국산 Lidocaine 알칼리화를 위한 Bicarbonate 혼합시 침전유발 최소량에 대한 고찰

Background : When local anesthetics for regional anesthesia is used, usually small amounts of bicar-bonate are added for rapid onset. This addition gives the mixed solution a more alkaline pH. The following result is an increased unionized form of the local anesthetic and rapid penetration of the drug into tiissue. Unfortunately, no data about adequate mixing volumes of domestic lidocaine and bicarbonate is available. Methods : We examined six mixing pairs of two kinds of 2% lidocaines and three kinds of 8.4% bicarbonates for minimum volumes of bicarbonate to cause a precipitation of 2% 20 ml lidocaine. Results : The mean volumes of bicarbonate to cause precipitation were 1.54 ml for Kwang-Myung lidocaine with Kwang-Myung bicarbonate, 2.90 ml for Kwang-Myung lidocaine with Dae-Won bicarbonate, 2.73 ml for Kwang-Myung lidocaine with Je-Il bicarbonate, 0.97 ml for Je-Il lidocaine with Kwang-Myung bicarbonate, 1.26ml for Je-Il lidocaine with Dae-Won bicarbonate and 1.39 ml for Je-Il lidocaine with Je-Il bicarbonate. Conclusions : We conclude that the Kwang-Myung lidocaine and the Je-Il lidocaine could cause precipitation when mixing with a smaller bicarbonate volume than foreign textbook recommended. Thermomicrobium roseum 유래 Alcohol Dehydrogenase의 정제 및 특성

Alcohol dehydrogenase from Thermomicrobium roseum was purified to electrophoretic homogeneity approximately 77-fold with a 38% activity yield by adenosine 5'-monophosphate and Mono Q column chromatography. The molecular weight of the enzyme was determined to be approximately 43,000 by SDS-polyacrylamide gel electrophoresis and 88,000 by gel chromatography, indicating a homodimeric structure. The Km and Vmax values of the enzyme for ethanol were 24.2 mM and 12.8 ㎛ol/min/mg, respectively. Pyrazole notably inhibited the enzymatic activity. Ca2+ ion activated the enzyme activity, but Cu2+ inhibited. The activity of the enzyme was optimal at pH 10.0 and 60℃.

Background and purpose: The use of antiepileptic drugs for a long period is a known risk factor for bone loss and pathological fracture. An increased risk of fractures due to long-term treatment with antiepileptics has been attributed to the alterations in the levels of circulating calcium, calcitropic hormones and high homocysteine levels. The purpose is of this study to report bone fracture in an epileptic patient on chronic use of phenytoin may be associated with lower BMD (bone mineral density) and high plasma concentration of homocysteine. Case: We report a case of a 35 year old young male who had been on phenytoin for 31 years presenting with a pathological femur fracture and frequent complex partial seizures. The plasma homocysteine level was increased as much as 15.93 μmol/L. BMD of the lumbar spine and femur head were low. MRI of the thigh showed fibrodysplasia with cystic degeneration and pathologic fracture in the right proximal femur. Conclusion: A patient on chronic use of phenytoin has alterations in bone metabolism characterized by lower BMD of the lumbar spine and femur and high plasma concentration of homocysteine. An increased risk of bone fracture in an epileptic patient may be associated with osteoporosis and high homocysteine.
다운증아 어머니의 기대성향이 비계화 교수와 문제해결학습에 미치는 효과

이 연구는 어머니-다운증아의 상호작용을 통한 문제해결과정에서 어머니의 기대성향이 어머니 교수중재의 양과 수준에 미치는 영향과 다운증아의 적목피라미드 문제해결과제에 미치는 효과를 밝히는데 두고 다운증아 어머니의 기대성향에 따른 어머니의 비계화 교수와 아동의 문제해결학습간의 관계를 탐색하고자 하는 것으로,만 6세인 다운증아 남아 3 명과 여아 3명인 전체 6 명 그리고 그 어머니들을 대상으로 하였다. 어머니-아동의 문제해결상호작용과정을 비디오로 촬영하여,어머니의 교수중재는 중재 수준,중재 양,유관 및 비유관 여부를 중심으로 그리고 아동의 문제해결은 성패를 중삼으로 양적 · 질적 분석을 병행 하였다. 연구결과 어머니의 기대성향이 긍정적일수록 아동의 문제해결 성패에 기반하여 중재 양과 중재 수준을 조절하는 유관 중재를 많이 사용하고 문제해결학습에서 부정적 기대성향의 다운증 자녀보다 더 긍정적 학습결과를 보인다. 또한 어머니의 비계화 교수 변인과 자녀의 문제해갤학습 변인사이에는 상호 관련작 양방향성 효과가 있다. The purpose of this study was to examine the relationships between maternal expectancy and maternal use of scaffolded instruction and problem-solving skills of children with Down syndrome. For this purpose, three boys and three girls with Down syndrome whose age were six years old and their mothers participated in this study. The mother-child interaction regarding problem-solving tasks was videotaped and analyzed. The qualitative and quantitative analyses were conducted to find out the condition and quantity of scaffolded instruction used by mothers, and degree of their contingency of the tasks. In addition, it was analyzed whether these children resolved given problems successfully or not. As a result, it was found that the mothers who had more positive expectancy relied more on contingent intervention that regulated the quality and quantity of instructions based on the success or failure of their child' s problem solving tasks, and that their children produced better learning outcome than the children of the mothers who had more negative expectancy. In addition, two-way interrelationships between the scaffolded instruction variable of the mothers and their children ’ s problem-solving learning variable were found.

Thunderclap headache is a hyperacute, severe headache that is so named because the pain strikes suddenly and severely, and is sometimes a sign of a medical emergency. We report a case of pheochromocytoma in the urinary bladder in a 48-year-old woman who presented with recurrent thunderclap headache after voiding and exercise. So we think thunderclap headache during or immediately after micturition may be an important diagnostic clue of bladder pheochromocytoma.

In this research, we have developed a new navigation scheme using computer vision technology. A line feature tacking-based scheme is developed in which the orientation of the line features on the ceiling are detected in the input image, and used for guiding the direction of motion of a robot. In order to detect line information robustly, preprocessing steps such as lens calibration, noise cancelling, feature enhancement are employed. An also, to enhance the accuracy and the speed, we employed Intel IPL(Image Processing Library) for the implementation. The developed vision-based navigation system is quite reliable so that it can be used in a variety of natural circumstances, for example, home, office, factory, and so on. The performance of the developed system has been tested in many real environments, and shows excellent results in both accuracy and speed.
