Objective Coronavirus disease 2019 (COVID-19) has notably altered the emergency department isolation protocol, imposing stricter requirements on probable infectious disease patients that enter the department. This has caused adverse effects, such as an increased rate of leave without being seen (LWBS). This study describes the effect of fever/respiratory symptoms as the main cause of isolation regarding LWBS after the COVID-19 pandemic.Methods We retrospectively analyzed emergency department visits before (March to July 2019) and after (March to July 2020) the COVID-19 pandemic. Patients were grouped based on existing fever or respiratory symptoms, with the LWBS rate as the primary outcome. Logistic regression analysis was used to identify the risk factors of LWBS. Logistic regression was performed using interaction terminology (fever/respiratory symptom patient [FRP] × post–COVID-19) to determine the interaction between patients with FRPs and the COVID-19 pandemic period.Results A total of 60,290 patients were included (34,492 in the pre–COVID-19, and 25,298 in the post–COVID-19 group). The proportion of FRPs decreased significantly after the pandemic (P < 0.001), while the LWBS rate in FRPs significantly increased from 2.8% to 19.2% (P < 0.001). Both FRPs (odds ratio, 1.76; 95% confidence interval, 1.59–1.84 (P < 0.001) and the COVID-19 period (odds ratio, 2.29; 95% confidence interval, 2.15–2.44; P < 0.001) were significantly associated with increased LWBS. Additionally, there was a significant interaction between the incidence of LWBS in FRPs and the COVID-19 pandemic period (P < 0.001).Conclusion The LWBS rate has increased in FRPs after the COVID-19 pandemic; additionally, the effect observed was disproportionate compared with that of nonfever/respiratory symptom patients.

Recently, the kinematic alignment has been introduced as a total knee arthroplasty alignment method. It was known to reduce the initial heterogeneity in patients. In this study, we studied the changes in the knee joint's kinematics according to the alignment method. Eight pairs of cadavers were tested. For each pair, one knee was randomly assigned to the kinematic alignment TKA group (KA) and the other to the mechanical alignment TKA group (MA). During KA, the sizes of femur and tibia resections were equivalent to implant thickness. MA was performed using conventional measured resection techniques. All specimens were mounted on a customized knee-testing system, and knee motions included rollback, patella movement, were measured by a motion-analysis system during flexion. The pattern of knee motion following KA was similar to the native knee. However, following MA, both medial and lateral rollback were decreased relative to those of the native knee. In the patella movement, we found no significant difference according to the alignment method, but the KA showed similar with native knee relatively.

Many devices were developed to simulate the movement of the knee joint. We designed the knee joint simulator which could move flexion to extension of the knee joint continuously based on the Oxford knee simulator, and investigated the knee joint kinematics using the designed knee simulator. This study introduced the methods and results of the biomechanical test of the knee joint through the evaluation of the designed knee simulator, the preparation process of the cadavers, motion analysis using motion capture cameras and optic markers, and measurement of the surface strain using the video extensometer. To obtain the results of the motion analysis, the medial and lateral rollback of the knee and patella movements were measured using 12 optic markers, also, the results showed high repeatability. The video extensometer could measure the strain of the ligamentous structures during flexion and extension of the knee joint in real-time, and, this showed high repeatability in the test.

본 연구에서는 추진기관 급속가열시험을 실시하여 그 특성을 분석 평가 하였다. 연소관은 카본 에폭시 복합재를 사용하였고 추진제는 둔감 특성을 향상 시켜주기 위해 HTPE 추진제를 사용하였다. 반응 형태를 정량적으로 판단하기 위해 음압 및 열유속 센서를 사용하였다. 급속가열 시험한 HTPE 모타들의 반응형태는 Type V 인 연소반응을 나타내었다. Fast cook-off test with rocket motors was performed and characteristics of the results were analyzed. The material of the motor case was carbon epoxy composite. The motor was loaded with HTPE propellants to improve the insensitive munitions characteristics. In the tests, sound pressure and heat flux sensors were used to determine the category of response according to the standard. The reaction response of all of the HTPE motors tested by fast cook-off was judged as Type V burning.

Objective-The Structured Interview for Prodromal Syndrome (SIPS) from Yale University is intended to diagnose prodromal syndrome of psychosis and to measure the severity of prodromal symptoms. Here, a Korean version of SIPS is presented, and its reliability, validity, and factor structures are examined using a representative Korean sample. Methods-The Korean version of SIPS was administered to 40 participants over a period of 1 year. The inter-rater reliability and internal consistency of the SIPS were then evaluated. In addition, its factor structure was investigated using principal-axis factor analysis. Concurrent validity was explored using Pearson correlation coefficients with the Positive and Negative Syndrome Scale (PANSS). Results-Of the 40 subjects, 12.5% developed psychotic disorders during the 1-year follow-up period. Inter-rater reliability was good (intra-class correlations=0.96), and internal consistency was acceptable (Cronbach’s alpha=0.83). A three-factor resolution displayed the best simple structure and accounted for 52.6% of all item variance. Factors 1 and 2 showed strong correlations with negative symptoms and cognitive dysfunction, respectively, on the PANSS. Factor 3 was not correlated with any factor on the PANSS. Conclusion-The Korean version of SIPS is a reliable instrument for the assessment of prodromal symptoms in subjects and may be used to evaluate prodromal psychosis.

<P>We introduce an opportunistic interference mitigation (OIM) protocol, where a user scheduling strategy is utilized in K-cell uplink networks with time-invariant channel coefficients and base stations (BSs) having M antennas. Each BS opportunistically selects a set of users who generate the minimum interference to the other BSs. Two OIM protocols are shown according to the number of simultaneously transmitting users per cell, S: opportunistic interference nulling (OIN) and opportunistic interference alignment (OIA). Then, their performance is analyzed in terms of degrees-of-freedom (DoFs). As our main result, it is shown that KM DoFs are achievable under the OIN protocol with M selected users per cell, if the total number of users in a cell, N, scales at least as SNR<SUP>(K-1)M</SUP>. Similarly, it turns out that the OIA scheme with S(<;M) selected users achieves KS DoFs, if N scales faster than SNR<SUP>(K-1)S</SUP>. These results indicate that there exists a trade-off between the achievable DoFs and the minimum required N. By deriving the corresponding upper bound on the DoFs, it is shown that the OIN scheme is DoF-optimal. Finally, numerical evaluation, a two-step scheduling method, and the extension to multi-carrier scenarios are shown.</P>
