        Lateral Third Infraclavicular Implantation of the Vagal Nerve Stimulation Generator Through Axillary Incision

        Objective : This report describes the clinical study of the surgical method of lateral third infraclavicular implantation of vagal nerve stimulation (VNS) generator through the axillary wrinkle incision. Methods : In a retrospective study, the data for 20 patients with medically intractable epilepsy treated by this approach were examined. The mean age was 31.4 years (range : 14-50), and the mean follow-up period was 12.15 months (range : 4-21 months). The male to female ratio was 2.3 : 1. The subcutaneous pocket for the generator was located in the lateral third infraclavicular area through the axillary wrinkle. Our method was a modification of the standard VNS generator implantation in the mid-infraclavicular pocket through anterior axillary incision. Results : There were the excellent or good cosmetic satisfaction in 95% of the cases and fair in 5%. The generator was located outside the lung field in 15%, periphery of the lung field in 45%, and crossed over the lung field in 40%. Discomfort from shoulder motion occurred transiently in 35% of cases. Other complications were minimal. Conclusion : These results demonstrate that the lateral third infraclavicular apporach will offers cosmetic benefits and reduction of obscuration of the lung field without serious complications. Thus, this technique provides an attractive alternative among the surgical techniques for the vagal nerve stimulation.

        천연한방소재인 연교 추출물의 미백 효과에 관한 연구

        본 연구는 연교 추출물과 연교 추출물에서 분리한 활성물질의 멜라닌 생성 억제 효과에 관한 것이다. 연교 추출물은 0.1~1.0% (v/v)에서 세포독성이 없는 것을 확인하였으며 이를 바탕으로 멜라닌 생성억제 실험결과 1.0 %에서 63.1±3.1% 저해 효과를 확인하였다. 또한 활성물질을 분리하여 <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, Mass analysis 등의 기기분석 결과 리그난 계열의 활성물질인 4-[(3,4-Dimethoxyphenyl)methyl)]dihydro-3-[(4-hydroxy-3-methoxyphenyl)methyl]-2(3H)-furanone (arctigenin)으로 동정되었다. 활성물질에 대한 멜라닌 저해효과를 확인한 결과 농도 의존적으로 3.0 μg/mL에서 52.7±3.1%를 저해하는 것을 확인하였다. 또한 연교 추출물을 3 % 함유한 크림의 미백효과에 대한 임상실험을 진행한 결과 전문의의 육안평가와 기기평가 모두 통계적으로 유의한 결과를 얻을 수 있었다. 결론적으로 연교 추출물은 미백 효능을 갖는 화장품 소재로서의 개발 가능성이 클 것으로 기대된다. The aim of this study was to investigate the inhibitory effects of Forsythia suspensa fruit extracts (FSfE) (0.1~1.0%) and their active component on melanogenesis. FSfE dose-dependently inhibited melanin synthesis (up to 63.1±3.1% at the concentration of 1.0%) without cell cytotoxicity. We purifed one active compound from FSfE and identified its structure. It was identified as 4-[(3,4-Dimethoxyphenyl)methyl)]dihydro-3-[(4-hydroxy-3-methoxyphenyl)methyl]-2(3H)-furanone (arctigenin) by <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, and Mass analysis. Arctigenin also inhibited melanin synthesis in a dose dependent manner (up to 52.7±3.1% at the concentration of 3.0 μg/mL). In order to verify the whitening activity of the cream containing 3% FSfE, we performed the clinical test with twenty five female volunteers for 8 weeks. Bioengineering analyses and visual assessment by doctors at the initial time point and 8 weeks after applications showed that cream containing FSfE have significant whiteining effect (paired t-test). From the results, we conclude that the FSfE can be used as a useful whitening agent.

      • 소뇌교각부 유표피낭종에 대한 임상적 고찰

        Epidermoid tumor is rare. We reviewed 12 surgically treated intracranial epidermoid tumors of the cerebellopontine angle (CPA) between 1992 and 2005. Total resection was achieved in 11 cases (91%); in one case, some parts of the cyst capsule were left because the cyst adhered to the brainstem, vascular structure and cranial nerves from the cerebellopontine angle. The most common symptom was trigeminal neuralgia (9 cases). On their latest clinical and radiological follow-up examinations (mean 27.1months), most cases had good results without specific postoperative complications (91%). However, in one case, tumor regrowth was observed one year later and re-operation was done with a favorable result. Although epidermoid tumors in CPA are very difficult to resect totally for its surrounding neurovascular structure, a total surgical removal was possible to reduce tumor regrowth and improve outcome.

        노니 추출물의 주름개선 효과연구

        주름생성의 주요 원인은 자외선, 공기 오염, 담배 연기, 스트레스 등을 들 수 있다. 특히, 자외선은 피부에 다양한 형태로 영향을 주어 깊은 주름, 잔주름, 피부거침, 피부건조와 같은 현상을 발생시켜 피부노화를 유발시킨다. 이러한 주름 생성 해결 및 피부 노화 문제의 해결은 화장품에서 가장 활발하게 연구되는 분야 중 하나이다. 최근에 열대 식물인 노니로부터 추출한 유효성분이 주름 개선에서 가장 중요한 콜라겐 생합성을 촉진하는 효과를 확인하였으며, 특히 노니 추출물로부터 분리한 스코폴레틴(scopoletin)이 이러한 효과를 주는 활성 성분임을 확인하였다. 스코폴레틴은 인체 섬유아 세포를 이용한 콜라겐 합성 촉진 효과 시험에서 농도 의존적으로 콜라겐 합성을 촉진하는 활성을 보였다(0.2 μg/mL-89.5% 콜라겐 생합성 촉진. 또한, 주름 개선 효과를 확인하기 위해 3% (v/v) 노니 추출물을 함유한 크림을 이용하여 12주 동안 임상연구를 수행하여, 안면의 주름 감소 정도를 측정하였으며 이 제품이 주름을 감소시켜주는 뛰어난 효과가 있다는 것을 확인하였다. 이러한 결과들은 노니 추출물이 주름 개선효과가 매우 높으며, 노화 방지 원료로서 이용 가능성이 매우 높다는 것을 보여주는 것이라 할 수 있다. Wrinkle formation is mainly attributed to the environmental factors such as UV rays, air pollution, smoking and stress etc. Especially, UV rays induce premature skin aging which is characterized by deep wrinkle, leathery dryness etc. Recently, researches on the wrinkle formation and its prevention have been the main theme in cosmetics fields. We have studied the various plant extracts having anti-wrinkle effects and finally showed that Noni (Morinda citrifolia) extracts have the efficacy of promoting the type I collagen synthesis in normal human fibroblast, using PICP assay. We purified one active compound from Noni extracts and identified its structure. It was identified as 6,7-Dimethoxy-2H-1-Benzopyran-2-one; scopoletin by <sup>1</sup>H-NMR, <sup>13</sup>C-NMR, IR, Mass analysis. Scopoletin increased collagen synthesis in a dose dependent manner (89.5% at 0.2 μg/mL). In order to verify the anti-aging effectiveness of the cream containing 3% noni extracsts, we performed the in vivo test with some female volunteers for 12 weeks. It reduced the signs of aging, especially face wrinkles. From these results, we conclude that the noni extracts could be used as an useful anti-wrinkle agent.

        <P><B>Background</B></P><P>The first Korean individual diploid genome sequence data (KOREF) was publicized in December 2008.</P><P><B>Results</B></P><P>A Korean genome variation analysis and browsing server (Gevab) was constructed as a database and web server for the exploration and downloading of Korean personal genome(s). Information in the Gevab includes SNPs, short indels, and structural variation (SV) and comparison analysis between the NCBI human reference and the Korean genome(s).       • Control of naturally coupled piezoelectric and photovoltaic properties for multi-type energy scavengers

        <P>In this paper, we present a simple, low-cost and flexible hybrid cell that converts individually or simultaneously low-frequency mechanical energy and photon energy into electricity using piezoelectric zinc oxide (ZnO) in conjunction with organic solar cell design. Since the hybrid cell is designed by coupled piezoelectric and photoconductive properties of ZnO, this is a naturally hybrid architecture without crosstalk and an additional assembling process to create multi-type energy scavengers, thus differing from a simple integration of two different energy generators. It is demonstrated that the behavior of a piezoelectric output is controlled from alternating current (AC) type to direct current (DC)-like type by tailoring mechanical straining processes both in the dark and under light illumination. Based on such controllability of output modes, it is shown that the performance of the hybrid cell is synergistically enhanced by integrating the contribution made by a piezoelectric generator with a solar cell under a normal indoor level of illumination. Our approach clearly demonstrates the potential of the hybrid approach for scavenging multi-type energies whenever and wherever they are available.         실천공학 교육 방법론 : 직업훈련교사에 대한 단기교육의 만족도 결정요인 -개도국의 국내초청 직업훈련을 중심으로

        본 연구는 직업훈련교사에 대한 단기교육의 만족도를 결정짓는 요인을 살펴보는 것을 목적으로 하며, 이를 위하여 개발도상국 직업훈련교사에 대한 국내초청 단기교육의 사례를 분석하였다. 실증분석은 2008년부터 2011년까지 개발도상국 직업훈련교사들의 직업능력역량 강화를 위하여 한국산업인력공단에서 주관하는 ``ODA 국내초청 직업훈련 프로그램``에 참가한 개발도상국 직업훈련교사 226명의 설문지에 대해서 실시되었다. 실증분석결과 훈련 전수요조사 정도가 높을수록, 훈련기간이 길수록, 훈련생의 전공과 일치되고 학습수준에 적합할수록 직업훈련 단기교육에 대한 만족도가 높았다. 본 연구의 결과를 토대로 ODA 국내초청연수와 직업훈련교사 단기교육에 대한 시사점을 도출하면, 교육생의 인적특성을 고려한 프로그램이 개발되어야 하고, 교육생의 초기 학습 수준과 적성을 반영할 수 있는 세분화된 교육과정개발이 필요하다고 요약될 수 있다. The purpose of this study is to research factors determining satisfaction on the short-term training courses offered to vocational school teachers, and to analyze the case of the overseas training for developing countries. An empirical study is performed by surveying 226 trainees having participated in international HRD program of the HRD Korea during 2008∼2011. The results show that the training demand survey, training period and fitting to the trainees` major and the educational level positively affect the satisfaction on the short-term training for teachers of vocational schools. The implication of this study is that programs customized to personal attributes and educational level are needed in the ODA overseas training and the short-term training for teachers of vocational schools.
