        조혈모세포이식 환자에서 침습성 진균 감염에 대한 Micafungin의 예방 효과 및 안전성

        Background: Micafungin, a potent inhibitor of 1,3-β-D-glucan synthase, is a novel antifungal agent of the echinocandin class. In vitro study showed that micafungin was effective against Aspergillus species as well as Candida species, but clinical data on the prophylactic efficacy against invasive fungal infections (IFIs) other than candidiasis are still lacking. Materials and Methods: We identified 60 consecutive adult hematopoietic stem cell transplantation (HSCT) recipients who received at least 3 doses of micafungin during neutropenic period. Micafungin was started as an alternative in patients who were intolerant or had adverse events (AEs) to primary prophylactic antifungal agents. We retrospectively reviewed the medical records and analyzed the efficacy and safety of micafungin for prophylaxis against IFIs. Results: The patients either had autologous (n=9) or allogeneic (n=51: 1 syngeneic, 24 sibling, 26 unrelated donor) HSCT. Itraconazole oral solution (n=58) was the most frequently used first line antifungal agent for prophylaxis and was administered for median 11 days. The most frequent cause of switch to micafungin was vomiting (n=42). The duration of neutropenia and micafungin administration was median 13 and 12 days, respectively. A successful outcome was achieved in 45 (75%) patients. Empirical antifungal therapy was initiated in 13 (22%) patients. There were 2 cases (3.3%) of breakthrough fungal infections which comprised a probable invasive pulmonary aspergillosis and a possible invasive fungal sinusitis. There was no case of invasive candidiasis. A total of 53 (88%) patients experienced at least one AE regardless of causality during micafungin administration. The most frequent AEs were hypokalemia, vomiting, diarrhea, and elevated serum aspartate aminotransferase or alanine aminotransferase. Among the aforementioned AEs, only 1 case of diarrhea could be classified as a probable relation with micafungin when causality was assessed. There was no AEs that caused discontinuation of micafungin. Conclusions: Micafungin seems to be a safe and effective agent for prophylaxis of IFIs including aspergillosis as well as candidiasis in HSCT recipients. However, further large, prospective, and randomized comparative studies are warranted for aspergillosis.

      • 주파수 영역에서 다변수 제어 분석

        본 연구는 2개 이상의 입력과 출력으로 표현되는 다변수 시스템의 제어에 필요한 설계 요구 조건을 주파수 영역에서 분석하여 시스템에 적용하는 방법에 대하여 알아본다. 설계 요구 조건은 정상편차, 절점 주파수(cross-over frequency), 다변수에 의한 최대 및 최소 특이 값(singular value), 그리고 교란 및 잡음제거 등이며 목표 루프(target loop)는 Kalman 필터가 적용된다. 이러한 설계 요구 조건을 만족시키는 제어장치를 설계하는 주파수 영역에서의 분석은 LQG/LTR(Linear-Quadratic-Gaussian with Loop-Transfer-Recovery) 기술을 근거로 적용된다. 제어장치를 적용한 시스템의 폐루프 및 민감도 전달함수에 대한 특이 값을 주파수에 영역에서 분석하고 시간 영역에서 입력과 출력의 과도응답의 정확성과 반응속도에 대하여 알아본다. The main purpose of this study is an analysis of multi-variable control in frequency domain. This analysis is based on LQG(MBC)/LTR methodology. Target loop is designed by Kalman filter which includes the design specifications such as cross-over frequency, identification of maximum and minimum singular values, rejection of sensor noise in high frequency domain and disturbance in low frequency domain. Closed loop and sensitivity transfer function are analyzed by singular values in frequency domain and transient responses of input and output are also analyzed in time domain.

      • 사람 비만세포주에서 Clozapine과 Fluoxetine에 의한 케모카인 및 케모카인 수용체의 상이한 발현

        Objectives : It has been suggested that the immune system has a role in the pathophsiology of neuropsychiatric diseases. There are some reports that clozapine and fluoxetine affect on cytokine networks. Recently, it has been known that chemokines have chemotactic effect, and modulate a number of biological responses including the process of inflammation and the maintenance of CNS homeostasis. In the present study, we investigated the effect of clozapine or fluoxetine on the expression of chemokines and their receptors. Methods : Human mast cells(HMC-1) was incubated with phorbol myristate acetate(PMA, 100ng/㎖) and calcium ionophore A23187(350ng/㎖) for 3 hours. Colzapine(10^-7M) or fluoxetine(10^-7M) were pretreated for 1 hour. RT-PCR was performed for the detection of expression of various chemokines and chemokine receptors. Results : after treatment of clozapine, the expression of MCP-1, MIP-1β, CCR3,CXCR2, CXCR3 and CXCR4 were lower than the PMA/Ca ionophore-treated group, while the expression of MIP-1α, RANTES and IL-8 were not changed. However, fluoxetine was not affected the changes of expression of various chemokines and chemokine receptors. Conclusion : This result indicates that clozapine may affect on the expression of chemokines and chemokine receptors. Therefore, this study could, in part, provide the important basic data on the explanation of side effects of clozapine, such as fever and pancreatitis.

      • 준설토의 침강특성에 관한 실험적 연구

        Column tests in the laboratory were preformed to investigate characteristics of settling process of dredged soil sampled from in-situ. Test results were analyzed by using the existing theories on discrete settling and hindered settling. From column tests of monitoring the interface with time, settling was found to be a linear process with time and the settling rate was increased with initial water content of slurry. The settling rate was also observed to decrease with increasing initial height of slurry. Most of settling process were composed of flocculation, hindered settling and self-weight consolidation. On the Other hands, flocculation of soil during settling was observed and it was found that the size and density of flocculated particles could be analyzed by using the method proposed by Richardson and Zaki.

        Gastritis cystica profunda (GCP) is a disease characterized by hyperplastic and significant extension of cystic dilatation of the gastric mucous glands, which results in a cystic lesion in gastric submucosa. It often occurs on the part of gastroenterostomy, but can be found in the stomach without any previous surgery. GCP has variable gross finding including solitary polyps, diffuse ones, submucosal tumors, and rare giant gastric mucosal fold. It is difficult to tell GCP from a cancerous lesion by gross finding that the disease demands a necessary tissue biopsy, though GCP is commonly showed as a benign in the progress. We report a case of GCP in polypoid types diagnosed by endoscopic polypectomy.

        The purpose of this investigation was to develop a new carrier using chitin and glucan as the model for Aspergillus oryzae cell wall. In case of the ratio of chitin and glucan(3/1), the activity of the immobilized protease had higher than that of model(chitin/glucan=1/3) and carriers were formed gel. In case of soluble chitin and glucan treated with calcium carbonate, there were a 1.8-fold increase in activity of immobilized protease compared with untreated chitin. The scanning electron microscope image showed that a pore size of this carrier had a homogenized pore size and a smooth surface.

      • 준설토의 건조수축에 의한 강도증가 특성과 지지력에 관한 연구

        This research is results of experimental and numerical works on characteristic of strength increase and bearing capacity in dredged and reclaimed soil due to desiccation shrinkage. For a soil sampled from southern coastal area in Korea, basic soil property tests and standard consolidation test with falling head permeability tests were carried out to obtain consolidational characteristics of soil. Double cone penetration test. laboratory vane test and unconfined compression test were also performed to investigate the change of shear strength with degree of desiccation. Model tests were performed in 1G environment and 30G level artificially accelerated condition by using the centrifuge model test facilities to investigate the bearing capacity of desiccated ground. Test results were analyzed by using the bearing capacity of desiccated ground. Test results were analyzed by using the theoretical and load-settlement characteristics method proposed by Meyehof & Hanna(1978). On the other hands, the numerical technique, using the finite strain consolidation theory considering the effect of desiccation was used to estimate the appropriate time of using heavy construction equipments in field with respect to strength increase due to desiccation.

        「All things possibly could be measured.」, measurement is a essential part in all kinds of industry, especially in manufacturing. Korean Industrial Standard has been established and announced in September, 1961. Measurement has been emphasized. But until now, measurement hasn't been located to the core of manufacturing in domestic. So, this study suggests improvement guide line for the multilateral problems of measurement system based on the investigation of 146 small and medium-sized enterprises in April, 2002.

        1. The aim of this study The aim of this study is to systematically research the teaching of physical education to mentally reatared children. The research includes a review of physical education programs for the mentally retarded, a study of the facilities available, and a presentation of the actual physical activities carried out in special education classrooms and schools. 2. The method of this study Written questionaries were distributed to sixty teachers teaching either special classes of retared children or in special education schools in the Chung Cheong Nam and Chung Cheong Buk Provinces. The sixty teachers were selected randomly. From the 60 questionsires returned four were not able to be used and so the final data is based upon 56 respondents. All the statistics are expressed as percentages. 3. The results The investigation of the actual state of physical education for retared children and the activities they engage in yielded the following information: 1) The nature, extent, and causes of of retardation are diverses and complex. Each child poses very special individual problems. For this reason it is difficult to design a single curriculum for physical education that will be suitable for all the children in a class. 2) Most of the retarded children come from the lower class. Their home environment is not good and dvelopment. It appears that the parents of trainable mentally retarded children are more interested in their child's development than are the parents of educable mentally retarded children. 3) The mentally retarded children are generally taught according to the syllabus for normal children ; but they should really be taught according to their own special needs according to their individual handicap. 4) The attitude of the mentally retarded children toward physical education is good. The educable mentally retarded children prefer physical activities like gmaes and sports, whereas the trainable mentally retarded children prefer more passive activitities like recreation and dancing. 5) Physical facilities for mentally retarded children are deficient. Facilities must be developed as soon as possible. Facilities that now exist in special schools are not well used because of the lack of specially trained physical education teachers. It seems to me that an awareness of the importance ofphysical education for the retarded is lacking. 6) Physical education teachers' co-operation and the development of special physical education methodology are lacking. The development of special physical education programs for mentally retarded children is an urgent issue. In light of the results outlined here, it is clear that the place of physical education in special education has not been professionally developed. The importance of physical education for mentally retarded children has not been understood and the result is that their education has a serious lack.
