      • 비확공성 골수강내 금속정을 이용한 폐쇄성 경골 간부 골절의 치료

        Most of closed fractures of the tibial shaft can be treated successfully with medullary nails. Reamed intramedullary nailing technique has become a good alternative method in treating the close tibial fracture. Unreamed interlocking intramedullary nailing has been employed successfully in the treatment of open tibial fractures. This study was undertaken to evaluate the result of interlocking intramedullary nailing without reaming in the treatment of closed fractures of the tibial shaft. From 1995 through 1998, fifty four cases of the closed, displace tibial fractures were treated by an unreamed intramedullary nailing and the average follow-up period was 1 year and 5 months(ranged from 12 months to 27 months). The results were as follows : 1. Fifty four cases were treated with closed, unreamed nailing technique and the nail which has 1mm smaller diameter than isthmic width of unaffected tibia was choosen. 2. The average time to bony union was achieved at 17 weeks without abundant periosteal b callus formation. 3. Eight tibiae were required the additional procedure such as autogenous bone graft, skin graft or local skin flap. We also have observed the screw breakage of ten tibiae but there were no intraoperative jamming or incarceration and no significant complicaton such as bending or breakage of the nail, shortening or non-union of the fractures. In conclusion, unreamed technique is an attractive method to treat the close, displaced fractures of the tibial shaft. 경골 간부 골절의 치료에 있어서 견고한 내 고정으로 조기 관절운동 및 체중 부하를 통해 골절 유합 및 기능 회복을 도모할 수 있는 골수강내 금속정 고정술이 널리 사용되어지고 있다. 골수강내 금속정술의 장점은 술 후 조기 관절운동과 체중 부하가 가능하여 이환율(morbidity)을 감소시키며 불유합의 발생률이 낮아는 것이다. 경골 간부 골절의 치료에 있어서 비확공성 금속정 고정술은 주로 심한 연부 조직 손상을 동반한 골절 및 개방성 골절의 치료에 적용되어지고 있으며, 대부분의 폐쇄성 경골 간부 골절의 치료로는 골수강 확공 후 골수강을 충만시키는 금속정을 삽입하는 확공정 금속정 고정술이 시행되어지고 있는 경향이다. 저자들은 폐쇄성 경골 간부 골절에 대해서 비확공성 금속정 고정술의 유용성 여부를 알아보기 위해서 동국대학교 의과대학 정형외과학교실에서 치료했던 54례를 평균 1년 5개월 추시하여 골 유합 기간, 합병증의 발생에 대해서 분석하였다. 본 연구에서 비확공성 금속정 고정술시 골 유합 기간은 확공성 금속정 고정술시와 비교하여 유의한 차이가 없음을 알 수 있었으며, 확공을 하지 않음으로써 발생할 수 있는 jamming이나 가는 직경의 금속정을 선택함으로써 우려되는 금속정의 휨, 부러짐 등의 합병증은 발생하지 않았다. 결론적으로 폐쇄성 경골 간부 골절의 치료에 있어서도 비확공성 골수강내 금속정 고정술은 유용한 방법으로 사료된다.

        Dokhwaljihwang-tang inhibits LPS-induced inflammatory cytokine production in peripheral blood mononuclear cells.

        <P>OBJECTIVES: Dokhwaljihwang-tang, one of the prescriptions in Four Constitution medicine, has been used to treat neurological disorders. This work was aimed to examine the effect of Dokhwaljihwang-tang on lipopolysaccharide-induced inflammatory cytokine production in peripheral blood mononuclear cells from the lesser yang subjects. METHODS: The inflammatory cytokines, interleukin 1 beta, interleukin 4 and tumor necrosis factor were measured using the ELISA. The interleukin 1 beta mRNA expression was evaluated with reverse transcriptase-polymerase chain reaction. The expression level of nuclear factor kappa B was examined by western blotting. RESULTS: Dokhwaljihwang-tang significantly inhibited interleukin 1 beta and interleukin 4 production (p<0.05). When Dokhwaljihwang-tang was pre-treated at the dose of 1 mg/ml, it not only decreased the interleukin 1 beta mRNA expression, but also inhibited the activation of nuclear factor kappa B. DISCUSSION: These results suggest the potential of Dokhwaljihwang-tang as a source of pharmaceutical acupuncture for neuronal inflammatory diseases.</P>

      • Action of enzyme food, Green Life Enzyme on systemic and local anaphylaxis

        We studied the inhibitory effect of Green Life Enzyme (GLE) on compound 48/80-induced anaphylactic response in a murine model. GLE inhibited compound 48/80-induced systemic anaphylactic shock at the dose of 10 g/kg by 87.5%. When GLE was given as pre-treatment at concentrations ranging from 0.01 to 1.0 g/kg, it inhibited passive cutaneous anaphylaxis activated by anti-dinitrophenyl (DNP) IgE. In addition, GLE (0.1 mg/ml) inhibited anti-DNP IgE-induced tumor necrosis $factor-{\alpha}$ production from mast cells by 69% compared to saline value. These results indicate that GLE may possess anti-anaphylactic and anti-inflammatory activity.

        p-coumaric acid, an active ingredient of Panax ginseng, ameliolates atopic dermatitis-like skin lesions through inhibition of thymic stromal lymphopoietin in mice

        Background: Atopic dermatitis (AD) is associated with chronic skin inflammatory reactions. p-coumaric acid (pCA) is an active ingredient of Panax ginseng Meyer (Araliaceae). Methods: Here, we estimated an anti-AD effect of pCA on activated mast cells, activated splenocytes, and a mouse model of AD. Cytokines levels were measured by ELISA and protein activation was analyzed by Western blotting. 2,4-dinitrofluorobenzene (DNFB) was used to induce AD-like skin lesions. Results: The treatment with pCA suppressed the productions and mRNA expressions of thymic stromal lymphopoietin (TSLP), TNF-ɑ, IL-6, and IL-1β in HMC-1 cells. pCA downregulated the expressions of RIP2 and caspase-1, phosphorylated-(p)p38/pJNK/pERK, and pIKKβ/pIkBɑ/NF-κB in HMC-1 cells. pCA also decreased the productions of TSLP, TNF-ɑ, IL-6, IL-4, and IFN-γ in the supernatant of stimulated splenic cells. Comparing to DNFB-sensitized control group, pCA-treated group alleviated pathological changes of AD-like lesions. pCA decreased the proteins and mRNA expressions levels of TSLP, IL-6, and IL-4 in the skin lesions. Caspase-1 activation was also downregulated by pCA treatment in the AD-like lesions. The serum levels of histamine, IgE, TSLP, TNF-ɑ, IL-6, and IL-4 were suppressed following treatment with pCA. Conclusion: This study suggests that pCA has the potential to improve AD by suppressing TSLP as well as inflammatory cytokines via blocking of caspase-1/NF-κB signal cascade.

        Therapeutic potential of BaekJeol-tang in arthritis

        Osteoarthritis (OA) is the most common form of arthritis, and is a public health problem throughout the world. It remains the major cause of disability in the elderly, affecting about 60% of men and 70% of women above the age of 65. Even though there are many medicines for treatment of OA throughout the world, therapeutic potential of traditional Korean medicine (TKM) is not well known. Here we discussed the therapeutic potential of BaekJeol-tang (BJT, a prescription of TKM). Celecoxib is approved in one or more countries worldwide for the relief of the signs and symptoms of OA, rheumatoid arthritis, and juvenile rheumatoid arthritis. Although celecoxib inhibited mRNA expression of matrix metalloproteinase (MMP)-3 and increased mRNA expression of tissue inhibitors of metalloproteinase (TIMP)-1, celecoxib did not significantly inhibit IL-$1{\alpha}$-induced release of glycosaminoglycan (GAG) and only slightly reduced type II collagen. On the other hand, BJT significantly inhibited the release of GAG and type II collagen as well as regulated the mRNA expression of MMP-3 and TIMP-1. Through this article, we have tried to provide some therapeutic potential of BJT in arthritis.

      • Use of Electroacupuncture at ST36 to Inhibit Anaphylactic and Inflammatory Reaction in Mice

        <P><I>Objective:</I> Electroacupuncture (EA) has been used to treat myalgia, allergy and gastroenteropathy in Korea. To determine whether EA can treat anaphylactic and inflammatory reactions, the effect of EA was investigated in a murine model. <I>Methods:</I> EA stimulation of the ST36 acupoint was performed for 10 min. Using a passive cutaneous anaphylaxis (PCA) model, the antianaphylactic effects of EA were examined. Interleukin-6 and tumor necrosis factor-α were measured using the ELISA method. The level of nuclear factor (NF)-κB/RelA protein and NF-κB DNA-binding activity was determined using the Western blot analysis and the transcription factor enzyme-linked immunoassay method. <I>Results:</I> EA inhibits PCA and β-hexosaminidase release, IL-6 secretion on the PCA, and in addition, EA reduces NF-κB DNA-binding activity. <I>Conclusion:</I> These results indicate that EA may possess antianaphylactic and antiinflammatory properties.</P><P>Copyright © 2007 S.         <P>Thymic stromal lymphopoietin (TSLP) is a cytokine implicated in the pathogenesis of allergic diseases such as asthma, atopic dermatitis and allergic rhinitis. Curcumin has various effects such as antidepressant, antioxidant, antihyperglycemic, antitumor and anti-inflammatory. However, the effect of curcumin on the production of TSLP has not been clarified. Thus, we investigated how curcumin inhibits the expression and production of TSLP in the human mast cell line, HMC-1 cells.</P>

      • Anti-fatigue effects of porcine placenta and its amino acids in a behavioral test on mice

        <P> Placenta extracts are used for their health benefits; however, the anti-fatigue effects of placenta have not been elucidated. Thus, we investigated the anti-fatigue effects of porcine placenta extract (PE) and the amino acids present in the PE (glycine, Gly; proline, Pro; glutamic acid, GA; and arginine, Arg) using a forced swimming test (FST) and a tail-suspension test (TST) on mice. Whole PE or individual amino acids decreased immobility times in the FST. PE, Pro, and Arg all lowered blood levels of lactic acid and alanine aminotransferase (ALT). PE and Gly improved glycogen content and catalase activity. As determined from the serum after the FST: PE regulated the effects of interferon (IFN)-γ and tumor necrosis factor (TNF)-α; GA regulated the effects of IFN-γ; Gly and Arg regulated the effects of interleukin (IL)-6; and all of the amino acids present in PE regulated the effects of TNF-α. As determined from the spleen after the FST: Gly and Arg regulated the effects of IL-1β; Gly, Pro, and Arg regulated the effects of IL-6; PE and all of the amino acids present in PE regulated the effects of TNF-α. After the TST, PE and all of the amino acids present in PE reduced immobility duration as well as levels of aspartate aminotransferase and ALT. As determined from the serum after the TST: PE and Gly regulated the effects of TNF-α; Gly and Arg regulated the effects of IL-1β; Gly, Pro, and Arg regulated the effects of IL-6; PE and all of the amino acids present in PE regulated the effects of TNF-α. These results suggest that PE should be considered a candidate anti-fatigue agent. </P>

        Taurine, a major amino acid of oyster, enhances linear bone growth in a mouse model of protein malnutrition.

        <P>Oysters (Oys) contain various beneficial components, such as, antioxidants and amino acids. However, the effects of Oys or taurine (Tau), a major amino acid in Oys on bone growth have not been determined. In the present study, we evaluated the effects of Oys or Tau on linear bone growth in a mouse model of protein malnutrition. To make the protein malnutrition in a mouse, we used a low protein diet. Growth plate thickness was increased by Oys or Tau. Bone volume/tissue volume, trabecular thickness, trabecular number, connection density, and total porosity were also improved by Oys or Tau. Oys or Tau increased insulin-like growth factor-1 (IGF-1) levels in serum, liver, and tibia-growth plate. Phosphorylations of Janus kinase 2 (JAK2) and signal transducer and activator of transcription 5 (STAT5) were increased by Oys and by Tau. These findings show that Oys or Tau may increase growth plate thickness by elevating IGF-1 levels and by promoting the phosphorylations of JAK2-STAT5, and suggest that Oys or Tau are growth-promoting substances of potential use in the food and pharmaceutical industries.</P>
