines. Especially interleukin-1 (IL-1β), a cytokine present in periodontal ligaments of experimentally moved teeth, elicits bone resorption. In these processes, IL-1-induced bone resorption is mediated by interleukin-6 (IL-6) and granulocyte macrophage-colony stimulating fector (GM-CSF) secreted from osteoblasts. Periodontal ligament cells, which function as an anchorage for tooth, lie between alveolar bone and cementum. Therefore cytokines produced in the periodontal ligament (PDL) cells may also directly affect alveolar bone resorption in orthodontic tooth movement. Here I have examined whether PDL cells express IL-1β,interleukin-6 (IL-6) and granulocyte macrophage-colony stimulating factor (GM-CSF) mRNA and secrete those cytokines in response to IL-1β. Finally I have investigated whether IL-6 produced from PDL cells induces osteoclast formation in mouse bone marrow cell cultures. The expression of mRNA was estimated by polymerase chain reaction (PCR). The concentration of cytokines was quantified using enzyme linked immunosorbent method and the osteoclasts in bone marrow cultures were identified by tartrate resistant acid phosphatase (TRAP) stain. As results of these studies, IL-1βstimulated the expression of IL-1β, IL-6 and GM-CSF mRNA in PDL cells. 0.05 ng/ml IL-1βalso induced maximum production of Il-6 and GM-CSF in these cells. After an addition of IL-1β(0.05 ng/ml), IL-6 production increased from 2 hours to 8 hours and GM-CSF production also increased from 4 hours to 8 hours. IL-6 (100 ng/ml) increased the number of TRAP positive multinucleated cells in the presence of soluble interleukin-6 receptor (sIL-6R, 100 ng/ml). These results suggest that IL-1βmay stimulate alveolar bone resorption by inducing IL-6 and GM-CSF production in PDL cells which enhance osteoclast differentiation and IL-6 enhances osteoclast formation in the presence of sIL-6R. And this process by IL-1βmay be closely associated with alveolar bone resorption induced by orthodontic force.

The purpose of this study was to investigate the effect of various electroacupuncture duration induced by acupuncture point-Zusanli(S36) electrical stimulation on inhibition of amplitude of digastric electromyogram (dEMG)evoked by noxious electrical stimuli around the mental foramen. <Experiments were conducted on adult rats (SD) were anesthetizde with> intraperitoneal sodium pentobarbital in an initial dose of 50mg/kg and maintenance doses of 4.5mg/kg/h were given through a cannula in the femoral vein using a constant infusion pump. A pair of stimulating electrodes were inserted for noxious stimuli around the mental foramen. An irritant electronic stimuli pulse (0.2 Hz,0.1 ms duration) was produced with an intensity of about 1.5-2 times threshold for evoking the dEMG. The anterior belly of the digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for dEMG recording. Acupuncture point stimulation on Zusanli (2 Hz, 250㎲, biphasic pulse. 2 V) was delivered by Dental Electronic Anesthesia (3M,U.S.A). For periods of electronic stimulation of 10, 20, and 30min, the amplitudes of dEMG were measured on the oscilloscope and on the monitor connected to the amplifier. The following results were obtained: The dEMG was decreased to 73.4% of that in the control set after 10 min electroacupunture stimulation (Group Ⅰ): The dEMG was decreased to 77.1%(10min), 54.0% (20min)of that in the control set after 20minutes of electroacurunture stimulation (Group Ⅱ). The dEMG was decreased to 73.7% (10min), 61.9%(20min), 76.2%(30min) of that in the control set after 30 min of electroacupunture stimulation (Group Ⅲ). From these results, it may be that in the electroacupuncture stimulation on the Zusnali resulted in a reduction of amplitude of daEMG and that the most effective electroacupuncutre stimulation period was 20min.

본 연구의 목적은 한국인 부정교합자에서 하악 치열궁간의 형태적 차이를 알아보고자 하였다. Ⅰ급 114명. Ⅱ급 119명, Ⅲ급 135명의 부정교합자를 대상으로 하악 모형의 교합면을 복사한 후 13개의 접촉점 중에서 가장 협측면으로 위치한 부분을 digitize 하였고 각 치아의 브라켓 위치에 해당하는 점을 하악치아의 두께에 의거하여 4개의 선계측과 2개의 비율을 측정하였다. 치열궁의 형태는 square ovoid, tapered형으로 분류하여 그 빈도를 조사한 결과 Angle 분류와 남녀 성별에 따른 특별한 치열궁 형태가 존재하지 않았으나, 특정 치열궁 형태의 빈도는 Angle 분류에 따라 다르게 나타났다. 치열궁 크기의 계측항목에서는 대구치간 폭경에서만 남녀 성별 차이를 보였으며, 치열궁 크기에 있어서 Ⅰ급과 Ⅲ급, Ⅱ급 부정교합군간에는 유의한 차이가 있었다. 치열궁 형태의 분포는 Ⅰ급과 Ⅲ급 부정교합군에서는 square, ovoid, tapered 형태 순이었고, Ⅱ급 부정교합군은 square, tapered ovoid 형태 순이었다. 치열궁 형태의 빈도분포에 있어서는 남녀간의 차이는 없었다. The purpose of this study was to clarify morphological differences among mandibular dental arch forms in Korean malocclusion patients. The sample in this study consisted of 114 Class Ⅰ, 119 Class Ⅱ, and 135 Class Ⅲ malocclusion cases. The most facial portions of 13 proximal contact areas were digitized from photocopied images of the mandibular dental arches. Clinical bracket points were calculated for each tooth based on the data of the mandibular tooth thickness. Four linear and two proportional measurements were undertaken. The dental arches were classified into square, ovoid, and tapered forms to compare the frequency distributions. Our results suggested that there was no single arch form specific to any particular Angle classification or sex. It appeared to be the frequency of a particular arch form that varies among the Angle classifications. In comparison of arch measure between male and female, there was no statistical difference except in the intermolar width. In comparison of arch size measurements among the different Angle classifications, there were statistically significant differences between Class Ⅰ and Class Ⅲ malocclusion groups and between Class Ⅱ and Class Ⅲ malocclusion groups. In comparison of frequency distribution of arch forms in Class Ⅰ and Ⅲ malocclusion groups, the square form demonstrated the highest distribution followed by the ovoid and tapered forms in that order. In the Class Ⅱ malocclusion group, the square form showed the highest distribution, followed by the tapered and ovoid forms in that order. There was no statistical difference in the frequency distribution of arch forms between male and female groups.
개 비장 질환의 미세세침흡입술을 통한 세포병리학 검사 4례

To evaluate the accuracy and diagnostic significance of cytopathological diagnosis of canine splenic disorders, cytopathological examination throgh fine-needle aspiration(FNA) referred to Veterinary Medical Teaching Hospital of Chungnam National University were retrospectived. Splenic neoplasm, hemolymphatic neoplasm, hyperplasia, extramedullary hematopoiesis and splenitis were diagnosed canine splenic disorders through cytopathological examination. The patient should be identified what coagulation profile are normal before aspiration, because complication such as hemorrhage could happen. The 4 cases were identified symmetric, asymmetric splenomegaly on abdominal ultrasonography and then were performed ultrasound-guided FNA. The 4 cases were diagnosed hemangiosarcoma(2), hyperplasia(1) and extramedullary hematopoiesis(1) by clinical signs, hematological examination, and cytopathological examination. Cytopathological examination through FNA was cost-effective, no gneral anesthesia, no complication, and non-invasive method of diagnosis for canine splenic disorders.
