        폐경여성에서의 내시경하 수술의 적응증

        연구목적: 폐경여성에서 자궁 내시경 수술의 적응증을 알아보고자 한다. 연구재료 및 방법: 2001년 3월부터 2009년 3월까지 순천향대학교 부천병원에서 내시경수술을 시행한 시행한 43명을 중심으로 후향적으로 환자의 임상적 특징, 검사결과, 수술 후에 합병증과 결과에 대해서 조사하였다. 결과: 내시경 수술을 통해서 내막조직검사 26명, 내막용종제거술 8명, 자궁근종제거술 7명, 비정상 자궁출혈과 자궁내막증식증 치료 1명, 자궁내종괴 1명, 자궁근종의 평균 크기는 3.2 cm, 재원기간은 평균 2일, 수술 후에 빈혈 수치는 1.1 g/dl이고, 평균 수술 시간은 36분이었다. 결론: 자궁내시경 수술의 적응증은 광범위하게 변화하고 있으며 순수한 자궁내근종, 자궁내막 하 근종, 자궁내막용 중, 자궁선근종, 자궁내막증식증, 물묵자궁증, 비정상자궁출혈이 내시경하 수술로 치료될 수 있다. 자궁내시경수술 시복부초음파가 수술시의 합병증을 줄인다. Objectives: To evaluate the spectrum of hysteroscopic surgery to be extended. Methods: Forty-three women who underwent hysteroscopic procedures with Urione(R) solution or normal saline as distension media between March 2001 and March 2009 were randomized. The clinical characteristics, laboratory data, and post-operative results were retrospectively analyzed. Results: The following hysteroscopic procedures were performed: endometrial biopsy, 26; endometrial polypectomy, 8; myomectomy, 7; ablation for dysfunctional uterine bleeding and endometrial hyperplasia, 1; and excision of an endometrial mass, 1. The mean size of the myomas was 3.2 cm. The average duration of hospitalization was 2 days. The average change in hemoglobin was 1.1 g/dl. The mean operative time was 36 minutes. Conclusion: The indications for hysteroscopic procedures can be extended. Pure intramural myomas, submucosal myomas, endometrial polyps, adenomyosis, endometrial hyperplasia, hydrometra, and abnormal uterine bleeding can be treated via a hysteroscope. Intra-operative transabdominal ultrasonographic guidance can be helpful during hysteroscopic procedures, and reduces the complication and morbidity rates. (J Korean Soc Menopause 2010;16:46-51)

        비대면 교육환경에서의 대학생의 대학 몰입 예측 요인 연구: 교육 실재감을 중심으로

        본 연구는 코로나-19 사태로 인해 대학의 교육환경이 비대면으로 전환되고 대학생 중도탈락 문제가 크게 대두되었던 상황에서, 대학생의 중도탈락을 결정하는 주요 요인으로 알려진 대학 몰입의 예측 요인을 분석하고자 하였다. 즉, 본 연구의 목적은 비대면 교육환경에서 대학생의 대학 몰입을 예측하는 요인을 탐색하고, 이를 교육 실재감을 중심으로 분석하는 데 있다. 이를 위해 Tinto의 대학생 통합 모형과 사회적 실재감, 인지적 실재감, 교수 실재감으로 구성되는 교육 실재감(Garrison, Anderson & Archer, 2000)을 본 연구의 분석의 틀로 설정하였다. 아울러, 코로나-19 사태 이후 대학에 입학한 소위 ‘코로나 학번’ 학생들의 중도탈락이 높다는 문제의식하에, 교육 실재감과 대학 몰입간 관계가 코로나 학번 여부에 따라 달라지는지 살펴보고자 조절 효과 또한 검증하였다. 자료 수집은 2021년 10월 서울 소재 A 대학교 재학생을 대상으로 온라인 설문조사를 통해 이루어졌으며, 864명의 학생 자료를 분석하였다. 분석 결과, 교육 실재감 가운데 인지적 실재감과 교수 실재감이 비대면 교육환경에서 대학생의 대학 몰입을 정적인 관계에서 예측하는 것으로 나타났으며, 이외에도 주관적 계층의식, 학생-학생 비대면 상호작용, 대학의 조직 커뮤니케이션에 대한 인식, 대학의 온라인 학습관리시스템 지원에 대한 인식, 대학의 비대면 서비스 및 프로그램 지원에 대한 인식이 대학 몰입을 정적인 관계에서 예측하는 것으로 나타났다. 한편, 교육 실재감과 대학 몰입 간 관계에서 코로나 학번 여부의 조절 효과는 통계적으로 유의하지 않았다. 이와 같은 결과를 바탕으로 본 연구는 인지적 실재감과 교수 실재감의 지원 등을 포함하여 비대면 교육환경에서 대학생의 대학 몰입을 향상시키기 위한 실천적 시사점을 제언하였다. This study aimed to explore the factors predicting college students’ institutional commitment in the online learning environment, focusing on educational presence (social presence, cognitive presence, and teaching presence). At the beginning of the COVID-19 pandemic, the issue of student dropout emerged in Korean higher education. Against this background, this study adopted Tinto’s student integration model and the concept of educational presence (Garrison, Anderson, & Archer, 2000) as the theoretical framework. In addition, this study investigated the moderating effects of “the COVID-19 cohort,” who started their college life during the pandemic, on the relationship between educational presence and institutional commitment. Data were collected in October 2021 via an online survey with 864 students at “A” university located in Seoul, Korea. The main findings of this study are as follows: Cognitive presence, teaching presence, subjective perception of socioeconomic status, non-face-to-face interactions among students, student perception of organizational communication from university, student perception of support for the online learning management system (LMS), and student perception of support for online services and programs were found to predict institutional commitment. However, there were no moderating effects of the COVID-19 cohort on the relationship between educational presence and institutional commitment. Accordingly, this study suggests practical implications to improve institutional commitment in the online learning environment, including support for cognitive presence and teaching presence.

        Rho Guanine Nucleotide Exchange Factor 4 (Arhgef4) Deficiency Enhances Spatial and Object Recognition Memory

        Guanine nucleotide exchange factors (GEFs) play multiple functional roles in neurons. In a previous study, we reported that Arhgef4 (Rho guanine nucleotide exchange factor 4) functioned as a negative regulator of the excitatory synaptic function by sequestering postsynaptic density protein 95 (PSD-95). However, the role of Arhgef4 in behavior has not been examined. We performed comprehensive behavioral tests in knockout (KO) mice to investigate of the effects of Arhgef4 deficiency. We found that the expressed PSD-95 particle size was significantly increased in hippocampal neuronal cultures from Arhgef4 KO mice, which is consistent with the previous in vitro findings. Arhgef4 KO mice exhibited general motor activity and anxiety-like behavior comparable to those of the wild type littermates. However, spatial memory and object recognition memory were significantly enhanced in the Arhgef4 KO mice. Taken together, these data confirm the role of Arhgef4 as a negative synaptic regulator at the behavioral level.

      • Protection Design of DC Smart Grid

        Recently, introduction of renewable energy such as photovoltaics and wind energy has gained precedence for greenhouse gas reduction and oil substitution. However, due to the power fluctuation of renewable energy sources, voltage and frequency deviation are problematic in power systems. From the background, smart grids are attracting attention. Already, a smart grid based on AC grid is proposed. However, no study on research is presented or published on a smart grid based on DC grid. DC smart grid has been presented by the authors. The DC smart grid is robust to ac-side fault conditions. However, dc faults in the DC smart grid are not considered in previous papers. This paper analyzes dc faults, their transients, and the resulting protection issues in the DC smart grid. Over-current fault are analyzed in detail and provide an insight into protection system design. The effectiveness of the proposed method is performed by using PSIM environment.

      • Effect of macro and micro fiber volume on the flexural performance of hybrid fiber reinforced SCC

        The aim of this study is to investigate the flexural performance of hybrid fiber reinforced self-compacting concrete (HFRSCC) having different ratio of micro and macro steel fiber. A total of five mixtures are prepared. In all mixtures, the sum of the steel fiber content is 1% and also water/binder ratio is kept constant. The amount of high range water reducer admixture (HRWRA) is arranged to satisfy the workability criteria of self-compacting concrete. Four-point bending test is carried out to analyze the flexural performance of the mixtures at 28 and 56 curing days. From the obtained load-deflection curves, the load carrying capacity, deflection and toughness values are investigated according to ASTM C1609, ASTM C1018 and JSCE standards. The mixtures containing higher ratio of macro steel fiber exhibit numerous micro-cracks and, thus, deflection-hardening response is observed. The mixture containing 1% micro steel fiber shows worst performance in the view of all flexural parameters. An improvement is observed in the aspect of toughness and load carrying capacity as the macro steel fiber content increases. The test results based on the standards are also compared taking account of abovementioned standards.

        Freeze-thaw resistance and sorptivity of self-compacting mortar with ternary blends

        This paper investigated the influence of binary and ternary blends of mineral admixtures in self-compacted mortar (SCM) on the fresh, mechanical and durability properties. For this purpose, 25 mortar mixtures were prepared having a total binder content of 640 kg/cm3 and water/binder ratio between 0.41 and 0.50. All the mixtures consisted of Portland cement (PC), fly ash (FA) and silica fume (SF) as binary and ternary blends and air-entrained admixture wasn’t used while control mixture contained only PC. The compressive and tensile strength tests were conducted for 28 and 91 days as well as slump-flow and Vfunnel time tests whilst freeze-thaw (F-T) resistance and capillary water absorption tests were made for 91-day. Finally, in general, the use of SF with FA as ternary blends improved the tensile strength of mortars at 28- and 91-day while the use of SF15 with FA increased the compressive strength of the mortars compared to binary blends of FA. SCM mixtures with ternary blends had lower the sorptivity values than that of the mortars with binary blends of FA and the control mixture due to the beneficial properties of SF while the use of FA with SF as ternary blends induced the F-T resistance enhancement.
