        Korean Clinical Practice Guidelines for Aneurysmal Subarachnoid Hemorrhage

        Despite advancements in treating ruptured cerebral aneurysms, an aneurysmal subarachnoid hemorrhage (aSAH) is still a grave cerebrovascular disease associated with a high rate of morbidity and mortality. Based on the literature published to date, world-wide academic and governmental committees have developed clinical practice guidelines (CPGs) to propose standards for disease management in order to achieve the best treatment outcomes for aSAHs. In 2013, the Korean Society of Cerebrovascular Surgeons issued a Korean version of the CPGs for aSAHs. The group researched all articles and major foreign CPGs published in English until December 2015 using several search engines. Based on these articles, levels of evidence and grades of recommendations were determined by our society as well as by other related Quality Control Committees from neurointervention, neurology and rehabilitation medicine. The Korean version of the CPGs for aSAHs includes risk factors, diagnosis, initial management, medical and surgical management to prevent rebleeding, management of delayed cerebral ischemia and vasospasm, treatment of hydrocephalus, treatment of medical complications and early rehabilitation. The CPGs are not the absolute standard but are the present reference as the evidence is still incomplete, each environment of clinical practice is different, and there is a high probability of variation in the current recommendations. The CPGs will be useful in the fields of clinical practice and research.

        보호회로가 탄성적으로 부착된 전지 팩

        디지털 컨버전스로 대표되는 모바일 기기의 진화에 따라 카메라, MP3, TV, 게임기 등의 기능이 휴대폰에 탑재되었으며, 이에 따른 모바일 기기의 소비전력 증가는 차세대 초고용량 리튬이온전지의 개발을 촉진시키고 있다. 또한 환경 규제 및 유가 상승으로 인하여 하이브리드 자동차에 대한 수요가 증가하고 있고, 이에 따라 중대형 전지에 대한 관심이 집중되면서 전지의 저가격화, 고출력화, 고안전화를 이루기 위한 노력이 계속되고 있다. 본 연구에서는 리튬이온전지 패킹에 관한 기술로서 공정축소 및 생산성을 향상시킬수 있고 전지본체 CELL을 제외한 나머지 부품을 재활용 할 수 있는 특허기술을 제시하였다. 보호회로가 탄성적으로 부착된 리튬이온전지 팩은 보호회로와 전지본체의 순간적인 단락을 방지하고, 보호회로와 전지본체의 전기적인 연결을 용이하게 수행할 수 있도록 탄성적으로 부착된 전지 팩을 제공한다. As mobile devices evolve and digital convergence trend is here to stay, mobile phones are built with multiple functions including cameras, MP3s, TVs and game consoles. As a consequence, such multi-functional mobile phones come to spend more power, facilitating development of next-generation ultra-capacity lithium ion battery. In addition, environmental regulations and rising oil prices cause demand for hybrid cars to keep rising. Accordingly, more and more attention is being paid to medium and large batteries and more efforts are being made to realize lower battery prices, higher outputs and stability. This study presented a patent technology related to the lithium ion battery packing that allows reducing processes related, increasing productivity and recycling parts other than the body. The lithium ion battery pack to which protection circuits are elastically attached provides short circuit protection for the circuit and the body and makes electric connection of the circuit and the body easier.

        Perioperative Modulating Factors on Astigmatism in Sutured Cataract Surgery

        Purpose: To evaluate the factors that affect postoperative astigmatism and post-suture removal astigmatism, and to evaluate the risk factors associated with astigmatism axis shift. Methods: We performed a retrospective chart review of 130 eyes that had undergone uneventful phacoemulsification cataract surgery. Preoperative astigmatism was divided into four groups (Groups I, II, III, and IV) according to the differences between the axis of preoperative astigmatism (flattest axis) and the incision axis (105 degrees). We analyzed the magnitude and axis of the induced astigmatism after the operation and after suture removal in each group. We also analyzed the factors which affected the postoperative astigmatism and post-suture removal astigmatism in each sub-group of Groups I, II, III, and IV, excluding postoperative or post-suture removal axis shift (specifically, Group I(WAS), II(WAS), III(WAS), and IV(WAS)). We identified the variables associated with the prevalence of postoperative astigmatism axis shift and those associated with the prevalence of post-suture removal axis shift. Results: An increase in the magnitude of postoperative astigmatism was associated with an increase in the preoperative magnitude of astigmatism in Groups I(WAS), II(WAS), and III(WAS) (p<0.05), and with an increase in the corneal tunnel length in Group I(WAS). A decrease in the magnitude of postoperative astigmatism was associated with an increase in the corneal tunnel length in Groups III(WAS) and IV(WAS) (p<0.05). An increase in the magnitude of post-suture removal astigmatism was associated with an increase in the magnitude of postoperative astigmatism in Groups I(WAS) and IV(WAS) (p<0.05), and with late suture removal in Group IV(WAS) (p<0.05). A decrease in the magnitude of post-suture removal astigmatism was associated with late suture removal in Groups I(WAS) and II(WAS). A logistic regression analysis showed that the prevalence of post- suture removal astigmatism axis shift was associated with increased corneal tunnel length, decreased magnitude of postoperative astigmatism, and early suture removal. Conclusions: In order to reduce postoperative and post-suture removal astigmatism, we recommend a short corneal tunnel length and late suture removal in patients with Group I(WAS) characteristics, late suture removal in Group II(WAS)-like patients, long corneal tunnel length in Group III(WAS)-like patients, and long corneal tunnel length and early suture removal in patients with characteristics of Group IV(WAS).

        Original Article : Anterior Segment Parameters Using Pentacam and Prediction of Corneal Endothelial Cell Loss after Cataract Surgery

        Purpose: We evaluated various preoperative anterior segment parameters measured with a Pentacam rotating Scheimpflug camera and compared them with those of conventional methods. We also evaluated the effect of different parameters on corneal endothelial cells after cataract surgery. Methods: Pentacam examination was performed in 88 eyes from 88 patients to evaluate central anterior chamber depth (ACDpentacam), nuclear density (Densitometrypentacam), anterior chamber volume (ACV), and lens thickness (LTpentacam). We compared values of ACDpentacam with those of ultrasound (ACDsono) and also compared Densitometrypentacam values with those of Lens Opacities Classification System (LOCS III) classification. We evaluated the effect of the following preoperative values measured with Pentacam on postoperative endothelial cell loss: pupil size measured both preoperatively and before capsulorrhexsis (PupilCCC), amount of viscoelastics, and LT measured by ultrasound (LTsono). Results: A significant concordance was found between the two grading methods of nuclear opacity: Densitometrypentacam and LOCS III classification (τb=0.414, p=0.000). We also found a positive correlation between ACDpentacam and ACDsono (r=0.823, p=0.000) and between ACDpentacam and ACV (r=0.650, p=0.000). There were significant differences between the results of LTpentacam and LTsono. The final regression model identified Densitometrypentacam, viscoelastics and PupilCCC as independent predictors of decreased postoperative corneal endothelial cell density (CD) at postoperative day 3, and Densitometrypentacam, viscoelastics, and ACV as independent predictors of decreased CD two months postoperatively (p<0.05). Conclusions: Good agreement was found between all results obtained with the Pentacam and conventional methods except LT. Analyzing anterior chamber parameters preoperatively using Pentacam could be helpful to predict postoperative endothelial cell loss.

        A preliminary investigation into the decomposition rate of wild boar carcasses in forest habitats

        The estimation of the postmortem interval (PMI, the time that has elapsed since the death) is a critical issue for the biosecurity enforcement officers who implement to the timely establishment of biosecurity zone for preventing susceptible animals from disease transmission given the rapid occurrence of an infectious disease. Increasing attention has been paid to PMI of wild boar (Sus scrofa) carcasses associated with African swine fever epidemics in Korea since October 2019 to explain the geographical transmission of the disease, as well as to provide potential target animals for prevention measures in terms of farm biosecurity. This study is, to the best of our knowledge, the first to describe the decomposition process of wild boars in different microhabitats in the country. In the present comparative study, we obtained field data from the decomposition process of the wild boar and domestic pig carcasses continuously exposed aboveground in natural environment settings allowing animal scavenging. This study compared the pattern of decomposition in 16 wild boar carrion and 10 domestic pig carcasses placed between August and November 2019. Quite differences in decomposition rate measured by total body score and tissue s gross morphology over time were observed between wild boars and domestic pigs. Overall, the wild boar decomposed much more slowly than the domestic pig throughout the end of the experimental period. In addition, color changes to the skin were easily seen in domestic pigs, whereas there is much variation in the wild boar, especially carcasses placed in late autumn. Moreover, some wild boar carcasses did not show any sign of bloating. These results indicate that decomposition rates derived from forensic taphonomic studies on domestic pigs may be not directly applicable to the wild boar, hence there is a need to develop regional decomposition models to be employed in different geographical situations to increase the accuracy of PMI of wild boar carcasses.

        Population viability analysis to estimate the needed number of capture-and-remove wild boars for control of African swine fever in the Republic of Korea

        Since the first detection of the African swine fever (ASF) virus in the Republic of Korea in 2019, the Korean government has applied interventions, including fencing, increasing the biosecurity level at domestic pig farms, and the capture-and-removal of wild boars. In particular, wild boars are an important risk factor for ASF control because they can spread disease among susceptible animals, such as wild boars or domestic pigs. A capture-and-removal method aims to reduce the likelihood of ASF transmission from wild boars to domestic boars or among wild boars by decreasing the number of susceptible wild boars. This study estimated the required number of wild boars captured and removed for ASF control using population viability analysis. Population factors, such as a life span, sex ratio, or an inbreeding depression with different capture-and-removal proportions of wild boars, were included in the analysis. Ten scenarios with different capture-and-removal proportions of wild boars and different periods of culling were considered. According to the results, a method in which 75% of wild boars are captured-and-removed for at least three years showed long-term effectiveness for more than ten years. The current ASF control method, in which 33% of wild boars are captured-and-removed, decreased the number of wild boars for three years, after which the wild boar population increased to more than its initial number. Given the limited human and material resources for controlling ASF in the Republic of Korea, it is recommended that resources be prioritized to increase the capture-and-removal proportion of wild boars to take full advantage of the ASF-control effectiveness.

        단안의 시력감소과 비전형적인 시야장애를 보인 뇌하수체 종양 1예

        목적: 뇌하수체 종양의 임상양상이 단안의 시력과 시야장애를 보이는 경우는 드물며 본 증례와 같은 비전형적인 시야장애를 보이는 경우는 국내에 보고된 바가 없어서 저자들은 1예를 보고하고자 한다. 대상과 방법: 65세 남자가 약 2개월 전부터 진행되는 우안의 시력장애를 주소로 내원하였다. 시력은 우안의 교정시력이 0.32였고 좌안의 교정시력이 0.6이었다. 양안의 경도의 백내장이 관찰되었으나 안저검사 및 시신경에서는 특이한 이상이 없었고, 좌안은 전혀 시력이나 시야장애를 호소하지 않았다. 반복된 시력검사와 시야검사, 시유발전위검사에서 점점 악화되는 양상을 보여 우측 구후 시신경염이나 시신경의 종양을 의심하고 안와 전산화 단층촬영을 실시하였다. 우측 안구 및 시신경에는 이상소견이 없었으나 몇 개의 단층에서 뇌하수체의 비정상적인 음영을 관찰하여 뇌자기공명영상을 다시 하여 뇌하수체 종양으로 확진되었다. 환자는 신경외과의 절제 수술 후 경과관찰 중이다. 결과: 뇌하수체 종양은 다양한 임상양상과 시야변화를 보일 수 있으나 단안에 국한된 주증상을 보이는 경우는 매우 드물며 본 증례와 같은 시야결손을 보이는 경우는 안과 의사의 진단이 매우 중요할 것으로 사료된다. Purpose: We report a rare case of pituitary tumor showing decreased visual acuity and atypical visual field defect in one eye. Methods: A 65-year-old patient presented with decreasing visual acuity in right eye. There were mild cataratous change of lens in both eyes and no abnormal findings of retina and optic nerve in fundus examination except mild epiretinal membrane in left eye. And there showed an atypical constricted visual field and abnormal wave in visual evoked potential in right eye. Orbit CT was done to observe optic nerve lesion but there was no abnormality in optic nerve. But we found an abnormal enhancement in pituitary gland. Pituitary tumor was confirmed in brain MRI and the patient was undergone neurosurgical excision of tumor. Results: Pituitary tumor shows variable clinical signs and visual field defect. But signs confined to unilateral eye is rare and visual field defect such as our case is atypical. So ophthalmologists need more attention in diagnosis of patients with atypical visual field defect.
