Introduction: Peripheral T-cell lymphoma is a disease that accounts for approximately 5-20% of all non-Hodgkin`s lymphomas. The organs that it mainly affects are known to be the skin, liver, and lungs, and invasions in muscles and pleural fiuids are relatively unknown. This paper reports a rare case of atypical peripheral T-cell lymphoma that was confirmed through special immunochemical staining in muscular tissues, Nasal polyp and pleural fiuids` examination. Case Report: A 79 year old woman was transferred to the author`s hospital with continual febricity for two months and Right Shoulder pain. In the Laboratory tests, Alkaline Phosphatase, Aspartate aminotransferase and C reactive Protein was elevated. In the proton computed tomography in the Nasal cavities, Sphenoidal sinus, Both Pleura and Right infraspinatus muscle, the SUVmax increased and showed the possibiity of malignancy. In the tissue biopsy on the soft tissue in the right infraspinatus muscle, atypical cells that were suspected as lymphoma were shown, but their necrosis was so severe that special immunohistochemical staining was impossible. In the biopsy on the left nasal cavity, the CD3, CD56, Ki-67, UCHL-1, and Granzyme B were all positive, so the condition was diagnosed as peripheral T-cell lymphoma. Also, Same diagnosis was confirmed in the pleural fiuid`s cell block examination. Conclusion: The authors experienced a rare form of peripheral T-cell lymphoma that invaded the pleural fiuid and the skeletal muscle. Thus, when there are symptoms such as pain and edema related to the skeletal muscle of an elderly patient, an active biopsy on associated parts of the body and confirmation of tumor invasion of other parts of the whole body are considered useful in making a diagnosis.
Background/Aims: APASL and KASL guideline recommend only two drugs for naive CH-B patients. Tenofovir (TDF) is one of two nucleoside analogues which show potent antiviral effi cacy and scarce SNP associated with drug-resistance. TDF was permitted as a therapeutic drug for CH-B patients only after Jan 2013. We still need more data related to tenofovir effi cacy and adverse effect. We want to compare the early effi cacy and Antiviral effects of Tenofovir with those of Telbivudine. Methods: The patients were enrolled from January 2013 to August 2014. One arm (n=22) was administered with Tenofovir and the other arm (n=15) with Telbivudine. The patient who experienced antiviral therapy before the enrollment was ruled out. There was no signifi cant difference in baseline characteristics except for Age. We used T-test and Mann-Whitney u test as statistical methods. Results: The log(HBV DNA) level of TDF vs LdT group were 6. 58+-1. 97 vs 6. 69+- 0. 97 at baseline (p=0. 845), 3. 39+-0. 95 vs 4. 67+-2. 61 at 1 month(p=0. 58) and HBV DNA level difference between baseline and 1 month of treatment were 3. 35+-1. 30 vs 2. 06+-1. 97(p=0. 058). The Albumin level of TDF vs LdT group were 4. 56+-0. 25 vs 4. 23+-0. 33 at 9 months (p=0. 003). The ALT level of TDF vs LdT group were 51. 90+- 383. 18 vs 30. 53+-13. 39 at 3 months (p=0. 045), 50. 40+-31. 52 vs 32. 73+-17. 76 at 6 months (p=0. 060) and 42. 90+-29. 62 vs 24. 15+-8. 58 at 9 months (p=0. 035). Conclusion: Tenofovir had superior effect in terms of viral suppression during early phase of treatment. Tenofovir had better biochemical response in the first year of treamement. Tenofovir was recommended as a primary therapy, furthermore it should be chosen for the urgent situation because it`s viral suppression appeared more rapidly.

A nasal-type extranodal natural killer/T-cell lymphoma is considered an aggressive form of non-Hodgkin`s lymphoma, with approximately half of all patients relapsing during the follow-up period, and most relapses occurring within the first 2 years of remission. Here we report an unusual case of a 42-year-old man who experienced recurrence in single pleura after 8 years of remission.
