        Background: This study evaluated the efficacy of administering Donepezil(Aricept^(?)) for improving cognitive function, activities of daily living, and quality of life in patients with post-stroke dementia. Methods: Patients(n=99; mean age, 69.89 years; 48.5% men) with post-stroke dementia, according to criteria of the DSM-IV and MRI, were treated with donepezil 5mg/day or 10mg/day during 24 weeks. They were examined using Mini-mental Status Examination(MMSE), Chinical Dementia Rating(CDR), Barthel Activities of daily living(B-ADL), Instrumental Activities of daily living(I-ADL), Short Form 36 Health Survey-Korean(SF-36-K), and Geriatric Depression Scale(GDS) at before treatment and 24 weeks after treatment. Results: The 97 patients completed the trial. At 24 weeks, they showed significant improvement in MMSE(p<0.001), CDR(p<0.001), B-ADL(p=0.002), I-ADL(p<0.001), and SF-36-K(p=0.003) except GDS(p=0.288). Donepezil was well tolerated. There was no withdrawal due to adverse events. Conclusion: This results demonstrate that donepezil was beneficial in improving the cognitive function, activities of daily living, and quality of life in patients with post-stroke dementia and was well toleraetd.

        Chemopreventive effect of tolfenamic acid on KB human cervical cancer cells and tumor xenograft by downregulating specificity protein 1

        Earlier studies have shown that tolfenamic acid (Tol) exhibits anticancer activity in several cancer models by inhibiting tumor growth and angiogenesis. However, the chemopreventive effect of Tol on a cervical cancer model and the underlying mechanism of action are unknown. In this study, Tol was found to inhibit cell proliferation by inducing apoptosis without affecting cyclo-oxygenase 2 expression, but ampiroxicam did not. Tol decreases the specificity protein 1 (Sp1) mRNA and its promoter activity in KB cervical cancer cells, and the downregulation of Sp1 protein by affecting several proteins that contain GC-rich sites on their promoters. Studies using small interference RNA and an Sp1-specific inhibitor (mithramycin A) confirmed that the decrease in Sp1 by Tol affects survivin and p27. Tol also inhibited tumor growth and Sp1 protein in athymic nude mice xenografts. These results show that Tol could be a potent anticervical cancer drug that acts by regulating Sp1 protein and its downstream pathways.

      • Clonidine 전투약이 두부고정핀 거치시 혈역학 반응에 미치는 영향

        서론: 신경외과 환자중 두부고정핀을 거치하는 환자에서 경구용 clonidine 3 ㎍/㎏을 전투약한 경우 두부고정핀 거치에 의한 혈압 및 심박수에 미치는 영향을 알아보고자 본 연구를 시행하였다. 방법: 미국마취과학회 환자분류 1, 2급에 해당하는 제충 50∼70 ㎏, 20∼62세의 전신마취를 필요로 하는 두개강내 수술환자중 두부고정핀을 거치하는 40명으로 술전 혈압이 정상이고 순환계, 호흡계 및 자율신경계 질환의 기왕력이 없는 환자를 대상으로 하였다. 대상환자는 두군으로 나누어 대조군(n=20, 제1군), clonidine군(n=20, 제2군)으로 무작위로 나누었으며, 전투약으로는 대조군에서는 glycopyrrolate 0.2 ㎎을 수술실 입실 한시간 전에 근주하였으며, clonidine군에서는 clonidine 3 ㎍/㎏을 수술실 입실 90분전에 경구투여하고 glycopyrrolate 0.2 ㎎은 수술실 입실 한시간 전에 근주하였다. 본 연구에서 측정하고자 하는 혈압 및 심박수는 제1군 및 제2군 모두에서 두부고정핀에 의한 고정이 시작되기 전에 측정한 측정치를 대조치(C)로 하고 고정핀을 고치한 직후부터 1분 간격으로 10분(T1∼T10)까지 10회에 걸쳐 수축기혈압, 이완기혈압, 평균동맥압 및 심박수를 관찰하여 대조치와 비교한 후 대조치에 따른 백분율로 증감을 보았다. 또한 제1군과 제2군을 비교하여 clonidine 투여의 효과도 비교하였다. 결과: Clonidine 3 ㎍/㎏의 투여군과 대조군을 비교시 수축기혈압은 두부고정핀 거치후 3분(P<0.05), 6분(P<0.05), 7분(P<0.05), 9분(P<0.05), 10분(P<0.01)에, 이완기혈압은 1분, 3분(P<0.05), 7분(P<0.05), 9분(P<0.01), 10분(P<0.05)에, 심박수는 1분(P<0.05), 7분(P<0.01)에 의미있는 차이를 볼 수 있었다. 결론: Clonidine 3 ㎍/㎏의 전투약은 두부고정핀의 거치에 의해 발생되는 혈압의 상승 및 심박수의 증가를 의미 있게 감소시키는 효과를 발견할 수 있었다. Backgroud: The aim of this study was to observe the effect of premedication with oral clonidine on hemodynamic changes in patients undergoing pin head-holder application for neurosurgical operation. Methods: Sixty ASA Ⅰ/Ⅱ patients in a double-blind prospective study were assigned randomly to receive placebo (Group 1, m=20) or clonidine (Group 2, 3 ㎍/㎏/po, m-20) 90 minutes before surgery. Blood pressure (BP) and heart rate (HR) were monitored continuously during and immediately after anesthetic induction. Anesthetic induction was achieved with vecuronium (0.1 ㎎/㎏), thiopental sodium (5 ㎎/㎏) and N_2O+O_2+isoflurane, maintained with N_2O+O_2+isoflurane. Hemodynamic response such as systolic BP, diastolic BP, mean arterial pressure and HR to pin head-holder application was checked just before application (control value), 1 to 10 min after application with or without clonidine administration and compared between groups. Results: In clonidine treated group (Group 2), at 3 min (P<0.05), 6 min (P<0.05), 7 min (P<0.05), 9 min (P<0.05), 10 min (P<0.01) in systolic BP, at 1 min (P<0.01), 3 min (P<0.05), 7 min (P<0.05), 9 min (P<0.01), 10 min (P<0.05) in diastolic BP, and at 1 min (P<0.05), 7 min (P<0.01) in HR after pin head-holder application were significantly lower than control group (Group 1). Conclusions: We conclude that preoperative oral treatmetn with clonidine 3 ㎍/㎏ reduced hemodynamic changes effectively after pin head-holder application.

      • 임상적으로 ITP로 진단받은 환아에게 골수검사의 필요성 유무

        Objective : Bone marrow examination is controversial in childhood idiopathic thrombocyropenic purpura(ITP). The purpose of this study is whether bone marrow examination is needed or not in pediatirc patients clinically diagnosed ITP. Method : To asses the need for bone marrow examination, we followed up 64 patients with presumed ITP. In this patients, Bone marrow examinations were performed in 37 patients at initial diagnosis of ITP and 27 patients are not preformed bone marrow examination at initial diagnosis of ITP. In all 64 patients, the onsets of the disease were abrupt and congenital anomalies were not combined. Physical examination most often reveals mucocutaneous pethechiae and bruise, but lymph node, liver, and spleen were not enlarged. in addition, the complete blood cell count were normal, except for the thrombocytopenia. Results : 1. When clinical ITP was diagnosed initially, bone marrow examinations were preformed in 37 patients. Among these patients, the results of bone marrow examination were not consistent with ITP in 4 patients. In 4 patients, one patient is diagnosed as transient erythroid hypoplasia, another was diagnosed as decreased megakaryocyte, the others were diagnosed as hypocelluar marrow. During following up 37 patients, none had leukemia, aplastic anemia, and other diagnosis. 2. When clinical ITP is diagnosed initially, bone marrow examinations were not performed in 27 patients. these patients were treated with intravenous globulin(IV-G). 5 patients did not response with IV-G. In these 5 patients, only 2 patients progressed to chronic ITP, but were not diagnosed as other disease. Conclusion : Proper history and physical examination as well as a complete blood cell count are reliable means of diagnosing ITP. Bone marrow examination must be considered carefully to ITP patients who are clinically diagnosed.

        The effectiveness of exclusive bus lane is hot potatoes in practical as well as academic field. In particular, that of Kyoungbu Expressway implemented from 1st of October in 2009 has been the major target of critics due to the reason that it has worsened the road condition by shrinking the road capacity of general lane. By setting the model of users` choice under the imperfect substitutability between modes this paper made clear under what condition efficiency of exclusive bus lane could be effective instrument to handle the road condition, Furthermore, this paper evaluated the equity among different mode users and among regions, This paper shows that users` preferences to private car is key factor in determining the efficiency of exclusive bus lane. Thus, exclusive bus lane might not be better choice to cope with road congestion when users` preference to private car is strong. In this case, exclusive bus lane accompanied by road expansion or road expansion only might be more effective policies than exclusive bus lane only. However, exclusive is not regressive policy option, since public transport user could be more benefitted by that policy. By applying the analytical model to the situation of Kyoungbu Expressway, this paper shows that exclusive bus lane has greatly increased the public transport users` benefit even though it decreased the benefit of private car users slightly. Thus, average users` benefit might be increased with express bus lane. Furthermore this paper shows that users` benefit of Seoul city has increased with implementation of express bus lane. This paper also suggested that flexible operation would be desirable to enhance the efficiency of express bus lane and also suggested that better equipment of public transport system could be desirable way to derive the consent to express bus lane from the private car users.

      • 서울시 물류,유통 관련 인천광역시와의 전략적 협력방안 연구

        Seoul has experienced urban sprawl and change in industrial organization which has resulted in increase of costs of congestion and freight transport. On the other hand, infrastructures for logistics and distribution between Seoul and Incheon are being and will be constructed in the near future, which is expected to lower the cost of logistics. It is needed to lower the cost of logistics by using the newly constructed infrastructure effectively to obtain competitiveness in Mega City Region. This report deals with the strategy for cooperation in logistics and distribution system between Seoul and Incheon by investigating the general conditions surrounding Seoul and Incheon. By analyzing the volume of freight and traffic of truck between Seoul and Incheon, we found that interdependence between two regions has increased and intensified. In particularly Seoul has played the role of demand for freight from Incheon. This shows necessity for the effective usage of infratructures and for the cooperation between two regions. We derived establishment of green-growth-oriented logistics and distribution system as the basic directions for cooperation between Seoul and Incheon. For accomplishment of this goal, we suggested three tasks for cooperation between two regions, which are establishment of effective freight transport system, effective allotment of function of infrastructures for transport of freight, and establishment of environment-friendly green logistics system. For establishment of effective freight transport system, firstly, infrastru ctures newly-planned should be constructed as scheduled. Secondly, exclusive lanes for trucks should be established in expressway connecting the main points of logistics and distribution in Seoul and Incheon, which is expected to lower the transport cost of freight between two regions. For the effective allotment of function of infrastructures for transport of freight, firstly, function of Kimpo international airport should be strengthened due to the accessibility to Seoul and slack capacity of Kimpo international airport. Secondly, role of expressways which are existing and planned to be constructed should be alloted according to the destination of expressway between Seoul and Incheon. For establishment of environment-friendly green logistics system, firstly transportation demand management (TDM) should be strengthened which is expected to result in lowering the transport cost as well as congestion cost. For example cordon-pricing could be implementated as in the case of Stockholm. Toll revenue could be returned to Incheon, because benefit from the alleviated congestion by such tolling scheme is expected to belong to Seoul. Such toll revenue could be expended for infrastructure of public transport in Incheon. Secondly, affirmative intervention and financial support of central government are needed, since implementation of congestion charging scheme might be costly.

        Gallamine Triethiodide 가 안압에 미치는 영향

        The purpose og this experiment was to evaluate the effect of gallamine triethiodide on the intraocular pressure during general anesthesia for intraocular surgery. Twenty parionts in the American Society of Anesthesiologists, physical status I and II, aged from 15 to 65 years with no eye or kidney diseases were stydied during anesthesia for elective surgery. All were premedicated with 50mg of meperidine and 0.4mg of atropine. Before inducation intraocular pressure was measured under topical anesthesia with 0.5% tetracaine in the eye control value determined. Anesthesia was indyced with 5mg/kg of 2.5% thiopental and I mg/kg of succinycholine. The intraocular pressure was measured after endo-tracheal intubation.Anesthesia was maintained with 2mg/kg of meperidine, nitrous oxide and oxygen. On the return of spontaneous respiration followig succinycholine, 2mg/kg of gallamine triethiodide was adminidtered. Subsequent measurements were made 10 and 20 minutes after gallamine triethiodide administration, respectively. The results were as follows; 1) Succinycholine caused a significant rise in intraocular pressure. 2) Gallamine trierhiodide cause a slight decrease in intraocular pressure 10 and 20 minutes after administration.
