농촌병원과 도시 종합병원 외래환자의 수진행동에 관한 비교연구

In order to know the sick role behavior of outpatients who visiting the Hospitals in City and Rural, this questionnaire survey was conducted from Dec, 1990 to Feb, 1991 in the five selected hospitals; 3 general hospitals in Taejeon City al d 2 general hospitals in Chungnam provincial for 876 outpatients. The major findings of this survey were as follows: 1. A total of 49.3 % of the study population were male and female were 50.7 % and average age were 40.14 years old. 2. The average time spent on diagnosis and treatment in hospitals was 138.6 minutes, the spent times were longer in hospitals in city than one in Rural area. 3. The average time spent on diagnosis and treatments by departments was 168.7 min. in internal medicine, 136.1 min. in orthopedic surgery, 104.4 min. in obstetrics and gynecology, 86.8 min. in general surgery, 81.4 min. in pediatrics. 4. The previous medical facility from which patients were transport were 37.2 % in clinic, 16.4 % in hospital, 4.2 % in health center and subcenter and 38.8 % in no transfarted medical facility. 5. The Routes of hospital choice were 30.9 % in patients' own first choice, 35.8 in transferred from other facility on patients' own decision, 26.5 % in recommended by other medical facility. 6. The reasons of hospital choice were 32.1 % in previous experience and familiarity, 26.6 % in bad treatments in previous hospitals, 28.0 % in good facility and equipment. 7. The dissatisfaction against currently treating hospital were 58.9 % in long waiting time, 16.4 % in insufficient explanation of disease, 16.5 % in unkindness, 13.4 % in diagnosis time is too short, 26.6 % in no dissatisfaction. 8. The attitude on medical care delivery system were 34.8 % in acceptable, 23.4 % in not acceptable, 36.5 % in don't know.
급성 충수염으로 진단된 예측 불가 염증성 맹장 종물에 대한 외과적 고찰

Purpose: An unexpected inflammatory cecal mass of uncertain etiology was encountered during surgery for presumed appendicitis. In this case, the surgeon couldn't exclude differential diagnosis which were severe appendicitis involving the cecal region, diverticular disease, other inflammatory bowel diseases, and neoplasm. Therefore, this finding leads to a therapeutic dilemma for the surgeon, when making surgical decisions during the operation. The aim of this study was to resolve this dilemma, a retrospective review was undertaken in this study. Methods: A retrospective study was performed to review the management of this problem at Soonchunhyang University Chunan Hospital for eight and half years from July 1991 to December 1999. A review of the emergency operation records identified 50 patients who had undergone ileocecal resection or right hemicolectomy for an inflammatory ileocecal mass of uncertain etiology when operated on for probable appendicitis. Results: Altogether, 39 patients underwent ileocecal resection, and 11 patients underwent right hemicolectomy. The final pathologic diagnosis was appendiceal abscess in 32 patients (63%), cecal diverticulitis in 7 patients (14%), neoplasm in 6 patients (12%), appendiceal phlegmon in 3 patients (6%), ileal tuberculosis in 1 patient (2%) and Anisakiasis in another patient (2%). The neoplasm of the 6 neoplasm patients was ascending colon cancer for 4 patients and cecal cancer for 2 patients. There was no mortality in this group. Conclusion: Although most inflammatory cecal masses are caused by benign disease, all cases in which the intraoperative diagnosis is unclear, any pathologic diagnosis including neoplasm cannot be ruled out. So this suggest ileocecal resection or right hemicolectomy to remove a possible underlying malignancy, and to decrease morbidity and mortality.

수두(chicltenpo즈) 및 대상포진(shingles)의 예방을 위해 사용되는 수두 생 바이러스 백근은 현재 국내 -외 제조사들이 정상사람 2배체 폐 태아 세포 주들을 이용하여 바이러스를 배양 · 증식시켜 생산하고 있으나, 품질관리를 쒸한 역가 시험에는 t·fRC튼 띤 LB딘EL 세포 주외에 제조사가 설정한 세포 주를 이용하고 있으며, 역파신험 방법인 fH Ui'rFO Plaque 3SSaIT에도 조사별로 다소 차이가 있는 시헌법을 사용하고 있다. 그러므로 일반적으로 세포 주간우 감수성 차이, 계대력(passa딩e level)간의 차이,varicelta-zoster virus(4'2V) diluent medium 등에 의한 차이에 의해 역가시힘 결과에 많은 차이를 나타낼 수 있다. 이에 본 연구에서는 역가 시험 결과꼭 편차를 촉소화 할 수 있는 시헌방법을 극힙하고자 Ok퍼 백신 주와 세로 주간 및 기타 요인에 따른 차이를 비교 분석하고 이를 표준확하여 일관겅있는 국가 및 제조사의 품질관리예 활용하고 나아가 수두 생 바이러스 백신 역가시헌 국가 표준품 거달에 활용하고자 하는데 목적이 운.다. 수두 생 바이러스 빅신의 역가 시험에 영향을 띠칠 수 꿀.는 모민으로 우선 바이러스 흡착에 사용되는 1#Z)·'diluent medium이 가장 큰 오인으로 확인되.고. 현개까기 최종적으로 if4 sucrose, 10% FBS, 0.1% sodiun) rrlonog)utamate in PBS(-), fIH 7.2가 가당 두수글 L'z).「 린luent medium 조성으로 확인되었단. 또한 '·:~l포 주별로 다소의 차이는 잇.으나 최대 바이러스 흡착시간은 90분으론:÷잰착인 되었다. 다른 negati)Fe effect를 보일 수 있는 요인으로 항생물질 및 항츤 제인 fuugiBone이 곡이러스의 역가에 영향을 미치는 것으로 확인되었다 따라서 이러한 항생물 띤 항균제를 standard solid agarose overla?·(5.tO) 시헌방법에는 사용하지 않는 것이 역가 시험의 편차를 줄일 수 있는 요인이라 판단된다. 최적의 조건하데서 Oka 수두 백신 주의 j 종의 세포 쭈에 대한 역가시험 수행 결과 각 세포 주의 감수성의 차이는 크지 않았으나, 반복시헌에 대한 표준편차는 다소 차이를 나타났다. Until now in Korea, the international and domestic vaccine maufacturers produce and sale live attenuated daricella vaccines, used to prevent chickenpox and shinglees, which are propagated in and done the potency test with various originated human diploid embryonic lung cells. Also manufacturers perform in vitro plaque assay with a little different methods to their quality control tests. Therefore, in general, there is slight or big difference among in vitro plaque assay results because of the difference in susceptibility of various originated human diploid embryonic lung cells and in assay methods. The purpose of this study is to confirm what factors occurring deviation to establish the standardized method which minimize deviation intracellularly and intercellularly. Eventually the standadized method will be used in consistent national and manufacturer's quality control and moreover intra and inter laboratory's validation assay of national standard for live varicella vaccine candidate. We confirmed VZV diluent medium was the biggest factor influencing in vitor plaque assay and the excellent composition of VZV diluent medium was final 5% sucrose, 10% FBS, 0.1% sodium monoglutamate in PBS(-), pH 7.2. We confirmed 90 minutes was maximum and optimum VZV adsorption time, but a little different among cells Also we recommended not use antibiotics and antimycotic, fungizone in standard solid agarose overlay assay in varicella potency test. Eventually 5 different originated human diploid embryonic lung cells showed little difference in susceptibility to Oka vaccine strain, but showed a lot of difference in reproducibility.
상징적 공간 탐사 기법을 이용한 상향적 및 하향적 주의 효과 측정

상징적 공간 탐사과제를 이용하여 시각적 주의의 상향적 및 하향적 요소를 3개의 실험을 통하여 측정하였다. 실험 1에서는 탐사자극 이전에 돌발자극을 제시하여 주의가 상향적으로 유도되도록 조작하였으며, 실험 2에서는 1차 탐색과제로 단순 색으로 정의된 표적자극과 방해자극을 제시하여 주의의 상향적 효과와 하향적 효과 모두를 측정하였으며, 실험 3에서는 탐색과제에서 탐색자극의 상향적 정보 차이를 최소화한 채 하향적 효과를 주로 측정하였다. 실험결과, 상향적 및 하향적 주의효과가 상징적 공간 탐사 기법을 통하여 측정 가능함이 나타났으며, 기존의 연구 결과와 유사한 방식으로 이 두 기제의 시간적 및 공간적 작용 방식에 각기 차이가 있음을 보였다. 이러한 결과는 공간적 주의가 아날로그적 공간 표상 뿐 아니라 상징적인 공간 표상과 같은 다양한 공간 표상 수준에서 작동할 가능성을 시사해 준다. Three experiments were performed to examine bottom-up and top-down attentional effects, using a symbolic spatial probe task. Experiment 1 measured bottom-up factors. Probe responses were faster when the arrow pointed to an abrupt onset location than to blank locations, indicating that symbolic spatial representation is affected by the bottom-up activation of an abrupt onset stimulus. In experiment 2, both top-down and bottom-up attentional effects were measured. Probe results showed that the target location received more attention with both SOA conditions (133msec and 317msec). In experiment 3, we measured top-down control of attention, and found again the benefit of the target location, but only with the long-SOA condition (317msec). The symbolic spatial task in the current study could measure the bottom-up and top-down mechanisms of visual attention that develop differently over time and space. Moreover, these results suggest that spatial attention operates on many different spatial representations such as a symbolic spatial representation as well as an analogical spatial representation.

In comparison with gasoline engine, LPG direct injection engine can have advantage in emission and fuel efficiency, and solve the problems which conventional LPG engine has, like power decrease and back fire etc. So, Direct injection engine technology is applied to LPG engine, it can show the solution of ULEV emission regulation. In this paper, the in-cylinder spray characteristics of LPG fuel has been studied in the CVC as a preliminary research for the development of low emission and high fuel efficiency of LPG engine in near future. During the direct injection of LPG fuel into the inside of CVC, both droplet development and vaporization characteristics according to the injection condition have been photographed by the high speed shadow graph and schlieren methods. The results are compared with the case of gasoline fuel.
산소 혼합비에 따른 Ba_(0.7)Sr_(0.3)TiO₃박막의 유전체 특성

Ba과 Sr의 화학당량비를 조절하여 Ba_(0.7)Sr_(0.3)TiO₃분말 시료를 제조하여 각각 1,200℃와 1,300℃에서 calcining과 sintering을 실시한 후 3 inch 성형틀로 Ba_(0.7)Sr_(0.3)TiO₃인 sputtering target를 제작하였다. MIM구조의 capacitor를 위하여 하부전극을 Pt(1500Å)/TiN(2000Å)/SiO₂/Si로 하고, 제작한 target를 이용하여 RF sputtering방법으로 BST박막을 증착하였다. 제작한 박막의 결정구조를 X-선 회절계(XRD)로 분석한 후 Al을 상부전극으로 증착하여 축전기구조로 C-V와 I-V의 전기적 특성을 얻었다. 제작된 박막의 결정구조를 XRD 측정으로 확인한 결과 550℃ 이상의 기판온도에서 결정성장이 양호함을 보였다. 유전상수와 유전손실이 기판온도와 sputtering 가스의 산소 혼합비에 영향을 받는 것으로 확인되고 I-V를 통한 전기적 특성에서 전도기구가 Schottky 장벽에 의한 것과 높은 인가전압에서 space charge limited (SCL) 전도 특성을 보였고, 누설전류의 값은 표면구조의 영향으로 산소가스의 증가에 따라 감소하였다. Ba_(0.7)Sr_(0.3)TiO₃powders were prepared by changing stoichiometry of Pb and Ba. After they were calcined and sintered at 1200℃ and 1300℃ respectively, BST sputtering targets were made in 3 inch mould. For MIM structure capacitors, Pt(1500Å)/TiN(2000Å)/SiO₂/Si were used for bottom electrodes and the BST thin films were sputtered on bottom electrodes. After we analysed their structures by XRD, Al was evaporated on the BST films for MIM capacitor and we investiaged the electrical properties by measuring of C-V and I-V. We found that the structures of the films showed good crystalllinity above 550℃ from the results of XRD measurements. We supposed that the dielectric constant and dielectric loss of films were affected with substrate temperature and oxygen ratio of sputtering gas. In I-V behavior, we supposed that the conduction mechanism was Schottky conduction and space charge limited(SCL) conduction in high voltage range.
부착규조류를 이용한 남한강 하류수계의 생물학적 수질평가

The study was the research about the epilithic diatoms and physco-chemical factors at 5 sites of South Han river. The density of Chlorophyll-a showed from site 2 showing the highest value and the rest sites showed 12.9-10.9ug·cm^(-2), its values are a relatively low value from site 2. The values of T-N was 2.494-4.037mg·ℓ^(-1), showing from site 4 the highest value. T-P was 0.017-0.041mg·ℓ^(-1) showed the highest value from site 4. Site 5 from the DAIpo was 81.2, showing the highest value and rest of sites was from 41.6-57.0. When DAIpo was divided into grades from A to D, site 5 was B, the rest sites were C. In the grades of TDI, site 1 and 2 were grade C, site 3 and 4 showed grade D, and site 5 showed grade B. As shown above, DAIpo and TDI did not match in site 3 and 4, and the water was relatively polluted compared to la and Ib in BOD. The difference among BOD DAIpo, and TDI signifies that there has been a change in the indicator community, It can be concluded for DAIpo and TDI to be used together to determine the water pollution because DAIpo is instable with the water quality determination of the water mixed area.

본 연구에서는 관절 내시경 시스템 개발을 목표로 임상 교수들의 조언과 사용자의 편의를 우선으로 하는 PC기반 GUI 형태의 관절 내시경 시스템 설계에 초점을 맞추어 연구하였다. 관절 내시경 시스템의 성능 향상을 위한 모듈 설계하여 검증을 통한 시험 및 시뮬레이션 분석을 하였다. 또한 임상 교수의 관절 내시경 시스템 개발 참여로 사용자 중심의 편리한 시스템 구성 및 관련 H/W 및 S/W 기술 축적뿐만 아니라 PC기반 GUI 형태의 윈도우 S/W 개발로 해외 시장개척을 위한 경쟁력이 강화될 것으로 기대된다.
