Living with Atrial Fibrillation: An Analysis of Patients' Perspectives

Purpose: The aim of this study was to determine the perceptions of patients with atrial fibrillation regarding the disease, to reveal their feelings, thoughts and wishes, and to investigate their perspectives and coping behaviors towards their condition. Methods: Phenomenological methodology was used. The study population consisted of a total of 225 patients treated by the cardiology department of a university hospital, while the study sample consisted of 32 patients who met the inclusion criteria. A semistructured interview addressed perceptions of patients with atrial fibrillation regarding the disease. Data were collected by asking the participants the three questions on the In-depth Individual Interview Form. Data were analyzed using the continuous comparative method of Colaizzi. Results: In the study sample, 50.0% of participants were female, 69.0% were married, and the mean age was 66.90 years (± 7.90 years). As a result of the content analysis, four main themes and 15 subthemes were identified: patient's mental status regarding the disease, patient's social status regarding the disease, patient's physical condition regarding the disease, and disease management and coping with the disease. The study found that individuals with atrial fibrillation faced major limitations in their daily living activities and social lives due to the disease symptoms and warfarin use. Conclusions: Patients need to be provided with relevant individual training and counselling so that they lead more satisfactory lives. In addition, appropriate health appointment and monitoring systems should be developed for patients to reduce the problems associated with frequent follow-up appointments.
Reliability and fatigue life evaluation of railway axles

The axle is one of the most important components of a rail vehicle which transmits the weight of the vehicle to the wheels, meets the vertical and horizontal loads formed during static and dynamic moving, and carries the driving moment and braking moment. The prediction of fatigue failure of axles plays an important role in preventing fatigue fractures. Varying loads on components lead to cumulative failure in the mechanism. In this study, failures in axles of rail vehicles serving the Istanbul Transportation Co. have been investigated. Statistical evaluation of real life values has been performed by taking into account the kilometer and load cycle. Equivalent stresses have been used to derive life equations and diagrams by using one of the cumulative life theories known as the Palmgren-Miner method. Finally,theoretical and practical Wohler diagrams S-N (σ-N: stress-life) have been plotted to reveal error calculation.
A New Approach to the Treatment of Metastatic Paraganglioma: Sorafenib

Paragangliomas are relatively rare chromaffin cell tumors which may be cured throughresection. Patients with paragangliomas may develop metastatic diseases. There isno consensus regarding refractory chemotherapy for treatment of metastatic disease. In this report, we presented a case of a 43-year-old woman who was admitted to thehospital with a history of episodic headaches, diaphoresis, and weakness. Elevatedplasma catecholamine levels and a right paraaortic mass were observed on computedtomography. The mass was excised, and a diagnosis of paraganglioma was confirmed. After 20 months of follow-up, local recurrence and metastases were detected in thethorax, abdomen, and skeletal system. Plasma and urinary catecholamine levels werehigh. Chemotherapy was administered, and no improvement was observed. Therefore,following this palliative conventional chemotherapy, sorafenib was administered forthree months, and, finally, positron emission tomography showed that the patient’slesions had completely regressed.

Background: Adrenal neoplasms are common and are incidentally discovered in 4-10% of abdominal imaging studies. Most of them are not cancerous and nonfunctional. Every adrenal incidentaloma is evaluated with function tests for functionality. In our study we aimed to investigate the relation of tumor markers which were measured for other reasons and function tests in nonfunctional adrenal adenomas retrospectively. Methods: Patients diagnosed with nonfunctional adrenal adenoma in the last three years in our research hospital were evaluated retrospectively. Data of 133 patients including function tests (renin -aldosterone-17OHprogesterone-testosterone-dehydroepiandrosteronesulphate levels, 24 hour urinary metanephrine and normetanephrine levels, overnight dexamethasone supression test) were evaluated. Fortyfi ve of these 133 patients had tumor markers (CEA, alpha-fetoprotein, CA 19-9, CA15-3, CA125) recorded. The relation between function tests and tumor markers are examined. Results: The data of 31 women and 16 men with mean age 55,54±10,64 years and 56,06±11,41 years respectively were evaluated. There was a negative correlation between age and CA-125 levels. A negative correlation was also found between testosterone and CA15-3. There was a positive correlation between urinary cortisole and CEA and also between aldosterone and CA19-9 levels. Conclusions: Tumor markers may be used as alternative tests for function tests. But future studies with big sample size are needed to confi rm this hypothesis.

Background: It has been reported that MPV is increased in diabetes while the status of MPV in prediabetes is not well known. It has been reported that decreasing level of paraoxonase-1 is related to increased probability of development of atherosclerosis and cardiovascular disease (CVD). In this study we compared the mean platelet volume (MPV), paraoxonase-1 (PON) and arylesterase (ARE) levels in patients with the diagnosis of prediabetes and diabetes mellitus (DM). Methods: Forty five prediabetic patients and 135 type 2 diabetic patients were enrolled into the study. Height, weight, BMI, age, gender, smoking, alcohol, exercise, family history of DM and CVD, childbirth, menopause status of all patients were recorded. Routine biochemical tests and A1c, lipid profiles, homocysteine, PON, ARE and MPV levels were performed. Results: Mean age of diabetics and prediabetic patients are 53.4 ±13.7 and 53.0±12.8 respectively. Age, weight, height, BMI and MPV levels were not significantly different in both groups but PON, ARE, A1c and HOMA levels were significantly different (p=0.0001). There were no significant differences in smoking, hypertension, hyperlipidaemia, exercise, coronary heart disease between the two groups. There was no significant correlation between MPV and PON, ARE but there was a negative correlation between Alc, glucose, WBC values and PON, ARE. We also found a negative correlation between triglyceride and PON levels and microalbuminuria and ARE levels. Conclusions: There was no difference in MPV level in both groups having different risk of atherosclerosis and probably because of the limited number of prediabetic patients in our study. However, lower levels of PON and ARE may have been caused by high risk of atherosclerosis in diabetic patients. It would be more reliable to analyse MPV, PON and ARE according to biochemical and clinical conditions.
Local Challenges in Global Korea : Rebalancing with Leverage

This paper analyzes the implications of global rebalancing in the post-crisis period for Korea and how high leverage in the household and SME sectors could affect this process. Consumption and investment in Korea appear in line with that of peers, but there are limits how much they could be sustained in the post crisis world to pick up the slack from a lower external demand. To avoid the build-up of vulnerabilities from high leverage, households have to increase their saving rate limiting consumption growth. On the other hand, exportoriented and large corporates may need to invest less with lower export demand increasing corporate savings further. This would leave SMEs in the non-tradable sector as the engines for investment, employment and household income growth, increasing the urgency to address their long-standing structural problems and weak balance sheets. Delaying the necessary adjustment would increase costs and financial vulnerabilities. Improving social safety nets and the pensions system would be important to manage the adjustment costs while increasing labor market flexibility.
THE CATENARY DEGREE OF THE SATURATED NUMERICAL SEMIGROUPS WITH PRIME MULTIPLICITY

In this paper, we formulate the set of all saturated numerical semigroups with prime multiplicity. We characterize the catenary degrees of elements of the semigroups we obtained which are important invariants in factorization theory. We also give the proper characterizations of the semigroups under consideration.
