        Systematic Review of Reciprocal Changes after Spinal Reconstruction Surgery : Do Not Miss the Forest for the Trees

        The purpose of this review was to synthesize the research on global spinal alignment and reciprocal changes following cervical or thoracolumbar reconstruction surgery. We carried out a search of PubMed, EMBASE, and Cochrane Library for studies through May 2020, and ultimately included 11 articles. The optimal goal of a truly balanced spine is to maintain the head over the femoral heads. When spinal imbalance occurs, the human body reacts through various compensatory mechanisms to maintain the head over the pelvis and to retain a horizontal gaze. Historically, deformity correction has focused on correcting scoliosis and preventing scoliotic curve progression. Following substantial correction of a spinal deformity, reciprocal changes take place in the flexible segments proximal and distal to the area of correction. Restoration of lumbar lordosis following surgery to correct a thoracolumbar deformity induces reciprocal changes in T1 slope, cervical lordosis, pelvic shift, and lower extremity parameters. Patients with cervical kyphosis exhibit different patterns of reciprocal changes depending on whether they have head-balanced or trunk-balanced kyphosis. These reciprocal changes should be considered to in order to prevent secondary spine disorders. We emphasize the importance of evaluating the global spinal alignment to assess postoperative changes.

        한국인 직무 스트레스 측정도구의 개발 및 표준화

        Background and Purposes: Over the past three decades, numerous studies performed in Korea have reported that job stress is a determinant risk factor for chronic diseases and work disability. Every society has its own culture and occupational climate particular to their organizations, and hence experiences different occupational stress. An occupational stress measurement tool therefore needs to be developed to estimate it objectively. The purpose of this study is to develop and standardize the Korean Occupational Stress Scale (KOSS) which is considered to be unique and specific occupational stressors in Korean employees. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002-2004). A total of 12,631 employees from a nationwide sample proportional to the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were administered. The KOSS was developed for 2 years (2002-2004). In the first year, we collected 255 items from the most popular job stress measurement tools such as JCQ, ERI, NIOSH and OSI, and 44 items derived from the a qualitative study (depth interview). Forty-three items of KOSS, in the second year, were retained for use in the final version of the KOSS by using Delphi and factor analysis. Items were scored using conventional 1-2-3-4 Likert scores for the response categories. Results: We developed eight subscales by using factor analysis and validation process: physical environment (3 items), job demand (8 items), insufficient job control (5 items), interpersonal conflict (4 items), job insecurity (6 items), organizational system (7 items), lack of reward (6 items), and occupational climate (4 items). Together they explained 50.0% of total variance. Internal consistency alpha scores were ranged from 0.51 to 0.82. Twenty-four items of the short form of the KOSS (KOSS-SF) were also developed to estimate job stress in the work setting. Because the levels of the subscales of occupational stress were gender dependent, gender-specific standard norms for both the 43-item full version and the 24-item short form using a quartile for the subscales of KOSS were presented. Conclusion: The results of this study suggest that KOSS might be an appropriate measurement scale to estimate occupational stress of Korean employees. Further and more detailed study needs to be conducted to improve the validity of this scale.

        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

      • 청소년기 태권도 수련생들의 체격 및 체력 요인별 발달 비교

        This study was intended to investigate changes in the physique and fitness by age in adolescence. For this purpose, it attempted to make the first measurement in June, 2001 and the second measurement in 2002 for a total of 15 persons(seven middle school students aged 13 to 14 years and 8 high school students aged 16 to 17 years) practicing Taekwondo at 'S' and 'C' Drill Centers located in South Kyungsang Province. Both measures were conducted over the 2-day period. An attempt was made to compare and analyze the growth rate of physique and fitness factors by age based on data from measurement. As a result, the following conclusion was drawn: First, it was found that there was a statistically significant difference in the change of height, weight and body fat percent before and after measurement in order to investigate the change of physique(P<.05). Among other things, it was found that there was a statistically significant difference in the change of height, weight between the period of middle school days(13 to 14 years old) and the period of high school days(16 to 17 years old)(P<.05). It was found that Taekwondo trainees grew more over the period of middle school days(13 to 14 years old) than over the period of high school days(16 to 17 years old). But it was found that there was not a statistically significant difference in body fat percent between both groups(P<.05). But body fat percent reduced over the period of middle school days{13 to 14 years old) but did remain almost unchanged. Second, an attempt was made to measure power(high jump test), agility(side step test), muscular endurance(sit-up) and cardiopulmonary endurance(1500-meter run) to investigate the change of physique. It was found that there was not a statistically significant difference in the change of power(high jump test) and muscular endurance(sit-up) before and after measurement(P<.05). But it was found that there was a statistically significant difference on agility(side-step test) and cardiopulmonary(1500-meter run)(P<.05). It was found that there was a statistically significant difference in agility(side-step test), muscular endurance(sit-up) and cardiopulmonary endurance(1500-meter run) except power(high jump test) between he period of middle school days(13 to 14 years old) and the period of high school days(16 to 17 years old)(P<.05). The high jump test was conducted to investigate the change of power. As a result, it was found that power increase the period of middle school days(13 to 14 years old), whereas it decrease over the period of high school days(16 to 17 years old). On the other hand, the side-step test was conducted to investigate the change of agility. It was found that agility increased in Tawekwondo trainees over both the period of middle school days(13 to 14 years old) and the period of high school daysd(16 to 17 years old), the latter of which it was more or less improved. This study conducted the sit-up test to investigate the change of muscular endurance and the 1500-meter run test to investigate the change of cardiopulmonary endurance. In case of the 1500-meter run, cardiopulmonary endurance decreased over the period of middle school days(13 to 14 years old) but increased over the period of high school days(16 to 17 years old).

        보조호르몬요법을 받는 여성 유방암 환자의 모발 변화와 치료 실태에 대한 기초 연구

        Background: Hormone therapy, which includes tamoxifen and aromatase inhibitors, is the most common adjuvant therapy used for breast cancer. However, only a few studies have reported endocrine therapy induced alopecia. Objective: We investigated the effects of long-term adjuvant hormone therapy on hair in patients with breast cancer, in addition to patients’ concerns and current treatment for hair loss. Methods: Patients completed a questionnaire that included information on self-perceived hair changes after each adjuvant therapy session, distress, and current treatment for hair loss. Using a folliscope, we measured hair density and thickness in each patient and in healthy controls. Results: The study included 93 patients with breast cancer (mean age 51.9±9.8 years). The density and hair thickness were 106.36±21.85 hairs/cm2 and 0.07±0.01 mm in the patient group and 147.86±30.67 hairs/cm2 and 0.07±0.01 mm in the control group (n=98, mean age 52.10±8.40 years), respectively. The mean hair density was significantly lower in the patient group than in the control group; however, no statistically significant intergroup difference was observed in hair thickness. Among 76 patients who perceived hair changes after adjuvant therapy, 71.1% (n=54) were distressed with regard to hair changes. However, only 7.8% of the patients, including two who were treated by dermatologists, currently received treatment for hair changes. Conclusion: Dermatologists should be familiar with hair changes in patients with breast cancer and provide appropriate education to encourage patients to consult dermatologists for hair loss and thinning after breast cancer treatment.         펠던크라이스 기법, 탄성밴드 운동 및 복합운동 적용이 현대무용전공자의 체간근력에 미치는 영향

        This study is carried out to examine the effect of applying Feldenkrais method, elastic band training and combined training on trunk muscle. The subjects of this study were 32 modern dance majors and they were divided into four groups which consists of 8 peoples each, Feldenkrais method group(modern dance + Feldenkrais method), elastic band training group(modern dance + elastic band training), combined training group(modern dance + Feldenkrais method + elastic band training), and control group(modern dance). Measurement variables which are trunk muscle were measured before training, and after 4 and 8 weeks training and the results are as follow: In the left muscle strength change at Centaur 0°, Feldenkrais method group, elastic band training group and combined training group showed significant increase after 8 weeks training compared to before training. In the right muscle strength change, Feldenkrais method group and combined training group had significant increase after 8 weeks training. In the left muscle strength change at Centaur 45°, Feldenkrais method group and combined training group showed significant increase after 8 weeks training compared to before training and after 4 weeks training. Also elastic band training group showed significant increase after 8 weeks training compared to before training. In the right muscle strength change, Feldenkrais method group and elastic band training group significantly increased after 8 weeks training compared to before training. In the left muscle strength change at Centaur 90°, Feldenkrais method group and combined training group showed significant increase after 4 and 8 weeks training compared to before training. In the right muscle strength change, Feldenkrais method group showed significant increase after 4 and 8 weeks training compared to before training, and elastic band training group significantly increased after 8weeks training compared to before training. In the left muscle strength change at Centaur 135°, there was no significant changes. In the right muscle strength change, elastic band training group and combined training group showed significant increase after 8 weeks training compared to before training. In the left muscle strength change at Centaur 180°, combined training group showed significant increase after 4 and 8 weeks training compared to before training. In the right muscle strength change, combined training group showed significant increase after 4 and 8 weeks training compared to before training. From above results, applying 8 weeks regular program of Feldenkrais method, elastic band training and combined training to modern dance majors showed positive changes on improvement of trunk muscle strength and physical strength. Also this study implemented to verify the effects of trunk muscle strength and body balance through supplementary training program, and showed the effects in the training group and control group after 8 weeks trunk muscle strengthening program. Feldenkrais method, elastic band training and combined training are very effective to improve trunk muscle strength of modern dance majors so that continuous implementation and application of these effective training program shall be provided for strengthening trunk muscle.

        Three-dimensional analysis of soft and hard tissue changes after mandibular setback surgery in skeletal Class III patients

        하악 수술로 치료한 골격성 3급 부정교합 환자의 연조직 변화를 3차원적으로 분석하였다. 수술전과 수술후에 CT를 촬영하고, 연조직과 경조직을 각각의 임계값에 따라 segmentation였다. FH.평면, 정중시상면, PNS를 포함하는 전두면을 기준으로 공통 좌표계를 구성하고, 이 좌표계를 기준으로 술전, 술후 영상을 위치시켰다. 술후의 변화를 측정하기 위해 각각의 모형에 대해 전두면에 평행한 grid를 형성하였다. Grid내의 교점에서 골조직과 연조직 모형에 투사하여 만나는 점의 좌표값을 구하고, 이를 바탕으로 술후의 변화를 측정하였다. 하악골 후퇴술시 안모의 변화는 하악골 부분에서만 발생한 것이 아니라, 구각부에서도 관찰되었다. 하악골 부위의 연조직 변화는 대응되는 골조직 이동량에 따른 상대적인 값으로 계산하였다. 정중시상면에서의 변화율은 $77\~102\%$로 나타났다. (p<0.05). 정중시상면 이외의 부분의 변화양상도 이와 유사하였다. 구각부에서의 변화는 하악골의 이동을 대표할 수 있는 점의 이동량에 대한 상대적인 값으로 계산하였다. 정중시상면에서의 변화는 B점을 기준으로 $14\~29\%$이고, Pog점을 기준으로 $17\~37\%$, grid상 83번째 점을 기준으로 $11\~22\%$로 관찰되었다.(p<0.05) The three-dimensional (3D) changes of bone, soft tissue and the ratio of soft tissue to bony movement was investigated in 8 skeletal Class III patients treated by mandibular setback surgery. CT scans of each patient at pre- and post-operative states were taken. Each scan was segmented by a threshold value and registered to a universal three-dimensional coordinate system, consisting of an FH plane, a mid-sagittal plane, and a coronal plane defined by PNS. In the study, the grid parallel to the coronal plane was proposed for the comparison of the changes. The bone or soft tissue was intersected by the projected line from each point on the gird. The coordinate values of intersected point were measured and compared between the pre- and post-operative models. The facial surface changes after setback surgery occurred not only in the mandible, but also in the mouth corner region. The soft tissue changes of the mandibular area were measured relatively by the proportional ratios to the bone changes. The ratios at the mid-sagittal plane were $77\~102\%(p<0.05)$. The ratios at all other sagittal planes had similar patterns to the mid-sagittal plane, but with decreased values. And, the changes in the maxillary region were calculated as a ratio, relative to the movement of a point representing a mandibular movement. When B point was used as a representative point, the ratios were $14\~29\%$, and when Pog was used, the ratios were $17\~37\%(9<0.05)$. In case of the 83rd point of the grid, the ratios were $11\~22\%(p<0.05)$.

It usually affects cervicofacial, thoracic and abdominal tissue. Cervicofacial type has the highest percentage of occurrence with 50%. Actinomycosis frequently occurs following dental extraction, jaw surgery, chronic infection or poor oral hygiene. It may also be considered as an opportunistic infection in immunocompromised patients such as malignancy, human immunodeficiency virus infection, diabetes mellitus, steroid usage or alcoholism. But, actinomycosis rarely occurs in adults with normal immunity and rare in the esophagus. We report an unusual case of esophageal actinomycosis which was developed in a patient with normal immunity and improved by therapy with intravenous penicillin G followed oral amoxicillin, and we also reviewed the associated literature.

        A Case of Occupational Hypersensitivity Pneumonitis Associated with Trichloroethylene

        Trichloroethylene (TCE) is a toxic chemical commonly used as a degreasing agent, and it is usually found in a colorless or blue liquid form. TCE has a sweet, chloroform-like odor, and this volatile chlorinated organic chemical can cause toxic hepatitis, neurophysiological disorders, skin disorders, and hypersensitivity syndromes. However, the hypersensitivity pneumonitis (HP) attributed to TCE has rarely been reported. We hereby describe a case of HP associated with TCE in a 29-year-old man who was employed as a lead welder at a computer repair center. He was installing the capacitors on computer chip boards and had been wiped down with TCE. He was admitted to our hospital with complaints of dry coughs, night sweats, and weight losses for the past two months. HP due to TCE exposure was being suspected due to his occupational history, and the results of a video-associated thoracoscopic biopsy confirmed the suspicions. Symptoms have resolved after the steroid pulse therapy and his occupational change. TCE should be taken into consideration as a potential trigger of HP. Early recognition and avoidance of the TCE exposure in the future is important for the treatment of TCE induced HP.
