      • Post Intensive Care Syndrome in Parents of Critically Ill Pediatric Patients

        Aim(s): Family members can suffer from psychological symptoms and social problems when their loved one is in the intensive care unit. The symptoms experienced by families of critically ill patients are called "post-intensive care syndrome-family." The purpose of this study was to describe the symptoms of post intensive care syndrome-family in parents of critically ill pediatric patients. This study examined (1) psychological and social problems, (2) changes in symptoms over 1 month, and (3) the differences in symptoms by the participants’ characteristics. Method(s): A longitudinal descriptive study design was used for a pediatric intensive care unit in a tertiary care hospital in Korea. Parents of children admitted to the pediatric intensive care unit for at least 48hours were questioned on the day of the child’s discharge from the pediatric intensive care unit and 1 month after discharge. Anxiety, depression, post-traumatic stress symptoms and social impacts were measured as symptoms of post intensive care syndrome-family. Result(s): Among 41 participants, 36 completed the follow-up survey. Most of the parents reported all three psychological problems, anxiety, depression, and post-traumatic stress symptoms, on the day of the child’s discharge from the pediatric intensive care unit (77.8%) and more than a half (58.3%) on 1 month after discharge. Social problems significantly increased over time (p=.04). Factors associated with symptoms of post intensive care syndrome-family were emotional support from the staff of the pediatric intensive care unit. Emotional support showed a significant negative correlation with post-traumatic stress symptom(p=.003) and social problems(p=.02). Conclusion(s): This study revealed a high prevalence of psychological symptoms and increased social problems among parents of critically ill pediatric patients after discharge from the pediatric intensive care unit. Emotional support from health care professionals can help reduce symptoms of post intensive care syndrome in parents of children in pediatric intensive care units.

        PROTECTION OF CELEBRITY RIGHTS: A COMPARATIVE ANALYSIS OF RELEVANT IPR LAWS IN US, UK AND INDIA

        This paper is a modest endeavour to explain a comparative analysis of laws and judicial trends for protection of celebrity rights from U.S., U.K. and India. This paper is based on the evaluation of multi jurisdictional regime of protection granted to celebrity rights, with the analysis of precautions undertaken for balancing the public and private interest while protecting celebrity’s persona and identity. Further, the need for international recognition and universal application of celebrity rights protection forms to be an important aspect of the paper. The concluding remarks derived are firstly, in comparative analysis it can be found that India is at nascent stage of development for protecting publicity rights with no specific protection granted, U.K. does not recognize the concept of publicity rights on one hand and on the other U.S. has given due recognition to publicity rights protection and provided statutory mandate of protection in its twenty two states; Secondly, Publicity right requires a specific statutory protection as other laws fail to protect the right in complete sense. Lastly, there is need of international treaty for publicity rights protection to provide the right international standard and develop uniform mandates of protection in different jurisdictions.

        Capital Structure Decisions Following Credit Rating Changes: Evidence from Japan

        Our study adds to the body of knowledge about the relationship between credit ratings and the capital structure of bond issuers. Using Bloomberg and Datastream databases and employing panel regression models, we study the capital structure changes of Japanese enterprises after credit rating changes by global rating agencies (S&P and Moody’s) as well as their local counterparts (R&I and JCR) from 1998 to 2016. We find that after rating downgrades, Japanese enterprises considerably reduce net debt or net debt relative to net equity, similar to the findings of Kisgen (2009), who focused on U.S. industrial firms. They do not, however, make adjustments to their financial structure as a result of rating improvements. In comparison to downgrades by S&P and Moody’s, Japanese corporations issue 1.89 percent less net debt and 1.50 percent less net debt relative to net equity after R&I and JCR rating downgrades. To put it another way, Japanese companies consider rating adjustments made by local agencies to be more significant than those made by global rating organizations. Our findings contradict earlier research that suggests S&P and Moody’s are more prominent in the investment community than R&I and JCR in Japan.

        Creative Apparel Design Process Approach Using Architectural and Artistic Influence

        Creativity is an ever-increasing research area related to fashion design, namely the documentation and dissemination of creative scholarship. The purpose of this project is to apply creative bridge or creative leap theory to the development of an existing designer’s garment collection using the creative design process of an architect. A single case study design was used to investigate the phenomenon of design process creative leap using a direct observation. Results indicate the difficult challenge of producing new and individualized silhouettes for the marketability of clothing, it is apparent that new methods of fostering creativity in designers through non-traditional approaches would benefit the education of apparel designers. Through targeted design practice, educators and practitioners can apply the principles of scientific inquiry into design processes to solve a problem, address a challenge, or create based on personal inspirations.

        MyRisk_Stroke Calculator: A Personalized Stroke Risk Assessment Tool for the General Population

        Background and Purpose There is a variety of stroke risk factors, and engaging individualsin reducing their own personal risk is hugely relevant and could be an optimal disseminationstrategy. The aim of the present study was to estimate the stroke risk for specific combinationsof health- and lifestyle-related factors, and to develop a personalized stroke-risk assessmenttool for health professionals and the general population (called the MyRisk_Stroke Calculator). Methods This population-based, longitudinal study followed a historical cohort formed fromthe 1992 or 1998 Santé Québec Health Surveys with information for linkage to health administrative databases. Stroke risk factors were ascertained at the time of survey, and stroke was determined from hospitalizations and death records. Cox proportional hazards models were used,modeling time to stroke in relationship to all variables. Results A total of 358 strokes occurred among a cohort of 17805 persons (men=8181) whowere followed for approximately 11 years (i.e., –200000 person-years). The following regressionparameters were used to produce 10-year stroke-risk estimates and assign risk points: for age (1point/year after age 20 years), male sex (3 points), low education (4 points), renal disease (8points), diabetes (7 points), congestive heart failure (5 points), peripheral arterial disease (2points), high blood pressure (2 points), ischemic heart disease (1 point), smoking (8 points), >7alcoholic drinks per week (3 points), low physical activity (2 points), and indicators of anger (4points), depression (4 points), and anxiety (3 points). According to MyRisk_Stroke Calculator, aperson with <50, 75, and 90 risk points has a 10-year stroke risk of <3%, 28%, and >75%, respectively. Conclusions The MyRisk_Stroke Calculator is a simple method of disseminating information to the general population about their stroke risk.

      • Personalized Activity Tracking System: Improving Situation Awareness for Anesthesiologist`s Task within the Surgical Team

        In surgery department, as tight daily activities make the critical and crucial situation, some immediate actions with careful handling are needed to be monitored. Therefore, every personnel should be aware of the environment and situation around, in the term of temporal, spatial, and personage. We proposed a personalized activity tracking system that help each personnel track down his or her activities, and perceive how each team member\`s activities can influence one another. Although we intended to make the system for each stakeholder\`s needs, we started designing with anesthesiologist for its significant involvement in the preoperative stage and its continuous role from before, during, and after the procedure. With the predefined scenarios, we conducted an observational study in the operating room as well as its environment, in order to get prototypical dimension of situation that lead us to identify system design variables and characterize each personnel\`s roles and responsibilities.

        Anorectal Manometry Versus Patient-Reported Outcome Measures as a Predictor of Maximal Treatment for Fecal Incontinence

        Purpose: This study aims to establish the ability of patient-reported outcome measures (PROMs) and anorectal manometry (ARM) in predicting the need for surgery in patients with fecal incontinence (FI). Methods: Between 2008 and 2015, PROMs data, including the Birmingham Bowel and Urinary Symptoms Questionnaire (BBUSQ), Short Form 36 (SF-36), Wexner Incontinence Score and ARM results, were prospectively collected from 276 patients presenting with FI. Spearman rank was used to assess correlations between specific PROMs questions and ARM assessments of sphincter motor function. Binomial regression analyses were performed to identify factors predictive of the need for surgery. Finally, receiver operating characteristic (ROC) curve analyses were performed to establish the utility of individual ARM and PROMs variables in predicting the need for surgical intervention in patients with FI. Results: Two hundred twenty-eight patients (82.60%) were treated conservatively while 48 (17.39%) underwent surgery. On univariate analyses, all 4 domains of the BBUSQ, all 8 domains of the SF-36, and the Wexner Incontinence Score were significant predictors of surgery. Additionally, maximum resting pressure, 5-second squeeze endurance, threshold volume, and urge volume were significant. On ROC curve analyses, the only significant ARM measurement was the 5-second squeeze endurance. PROMs, such as the incontinence domain of the BBUSQ and five of the SF-36 domains, were identified as fair discriminators of the need for surgery. Conclusion: PROMs are reliable predictors of maximal treatment in patients with FI and can be readily used in primary care to aid surgical referrals and can be applied in hospital settings as an aid to guide surgical treatment decisions.
