      • 재발성 복부 통증을 호소한 환자에서 복벽에 발생한 자궁내막증 1예

        Endometriosis is the abnormal growth of endometrial tissue outside the uterine cavity. Endometriosis in the abdominal scar following cesarean section is very rare condition among the extrapelvic endometriosis. The frequency of abdominal wall endometriosis secondary to cesarean section is from 0.03% to 0.8% in some reports. It is difficult to diagnosed in spite of the typical symptoms. The typical symptom is aching swelling in the area of the surgical scar, which is influenced by the phases of menstruation. The definitive diagnosis is established by pathologic analysis. Surgical excision remains the treatment of choice. The majority of patients presented from 1 to 2 years after the precipitating operation. We experienced an unusual case of abdominal wall endometriosis presented over 10 years after cesarean section, who was admitted to our hospital complaining of recurrent left lower abdominal pain. So, we report this case with a brief review of the concerned literatures.

      • 종합병원 간호조무사들의 직무만족도와 관련요인

        간호조무사들의 역할변화와 함께 이들의 직무만족도와 직무만족도에 미치는 요인 또한 변화하였을 것이므로 새로운 변화에 맞는 직무만족도 조사가 필요하다. 이 연구는 간호조무사들의 직무만족도를 평가하고, 직무만족도에 영향을 주는 요인을 규명하여, 의료기관의 효율적 운영과 인력관리 그리고 의료서비스의 질적 개선과 간호조무사 개인의 직무만족도를 높이는 방안을 탐색하기 위하여 수행되었다. 대구 광역시에 소재하는 종합병원 2곳, 대학병원 4곳에 근무하는 간호조무사 291명을 대상으로 직무만족도와 관련요인을 조사하였다. 교대근무 및 근무 장소에 따른 직무만족도에서 교대근무를 하지 않는 군이 하는 군에 비해 업무만족도가 유의하게 높았다.(p<0.05). 근로조건만족도는 교대근무를 하는 군이 안하는 군보다 유의하게 높았다(p<0.05). 의료기관만족도와 사회적만족도는 두 군간에 유의한 차이가 없었다. 근무 장소에 따른 만족도에서 직무만족도는 병실근무자가 가장 낮았으며, 근로조건만족도는 외래 근무자가 가장 낮았다. 의료기관만족도는 근무 장소에 따른 차이가 없었으며 사회적만족도는 병실근무자가 가장 낮았다(p<0.05). 간호조무사 업무 종사이유 여부에 따른 만족도는 이유가 있는 군이 이유가 없는 군보다 업무만족도가 유의하게 높았다(p<0.01). 간호조무사 업무 종사 이유에 따른 직무만족도에서 적성에 적합한 군의 업무만족도가 가장 높고 타 분야 취업이 곤란해서라는 군이 가장 낮았으며 두 군의 차이는 유의하였다(p<0.05). 이직 경험 유무에 따른 직무만족도는 이직한 적이 없는 군이 있는 군에 비해 업무만족도가 낮았고(p<0.05), 근로조건만족도는 이직한 적이 있는 군이 만족도가 높았다. 전직의사를 갖고 있는 정도에 따른 직무만족도는 전직의사가 없는 대상자가 업무만족도가 가장 높고 전직의사가 매우 높은 대상자가 업무만족도가 가장 낮았다(p<0.05). 교대근무자 즉, 병실근무자의 직무만족도가 현저히 떨어지고 동료 및 상사의 관계가 직무만족도에 미치는 영향을 고려할 때 교대근무제도의 개선과 간호사와 간호조무사간의 원활한 업무협조를 통한 직장 내 직무 만족도 향상방안을 마련하여 간호조무사가 자신의 업무에 보람을 느끼고 능률적으로 직무에 임함으로서 양질의 의료서비스를 국민들에게 제공할 수 있도록 해야 할 것이다. Background : The main aim of this study was to analyze the factors that could be used to predict job satisfaction of assistant nurses in general hospitals. Material and Methods : This study was based on a survey of assistant nurses in the Daegu region from January 1, 2006 to January 31, 2006. Originally 300 questionnaires were distributed and among them 291 were completed. The data was collected and used for the statistical analysis. Descriptive and inferential statistics, such as the frequency, t-test and one-way ANOVA were used for the analysis. Results : Among the respondents, the average score for job satisfaction was lowest in the university-trained nurses and highest in the nurses who had only graduated from high school. Conclusion : The working conditions, work place and motivation are significantly associated with job satisfaction. In particular, there was lower job satisfaction in those nurses who wanted to change their occupation.

        소아복부둔상시 저혈량성 쇽과 연관된 복부전산화 단층촬영술의 유용성

        Background: We reviewed retrospectively the clinical features and contrast material-enhanced computed tomography(CT) after blunt abdominal trauma demonstrated a characteristic hypoperfusion complex. Our purpose were to evaluate the hypothesis that children with this "hypoperfusion complex" CT finding were associated with a tenuous hemodynamic state, severe injury and a poor outcome. Methods: Between January 1996 and December 1999, 39 consecutive children who sustained blunt trauma and were suspected clinically of having intraabdominal injury underwent CT. Demographic data, blood pressure, indication for CT, pediatric trauma score(PTS) and Glasgow coma score(GCS) were recorded at the time of the initial examination. Results: Three of the 39 children(8%) demonstrated a characteristic finding at CT which was described as the hypoperfusion complex. The "hypoperfusion complex" was present in the 3 of the 39 traumatized children(8%), but was the main radiographic finding in the 3 of the 14 children with a severe pediatric trauma score(PTS<8) and in the 3 of the 6 children with a Glasgow coma score(GCS<8) who were examined with CT. Conclusion: The CT findings in all children with the hypoperfusion complex by definition included marked, diffuse dilatation of the intestine with fluid; abnormally intense contrast enhancement of the bowel wall, mesentery, kidney and/or pancreas; decreased caliber of the abdominal aorta and inferior vena cava(IVC); and moderate to large peritoneal fluid collection. This "hypoperfusion complex" is a relatively rarely observed in injured children but appears to be associated with severe injury and a poor outcome. The pediatric trauma score provides a useful profile of injury severity in children. Prompt diagnosis of hypovolemic shock is important so that supportive therapy can be instituted to prevent further metabolic abnormalities and their potential cardiotoxic effects. In summary, the intense multiorgan enhancement pattern seen in the hypoperfusion complex indicates tenuous hemodynamic stability and is associated with a poor outcom.

        상악 치아군의 저항중심의 위치에 관한 3차원 유한요소 해석

        최근 골내 고정 형태의 temporary anchorage device (TAD)를 많이 이용하게 되면서 다양한 위치로부터 그리고 강한 교정력을 이용할 수 있게 되었다. 이에 따라 치아군의 이동양상을 예측하고 치료계획을 세우기 위하여 다양한 치아군의 저항중심의 위치에 대한 이해가 필요하게 되었다. 본 연구에서는 3차원 유한요소해석을 이용하여 상악 4전치, 6전치 그리고 상악 전치열에서 3차원적 저항중심의 위치를 조사하고자 하였다. 이를 위하여 상악 전치열 14개 치아와 치근막 및 치조골의 3차원 유한요소모델을 제작하였고, 각 치아군별로 치관부를 협측, 설측 호선, 설측 splint wire로 고정하여 개별 치아이동을 최소화하고 적용된 힘이 치아에 고루 분산되도록 하였다. 상악 중절치 절단연의 중점에서 연장된 와이어 빔에 수직, 수평으로 100 g 또는 200 g의 힘을 가하여 치아의 변위를 해석하고, 각 치아군에 속한 치아들이 최대한 평행이동 되는 힘의 적용부위를 저항중심으로 정의하였다. 연구결과 상악 4전치군의 저항중심은 상악 중절치 절단연으로부터 치근방향 13.5 mm, 후방 12.0 mm, 상악 6전치군은 상악 중절치 절단연으로부터 치근방향 13.5 mm, 후방 14.0 mm에 위치하였으며 상악 전치열군의 저항중심은 상악 중절치 절단연으로부터 치근방향 11.0 mm, 후방 26.5 mm에 위치하였다. 본 유한요소 실험모델을 이용하여 얻은 결과는 교정치료의 효율성을 높일 수 있으리라 생각된다. Objective: The aim of this study was to investigate the 3-dimensional position of the center of resistance of the 4 maxillary anterior teeth, 6 maxillary anterior teeth, and the full maxillary dentition using 3-dimensional finite element analysis. Methods: Finite element models included the whole upper dentition, periodontal ligament, and alveolar bone. The crowns of the teeth in each group were fixed with buccal and lingual arch wires and lingual splint wires to minimize individual tooth movement and to evenly disperse the forces to the teeth. A force of 100 g or 200 g was applied to the wire beam extended from the incisal edge of the upper central incisor, and displacement of teeth was evaluated. The center of resistance was defined as the point where the applied force induced parallel movement. Results: The results of study showed that the center of resistance of the 4 maxillary anterior teeth group, the 6 maxillary anterior teeth group, and the full maxillary dentition group were at 13.5 mm apical and 12.0 mm posterior, 13.5 mm apical and 14.0 mm posterior, and 11.0 mm apical and 26.5 mm posterior to the incisal edge of the upper central incisor, respectively. Conclusions: It is thought that the results from this finite element models will improve the efficiency of orthodontic treatment.

        The purpose of this study is to examine the contentment for wind surfing class. 52 of male-student and 44 of female-student were participated in questionnaire which based on the necessary, contentment, curriculum, facility and teaching method and etc. The results are following like this : For a question asking about the contentment of wind surfing class there were significant difference between male and female student. About the question for curriculum of wind-surfing class there were not significant difference male and female student. On the question about facility it can not find the significant difference between male and female student. In addition female-student and male-student agreed to the question about teaching method. Lastly they were demanded to write something about improvement. They required to development of facility and incidental facility. Edpecially female student want to have special program for womens'. Therefore it must be needed to extend the facility, incidental facility and improved curriculum for the better wind-surfing class.

        감마선투과검사 장치의 자동 원격조작기 개발

        국내 감마선투과검사용 조사장치는 1,000여대로 2,500여명의 종사자가 대부분 수동으로 원격 조작하는 실정으로 작업의 효율화와 방사선피폭 저감화를 위하여 자동 원격조작기의 사용이 요구된다. 이에 연구팀은 실무에 적합한 토크와 안전계수로부터 54kgf·㎝의 출력을 지닌 24V, 200W의 BLDC모터를 이용하여 0.4∼1.2 m/s 범위의 선원 이동속도를 조절할 수 있는 구동장치와 RF 센서로 구동 속도 조절, 광전센서에 의한 선원 위치 및 timer로 노출시간을 제어할 수 있는 무선 원격조작기를 개발하였다. 이와 같이 개발한 자동원격조작기는 AC와 DC 겸용, 그리고 기존의 수동 원격조작기에 탈착이 가능하여 자동과 수동을 겸용할 수 있는 경제적 이점 등으로 활용도가 클 것으로 기대된다. Recently, gamma radiographic equipments have been used about 1,000 sets manually and operated by about 2,500 persons in Korea. In order for a radiography to work effectively with avoiding any hazard of the high level radiation from the source, many field workers have expected developing a wireless automatic remote exposure controller. The KITCO research team has developed an automatic remote exposure controller that can regulate the speed of 0.4∼1.2 m/s by BLDC motor of 24V 200W which has output of 54 kgf·㎝, suitable torque and safety factor for the work. And the developed automatic remote exposure controller can control rpm of motor, pigtail position by photo-sensor and exposure time by timer to RF sensor. Thus, the developed equipment is expected that the unit can be used in many practical applications with benefits in economical advantage to combine the use of both automatic and manual type because attachment is possible existent manual remote exposure controller, AC and DC combined use.

      • 토양환경지도 자료를 이용한 콩밭 토양의 화학성 비교

        It is important to interpret the soil chemical properties in order to effectively manage the farmland. This study was conducted to investigate the soil chemical properties of upland soybean field according to the soil series at 32 upland fields located in the parts of Gangwon-do. Soil sampling sites were selected by using the National Statistics DB and KSIS DB that is a soil environment information system provided by the Rural Development Administration. Soil samples were collected from 0~15 cm of top soil before seeding or transplanting, air-dried, passed through a 2 mm sieve, and analyzed for soil pH, EC, organic matter, available phosphorus (P2O5), exchangeable potassium, exchangeable calcium and exchangeable magnesium. The average chemical properties have exceeded the each nutrient optimum range level of RDA, except the organic matter in experimental soil 2015. And despite application of conventional fertilization, each soil series nutrient contents were change respectively. The results of this study suggest that conventional fertilization should be applied based on the analysis about the amount of accumulated nutrient contents in the soil.

        염산용액에서 비누화 PC88A에 의한 용매추출로 Gd와 Sm의 분리

        Solvent extraction experiments have been performed to separate Gd and Sm from chloride solution with PC88A and saponified PC88A. The effects of extraction conditions on the distribution coefficients of Gd and Sm were investigated. The distribution coefficients of Gd were higher than those of Sm in the experimental ranges of experimental conditions of this study. A solvent extraction model was developed to predict the distribution coefficients of metals and separation factor between Gd and Sm from the initial extraction conditions. The measured distribution coefficients of Gd and Sm with PC88A and saponified PC88A showed good agreements with the predicted values. (Received March 3, 2005)
