Background/Aims: Propofol induced sedation with bispectral index (BIS) monitoring has been reported to lead to higher satisfaction in patients and endoscopists during endoscopic submucosal dissection (ESD) procedures. There are no data, however, regarding the effi cacy of midazolam and meperidine (M/M) induced sedation with BIS monitoring during ESD. The purpose of this study was to evaluate whether M/M induced sedation with BIS monitoring could improve satisfaction and reduce the dose of M/M required during ESD. Methods: Between September 2009 and January 2010, 56 patients were prospectively enrolled and randomly assigned to a BIS group (n=28) and a non-BIS group (n=28). Patient and endoscopist satisfaction scores were assessed using the visual analog scale (0 to 100) following the ESD. Results: The mean satisfaction scores did not significantly differ between the BIS and non-BIS groups (92.3±16.3 vs 93.3±15.5, p=0.53) or endoscopists (83.1±15.4 vs 80.0±16.7, p=0.52). Although the mean meperidine dose did not differ (62.5±27.6 vs 51.0±17.3, p=0.18) between the two groups, the mean dose of midazolam in the non-BIS group was lower than in the BIS group (6.8±2.0 vs 5.4±2.1, p=0.01). Conclusions: BIS monitoring during ESD did not increase the satisfaction of endoscopists or patients and did not lead to an M/M dose reduction. These results demonstrate that BIS monitoring provides no additional benefi t to M/M induced sedation during ESD. (Gut Liver 2011;5:160- 164)

한국감성과학회의 홈페이지의 구성을 위하여 형식 문법(HTML ; Hyper Text Markup Language)의 작성은 넷스케이프(NETSCAPE)의 컴포저(Composer)를 이용하였으며, 필요한 그림 및 아이콘(Icon)은 Adobe Photoshop 4.0 버전으로 작성하였고, Map This 1.3버전을 이용하여 이미지 맵들을 구성하여 학회의 홈페이지를 만들었다. 감성과학회의 홈페이지를 시작으로 회원마당/가입안내, 학회안내, 행사일정 및 소식, 학회논문 검색, 국제정보를 하부구조로 구성하였으며, 각 하부구조 밑에 또 다른 하부구조를 구성하도록 하였다. 이렇게 본 학회의 홈페이지를 구축함으로써 쉽게 학회의 행사 및 소식을 회원에게 전달할 수 있으며, 학회의 가입 및 신청과 논문 투고요령, 학회강령을 각 회원 및 비회원에게도 편리하게 홍보할 수 있으며, 세계적인 수준의 학회로도 발전해 갈 수 있다.

<P>Abstract</P><P><I>Background and Aims:</I> Our aim was to investigate the incidence of tumor recurrence in cases in which there was a discrepancy in the diagnosis of complete resection by pathologists and colonoscopists, especially in the lateral resection margin. <I>Methods:</I> We reviewed 245 patients with colorectal tumors that were treated by endoscopic submucosal dissection (ESD) between March 2006 and June 2011. We evaluated the recurrence rate in cases judged as pathologically incomplete resection despite endoscopically complete resection. <I>Results:</I> Of the 24 cases with tumor cells on the lateral resection margin, the histologies of the colorectal tumors were found to be tubular adenoma (TA) with low-grade dysplasia (n = 19), TA with high-grade dysplasia (n = 3) and differentiated adenocarcinoma (n = 2). No tumor recurrence was observed in 22 patients after ESD. Four patients did not receive surveillance colonoscopy. The median tumor size was 28 mm (12–35) and the median follow-up period was 19 months (5–42). <I>Conclusions:</I> This retrospective analysis was limited by a short follow-up period. However, surveillance colonoscopy could be attempted without additional ESD in those cases in which incomplete resection on the lateral margin was judged pathologically, if endoscopic complete resection was grossly achieved.</P><P>Copyright © 2011 S. Is a Second-Look Endoscopy Necessary after Endoscopic Submucosal Dissection for Gastric Neoplasm?

Background/Aims: Second-look endoscopy is performed to check for the possibility of post-endoscopic submucosal dissection (ESD) bleeding and to perform prophylactic hemostasis in most hospitals; however, there is little evidence about the efficacy of second-look endoscopy. We investigated whether second-look endoscopy after ESD is useful in the prevention of post-ESD bleeding. Methods: A total of 550 lesions with gastric epithelial neoplasms in 502 patients (372 men and 130 women) were treated with ESD between August 18, 2009 and August 18, 2010. After the exclusion of three lesions of post-ESD bleeding within 24 hours, 547 lesions (335 early gastric cancers and 212 gastric adenomas) were included for the final analysis. Results: The occurrence rate of delayed post-ESD bleeding was not significantly different between the second-look group and the no second-look group (1% vs 2.5%, p>0.05). The only predictor of delayed bleeding was tumor size, regardless of second-look endoscopy after ESD (22.8±9.87 vs 15.1±10.47, p<0.05). There was no difference between the prophylactic hemostasis and nonprophylactic hemostasis groups, including the occurrence rate of delayed bleeding. In the second-look group with prophylactic hemostasis, the hospital stay was more prolonged than in the second-look group without prophylactic hemostasis, but there was no significant difference (p=0.08). Conclusions: Second-look endoscopy to prevent delayed bleeding after ESD provides no significant medical benefits. (Gut Liver 2015;9:52-58)

최근 PC의 성능이 향상되고 디지털 신호처리 기술이 발달함에 따라 복잡한 계산을 필요로 하는 입체음향 처리를 PC상에서도 구현하는 것이 가능하게 되면서, 멀티미디어 분야에서 입체음향에 대한 관심이 고조되고 있다. 특히 2채널 방식의 입체음향 기술은 공간활용성과 경제성에서 우수하여 많은 연구가 진행되고 있다. 2채널 재생 방식에 있어서 위치음 효과는 비교적 단순한 반면에, 이동음 효과는 특정 점에서만 측정된 머리전달함수를 이용하여 이동하는 음을 생성해야 하기 때문에 해결해야 할 문제점이 많다. 본 논문에서는 입체음향을 2채널로 재생할 때 이동음 효과가 부드럽게 생성되도록 하는 방법에 대해 다루고자 한다. 특히 부드럽게 궤적을 생성하는 방법, 그리고 이동음의 출력이 이산적인 공간에서 부드럽게 연결되도록 하는 방법에 대해서 제안하고 실제 테스트를 통해서 그 유용성을 보여준다. As it became possible to generate 3D sound on a PC environment due to the advances in computing performance and digital signal processing technology, 3D sound technology gains its focus in the multimedia area. Specifically, a two-channel based 3D sound technology is being studied by many researchers because of its space efficiency and economical structure. While the positional sound effect is simple in its implementation, the moving sound effect has many problems to be resolved as there are only discrete measured point of HRTF database. In this paper, we propose the method of generating smooth moving sound in a two-channel based 3D sound technique with respect to generating smooth trajectory, and the interpolation method of discrete measured HRTF data. We perform the tests in the PC environment and prove the utility of the proposed method.

Background/Aims: Internet has become an important source of medical information not only for doctors but also patients. However, information available in the Internet may provide wrong or even harmful knowledge to the public. The aim of this study was to evaluate the quality of Internet-based medical information about Barrett`s esophagus in Korea. Methods: The first 50 Internet links were retrieved from the Google using the key word `Barrett`s esophagus`. The quality of information from a total of 49 websites was evaluated using a checklist. Results: Among total 49 sites related to `Barrett`s esophagus`, only 4 sites (8.2%) were made by hospitals or clinics, and 11 sites (22.4%) were for patients. Of the 49 sites, only one web site (2.0%) had all HON CODE principles (authority, complementarity, confidentiality, attribution, justifiability, transparency of authorship, transparency of sponsorship, honesty in advertising and editorial policy). Sixteen Internet links (32.0%) had fair contents for the definition, and 24 links (48.0%) for the diagnosis, and 15 links (30.0%) for the treatment. Conclusions: Information about Barrett`s esophagus was incomplete in the majority of medical web sites. It will bring about confusion in patients who want to get information about Barrett`s esophagus from the Internet. There is a need for better evidence-based information about Barrett`s esophagus on the web. (Korean J Gastroenterol 2009;53:292-296)


두안면부 변형에 대한 Three-Dimensional CT의 진단적 의의

Recently, successful surgical reconstruction of congenital and acquired cramofacial deformities has become routine procedure in a number of centers. centers. The planning and evaluation of such operations are dependent on radiologic imaging for defining the aberrant anatomy. Traditional radiologic images, including CT scans, have restricted twe-plane presentaion. It has limited value for the patients with major craniofacial deformities. In this reason, three-dimensional display 9of surface(3-D CT image) derived from serial high-resolution computed tomographic scans has been investigated. Then, we have applied this method to 30 patients with craniofacial deformities, and following reults were obtained. 1. The 3-D CT image produces simulated photographs of the complex craniofacial deformitis and requires no operator intervention. 2. The direct visualization of anatomic details has reduced operation time and improved surgical result. 3. Life-size 3-D CT image produce of permit accurate dimensional measurement required for surgical planning.
