      • 비만학생들의 운동후 회복시 초과 산소소비량 및 지속시간에 관한 연구

        The purpose of this study was to examine the effect of two different intensity of continuous exercise on magnitude of excess post-exercise oxygen consumption(EPOC), energy consumption following a 20min treadmill exercise. The subjects of this study were eight super-obesity in middle school students who were above 13 years old. subjects participated in two different exercise intensity to measure respiratory gas, blood lactate concentration and eardrum temperature. The results of this study were as follows: 1. The Energy consumption of the high intensity (80% VO₂ max) exercise group (264.68 35.60cal) was greater than that of the low intensity (50%) exercise group(172.01 20.39 Cal)(p<.001). 2. The EPOC of the high intensity exercise group (22.36+ 6.82Cal ) was greater than that of the low intensity exercise group(15.39+5.27 Cal)(p<.001). 3. The EPOC duration of th high intensity exercise group(28.36=3.56min) was longer than that of the low intensity exercise group(17.23+1.25min)(p<.001) 4. The blood lactate concentration, respiratory exchange ratio and temperature of the high intensity exercise group were greater than those of the low intensity exercise group(p<.05,p<.01). EPOC and duration were increase with 80%VO₂max, 80% VO₂max as intensity exercise got higher this also showed that it had something to do with lactate concentration in blood and increase of body temperature in the conclusion. We considered that there was a difference in using methods of energy, as there was a similar difference in RER. We though this following with oxygen consumption after exercise, we could clarify that high intensity exercise was better than lower intensity exercise in sametime for those who are obese especially adolescents to maintain their weights on the aspect of effect.

        The purpose of this study was to investigate the effect of 12 wks before and after running exercise of obsese student in D high school in Pusan city, exercise group(7) performed 12 wks running exercise and control group(7),before and after subjects performed the maximal exercise thest on treadmile's 12 wks running exercise. Through the comparision and analysis of change serum lipids and apoliporotein metabolism, the results of this study were as follows. 1. In 12 wks before and agter running exercise at the rest time,exercise group was found to have a great decrease in TC(8%),TG(29%),LDL-C(7%) and to have a significant in crease in HDL-C(20%),FFA(74%). 2. At the rest, the control group was found to have a little decrease in TG, HDL-C,LDL-C and to have little changes in TC and FFA. 3. In 12wks before and after running exercise at the maximal exercise, the exercise group was found to have a significant decrease in TC(18%),TG(23) and to have a significant in crease in HDL-C(20%),LDL-(25%),FFA(23%) and TG and HDL-C were found to have a significant change. 4. At the maximal exerxcise , were found to have little change in the control group. 5. Apo A-I in the exercise group was found not to have the change signicantly. but to have anincrease 5% at the rest time, 6% at the maximal exercise. And the control group was almost found not to have change. 6. Statically, Apo B in the exercise group was found not to have a signficant change, but to have the increase 12%at the maximal exercise and the control group was found not to have a change.

        The purpose of this study was to investigate the effect of 12 wks before and after step test of obsese student in D high school in Pusan city, exercise group(7) performed 12 wks step test and control group(7), before and after subjects performed the maximal exercise thest on treadmile's 12 wks step test. Through the comparision and analysis of change serum lipids and apolipoprotein metabolism, the results of this study were as follows. 1.In 12 wks before and after step test at the rest time, exercise group was found to have a increase In TC(12.7%), TG(9.76%), LDL-C(13.8%) and to have a significant increase in HDL-C(11.3%), FFA(96.8%). 2.At the rest, the control group was found to have a little increase in TG(9.2%) and HDL-C(6.7%), and to have a significant increase in TC(9.2%), LDL-C(8.9%) and FFA(7.5%). 3.In 12 wks before and after step test at the maximal exercise, the exercise group was found to have a significant increase in TC(5.6%), HDL-C(20.2%) and FFA(54.3). 4.At the maximal exercise, were found to have a significant increase in TC(9.7%) and LDL-C(9.9%) in the control group. 5.In 12 wks before and after step test at the maximal exercise, the exercise group was found to have a significant increase in Apo A-I(8.0%). 6.Statistically, At the maximal exercise, were found to have a significant increase in Apo B(7.8%).

Through the comparision and analysis of change respiro-circulatory and serum lipids the results of this study were as follows. 1. In 12wks before and after running exercise, physique and body composition of exercise group was found to have a great decrease in %Fat(6%), Fat(kg)(6%), Triceps(7%) and to have a significant in crease in %LBM(6%). 2. Respiro-circulatory Capacity of exorcise group at the maximal exercise, was found to have a great decrese in VE(5%), RR(8%), RQ(5%). 3. In 12wks before and after running exercise at rest time, in seam lipids of exercise group was found to have a great decrease in TC(8%), TG(29%), LDL-C(7%) and to have a significant in crease in HDL-C(20%), FFA(74%). 4. At rest time, the control group was found to have a little decrease in TG, HDL-C, LDL-C and to have little change in TC and FFA. 5. In 12wks before and after running exercise at the maximal exercise, in serum lipids of the exercise group was found to have significant decrease in TC(18%), TG(23%) and to have a significant in crease in HDL-C(20%), LDL-C(25%), FFA(23%) and TG and HDL-C were found to have a significant change. 6. At maximal exercise, was found to have little change in the control group.

      • 반복성 혈뇨(IgA 신병증 및 non-IgA 신병증)와 알레르기성 자반증 신염의 면역지표의 변동에 관하여

        A prospective study was conducted to see the changes of immune system in recurrent gross hematuria(IgA nephropathy and non-IgA nephropathy) and HSP nephritis in children. Study patients consisted of 60 children with recurrent gross hematuria and Henoch-Scho¨nlein purpura nephritis (8 IgA nephropathy, 24 non-IgA nephropathy and 28 HSP nephritis). The cellular immune indices(total T, T4, T8 cells and T4/T8 ratio) and humoral immune indices (IgG, A, M and E) were measured during the hematuric and non-hematuric period. Following results were obtained. The proportion of T4 cells of peripheral blood during the hematuric period of patients with IgA nephropathy rose to 35.4±14.9% from the non-hematuric value of 20.0±9.1%. The T4/T8 ratio during hematuric period of patients with IgA nephropathy rose to 1.51±0.77 from non-hematuric value of 0.73±0.33. The values of serum IgA and IgE during hematuric period of patients with IgA nephropathy rose to 237±106 ㎎/dl, 231±226 IU/dl from non-hematuric values of 140±10, 28±23, respectively. These changes of cellular and humoral immune indices showed statistically significant differences(p<0.05). However, these changes were not found in patients with non-IgA nephropathy nor HSP nephritis. In conclusion, it can be said that the immune mechanism involved in IgA nephropathy is different from that of HSP nephritis.

      • 심한 골병변으로 발현된 기능성 낭종성 부갑상선 선종

        낭종성 부갑상선 선종과 심한 골병변을 동반한 부갑상선 기능항진증은 매우 드문 질환으로서, 저자들은 양측 고관절의 통증을 초기 주소로 내원한 환자에서 고칼슘혈증과 부갑상선 호르몬 증가, 골병변의 방사선적 소견을 통해 부갑상선 기능항진증을 진단하고, 경부 초음파와 컴퓨터 단층 촬영, 부갑상선 스캔검사 및 수술중 부갑상선 낭종액 검사 등을 통해 기능성 부갑상선 낭종의 한 종류인 낭종성 부갑상선 선종을 진단하고 수술적 제거를 통하여 정상화된 1예를 경험하였다. A cystic parathyroid adenoma is rare. A case of primary hyperparathyroidism, with the cystic formation of a parathyroid adenoma and a severe bony lesion, is reported. A 52-year-old male was admitted due to pain in both hips and for evaluation of hypercalcemia. The plasma level of the intact parathyroid hormone (iPTH) was elevated to 1424 pg/mL. Ultrasonography and the computed tomography revealed a parathyroid cyst on the left thyroid lower pole. Parathyroid scintigraphy detected a parathyroid adenoma. A radiograph showed a subperiosteal bone resorption on the phalanges, and a brown tumor (osteitis fibrosa cystica) on the femur shaft was noted. A surgical excision of the parathyroid adenoma was performed. The PTH level in the cystic fluid was increased. A histological examination confirmed a cystic parathyroid adenoma. The PTH level was normalized after the operation (J Kor SOC Endocrinol 18:214-220, 2003).

        2003년 국내 중증급성호흡기증후군 진료 현황 및 문제점 분석

        목적 : 2002년 말 중국에서 SARS가 발생한 이후 국내에서도 2003년 10월까지 총 3명의 추정환자, 17명의 의심환자가 보고되었다. 향후 추가적인 SARS의 유행이 우려되는 상황에서, 그간의 SARS 환자 진료에 있어서의 실질적인 준비사항, 진료 현황 등에 대한 조사를 통해 문제점을 파악하여, 향후 더 나은 대비가 될 수 있도록 개선점을 제시하고자 하였다. 재료 및 방법 : SARS로 의심되는 환자를 진료 경험이 있는 병원의료진을 대상으로 2003년 10월에 설문조사를 실시하였다. 설문에는 SARS 환자 진료 시의 실질적인 조치, 진료 현황, 병실, 응급실 및 외래에서의 격리 시설과 준비사항, 보건당국의 관리와 지원에 관한 사항을 포함하였다. 결과 : 대상이 되는 22개 병원 중 17개(17/22, 77.2%) 병원이 설문에 응하였다. SARS 환자를 위한 격리실은 응급실, 외래, 일반병실 및 중환자실에서 각각 9개(9/17, 52.9%), 5개(5/17, 29.4%), 15개(15/16, 93.7%), 4개(4/16, 25%) 병원에서 음압처리가 되어있지 않은 일인실 혹은 다인실이 사용되었고, 1개(1/16, 6.3%) 병원에서만 일반병실에서 음압격리실이 운영되었다. 입원환자의 진찰 시 개인보호구의 착용은 거의 모든 의료기관에서 이루어졌다. 보건당국에서 SARS지정병원의 시설 등을 사전에 확인한 곳은 1곳(1/12, 8.3%)이였고, 14개 병원(14/15, 93.3%)에서는 보건당국에 의뢰한 검사결과를 통하 받지 못하였다. 결론 : 의료기관에서 SARS 환자용 격리실뿐만 아니라 기존의 격리실 설비 등이 미흡하였으며, 특히 중환자실 및 외래의 준비가 더욱 부족하였다. 보건당국의 의료기관에 대한 종합적인 지원이 부족하였고, 병원과의 원활한 연계가 잘 이루어지지 않았다. SARS 만이 아닌 격리를 필요로 하는 질환의 적절한 진료를 위해 향후 병원 시설의 정비와 정부차원에서의 보다 구체적이고 실질적인 대책마련이 필요하다. Background : There was an worldwide outbreak of the Severe Acute Respiratory Syndrome (SARS) originated from China in late 2002. During that period three cases of suspected SARS and 17 cases of probable SARS were reported in Korea. With the concerns about the reemergence of SARS-coV transmission, it is important to be prepared for any possibility. So, this study is aimed to analysis the past measures in managing SARS and propose the amendatory plans to improve the preparedness. Materials & Methods : Questionnaires were collected among clinicians with any experience in managing the probable or suspected SARS cases in Oct. 2003. 17 out of 22 hospitals responded to the questionnaire. The contents in the questionnaire were practical activities, personal equipments, response plans, isolation facilities in emergency centers, outpatient clinics, general wards and intensive care units, and relationship with the public health department. Results : The dedicated isolation rooms in emergency centers, outpatient clinics, general wards, and intensive care units were prepared in 9 (9/17, 52.9%), 5 (5/17, 29.4%), 15 (15/16, 93.7%), and 4 (4/16, 25.0%) hospitals, respectively. Except for one hospital that newly made negative pressure room for SARS, single or multi-bed rooms without airborne infection control were used in all the other hospitals. The personal precaution principles were kept quite well in general wards. Before the designation of SARS hospital by the public health department prior evalution to see if the hospital was suitable for managing SARS was conducted in only 1 (1/12, 8.3%) hospital. The results of laboratory diagnosis were reported back in 1 (1/15, 6.6%) hospital. Conclusions : The isolation facilities which can control airborne infection were almost deficient not only for SARS but also for other respiratory transmissible diseases. For the infection control of transmissible diseases including SARS, more investment is needed on medical facilities and comprehensive support from the public health department required.

        미세변화신증후군에서 말초혈액단핵구가 사구체 상피세포의 Glomerular Basement Membrane Heparan Sulfate Proteoglycan의 유전자 발현에 미치는 영향 및 이의 측정을 위한 적량적 PCR법의 개발

        N/A Minimal Change Nephrotic Syndrome(MCNS) reflects a disorder of T-lymphocytes. These T-cells are thought to release a vascular permeability factor (UPF) that injures the glomerular epithelial cells (GECs). Glomerular epithelial cellular damage may lead to proteinuria in MCNS by decreasing the synthesis of polyanions such as heparan sulfate proteoglycan(HSPG) : these polyanions constitute most of the normal charge barrier to glomerular filtration of macromolecules such as albumin. This study evaluates the direct effect of supernatant of culture media of peripheral blood mono- nuclear cells(PBMC) which was isolated from children with MCNS to GBM HSPG mRNA expression in rats GEC. GEC were cultured until confluent. Supernatant of culture media of PBMC from each group of 3 chilren with MCNS, IgA nephropathy or normal healthy were added. Total RNA was extracted at 12, 24 and 72hrs after adding supernatant. RT-PCR using Rat Perlecan Domain-I(RPD- I) specific primers and beta-actin as internal controls was done. Densities and areas of GRM HSPG corresponding bands to beta-actin bands were measured. At 24 hrs, supernatant of culture media of PBMC from 3 children with MCNS caused 62, 70, and 75Vo reductions, respectvely, in GEC's GBM HSPG mRNA expression compared to normal children. However, supernatant of culture media of PRMC from 3 children with IgA nephropathy did not. In addition, reductions of GEC's GBM HSK' mRNA expressions caused by supernatant of culture media of PBMC from 3 children with MCNS were restored upto levels of normal children at 72hrs after adding supernatant. Mutant cDNA was synthesized as primers for competitive PCR to quantify GBM HSPG mRNA expression. Mutant template was 212 base pairs shorter than RPD-I, 497 base pairs. In conclusion, we found that supernatant of culture media of PBMC from children with MCNS reversibly suppressed GBM HSPG mRNA expression in rats GEC. This study suggests cytokines of PBMC from children with MCNS directly injures GEC and leads to decrease in synthesis of GBM HSPG by GEC in the pathogenesis of MCNS.

        Development of Near-Isogenic Lines (NILs) Conferring Xa4, xa5 and Xa21 Genes Resistant to Bacterial Blight (Xanthomonas oryzae pv. oryzae) in japonica rice Genetic Background

        Near-isogenic lines (NILs) carrying bacterial blight resistance genes (Xa4, xa5 and Xa21) were developed in japonica rice using Suweon345 as genetic background. NILs were selected by gene specific DNA markers and inoculation of K1 or K3a race. NILs conferring Xa4 were resistant to K1, K2, K3, and moderately resistant to K3a. NILs conferring xa5 were resistant to K1, K2, K3, and K3a. NILs having Xa21 were susceptible to K1, while resistant to K2, K3 and K3a. Target genes of NILs with the genetic background of Suweon345 were also confirmed by using eleven Philippines races and International Rice Bacterial Blight (IRBB) NILs carrying Xa4, xa5 and Xa21. All NILs had no significant difference from their recurrent parents in the major agronomic traits except for panicle length and brown rice 1,000 grain weight. Heading date of NILs ranged from Aug. 10 to Aug. 11, which was similar to that of recurrent parent, Suweon345. Culm length, number of grains per panicle and ratio of ripened grain of NILs were similar to those of Suweon345. Milled rice of NILs was ranged from 4.82 to 4.93MT/ha. These NILs will be useful for improving resistance to K3a race of bacterial blight pathogens in Korean japonica cultivars.
