Purpose: Hypertension is a common problem in maintenance hemodialysis (HD) patients. We assessed the effects of low sodium dialysate on changes of blood pressure in maintenance HD patients. Methods: Forty HD patients were enrolled in this cross-over study. All the patients underwent nine consecutive HD sessions with the dialysate contained 138 mEq/L sodium (conventional sodium HD), then concentrations of sodium were switched to match the patients average pre-HD plasma sodium measured during the conventional sodium phase (135 mEq/L for patients with sodium levels less than 137, 137 for patients with sodium levels over 137). Dry weight and dialysis prescription were not modified during the six weeks of the study. Results: There was a significant decrease in the interdialytic weight gain (2.4±0.9 kg vs. 2.0±0.7 kg, p<0.001) and the interdialytic thirsty in low sodium HD sessions compare to conventional sodium HD sessions. Pre-HD systolic and diastolic blood pressure (BP), post-dialysis systolic BP was similar in both periods of the study. The use of low sodium dialysate is associated with significantly lower systolic BP in patients with uncontrolled hypertension (n=10,157.1±3.6 mmHg vs. 148.0±9.4 mmHg, p= 0.011), but not in those with controlled hypertension. Cardio-thoracic ratio was significant decrease in low sodium dialysate HD (0.53±0.08 vs. 0.51±0.07, p=0.002). The episodes of intradialytic hypotension and related symptoms were not more frequent in low sodium dialysate HD. Conclusion: Low dialysate sodium concentration based on predialysis sodium levels of patients could reduce the pre-HD systolic BP, interdialytic thirsty and interdialytic weight gain in maintenance HD patients.

The objective of this research is to survey the perception trend analysis of resort among various accommodations by analyzing big data of resort-related data from social media (SNS) and to provide resort industry practical and useful information on how to respond on the result from the big data analysis. In order to do that, this paper analyzed the followings: 1) Network analysis with degree centrality, closeness centrality, betweenness centrality and eigenvector centrality by using Textom, UCINET6 and NetDraw, and 2) CONCOR analysis to find clusters and to see what kinds of relationships are present among them. As a result, words such as `event`, `resort`, `travel`, `package`, `family`, `family travel`, `reservation`, `goods`, `discount`, `preparation`, `tourist site`, `utilization`, `recommendation`, `ti-me` are top ranked in frequency ratio and all of centrality ratios. Especially, `event`, `resort` and `travel` played a key role. Therefore, from the point of suppliers` view, it suggests that the suppliers should prepare various information to customers regarding those words. Cluster of words of `resort`, `family`, `package`, `hotel`, `travel`, `family travel`, `jeju`, `re-servation`, `price`, `ocean`, `vacation`, `moment`, `spa`, `luxury` is organized of those very high on web visibility. This cluster is correlated to all of 5 other clusters. Consequently, the resort management companies in Jeju island could utilize the findings such that customers who want to visit a resort in ocean area are very interested in family travel package offered by the ocean resorts for their marketing promotions.
방풍통성산(防風通聖散)이 아토피 피부염을 유발한 동물모델의 피부 손상에 미치는 영향

Objectives : Atopic dermatitis has a close relationship with damage of skin barrier function. To investigate the effects of Bangpungtongsungsan(BT) extract to the skin damage on mice model after atopic dermatitis elicitation, this study was done through forcing injury to mice's skin. Methods : The BALB/c mice were distributed into three groups: control(CON) group, atopic dermatitis(AD)-elicited group, Bangpungtongsungsan(BT)-treated group. AD-elicited and BT-treated group were caused AD according to the method of Christophers E., Mrowietz and Minehiro. The BT extract was administered for 48 hours to BT-treated group. We observed changes of external dermal formation, eosinophils in vasculature, lipid formation in stratum corneum, distribution of ceramide, distribution of capillary, $I{\kappa}B$ kinase(IKK) and induce nitric oxide synthase(iNOS) mRNA expression. We used the statistical methods of student t-test(p<0.05). Results : After dispensing BT extract into the AD-elicited group, the number of eosinophil as an atopic index in mice noticeably decreased and dermal injury decreased. Also the decrease of hyperplasia, degranulated mast cells, angiogenesis and substance P were shown. The lipid lamellae, lipid protect formation, were repaired and the distribution of ceramide which inhibit protein kinase C(PKC) activation increased, and the PKC caused inhibition of nuclear $factor(NF)-{\kappa}B$ activation. As a result of inhibition of $NF-{\kappa}B$ activation, iNOS production were inhibited and apoptotic cell were increased. Moreover the decrease of IKK and iNOS mRNA expression in BT-treated RAW 264.7 cell were noted. Conclusion : BT mitigated skin damage on mice model after atopic dermatitis elicitation through recovering skin barrier function and inhibiting nuclear $factor(NF)-{\kappa}B$ activation.
PPAR-$\gamma$ 조절을 통한 여택통기탕(麗澤通氣湯)의 항알레르기와 항염증 효과

Objectives : The aim of this study is to investigate the antiallergic effects of YTT on allergic rhinitis by regulation of peroxisome proliferator-activated receptor gamma (PPAR-$\gamma$). Methods : The thirty rats were divided into three groups : normal group, control (allergic rhinitis elicited) group, sample (Yeotaectonggi-tang treated after allergic rhinitis elicitation) group. To induce allergic rhinitis in control group and sample group, rats were sensitized intraperitoneally with 0.1% ovalbumin(OVA) solution 3 times at intervals of 1 week. Then intranasal sensitization was performed by diffusing 0.1% ovalbumin(OVA) solution 3 times at intervals of 2 days. After that time rats in sample group were oral administration treated by YTT for 7days. The change of the amounts of eosinophil, substance P, MIP-2, PPAR-$\gamma$, IL-4, iNOS were observed in each group. we used the statistical method of ANOVA test(p<0.05). Results : The number of eosinophil in sample group noticeably decreased than control group. And the decrease of substance P and MIP-2 positive reaction were observed in mucosa. YTT inhibited IL-4 and iNOS production, mucus secretion, activation of mast cells and fibrosis remodeling by regulation of PPAR-$\gamma$ activation. Conclusion : According to the above results, it is considered that YTT mitigated mucosa damage on the mice model with allergic rhinitis by regulation of PPAR-$\gamma$.

Liddle`s syndrome is a rare inferited disease with characteristic clinical manifestations of hytertension an hypokalemic metabolic alkalosis. Markedly suppressed serum aldosterone and renin levels are important laboratory findings to differentiate this disorder from primary hyperaldosterionism. When Liddle et al. reported the disorder in 1963, they proposed aggressive Na+ absorption and increased excretion of K+ as the pathogenesis of the syndrome. Since then, specific mutaion in the epithelial Na+ channel located in the collecting duct of the kidney has been dlucidated as a disease mechanism. Liddle`s syndrome is inherited by an autosomal dominant trait and generally the onset of the syndrome is before the age of 20 with increased risk of premature death due to stroke or heart failure. Recently, however, a few cases of late onset and genetically proven non-familial cases with de novo mutation of β or γ Na+ channel have been reported. We report a case of seventy-one year old woman who had hypertension with hypokalemic metabolic alkalosis and was diagnosed as Liddle`s syndrome. Further evaluation revealed low renin and aldosterone levels. Primary aldosteronism, Cushing`s syndrome, glucocorticoid remediable aldosteronism and deficiency of 11 β-OHase and 17α-OHase were ruled out based on her laboratory data and history. Her hypertension and hypokalemia responded to amiloride treatment but not to spironolactone. (Korean J Nephrol 2003;22(4):464-468)

BACKGROUND/OBJECTIVES: We investigated the anti-osteoarthritic effects of deer bone extract on the gene expressions of matrix metalloproteinases (MMPs) and collagen type II (COL2) in interleukin-$1{\beta}$-induced osteoarthritis (OA) chondrocytes. MATERIALS/METHODS: Primary rabbit chondrocytes were treated as follows: CON (PBS treatment), NC (IL-$1{\beta}$ treatment), PC (IL-$1{\beta}+100{\mu}g/mL$ glucosamine sulphate/chondroitin sulphate mixture), and DB (IL-$1{\beta}+100{\mu}g/mL$ deer bone extract). RESULTS: The results of the cell viability assay indicated that deer bone extract at doses ranging from 100 to $500{\mu}g/mL$ inhibits cell death in chondrocytes induced by IL-$1{\beta}$. Deer bone extract was able to significantly recover the mRNA expression of COL2 that was down-regulated by IL-$1{\beta}$ (NC: 0.79 vs. DB: 0.87, P < 0.05) and significantly decrease the mRNA expression of MMP-3 (NC: 2.24 vs. DB: 1.75) and -13 (NC: 1.28 vs. DB: 0.89) in OA chondrocytes (P < 0.05).CONCLUSIONS: We concluded that deer bone extract induces accumulation of COL2 through the down-regulation of MMPs in IL-$1{\beta}$-induced OA chondrocytes. Our results suggest that deer bone extract, which contains various components related to OA, including chondroitin sulphate, may possess anti-osteoarthritic properties and be of value in inhibiting the pathogenesis of OA.
