      • Longitudinal Clinical Course of Preserved Ratio Impaired Spirometry Compared to COPD

        Background Preserved ratio impaired spirometry (PRISm) is a heterogeneous disease entity, and limited data are available about the prevalence, trajectories and prognosis of PRISm. We assessed the longitudinal clinical course of patients with PRISm compared to chronic obstructive pulmonary disease (COPD). Methods A retrospective study was conducted between January 2013 and December 2020 on PRISm and COPD patients who underwent chest CT imaging and longitudinal pulmonary function tests. We evaluated differences of acute exacerbation history and lung function changes between PRISm and COPD. Results Of the 716 patients, 45 and 671 patients had PRISm and COPD, respectively. PRISm patients had younger age, less smoking history, lower Charlson comorbidity index, and more history of tuberculosis and physician-diagnosed asthma, compared to COPD patients. PRISm did not differ significantly from COPD in the risk of acute exacerbation during longitudinal follow-up. PRISm patients show significant lower rates of FEV1 and FVC decline despite of lower baseline FVC, than COPD patients. Conclusion PRISm patients had no significant difference in risk of acute exacerbation, but significant slower decline of lung function during longitudinal follow-up, compared to COPD patients.

        Efficacy and Safety of Glecaprevir/Pibrentasvir in Korean Patients with Chronic Hepatitis C: A Pooled Analysis of Five Phase II/III Trials

        Background/Aims: Glecaprevir/pibrentasvir (G/P) is the first pan-genotypic direct-acting antiviral combination therapy approved in Korea. An integrated analysis of five phase II and III trials was conducted to evaluate the efficacy and safety of G/P in Korean patients with chronic hepatitis C virus (HCV) infection. Methods: The study analyzed pooled data on Korean patients with HCV infection enrolled in the ENDURANCE 1 and 2, SURVEYOR II part 4 and VOYAGE I and II trials, which evaluated the efficacy and safety of 8 or 12 weeks of G/P treatment. The patients were either treatment-naïve or had received sofosbuvir or interferon-based treatment. Efficacy was evaluated by assessing the rate of sustained virologic response at 12 weeks posttreatment (SVR12). Safety was evaluated by monitoring adverse events (AEs) and laboratory assessments. Results: The analysis included 265 patients; 179 (67.5%) were HCV treatment-naïve, and most patients were either subgenotype 1B (48.7%) or 2A (44.5%). In the intention-to-treat population, 262 patients (98.9%) achieved SVR12. Three patients did not achieve SVR12: one had virologic failure and two had non-virologic failures. Most AEs were grade 1/2; eight patients (3.0%) experienced at least one grade ≥3 AE. No serious AEs related to G/P treatment were reported, and grade ≥3 hepatic laboratory abnormalities were rare (0.8%). Conclusions: G/P therapy was highly efficacious and well tolerated in Korean patients with HCV infection, with most patients achieving SVR12. The safety profile was comparable to that observed in a pooled analysis of a global pan-genotypic population of patients with HCV infection who received G/P. (Gut Liver 2021;15:895-903)

      • 오리엔트종 잎담배의 화학성분에 의한 관능 특성 예측

        This study was conducted to evaluate the prediction of sensory characteristics of smoke from the leaf chemical compounds and characterize leaf chemical components for the best tobacco taste's leaves in oriental tobacco. For analytical and sensory evaluations, seventy two grades were used. Sensory evaluation of tobacco smoke for six attributes were scored on fifteen-point scale by $10{\sim}14$ expert panels trained to estimate smoking quality quantitatively. The major leaf chemical compounds to predict the sensory characteristics of smoke were ether extract for tobacco-like, nicotine for impact and total nitrogen/nicotine ratio for irritation, and total sugar for off taste & odor. Within ${\pm}20%$ range of difference, the predictable probabilities of sensory characteristics of smoke from the leaf chemical compounds were 87.5 % for off taste & odor and $94.4{\sim}98.6\;%$ for tobacco-like, impact and irritation. As a result of K-means cluster analysis on the basis of tobacco taste, the desirable leaf chemical compound contents were $5.9{\sim}8.3\;%$ in ether extract, $1.35{\sim}2.27\;%$ in nicotine and $1.17{\sim}2.24$ in total nitrogen/nicotine ratio. This study suggest that the some regression equations may be useful to predict the sensory characteristics of tobacco smoke with a few selected leaf chemical compounds in oriental tobacco and to select the oriental tobacco leaves by means of enhancing the tobacco taste of cigarette.

        This study was conducted to evaluate the prediction of sensory property of smoke from leaf chemical property and characterize leaf chemical components for the best tobacco taste's leaves in flue-cured tobacco. For analytical and sensory evaluations, one hundred and forty grades were used. The major leaf chemical components to predict the sensory property of smoke were nicotine for impact, irritation and off taste & odor, and total sugar/nicotine ratio for tobacco taste. Within ${\pm}20%$ range of difference, the predictable probabilities of sensory property of smoke form leaf chemical property were 80.0% for off taste & odor and $91.4{\sim}96.4%$ for impact, irritation and tobacco taste. As a result of K-means cluster analysis on the basis of tobacco taste, the desirable leaf chemical component contents were $2.77{\sim}3.55%$ in nicotine and $5.1{\sim}6.9$ in total sugar/nicotine ratio. This study suggest that the some regression equations may be useful to predict the sensory property of tobacco smoke from a few selected leaf chemical components in flue-cured tobacco and to select the flue-cured tobacco leaves for enhance the tobacco taste of cigarette.

        In this study, we have investigated the natural and puffing combustibility of cigarette brands and the compulsory ash-drop, and analysed factors related to the materials. Cigarette combustibility was greatly affected by the tobacco weight in cigarettes rod. Combustion rate of 4AOR cigarette made by flue-cured tobacco was 5.02 $^{mm}/_{min}\;and\;10\%$ faster than 1BO cigarette. Combustion pattern has interrelation with sugars, chlorine and ash contents among tobacco components. Combustion rate of 4DL, flue-cured tobacco was 55 seconds faster than 1CL tobacco extremely much in sugars contents, and ash drop was maintained at $21\%$ longer. Combustion time and burning rate were fast in cigarette made by expanded tobacco slice and tobacco stem and the formation of ash was poor. The length of puffed ash drop of expanded tobacco slice and tobacco stem were 18.6 mm and 25.2 mm and these forced ash drop length were 3.8 mm and 4.9 mm respectively. Puffed burning rate of general cigarette was $14.3\%$ faster than static burning rate and super slim type cigarettes was $21.3\%$ faster. Combustion rate and ash drop of domestic and foreign cigarette brands were quite different depending on tar level and cigarette types. The lower tar level, the faster combustion rate and the shorter ash drop.
